Although interferon (IFN) based therapy for recurrent hepatitis C virus (HCV) infection after liver transplantation has been widely accepted, it induces various adverse effects such as thrombocytopenia, resulting in its interruption. Recently, concomitant splenectomy at the time of living donor liver transplantation (LDLT) has been tried to overcome this problem, but this procedure leads to several complications such as excessive intraoperative bleeding and serious infection. A 60-year-old female received LDLT using a left lobe graft from her second son for liver failure caused by hepatitis C-related cirrhosis. Six months after LDLT, she was diagnosed as recurrent HCV infection by liver biopsy. IFN monotherapy was started from 7 mo after LDLT and her platelet count decreased to less than 50 000/μL, which thus made it necessary to discontinue the treatment. We decided to attempt laparoscopic splenectomy (LS) under general anesthesia. Since intra-abdominal findings did not show any adhesion formations around the spleen, LS could be successfully performed. After LS, since her platelet count immediately increased to 225 000/μL 14 d after operation, IFN therapy was restarted and we could convert the combination therapy of IFN and ribavirin, resulting in no detectable viral marker. Inconclusion, LS can be performed safely even after LDLT, and LS after LDLT is a feasible and less invasive modality for thrombocytopenia caused by antiviral therapy.

AIM:To prevent pancreatic leakage after pancreaticoje-junostomy,we designed a new standardized technique that we term the "Pair-Watch suturing technique".METHODS:Before anastomosis,we imagine the faces of a pair of watches on the jejunal hole and pancreatic *** first stitch was put between 9 o'clock of the pancreatic side and 3 o'clock of the jejunal side,and a total of 7 stitches were put on the posterior wall,followed by the 5 stitches on the anterior *** this technique,twelve stitches can be sutured on the first layer anastomosis regardless of the caliber of the pancreatic *** all cases the amylase activity of the drain were measured.A postoperative pancreatic fistula was diagnosed using postoperative pancreatic fistula ***:From March 2007 to July 2008,29 consecutive cases underwent pancreaticojejunostomy using this *** examination results showed pancreatic carcinoma(n=14),intraductal papillary-mucinous neoplasm(n=10),intraductal papillary-mucinous carcinoma(n=1),carcinoma of ampulla of Vater(n=1),carcinoma of extrahepatic bile duct(n=1),metastasis of renal cell carcinoma(n=1),and duodenal carcinoma(n=1).Pancreaticojejunal anastomoses using this technique were all watertight during the surgical *** mean diameter of main pancreatic duct was 3.4 mm(range 2-7 mm).Three patients were recognized as having an amylase level greater than 3 times the serum amylase level,but all of them were diagnosed as grade A postoperative pancreatic fistula grading and required no *** of the cases developed complications such as hemorrhage,abdominal abscess,and pulmonary *** was no postoperative ***:Our technique is less complicated than other methods and very secure,providing reliable anastomosis for any size of pancreatic duct.

Guide vanes are installed in the Wells turbine in order to improve its efficiency, self-rotating characteristics and off design performance with stall. This work attempts to explain the role of these guide vanes on the basis of momentum theory. It is shown that the upstream vanes are more effective in enhancing efficiency than the downstream ones. A design method for guide vanes is suggested based on experimental data and potential theory. Experimental studies carried out by the author confirm the theory proposed.

The incorporation of the saturation of the tangential contact stress with the increase of the normal contact stress is required for the analysis of the friction phenomenon of solids and structures subjected to a high normal contact stress,which cannot be described by the Coulomb friction condition,in which the tangential contact stress increases linearly with the increase of the normal contact *** this article,the subloading-friction model,which is capable of describing the smooth elastic-plastic transition,the static-kinetic transition,and the recovery of the static friction during the cease of sliding,is extended to describe this ***,some numerical examples are shown,and the validity of the present model will be verified by the simulation of the test data on the linear sliding of metals.

Influence of agricultural activities,forest fires and agro-industries on air quality in Thailand

Annual and monthly-based emission inventories in northern, central and north-eastern provinces in Thailand, where agriculture and related agro-industries are very intensive,were estimated to evaluate the contribution of agricultural activity, including crop residue burning, forest fires and related agro-industries on air quality monitored in corresponding provinces. The monthly-based emission inventories of air pollutants, or, particulate matter（PM）, NOx and SO2, for various agricultural crops were estimated based on information on the level of production of typical crops： rice, corn, sugarcane, cassava, soybeans and potatoes using emission factors and other parameters related to country-specific values taking into account crop type and the local residue burning period. The estimated monthly emission inventory was compared with air monitoring data obtained at monitoring stations operated by the Pollution Control Department, Thailand（PCD） for validating the estimated emission inventory. The agro-industry that has the greatest impact on the regions being evaluated, is the sugar processing industry, which uses sugarcane as a raw material and its residue as fuel for the boiler. The backward trajectory analysis of the air mass arriving at the PCD station was calculated to confirm this influence. For the provinces being evaluated which are located in the upper northern, lower northern and northeast in Thailand, agricultural activities and forest fires were shown to be closely correlated to the ambient PM concentration while their contribution to the production of gaseous pollutants is much less.

