Microchimerism in recurrent miscarriage

Maternal-fetal cell exchange during pregnancy results in acquisition of microchimerism, which can durably persist in both recipients. Naturally acquired microchimerism may impact maternal-fetal interaction in pregnancy. We conducted studies to ask whether microchimerism that a woman acquired from her own mother is detectable before or during pregnancy in women with recurrent miscarriage. Fetal microchimerism was also assayed. Women with primary idiopathic recurrent miscarriage （n=23） and controls （n=31） were studied. Genotyping was conducted for probands, their mothers and the fetus, a non-shared polymorphism identified and quantitative polymerase chain reaction performed to measure microchimerismin peripheral blood mononuclear cells. Preconception comparisons were made between recurrent miscarriage subjects and controls, using logistic regression and Wilcoxon rank sum. Longitudinal microchimerism in subsequent pregnancies of recurrent miscarriage subjects was described. There was a trend toward lower preconception detection of microchimerism in recurrent miscarriage versus controls, 6% vs. 19% （1/16 vs. 6/31, P=0.2）. during pregnancy, 3111 （27%） of recurrent miscarriage subjects who went on to have a birth had detection of microchimerism from their own mother, whereas neither of two subjects who went on to miscarry had detection （0/2）. This initial data suggest that microchimerism from a woman＇s own mother, while detectable in women with recurrent miscarriage, may differ from controls and according to subsequent pregnancy outcome. Further studies are needed to determine the cell types,quantities and any potential functional role of microchimerism in recurrent miscarriage.

Post electrical or lightning injury syndrome: a proposal for an American Psychiatric Association's diagnostic and Statistical Manual formulation with implications for treatment

In the past, victims of electrical and lightning injuries have been assessed in a manner lacking a system- atic formulation, and against ad hoc criteria, particularly in the area of neuropsychological disability. In this manner patients have, for example, only been partially treated, been poorly or incorrectly diagnosed, and have been denied the full benefit of compensation for their injuries. This paper contains a proposal for diagnostic criteria particularly for the neuropsychological aspects of the post injury syndrome. It pays attention to widely published consistent descriptions of the syndrome, and a new cluster analysis of post electrical injury patients. It formulates a proposal which could be incorporated into future editions of the American Psychiatric Association＇s diagnostic and Statistical Manual （dSM）. The major neuropsycholog- ical consequences include neurocognitive dysfunction, and memory subgroup dysfunction, with ongoing consequences, and sometimes including progressive or delayed psychiatric, cognitive, and/or neurological symptoms. The proposed diagnostic criteria insist on a demonstrated context for the injury, both specifying the shock circumstance, and also physical consequences. It allows for a certain delay in onset of symptoms. It recognizes exclusory conditions. The outcome is a proposal for a dSM classification for the post electrical or lightning injury syndrome. This proposal is considered important for grounding patient treatment, and for further treatment trials. Options for treatment in electrical or lightning injury are summarised, and future trials are foreshadowed.

Background:Excessive heat exposure can lead to hyperthermia in humans,which impairs physical performance and disrupts cognitive *** heat is a known physiological stressor,it is unclear how severe heat stress affects brain physiology and ***:Eleven healthy participants were subjected to heat stress from prolonged exercise or warm water immersion until their rectal temperatures(T_(re))attained 39.5℃,inducing exertional or passive hyperthermia,*** a separate trial,blended ice was ingested before and during exercise as a cooling *** were compared to a control condition with seated rest(normothermic).Brain temperature(T_(br)),cerebral perfusion,and task-based brain activity were assessed using magnetic resonance imaging ***:T_(br)in motor cortex was found to be tightly regulated at rest(37.3℃±0.4℃(mean±Sd))despite fluctuations in T_(re).With the development of hyperthermia,T_(br)increases and dovetails with the rising T_(re).Bilateral motor cortical activity was suppressed during high-intensity plantarflexion tasks,implying a reduced central motor drive in hyperthermic participants(T_(re)=38.5℃±0.1℃).Global gray matter perfusion and regional perfusion in sensorimotor cortex were reduced with passive *** function was poorer under a passive hyperthermic state,and this could relate to compromised visual processing as indicated by the reduced activation of left lateral-occipital ***,ingestion of blended ice before and during exercise alleviated the rise in both T_(re)and T_(bc)and mitigated heat-related neural ***:Severe heat exposure elevates T_(br),disrupts motor cortical activity and executive function,and this can lead to impairment of physical and cognitive performance.

