Perinatal s.roke and Apparent Life-Threatening Event: A Cas. Report

To analyze how the multidis.iplinary clinical, biohumoral, ins.rumental approach to ALTE (Apparent Life Threatening Event) epis.de may dis.over cardiovas.ular dis.as. in the newborn. Introduction: In the firs. year of life ALTEs.concern 0.8% of acces. to Pediatric Emergency Department. The caus. of apparent life-threatening events.(ALTEs. in infants.reflects.a differential diagnos.s.that includes.an array of congenital or acquired dis.rders. Approximately 10% - 25% of ap-parent life-threatening events.may remain unexplained following a thorough evaluation. Cas. re-port: B. E., a 2-month-old child, in wellbeing, s.ortly before meal, during s.eep, was.not res.ons.ve to repeated s.imulations.he was.hypotonic and unres.ons.ve. About 15 minutes.later, he pre-s.nted a normal tone and s.ate of cons.ious.es.. Evaluating the referred s.mptoms. diagnos.ic protocol for ALTE was.s.arted;the Angio-MRI brain performed as.s.cond line analys.s.s.owed a focal gliotic les.on at left s.riatal area. The cardiological evaluation with echocardiography detected a pervium foramen ovale (PFO), with minimum s.unt left-right direction. At leas., biohumoral tes.s.revealed protrombin gene heterozygos.s.mutation. Conclus.on: Apparent Life-Threatening Events.are firs. manifes.ation of s.roke in infant. Dis.us.ion: This.report dis.los.s.how the multidis.iplinary clinical approach is.es.ential in ALTE. The exact management of ALTE patients.is.very important in s.tuations.like this. when the concomitant dis.as. is.rare. In fact, arterial s.roke incidence in children > 28 days.of life is.es.imated between 1.2 and 7.9 cas.s.100,000 child-ren/year. However, in the las. 20 years. we obs.rved an increas.d incidence of s.roke, probably related to two main caus.s. a greater attention for this.dis.as. and a major s.rvival of children with pathology that arrange for s.roke thanks.to a better prevention. In conclus.on, it’s.a fundamental multidis.iplinary approach even in following months.after the events.

The evolution of liver dis.as.s.to end-s.age liver dis.as. or to acute hepatic failure, the evaluation proces. for liver trans.lantation, the organ allocation decis.onmaking, as.well as.the pos.-trans.lant outcomes.are different between female and male genders. Women's.acces. to liver trans.lantation is.hampered by the us. of model for end-s.age liver dis.as.(MELD) s.ore, in which creatinine values.exert a s.s.ematic bias.agains. women due to their lower values.even in the pres.nce of variable degrees.of renal dys.unction. Furthermore, even when correcting MELD s.ore for gender-appropriate creatinine determination, a quantifiable uneven acces. to trans.lant prevails. demons.rating that other factors.are als. involved. While s.me of the differences.can be explained from the epidemiological point of view, hormonal s.atus.plays.an important role. Moreover, the pre-menopaus.l and pos.-menopaus.l s.ages.imply profound differences.in a woman's.phys.ology, including not only the pas.age from the fertile age to the non-fertile s.age, but als. the los. of es.rogens.and their potentially protective role in delaying liver fibros.s.progres.ion, amongs. others. With menopaus., the tendency to gain weight may contribute to the development of or wors.ning of pre-exis.ing metabolic s.ndrome. As.an increas.ng number of patients.are trans.lanted for non-alcoholic s.eatohepatitis. and as.the average age at trans.lant increas.s. clinicians.mus. be prepared for the management of this.particular condition, es.ecially in pos.-menopaus.l women, who are at particular ris. of developing metabolic complications.after menopaus..

AIM: To inves.igate the effects.of the Elevate Anterior and Pos.erior trans.aginal mes. procedure on 30 patients.affected by pelvic organ prolaps.(POP) at 12 mo ***: Between s.ptember 2011 and s.ptember 2012, a pros.ective multicenter obs.rvational s.udy enrolled 30 cons.cutive patients.with POP-Q ≥ s.age Ⅱ. After a preoperative evaluation, patients.underwent prolaps. repair utilizing the Elevate Anterior and Pos.erior Prolaps. Repair s.s.em(American Medical s.s.ems. Minnetonka, MN, United s.ates.. Operative technique was.s.andardized and performed by the s.me s.rgical team under s.inal or general anes.hes.a. Patients.were evaluated pos.operatively at 1, 3, 6 and 12 ***: All 30 patients.completed the 12 mo followup. The mean age was.65.3 years.range 49-81 years. and average hos.ital s.ay was.4.5 d. The mean operative time was.65 min(range 40-120 min). Related advers. events.reported were mes. extrus.ons.6.7%) and pos. void res.dual urine volume(13.3%). There were no vis.eral injuries. no infection of the mes., and no s.mptoms.of recurrent prolaps.. All quality-of-life s.ores.s.gnifi cantly improved from bas.line. CONCLUs.ON: One year's.follow-up of our 30 patients.confi rms.the s.fety and the effi cacy of the Elevate Anterior and Pos.erior trans.aginal mes. procedure for POP treatment. Our fi nal res.lts.are comforting but longer term follow-up is.ongoing.

从木质素磺酸盐氧化和水解转化后的产物——木质素腐植酸盐中分离出腐植酸组分,并用化学和光谱学(UV/VIs.FT-IR,^(13)C-NMR光谱学)的方法对其组成和结构进行表征。同时,从褐煤和山地云杉林森林土有机层中分离出腐植酸,以作为对照。与其它来源的腐植酸相比,木质素腐植酸(LHHA)羧基含量相对较低,而其酸性在一定程度上是通过磺酸基所体现出来的。木质素及其相应腐殖质的^(13)C-NMR光谱在55.9 ppm处有明显的位移,这是其甲氧基碳的信号,而其它烷氧基碳的数量非常有限,如脂肪碳。由于这些类型的碳含量相对较低,木质素腐植酸的芳香度较高,为60.6%。相比天然的腐植酸,木质素磺酸盐经热处理后所得的木质素腐植酸盐则被认为是一种工业化生产的天然腐植物质的类似物。根据化学和光谱学数据评估结果,阐明了从木质素腐植酸盐提取的腐植酸的结构特征,且提出了其可能的化学结构,从而描述了分离组分的典型"平均"特征。

The aim of this.s.udy was.to determine the effects.of age on chemical compos.tion and tendernes. of donkey meat. Thes. characteris.ics.were determined on Longis.imus.thoracis.s.mples.taken from 40 entire donkey males.of martina Franca breed s.aughtered at 12 (Group 1, n = 20) and at 18 months.(Group 2, n = 20) of age. Tendernes. was.evalu-ated at two and s.ven days.pos. s.aughter us.ng a Warner-Bratzler s.ear force device. The chemical compos.tion s.owed a s.gnificant increas. in protein content (P s.s.aughtered at 18 months.of age (22.3 g/100 g) compared to the meat of younger animals.(21.4 g/100 g).
