Complete mesocolic excision and central vascular ligation in colorectal cancer in the era of minimally invasive surgery

Since the 19th century,appropriate lymphadenectomy has been considered a cornerstone of oncologic surgery and one of the most important prognostic *** approach can be applied to any surgery for gastrointestinal *** surgery for colon and rectal cancer,an adequate portion of the mesentery is removed together with the segment of bowel affected by the *** adequate number of lymph nodes to be removed is standardized and reported by several *** is mandatory to determine the appropriate extent of lymphadenectomy and to balance its oncological benefits with the increased morbidity associated with its execution in cancer *** review focuses on the concept of“complete mesenteric excision(CME)with central vascular ligation(CVL),”a radical lymphadenectomy for colorectal cancer that has gained increasing interest in recent *** aim of this study was to evaluate the evolution of this approach over the years,its potential oncologic benefits and potential risks,and the improvements offered by laparoscopic *** advantages of CME are improved local-relapse rates due to complete removal of the intact mesocolic fascia and improved distance recurrence rates due to ligation of vessels at their origin(CVL)which guarantees removal of a larger number of lymph *** development and worldwide diffusion of laparoscopic techniques minimized postoperative trauma in oncologic surgery,providing the same oncologic results as open *** has been widely applied to colorectal cancer surgery;however,CME entails a technical complexity that can limit its wide minimally-invasive application. This review analyzesresults of these procedures in terms of oncological outcomes, technical feasibilityand complexity, especially within the context of minimally invasive surgery.

Progressive aging of the population and prolonga tion of life expect ancy have led to the ris ng prevalence of heart failure (HF)。Despitethe improvements in medical therapy，the mortality rate of this condition has remained unaccept ably high，becoming t heprimary cause ofdeath in the elderly popul at ion。Almost half of patient swith signs and symptoms of HF are f ound to have a nearly normal ejection fraction，which delineates a distinctclinical syndrome 。known as HF with preserved ejection fraction(HF-PEF) 。While early research focused on the importance of diastolicdys function，more recent studies reported the pathophysological compl exity of the disease with multiple cardiovas 。cular abnormalities contributing to its development and progression。HF-PEF is a challenging maj or health problem with yet no solution as there is no evidence。Based treatment which improves clinical out comes。This review summarizes the state of current knowledge on diagnosis，prognosis and treatment of HF。PEF，with particul ar insights on the pathological characteristics in the elderly population。

Biliverdin Reductase-A correlates with inducible nitric oxide synthasein in atorvastatin treated aged canine brain

Alzheimer’s disease is a neurodegenerative disorder characterized by progressive cognitive impairment and neuropathology. Recent preclinical and epidemiological studies proposed statins as a possible therapeutic drug for Alzheimer’s disease, but the exact mechanisms of action are still unknown. Biliverdin reductase-A is a pleiotropic enzyme involved in cellular stress responses. It not only transforms biliverdin-IX alpha into the antioxidant bilirubin-IX alpha but its serine/threonine/ tyrosine kinase activity is able to modulate cell signaling networks. We previously reported the beneficial effects of atorvastatin treatment on biliverdin reductase-A and heme oxygenase-1 in the brains of a well characterized pre-clinical model of Alzheimer’s disease, aged beagles, together with observed improvement in cognition. Here we extend our knowledge of the effects of atorvastatin on inducible nitric oxide synthase in parietal cortex, cerebellum and liver of the same animals. We demonstrated that atorvastatin treatment （80 mg/day for 14.5 months） to aged beagles selectively increased inducible nitric oxide synthase in the parietal cortex but not in the cerebellum. In contrast, inducible nitric oxide synthase protein levels were significantly decreased in the liver. Significant positive correlations were found between biliverdin reductase-A and inducible nitric oxide synthase as well as heme oxygenase-1 protein levels in the parietal cortex. The opposite was observed in the liver. Inducible nitric oxide synthase up-regulation in the parietal cortex was positively associated with improved biliverdin reductase-A functions, whereas the oxidative-induced impairment of biliverdin reductase-A in the liver negatively affected inducible nitric oxide synthase expression, thus suggesting a role for biliverdin reductase-A in atorvastatin-dependent inducible nitric oxide synthase changes. Assessment and impact of diastolic function by echocardiography in elderly patients

Doppler echocardiography is the gold standard for assessment of diastolic dysfunction, which is increasingly recognised as a cause of heart failure, especially in the elderly. Using a combination of Doppler echocardiography techniques, it is possible to identify grades of dia- stolic dysftmction, estimate left ventricular filling pressures and establish the chronicity of diastolic dysfunction. These physiologi- cally-derived measures have been widely validated against invasive measurements of left heart pressures and have been shown to be prog- nostically valuable in a wide range of clinical settings. This review explores the mechanisms, and approaches to the assessment of diastolic dysfunction in the elderly. The challenge for clinicians is to identify pathophysiological changes from those associated with normal ageing. When used in combination, and taking age into account, Doppler echocardiographic parameters are helpful in the assessment of dyspnoea in older patients and provide prognostic insights.