Several studies revealed that gut microbiota are associated with various human diseases,e.g.,metabolic diseases,allergies,gastroenterological diseases,and liver *** liver can be greatly affected by changes in gut microbiota due to the entry of gut bacteria or their metabolites into the liver through the portal vein,and the liver-gut axis is important to understand the pathophysiology of several liver diseases,especially non-alcoholic fatty liver disease and hepatic ***,gut microbiota play a significant role in the development of alcoholic liver disease and *** on theseprevious findings,trials using probiotics have been performed for the prevention or treatment of liver *** this review,we summarize the current understanding of the changes in gut microbiota associated with various liver diseases,and we describe the therapeutic trials of probiotics for those diseases.

The prognosis of patients with hepatocellular cardnorna （HCC） accompanied by portal vein tumor thrombus （PVTT） is generally poor if leo untreated： a median survival time of 2.7-4.0 mo has been reported. Furthermore, while transcatheter arterial chemoembolization （TACE） has been shown to be safe in selected patients, the median survival time with this treatment is still only 3.8-9.5 mo. Systemic single-agent chemotherapy for HCC with PVTT has failed to improve the prognosis, and the response rates have been less than 20%. While regional chemotherapy with low-dose cisplatin and 5-fluorouracil or interferon and 5-fluorouracil via hepatic arterial infusion has increased the response rate, the median survival time has not exceeded 12 （range 4.5-11.8） mo. Combined treatment consisting of radiation for PVTT and TACE for liver tumor has achieved a high response rate, but the median survival rates have still been only 3.8-10.7 mo. With hepatic resection as monotherapy, the 5-year survival rate and median survival time were reportedly 4%-28.5% and 6-14 mo. The most promising results were reported for combined treatments consisting of hepatectomy and TACE, chemotherapy, or internal radiation. The reported 5-year survival rates and median survival times were 42% and 31 mo for TACE followed by hepatectomy; 36.3% and 22.1 mo for hepatectomy followed by hepatic arterial infusion chemotherapy; and 56% for chemotherapy or internal radiation followed by hepatectomy.

Objective:Cytopathic effects and local immune response were analyzed histologically in prostatic cancer(PCa)with in situ herpes simplex virus-thymidine kinase(HSV-tk)/ganciclovir(GCV)gene therapy(GT).Methods:Four high-risk PCa patients who received HSV-tk/GCV GT were *** two cycles of intraprostatic injection of HSV-tk and administration of GCV,radical prostatectomy was ***-fixed,paraffin-embedded sections were evaluated using *** with hormone therapy(HT,n=3)or without neoadjuvant therapy(NT,n=4)that were equivalent in terms of risk were also examined as ***-positive cells were counted in at least three areas in cancer *** indices(LI)were calculated as percentage ***:ssDNA LI in GT increased,indicating apoptosis,as well as tumor-infiltrating lymphocytes and CD68-positive macrophages,compared with their *** cases showed significantly higher numbers of single-stranded DNA(ssDNA)LI,CD4/CD8-positive T cells and CD68-positive macrophages including M1/M2 macrophages than HT or NT ***,there was no significant difference in CD20-positive B cells among the types of *** were strong correlations between CD8+T cells and CD68+macrophages(ρ=0.656,p<0.0001)as well as CD4+T cells and CD20+B cells(ρ=0.644,p<0.0001)in PCa with ***:Enhanced cytopathic effect and local immune response might be indicated in PCa patients with HSV-tk/GCV gene therapy.

ANALYSIS ON PSEUDO-STEADY INDENTATION CREEP

Theoretical analysis and finite element （FE） simulation have been carried out for a constant specific load rate （CSLR） indentation creep test. Analytical results indicate that both the representative stress and the indentation strain rate become constant after a transient period. Moreover, the FE simulation reveals that both the contours of equivalent stress and equivalent plastic strain rate underneath the indenter evolve with geometrical self-similarity. This suggests that pseudo-steady indentation creep occurs in the region beneath the indenter. The representative points in the region are defined as the ones with the equivalent stress equal to the representative stress. In addition, it is revealed that the proportionality between indentation strain rate and equivalent plastic strain rate holds at the representative points during the pseudo-steady indentation creep of a power law material. A control volume （CV） beneath the indenter, which governs the indenter velocity, is identified. The size of the CV at the indented surface is approximately 2.5 times the size of the impression. The stress exponent for creep can be obtained from the pseudosteady indentation creep data. These results demonstrate that the CSLR testing technique can be used to evaluate creep parameters with the same accuracy as conventional uniaxial creep tests.
