Objective:To investigate the prevalence of non-tuberculosis mycobacteria(NTM)among the samples deposited from the National Tuberculosis Reference Laboratory of Iran between 2011 and ***:The study evaluated the prevalence of NTM among specimens from patients with pulmonary tuberculosis symptoms(n=15771)deposited at the National Tuberculosis Reference Laboratory of Iran from 2011 to *** of Mycobacterium(M.)tuberculosis was based on presence of a 190-bp amplicon from IS6110 insertion sequence using Tb1 and Tb2 primers,and amplicon-negative specimens were tested for NTM and ***(refractory to IS6110 amplification)using restriction fragment length polymorphism PCR of hsp65 amplicon ***:A total of 7307(46.33%)*** and 658(4.17%)NTM specimens were found,the latter mainly comprising ***(10.18%),***(2.28%),***(8.97%),***(10.49%),***(4.71%),and ***(56.08%).Conclusions:As treatment for NTM differs from that for ***,accurate detection of Mycobacterium *** of public health significance.

BACKGROUND:Numerous drugs have been proposed to alleviate pain in patients with biliary colic,especially opioids,but still there is a tendency to use less narcotics because of their side effects and the unwillingness of some *** present study aimed to compare the analgesic effect of paracetamol combined with low-dose morphine versus morphine alone in patients with biliary ***:A randomized double-blind controlled trial was performed in 98 patients with biliary colic,recruited from two emergency departments from August 2012 to August *** patients were excluded and the remaining were randomized into two groups:group A received 0.05mg/kg morphine+1 000 mg paracetamol in 100 m L normal saline and group B received 0.1 mg/kg morphine+normal saline(100 m L)as *** scores were recorded using visual analogue scale(VAS)at baseline and 15 and 30 minutes after drug *** effects and the need for rescue medication(0.75 g/kg intravenous fentanyl)were also reported within 60 minutes of drug ***:Before the infusion,the mean±SD VAS scores were 8.73±1.57 in group A and8.53±1.99 in group *** 15 minutes after drug administration,the mean±SD VAS scores were2.16±1.90 in group A vs.2.51±1.86 in group B;mean difference was–0.35,and 95%CI–1.15 to 0.45(P=0.38).At 30 minutes the mean±SD VAS scores were 1.66±1.59 in group A vs.2.14±1.79 in group B;mean difference was–0.48,and 95%CI–1.20 to 0.24(P=0.19).The mean pain scores in the two groups at 15 and 30 minutes demonstrated no significant ***:Paracetamol combined with low-dose morphine may be effective for pain management in patients with biliary colic.

To the Editor： Rapid sequence intubation （RSI）, defined as the simultaneous administration of a sedative and a neuromuscular blocking agent, is the preferred method ofendotracheal intubation in the emergency department （ED）. Although angioedema and anaphylaxis are not common side effects of these drugs, they rarely happen. This report focuses on a 26-year-old woman, a known case of cerebral palsy and epilepsy, who admitted to the ED with complaints of fever and breathing discomfort. The patient had fever, cough, and purulent sputum for 3 days. Drug history included valproate sodium and gabapentin. No allergy to medication had reported.

背景:耐多药结核病(MDR-TB)是国家卫生系统一项艰巨的负担。这个问题仍然没有达成共识,在治疗规范、治疗效果和各种治疗方案上都有争议。方法:本研究报告介绍了伊朗第二次全国耐多药结核病人治疗的队列研究情况。这项研究包括2002—2006年间伊朗所有在我们中心记录的耐多药结核病例。所有患者都接受了由氧氟沙星,环丝氨酸,丙硫异烟胺和丁胺卡那霉素组成的标准的二线药化疗方案。根据药敏试验的结果,化疗方案中添加了乙胺丁醇和吡嗪酰胺。结果:43例确诊的耐多药结核病人接受了治疗,其平均年龄为44.38±19.05岁,其中27例(62.8%)是男性。23例(53.5%)为伊朗人,其余的都是阿富汗人。所有病人都是获得性耐多药结核病例。43例病人中,25例(58.1%)发生了严重的临床不良反应;29例(67.5%)治疗成功, 14例(32.5%)疗效较差(6例[14%]治疗失败和8例[18.6%]死亡)。在伊朗患者和因药物不良反应而改变早期治疗方案的患者中死亡率较高, P值分别为0.039和0.01。结论:与以往的研究相比,我们采用标准化疗方案治疗耐多药结核病,获得了更好的疗效。