Interaction of cetyltrimethylammonium bromide with cefixime trihydrate drug at different temperatures and compositions: Effect of different electrolytes

Herein,conductivity measurements have been carried out to explore the interaction between cetyltrimethylammonium bromide(CTAB,a cationic surfactant) and antibiotic drug(cefixime trihydrate(CMT)) in water and also in occurrence of inorganic salts(NaCl,Na2 SO4 and Na3 PO4) over the temperature range of 303.15-323.15 K with an interval of 5 *** all cases,two critical micelle concentrations(c^*) were achieved for the CMT-surfactant *** of CMT drug to CTAB solution decreases the values of c^* which indicates the interaction between CMT and *** values of c^* for CMT-CTAB mixture in the presence of salts are lower in magnitude compared to the aqueous medium which indicates that micellization of the CMT-CTAB mixed system is favorable in salt *** values of △Gm^0 were obtained to be negative indicating the spontaneity of the micellization process and the extent of spontaneity further increases by means of rising *** obtained outcomes from the ΔHm^0 and ΔSm^0 values disclose that the interactions between CMT and CTAB are mostly electrostatic along with hydrophobic in *** thermodynamic parameters of transfer and enthalpy-entropy compensation phenomenon were also determined and discussed in detail.

Intranasal nerve growth factor bypasses the blood-brain barrier and affects spinal cord neurons in spinal cord injury

The purpose of this work was to investigate whether, by intranasal administration, the nerve growth factor bypasses the blood-brain barrier and turns over the spinal cord neurons and if such therapeutic approach could be of value in the treatment of spinal cord injury. Adult Sprague-Dawley rats with intact and injured spinal cord received daily intranasal nerve growth factor administration in both nostrils for 1 day or for 3 consecutive weeks. We found an in-creased content of nerve growth factor and enhanced expression of nerve growth factor receptor in the spinal cord 24 hours after a single intranasal administration of nerve growth factor in healthy rats, while daily treatment for 3 weeks in a model of spinal cord injury improved the deifcits in locomotor behaviour and increased spinal content of both nerve growth factor and nerve growth factor receptors. These outcomes suggest that the intranasal nerve growth factor bypasses blood-brain barrier and affects spinal cord neurons in spinal cord injury. They also suggest exploiting the possible therapeutic role of intranasally delivered nerve growth factor for the neuroprotection of damaged spinal nerve cells.

Energy planning and solar plant site selections are vital strategic decisions and one of the most complex executive challenges in the interconnected *** is essential to study the potential renewable energy sources in Afghanistan to select the most sustainable sites for solar power production in populated *** study is based on the combination of a Geographic Information System,Remote sensing,and multi-criteria decision-making technique to evaluate the optimal placement of photovoltaic solar power plants in the Kabul province,capital of *** models,Analytical Hierarchy Process(AHP)and Analytical Network Process(ANP),were used to select suitable areas for establishing a solar power *** application of the proposed model has been made possible by integrating four constraints such as climate,environmental,topography,and economical which comprised twelve criteria:solar radiation,yearly average rainfall,land slope,aspect,land use,dust,geology and proximity to faults,main roads,Normalized difference vegetation index,urban areas river and water *** findings indicate that there is no considerable difference between the results of both models since both models identified more than 20%of the total area of Kabul province in suitable *** maps conclude that northern and southern parts of Kabul city and the eastern part of Kabul province came to the range of suitable *** can be concluded that Kabul province is a source of sufficient potential for producing solar *** results of this study can support the plans of the Afghanistan government in solar energy production and the implementation of photovoltaic power plants.

Topical delivery of nerve growth factor for treatment of ocular and brain disorders

Neurotrophins are a family of proteins that support neuronal proliferation, survival, and differentiation in the central and peripheral nervous systems, and are regulators of neuronal plasticity. Nerve growth factor is one of the best-described neurotrophins and has advanced to clinical trials for treatment of ocular and brain diseases due to its trophic and regenerative properties. Prior trials over the past few decades have produced conflicting results, which have principally been ascribed to adverse effects of systemic nerve growth factor administration, together with poor penetrance of the blood-brain barrier that impairs drug delivery. Contrastingly, recent studies have revealed that topical ocular and intranasal nerve growth factor administration are safe and effective, suggesting that topical nerve growth factor delivery is a potential alternative to both systemic and invasive intracerebral delivery. The therapeutic effects of local nerve growth factor delivery have been extensively investigated for different ophthalmic diseases, including neurotrophic keratitis, glaucoma, retinitis pigmentosa, and dry eye disease. Further, promising pharmacologic effects were reported in an optic glioma model, which indicated that topically administered nerve growth factor diffused far beyond where it was topically applied. These findings support the therapeutic potential of delivering topical nerve growth factor preparations intranasally for acquired and degenerative brain disorders. Preliminary clinical findings in both traumatic and non-traumatic acquired brain injuries are encouraging, especially in pediatric patients, and clinical trials are ongoing. The present review will focus on the therapeutic effects of both ocular and intranasal nerve growth factor delivery for diseases of the brain and eye.