It is know from literature that small additions(d Zn significantly improve the intrinsic ductility of *** exact role of each element,both qualitatively and quantitatively,and their combined effects,however,are poorly *** we achieved a much clearer view on the quantitative role of each element with respect to ductility improvement and on the collaborative effect,particularly of Ca and Zn in *** of our findings and conclusions are in disagreement with data and interpretation found in *** different alloys,namely,Mg-0.1 Ca,Mg-0.1 Ca-1 Al,Mg-0.05 Ca-1 Al,Mg-0.1 Ca-2 Al-1 Zn(all are in wt%)were selected for this *** alloys were treated such that *** grain sizes and textures were *** largely excludes the effect of extrinsic factors on ***-guided slip trace analyses reveal that the addition of Ca eases activation of prismatic and pyramidal II slip *** in-situ deformation experiments in SEM and atom probe tomography observations of grain boundaries direct evidence is given for the individual and synergetic effects of Ca and Zn on grain boundary cohesion as an important contribution to improve the ductility of these *** conclude that Ca reduces the slip anisotropy and ameliorates ductility,however,the weak grain boundary cohesion in the Mg-0.1 wt%Ca alloy limits the material’s tensile *** addition of Zn alters the Ca segregation at the grain boundaries and helps to retain their cohesive strength,an effect which thus enables higher ductility and *** further addition of Al primarily improves the *** results show that the balanced influence of reduced slip anisotropy on the one hand and increased grain boundary cohesion on the other hand allow to design a high strength high ductility rare-earth free Mg alloy.

Rangeland hillslope lengths:A case study at the Walnut Gulch Experimental Watershed,southeastern Arizona

Rangeland hillslopes provide much of the sediment supplied to channel systems and their lengths exert a fundamental constraint on hillslope diffusive ***,information regarding lengths of rangeland hillslopes,and how best to estimate them,is *** this study,three groups of watersheds(10 in total)were selected from the Walnut Gulch Experimental Watershed according to their geology,soil and vegetation *** 1 watersheds were at lower elevations dominated by shrubs,Group 3 were at high elevations dominated by grass,and Group 2 were mixed shrub and *** hillslope lengths were calculated from 1 m-resolution dEMs using three methods:a flow routing algorithm,slope-area relationships,and inverted relationship with drainage *** that characterize the current watersheds,including Hack's exponent and coefficient,watershed shape coefficient,channel concavity and steepness,and surface roughness,were quantified and related to hillslope *** shows:(1)estimated hillslope lengths were different for the three methods and between the three groups of watersheds;(2)hillslope lengths that measured from the flow routing algorithm for the ten selected watersheds primarily ranged from 30 to 100 m,with a median value of 63.0 m,which was 20%e50%greater than those derived from slope-area plots or drainage densities;(3)hillslope lengths estimated from the flow routing method were greater in Group 3 watersheds than in Group 2 and then in Group 1 *** attributed these differences in hillslope lengths to the historic epeirogenic pulses,watershed and drainage network morphology,and differences in vegetation characteristics;(4)measured hillslope lengths from the flow routing algorithm were best correlated with hillslope relief,then surface roughness,channel steepness and *** results would benefit the applications of hydrological and erosion models in rangelands.

Chronic liver disease is a major cause of morbidity and mortality worldwide and usually develops over many years, as a result of chronic inflammation and scarring, resulting in end-stage liver disease and its complications. The progression of disease is characterised by ongoing inflammation and consequent fibrosis, although hepatic steatosis is increasingly being recognised as an important pathological feature of disease, rather than being simply an innocent bystander. However, the current gold standard method of quantifying and staging liver disease, histological analysis by liver biopsy, has several limitations and can have associated morbidity and even mortality. Therefore, there is a clear need for safe and noninvasive assessment modalities to determine hepatic steatosis, inflammation and fibrosis. This review covers key mechanisms and the importance of fibrosis and steatosis in the progression of liver disease. We address non-invasive imaging and blood biomarker assessments that can be used as an alternative to information gained on liver biopsy.
