Introduction: The acromioclavicular joint is a superficial diarthrodial joint that surrounds the medial articular facet of the acromion and the distal portion of the clavicle. Due to its anatomy and biomechanics, it is highly susceptible to trauma and in young men who play contact sports, acromioclavicular dislocation is common. This article aimed to systematically review the literature and compare the surgical techniques used in the treatment of acromioclavicular dislocation in patients who practice sports. Methods: This systematic review was conducted according to the International Preferred Reporting Items for Systematic Review and Meta-Analyses guidelines. Eligible studies for this systematic review included articles in English or Spanish published between 2013 and 2023, which mention the occurrence of acromioclavicular dislocation during sports practices. Additionally, only studies that addressed the surgical treatment of acromion-clavicular dislocation and contained original data on the topic were included. Results: We found 144 eligible studies after searching the LILACS and PubMed databases. Based on the inclusion and exclusion criteria and the reviewers’ consensus, we selected four studies for the systematic review. 133 patients with AC joint displacement were evaluated. Mean Age: approximately 31.90 years. 81.92 of these injuries occurred during sports practice. Surgical Procedures Used: titanium plates fixation (49 patients), arthroscopy (24), single tunnel technique (30) and coracoid sling technique (30). The results of the visual analog scale and Constant-Murley scores varied between the techniques used. Twenty-two complications after surgical treatment were identified. Conclusion: A significant variability of operative techniques can be used in the surgical approach of acromioclavicular dislocation, such as arthroscopy, single tunnel, coracoid sling and titanium plates. The choice of a particular Artificial Neural Network (ANN) structure is a seemingly difficult task;worthy of relevance is that there is no systematic way for establishing a suitable architecture. In view of this, the study looked at the effects of ANN structural complexity and data pre-processing regime on its forecast performance. To address this aim, two ANN structural configurations: 1) Single-hidden layer, and 2) Double-hidden layer feed-forward back propagation network were employed. Results obtained revealed generally that: a) ANN comprised of double hidden layers tends to be less robust and converges with less accuracy than its single-hidden layer counterpart under identical situations;b) for a univariate time series, phase-space reconstruction using embedding dimension which is based on dynamical systems theory is an effective way for determining the appropriate number of ANN input neurons, and c) data pre-processing via the scaling approach excessively limits the output range of the transfer function. Background:Hemodialysis(HD) per se is a risk factor for *** the growing body of evidence on blood-flow restriction(BFR) exercise in HD patients,identification of possible risk factors related to the prothrombotic agent D-dimer is required for the safety and feasibility of this training *** aim of the present study was to identify risk factors associated with higher D-dimer levels and to determine the acute effect of resistance exercise(RE) with BFR on this ***:Two hundred and six HD patients volunteered for this study(all with a glomerular filtration rate of ytic exercise).RE repetitions included concentric and eccentric lifting phases(each lasting 2 s) and were supervised by a strength and conditioning ***:Several variables were associated with elevated levels of D-dimer,including higher blood glucose,citrate use,recent cardiovascular events,recent intercurrents,higher inflammatory status,catheter as vascular access,older patients(>70 years old),and HD ***,RE+BFR significantly increases D-dimer after 4 *** with borderline baseline D-dimer levels(400-490 ng/mL) displayed increased risk of elevating D-dimer over the normal range(≥500 ng/mL).Conclusion:These results identified factors associated with a heightened prothrombotic state and may assist in the screening process for HD patients who wish to undergo RE+BFR.D-dimer and/or other fibrinolysis factors should be assessed at baseline and throughout the protocol as a precautionary measure to maximize safety during RE+BFR.

