AIM:To study the angle between the circular smooth muscle(CSM) and longitudinal smooth muscle(LSM) fibers in the distal ***:In order to identify possible mechanisms for greater shortening in the distal compared to proximal esophagus during peristalsis,the angles between the LSM and CSM layers were measured in 9 *** outer longitudinal layer of the muscularis propria was exposed after stripping the outer *** inner circular layer of the muscularis propria was then revealed after dissection of the esophageal mucosa and the underlying muscularis *** of each specimen were taken with half of the open esophagus folded back showing both the outer longitudinal and inner circular muscle *** were measured every one cm for 10 cm proximal to the squamocolumnar junction(SCj) by two independent *** human esophagi were obtained from organ transplant donors and the angles between the circular and longitudinal smooth muscle layers were measured using micro-computed tomography(micro CT) and Image j ***:All data are presented as mean ± *** CSM to LSM angle at the SCj and 1 cm proximal to SCj on the autopsy specimens was 69.3 ± 4.62 degrees vs 74.9 ± 3.09 degrees,P = *** CSM to LSM angle at SCj were statistically significantly lower than at 2,3,4 and 5 cm proximal to the SCj,69.3 ± 4.62 degrees vs 82.58 ± 1.34 degrees,84.04 ± 1.64 degrees,84.87 ± 1.04 degrees and 83.72 ± 1.42 degrees,P = 0.013,P = 0.008,P = 0.004,P = 0.009 *** CSM to LSM angle at SCj was also statistically significantly lower than the angles at 6,7 and 8 cm proximal to the SCj,69.3 ± 4.62 degrees vs 80.18 ± 2.09 degrees,81.81 ± 1.75 degrees and 80.96 ± 2.04 degrees,P = 0.05,P = 0.02,P = 0.03 *** CSM to LSM angle at 1 cm proximal to SCj was statistically significantly lower than at 3,4 and 5 cm proximal to the SCj,74.94 ± 3.09 degrees vs 84.04 ± 1.64 degrees,84.87 ± 1.04 degrees and 83.72 ± 1.42 degrees,P =

Osteoporosis and People with Down Syndrome: A Preliminary Descriptive Examination of the Intellectual Disability Supplement to the Irish Longitudinal Study on Ageing Wave 1 Results

Background: People with Down syndrome (DS) are a high “at risk” group to develop osteoporosis. Increased morbidity associated with older age, higher prevalence of Alzheimer dementia, hypotonia, hypogonadism, and thyroid disease, are all features of DS and predispose this cohort to musculoskeletal complications. Methods: The Intellectual Disability Supplement to The Irish Longitudinal Study on Ageing (IDS-TILDA) is a representative study of the aging of adults with intellectual disability in Ireland. The sample (N = 753) included 147 people with DS. Data was gathered on participants’ health status, behavioural health, health screenings and activities of daily living. The prevalence of osteoporosis and related risk factors among people with DS was specifically examined. Findings: Of the 147 participants with DS, 9.4% reported a doctor’s diagnosis of osteoporosis;a much lower figure than prevalence of risk factors would suggest. Predisposing factors identified included higher than general population rates of thyroid disease (37.4%), epilepsy (19.3%), sedentary lifestyle (51.7%) and the majority of the females reporting having experienced menopause (61%). Bone health screening was low at (8%) despite the presence of such high levels of risk factors in this population. Conclusion: Given the risk factor findings and the hidden nature of osteoporosis, underreported incidence among people with Down syndrome seems probable. Further investigations and systematic screening are required.

Hepatocellular carcinoma(HCC)is the sixth most common cancer in the *** majority of patients with HCC present with unresectable *** patients have historically had limited treatment options secondary to HCC demonstrating chemoresistance to the currently available systemic ***, normal liver parenchyma has shown intolerance to tumoricidal radiation doses,limiting the use of external beam *** of these limitations,novel percutaneous liver-directed therapies have emerged. The targeted infusion of radioactive microspheres (radioembolization)represents one such therapy. Radioembolization is a minimally invasive transcatheter therapy through which radioactive microspheres are infused into the hepatic arteries that supply tumor. Once infused,these microspheres traverse the hepatic vascular plexus and selectively implant within the tumor *** within the arterioles, the 90Y impregnated microspheres emit high energy and low penetrating radiation doses selectively to the *** has recently shown promise for the treatment of patients with unresectable HCC. The objective of this review article is to highlight twocurrently available radioembolic devices(90Y,188Rh)and provide the reader with a recent review of the literature.

Determination of the Fatty Acid Profile of Breast Milk from Nursing Mothers in Bungoma County, Kenya

The fatty acid profile in breast milk of nursing mothers who participated in a Cohort for Vitamin A (COVA) study at the fourth and ninth month of lactation was investigated. Breast milk samples were collected by manual expression and stored at - 20°C until analysis. The fat was extracted from the milk and methylated using the American Oil Chemists’ Society (AOCS) Official Methods with modifications. The separation, identification and quantification of the fatty acid methyl esters was performed by gas chromatography coupled with mass spectrometry (GC-MS). Fat contents of human milk increased significantly between the fourth and ninth month of lactation 0.38 and 1.21 mg·mL-1 respectively;P 0.05) than the average percent of the total unsaturated FAs (48.93%). Similarly, at the 9th month the percentage average of the total saturated fatty acids (15.18%) was significantly lower (P < 0.05) than the average percent of the total unsaturated FAs (31.05%). The results obtained in this study demonstrated that the fat content in breast milk significantly increased in the 9th compared to the 4th month of lactation. The fatty profile was also significantly different with the omega 6 being the dominant at the 9th month compared with the omega - 9 being dominant at the 4th month of lactation.

Transepithelial leak in Barrett's esophagus patients: The role of proton pump inhibitors

AIM： To determine if the observed paracellular sucrose leak in Barrett＇s esophagus patients is due to their pro- ton pump inhibitor （PPI） use. METHODS： The in vivo sucrose permeability test was administered to healthy controls, to Barrett＇s patients and to non-Barrett＇s patients on continuous PPI thera- py. Degree of leak was tested for correlation with pres- ence of Barrett＇s, use of PPIs, and length of Barrett＇s segment and duration of PPI use. RESULTS： Barrett＇s patients manifested a near 3-fold greater, upper gastrointestinal sucrose leak than healthy controls. A decrease of sucrose leak was ob- served in Barrett＇s patients who ceased PPI use for 7 *** initial introduction of PPI use （in a PPI-na＇ive population） results in dramatic increase in sucrose leak, long-term, continuous PPI use manifested a slow spon- taneous decline in leak. The sucrose leak observed in Barrett＇s patients showed no correlation to the amount of Barrett＇s tissue present in the esophagus. CONCLUSION： Although future research is needed to determine the degree of paracellular leak in actual Barrett＇s mucosa, the relatively high degree of leak ob- served with in vivo sucrose permeability measurement of Barrett＇s patients reflects their PPI use and not their Barrett＇s tissue perse.
