Background Bermudagrass(Cynodon spp.)is one of the most common perennial warm-season forages grown in the Southeastern United *** incorporation of alfalfa(Medicago sativa)into bermudagrass pastures could serve as an alternative source of nitrogen(N)while also improving the nutritive value of the forage *** A 2-year grazing evaluation arranged in a randomized complete block design was conducted from May to September 2018(Year 1)and May–August 2019(Year 2)in Tifton,GA,*** objective was to compare the production of alfalfa–bermudagrass mixed pastures(BGA)with bermudagrass monoculture pastures with(BGN)or without(BG)the application of synthetic nitrogen on forage production and stocker cattle *** Differences were observed for average daily gain(p=0.10),gain per hectare(p=0.02)and stocking rate(p=0.01),in that BGA was greatest compared to BGN and BG,which did not differ from each ***,BGA had increased forage crude protein(p=0.01)and total digestible nutrients(p=0.01)compared to BGN and ***,BGA provided the greatest economic return per hectare compared to BG or *** These data illustrate improved animal performance and forage nutritive value using alfalfa–bermudagrass systems and offer a viable option for producers seeking alternatives to synthetic N sources for bermudagrass pastures in the Southeastern United States.

This article provides a review of tropical cyclone(TC)surface wind estimation from an operational forecasting ***,we provide a summary of operational forecast center practices and historical ***,we discuss current and emerging objective estimates of TC surface winds,including algorithms,archive datasets,and individual algorithm strengths and weaknesses as applied to operational TC surface wind forecast *** review leads to recommendations about required surface coverage—an area covering at least 1100 km from center of TC at a 2-km resolution in the inner-core,and at a frequency of at least once every 6 *** is enough coverage to support a complete analysis of the TC surface wind field from center to the extent of the 34-kt(17 m s^(-1))winds at 6-h *** also suggest future designs of TC surface wind capabilities include funding to ensure near real-time data delivery to operators so that operational evaluation and use are feasible within proposed ***,we suggest that users of archived operational wind radii datasets contact operational organizations to ensure these datasets are appropriate for their needs as the datasets vary in quality through time and space,even from a single organisation.

Background:Hormone sensitive lipase(HSL) is an enzyme that regulates adipose tissue lipolysis and plays an important role in chronic exerciseinduced changes in adipose tissue *** purpose of this study was to determine whether aerobic exercise intensity influences abdominal adipose tissue HSL gene expression in obese women under weight loss. Methods:Thirty women(body mass index(BMI) = 33.0±0.7 kg/m^2,age = 58±1 years) completed one of three 20-week interventions: caloric restriction alone(CR only,n = 8),CR plus moderate-intensity exercise(CR + moderate-intensity,45%—50%heart rate reserve(HRR). 3 day/week,n = 9),or CR plus vigorous-intensity exercise(CR + vigorous-intensity,70%—75%HRR,3 day/week,n = 13).Each group had a similar prescribed energy deficit comprised of underfeeding alone(2800 kcal/week for CR only) or underfeeding(2400 kcal/week) plus exercise(400 kcal/week).Body composition and maximal aerobic capacity(VO_2max) were measured,and subcutaneous abdominal adipose tissue samples were collected before and after the *** tissue HSL gene expression was measured by real time reversetranscriptase polymerase chain reaction. Results:All three interventions reduced body weight,fat mass,percent *** waist to a similar degree(all p < 0.01).In addition,all interventions did not change absolute VO_2 max,but increased relative VO_2 max(p < 0.05 to P < 0.01).Compared to *** CR only nor CR + moderate-intensity changed adipose tissue HSL gene expression,but CR + vigorous-intensity significantly increased adipose tissue HSL gene expression(p < 0.01).The changes of HSL gene expression levels in the CR + vigorous-intensity group were significantly different from those in the CR only(p < 0.05) and CR + moderate-intensity(p < 0.01) *** the whole cohort,changes in adipose tissue HSL gene expression correlated positively to changes in absolute(r = 0.55,p < 0.01) and relative(r = 0.32,p = 0.09) VO_2 max. Features of hepatocellular carcinoma in Hispanics differ from African Americans and non-Hispanic Whites

AIM To compare features of hepatocellular carcinoma(HCC) in Hispanics to those of African Americans and *** Patients treated for HCC at an urban tertiary medical center from 2005 to 2011 were identified from a tumor registry. Data were collected retrospectively, including demographics, comorbidities, liver disease characteristics, tumor parameters, treatment, and survival(OS) outcomes. OS analyses were performed using Kaplan-Meier *** One hundred and ninety-five patients with HCC were identified: 80.5% were male, and 22% were age 65 or older. Mean age at HCC diagnosis was 59.7 ± 9.8 years. Sixty-one point five percent of patients had Medicare or Medicaid; 4.1% were uninsured. Compared to African American(31.2%) and White(46.2%) patients, Hispanic patients(22.6%) were more likely to have diabetes(P = 0.0019), hyperlipidemia(P = 0.0001), nonalcoholic steatohepatitis(NASH)(P = 0.0021), end stage renal disease(P = 0.0057), and less likely to have hepatitis C virus(P < 0.0001) or a smoking history(P < 0.0001). Compared to African Americans, Hispanics were more likely to meet criteria for metabolic syndrome(P = 0.0491), had higher median MELD scores(P = 0.0159), ascites(P = 0.008), and encephalopathy(P = 0.0087). Hispanic patients with HCC had shorter OS than the other racial groups(P = 0.020), despite similarities in HCC parameters and treatment. CONCLUSION In conclusion, Hispanic patients with HCC have higher incidence of modifiable metabolic risk factors including NASH, and shorter OS than African American and White patients.