Characterisation of Meteorological Drought in Northern Nigeria Using Comparative Rainfall-Based Drought Metrics

Meteorological droughts occur when there is deficiency in rainfall;i.e. rainfall availability is below some acclaimed normal values. Hence, the greater challenge is to be able to obtain suitable methods for assessing drought occurrence, its onset or initiation and termination. Thus, an attempt was made in this paper to evaluate the performance of Standardised Precipitation Index (SPI) and Standardised Precipitation Anomaly Index (SPAI) to characterise drought in Northern Nigeria for purposes of comparison and eventual adoption of probable candidate index for the development of an Early Warning System. The findings indicated that despite the fact that the annual timescale may be long, it can be employed to obtain information on the temporal evolution of drought especially, regional behaviour. However, monthly timescale can be more appropriate if emphasis is on evaluating the effects of drought in situations relating to water supply, agriculture and groundwater abstractions. The SPAI can be employed for periodic rainfall time series though;it accentuates drought signatures and may not necessarily dampen high fluctuations due to implications of high climatic variability considering the stochastic nature and state transition of drought phenomena. On the other hand, the temporal evolution of SPI and SPAI were not coherent at different temporal accumulations with differences in fluctuations. However, despite the differences between the SPI and SPAI, generally at some timescales, for instance, 6-month accumulation, both spatial and temporal distributions of drought characteristics were seemingly consistent. In view of the observed shortcomings of both indices, especially the SPI, the Standardised Nonstationary Precipitation Index (SnsPI) should be looked into and too, other indexes that take into consideration the implications of global warming by incorporating potential evapotranspiration may be deemed more suitable for drought studies in Northern Nigeria.

Prevalence of Latent Tuberculosis Infection [LTBI] in Prison Officers: A Systematic Review and Meta-Analysis

This study aimed to perform a systematic review and meta-analysis to determine the LTBI prevalence in prison officers worldwide. A systematic search was performed in PubMed, WoS, Embase, and BVS, including all articles related to LTBI prevalence and risk factors. After critical evaluation and qualitative synthesis of the identified articles, a meta-analysis was used. Five studies carried out between 2012 and 2022 were included, with a total sample size of 1718 prison officers. The overall LTBI prevalence was 50% [95% confidence interval [CI]: 48% - 52%;n = 816], with high heterogeneity between studies. Smoking [OR = 1.76;CI 95% = 1.26 - 2.46] and males [OR = 2.08;CI 95% = 1.31 - 3.31] were positively related to a higher LTBI prevalence among prison officers. Thus, preventive measures and the rapid and accurate diagnosis of new cases should be emphasized to ensure tuberculosis control, especially among risk groups such as prison officers.

Background: Nigeria, a nation grappling with rapid population growth, economic intricacies, and complex healthcare challenges, particularly in Lagos State, the economic hub and most populous state, faces the challenge of ensuring quality healthcare access. The overview of the effect of quality improvement initiatives in this paper focuses on private healthcare providers in Lagos State, Nigeria. The study assesses the impact of donor-funded quality improvement projects on these private healthcare facilities. It explores the level of participation, perceived support, and tangible effects of the initiatives on healthcare delivery within private healthcare facilities. It also examines how these initiatives influence patient inflow and facility ratings, and bring about additional benefits and improvements, provides insights into the challenges faced by private healthcare providers in implementing quality improvement projects and elicits recommendations for improving the effectiveness of such initiatives. Methods: Qualitative research design was employed for in-depth exploration, utilizing semi-structured interviews. Private healthcare providers in Lagos involved in the SP4FP Quality Improvement Project were purposively sampled for diversity. Face-to-face interviews elicited insights into participation, perceived support, and project effects. Questions covered participation levels, support perception, changes observed, challenges faced, and recommendations. Thematic analysis identified recurring themes from interview transcripts. Adherence to ethical guidelines ensured participant confidentiality and informed consent. Results: Respondents affirmed active involvement in the SP4FP Quality Improvement Project, echoing literature emphasizing private-sector collaboration with the public sector. While acknowledging positive influences on facility ratings, respondents highlighted challenges within the broader Nigerian healthcare landscape affecting patient numbers. Respondents c
