Experimental investigating on the reflected waves from the caisson-type vertical porous seawalls

The hydrodynamic efficiencies of caisson-type vertical porous seawalls used for protecting coastal areas were calculated in this study. Physical models were developed to compare the wave reflection from vertical plane, semi-porous, and porous seawalls caused by both regular and random waves. Tests were carried out for a wide range of wave heights, wave periods, and different water depths (d=0.165, 0.270 and 0.375 m). The performance regarding the reflected waves from porous and semi-porous seawalls showed improvement when compared with those from the plane seawall. The reflection coefficients of the porous and semi-porous seawalls were calculated as 0.6 and 0.75, respectively, while the coefficient for the fully reflecting plane vertical wall was significantly higher (0.9). It was also observed that the reflection coefficient decreases with increase in wave steepness and relative water depth. In addition, the reduction in the reflection coefficient of porous and semi-porous seawalls, as compared to that of a plane seawall, was observed for both regular and random waves. New equations were also proposed to calculate the reflection coefficient of different types of seawalls with the aid of laboratory experiments. By verifying the developed equations using some other experimental data, it was validated that the equations could be used for practical situations. The results of the present study can be applied to optimize the design of vertical seawalls and for coastal protecting schemes.

Applying Finite Difference Method to Simulate the Performance of a Perforated Breakwater Under Regular Waves

Using a discretized finite difference method, a numerical model was developed to study the interaction of regular waves with a perforated breakwater. Considering a non-viscous, non-rotational fluid, the governing equations of Laplacian velocity potential were developed, and specific conditions for every single boundary were defined. The final developed model was evaluated based on an existing experimental result. The evaluated model was used to simulate the condition for various wave periods from 0.6 to 2 s. The reflection coefficient and transmission coefficient of waves were examined with different breakwater porosities, wave steepnesses, and angular frequencies. The results show that the developed model can suitably present the effect of the structural and hydraulic parameters on the reflection and transmission coefficients. It was also found that with the increase in wave steepness, the reflection coefficient increased logarithmically, while the transmission coefficient decreased logarithmically.

Hyaluronidase in sub-Tenon's anesthesia for phacoemulsification,a double-blind randomized clinical trial

AIM:To investigate the effect of hyaluronidase use on the quality of sub-Tenon's anaesthesia for phacoemulsification.· METHODS:This was a randomized,double-blind clinical trial which was conducted at Nikookari Eye Hospital for 5 ***-two eyes of candidates for phacoemulsification under sub-Tenon's anaesthesia were randomly allocated to two equal groups and received either 2 mL of lidocaine 2% solution with(LH),or without(L) addition of hyaluronidase(150IU/mL).Akinesia was assessed 15 minutes after sub-Tenon's *** and surgeon's satisfaction,as well as the postoperative pain(the visual analogue scale,VAS) were investigated after *** contingency tables(including the Chi-square or Fisher's exact tests,when appropriate) and parametric analysis(the independent samples t test) were used for statistical analysis.· RESULTS:Complete akinesia(33.3% vs 4.8%,P =0.04),as well as the patients(85.7% vs 57.1%,P =0.04)and surgeon's satisfaction(87.5% vs 52.4%,P =0.02) were significantly more frequent in LH than in L *** mean VAS was significantly lower in the same group(1.90±1.45 vs 3.00±1.55,P =0.04).· CONCLUSION:Addition of hyaluronidase to lidocaine solution for sub-Tenon's anesthesia significantly improves the ocular akinesia,enhances the intra-operative patients and surgeons' satisfaction,and attenuates the postoperative pain.·

目的:研究核黄素和紫外线(UVA)交联疗法对晚期大泡性角膜病的疗效。方法:15例人工晶状体大泡性角膜病变(PBK)患者参与研究。分别于角膜交联前和干预后1,6mo,对患者行裂隙灯检查、视敏度、异物调查表、角膜透明度分级、眼部疼痛度范围、分析仪角膜厚度分析及超声波角膜厚度测量。氯化钠溶液处理1wk后,去除8mm直径范围的中央角膜上皮,在角膜上频繁点滴核黄素30min,后行3min紫外线(UVA)照射交联疗法。结果:15例患者(5男,10女),平均年龄为66±13岁。平均随访时间6.2mo。治疗后1mo全眼角膜透明度统计结果明显优于术前(P<0.05)。6mo时8眼角膜透明度较术前更佳,5眼持平,2眼低于术前(P=0.218)。70%病患异物感减弱。处理后1mo平均中央角膜厚度(CCT)减少(P<0.05)。6mo时,除3眼外均出现渐进性肿胀,但统计显示CCT明显薄于术前(P=0.006)。统计显示术后1mo平均矫正远视力(CDVA)明显优于术前(P=0.010)。6mo后无显著差异(P=0.130)。1mo痛值统计结果明显优于术前(P=0.007)。但6mo平均痛值较1mo高,较术前无明显差异(P=0.070)。结论:术后1mo结果显示角膜胶原交联疗法可显著改善角膜透明度、角膜厚度和眼痛。但是,它对减少PBK患者疼痛和维持角膜透明度方面似乎并无长期持久的效果。这种方法延长了角膜移植的时间间隔,并提高了角膜内皮移植术(DSAEK)过程的可视化。