Moderators and mediators of antipsychotic response in delusional disorder:Further steps are needed

Delusional disorder(DD)has been traditionally considered a relatively rare and treatment-resistant psychotic *** the last decade,increasing attention has focused on therapeutic outcomes of individuals affected by this *** aim of this paper is to provide a synthesis of the literature addressing two very important questions arising from DD research:(1)For which patients with DD do antipsychotic medications work best(the moderators of response);and(2)What variables best explain the relationship between such treatments and their effectiveness(the mediators of response).We searched PubMed and Google Scholar databases for English,German,French and Spanish language papers published since *** also included a few classic earlier papers addressing this *** potentially moderating antipsychotic response in DD are gender,reproductive status,age,duration of illness,the presence of comorbidity(especially psychiatric comorbidity)and its treatment,brain structure,and genetics of neurochemical receptors and drug metabolizing *** and hormonal blood levels during treatment,as well as functional brain changes,are potential mediating ***,but not all,patients with DD benefit from antipsychotic *** the circumstances under which treatment works best can serve to guide optimal management.

Novel Scheme for Compressed Image Authentication Using LSB Watermarking and EMRC6 Encryption

In the current era, transmission and storing of medical data in the digital form is of great concern and thus the requirement for content authentication has aroused. As a solution to these, digital watermarking techniques and encryption schemes have been used to secure medical data like medical images. In this paper a combination of two algorithms to provide image authentication for medical images in the compressed format is proposed. In the proposed method, the watermark image is encrypted using the Enhanced modified RC6 block cipher (EMRC6) algorithm and the encrypted watermark image is watermarked using the simple Least significant Bit (LSB) watermarking technique. The watermarked output image shows no visual imparity and the watermark which has been extracted has no visual difference. The test results show that the watermarked image has high quality and the watermark is very secure. Also the PSNR value of proposed method is 44.966 on an average and 43.0633 for the existing system where LSB technique is integrated with MRC6 for security of watermark. Hence the work is aimed to increase the embedding volume and make the watermark more secure which is the basic requirement of medical image security.

Synovial lipomatosis: A rare cause of knee pain in an adolescent female

Synovial lipomatosis is a benign proliferative disease of the subsynovial adipose which can lead to a variety of presentations. Cases of synovial lipomatosis in children or adolescents are rare. This case report describes an adolescent patient with a rare bilateral presentation of synovial lipomatosis. She had been treated for years prior to her presentation for juvenile idiopathic arthritis. She presented with chronic bilateral knee pain, swelling, and mechanical symptoms. Bilateral MR imaging demonstrated effusions, hypertrophy of the synovium, and polyp-like projections of tissue with the same signal intensity as fat which is pathognomonic for synovial lipomatosis. Arthroscopic synovectomy and extensive debridement of polyp like fat projections of the right knee was performed. Histopathology was consistent with the synovial lipomatosis diagnosis. Postoperatively, the patient was satisfied with her outcome with improved pain relief and function in her right knee.

Objective:To review follow-up imaging after equivocal bone scans in men with castration resistant prostate cancer(CRPC)and examine the characteristics of equivocal bone scans that are associated with positive follow-up ***:We identified 639 men from five Veterans Affairs Hospitals with a technetium-99m bone scan after CRPC diagnosis,of whom 99(15%)had equivocal *** with equivocal scans were segregated into“high-risk”and“low-risk”subcategories based upon wording in the bone scan *** follow-up imaging(bone scans,computed tomography[CT],magnetic resonance imaging[MRI],and X-rays)in the 3 months after the equivocal scan were *** were compared between patients with a positive *** follow-up imaging after an equivocal bone ***:Of 99 men with an equivocal bone scan,43(43%)received at least one follow-up imaging test,including 32/82(39%)with low-risk scans and 11/17(65%)with high-risk scans(p=0.052).Of follow-up tests,67%were negative,14%were equivocal,and 19%were *** those who underwent follow-up imaging,3/32(9%)low-risk men had metastases vs.5/11(45%)high-risk men(p=0.015).Conclusion:While 19%of all men who received follow-up imaging had positive follow-up imaging,only 9%of those with a low-risk equivocal bone scan had metastases versus 45%of those with *** preliminary findings,if confirmed in larger studies,suggest follow-up imaging tests for low-risk equivocal scans can be delayed while high-risk equivocal scans should receive follow-up imaging.