目的:探讨超声乳化术中透明质酸酶对筋膜下麻醉的疗效。方法:该研究是一项双盲随机临床试验,在Nikookari眼科医院进行了长达5mo。行超声乳化术的候选患者中接受筋膜下麻醉的42眼随机分为均等的两组,分别应用 20g/L的利多卡因溶液2mL+150U/mL透明质酸酶(LH),和单纯20g/L的利多卡因溶液2mL( L)。筋膜下注射15min 后活动受抑制。患者和医生都非常满意,对于术后疼痛的抑制(视觉模拟评分,the visual analogue scale,VAS) 也同样满意。统计学分析采用列联表(包括卡方检验或 Fisher's精确检验) 和参数分析(独立样本t检验)。结果:活动抑制(33.3%vs4.8%,P=0.04)、患者满意度(85.7%vs57.1%,P=0.04)和医生满意度(87.5% vs52.4%,P=0.02),LH组明显高于L组。VAS在相同组内明显较低(1.90±1.45vs3.00±1.55,P=0.04)。结论:利多卡因溶液+透明质酸酶的筋膜下麻醉明显使得眼部活动受限,提高了患者和医生的满意度,并减轻了术后疼痛。

Guided Botulinum Toxin Injections for Limbs Spasticity:The Winning Duets

Botulinum toxin found its way into a lot of medical treatments improving the quality of life in many clinical *** treating spasticity,this toxin is injected in the affected muscle to decrease its tension and *** isolation of the affected muscles for injection is done either by anatomical palpation of the muscle which is also known as non-guided injection technique,or by using the ultrasound or the electromyography also known guided injection *** this study which is a prospective observational study that included patients over a 6 months period,28 patients aged between 2 and 78 years,with spasticity na?ve to botulinum toxin injections or had already been injected more than 3 months ago,were injected using guided injection *** optimal doses of botulinum toxin were administered based on the age,the severity of the condition,and affected muscle *** pediatric population these were also calculated according to the body *** patients were evaluated pre and 4 weeks post injection using the MAS Score(Modified Ashworth Scale).Statistical analysis showed a very significant response to treatment as compared to the pre-injection condition,for upper limbs,lower limbs,and all muscle groups with almost absent significant local or systemic side effects(with a p value less than 0.001).Conclusion:guided techniques for botulinum toxin injections showed a great efficacy in the treatment of spasticity along with no regional or systemic side effects and are relatively well tolerated by *** finding leads to a better management of spasticity and to a decrease in oral medication intake and secondarily a decrease in their possible side effects.

Dedicated Magnetic Resonance Imaging Sequences:Contribution in the Diagnosis of Focal Epilepsy in the Lebanese Population

There have been many advances in the diagnosis and management of focal epilepsies particularly with neuroimaging techniques and magnetic resonance imaging(MRI).Special MRI sequences can be employed to localize and resect the epileptogenic lesions responsible for focal *** study aims to show the benefit of dedicated epilepsy MRI sequences in the diagnosis of focal epilepsies.A general electric 1.5 Tesla MRI machine was used with standard and special ***,a Nihon Kohden electroencephalography(EEG)machine was *** is a prospective observational study that included 51 patients with focal epilepsies who had an initial negative brain *** underwent epilepsy MRI sequences along with a prolonged video EEG monitoring to localize the lesion,and then results were analyzed statistically using SPSS 22 *** majority of patients were males(62.75%)with a mean age of 30 *** grand majority of patients(74.5%,p value of 0.001)had their lesion localized by the epilepsy *** most commonly found pathology was mesial temporal sclerosis.A significant number of patients(23.5%)were sent for an epilepsy surgery(p value 0.002).This study shows the significant impact of dedicated epilepsy MRI sequences on the diagnosis and management of focal epilepsy in the Lebanese population.
