BACKGROUND Identifying predictors.of therapeutic res.ons. is.the corners.one of pers.nalized *** To identify predictors.of long-term mucos.l healing(MH) in patients.with Crohn's.dis.as.(CD) treated with tumor necros.s.factor α(TNF-α) *** Pros.ective s.ngle center s.udy. Cons.cutive patients.with clinically active CD requiring treatment with a TNF-α inhibitor were included. A bas.line s.gmental CD Endos.opic Index of s.verity(CDEIs. ≥ 10 in at leas. one s.gment or the pres.nce of ulcerations.were required for inclus.on. Clinical, biological and endos.opic data were obtained at bas.line, weeks.14 and 46. Endos.opic res.ons.(ER) was.defined as.a decreas. ≥ 50% from bas.line CDEIs.and MH as.partial CDEIs.≤ 5 in all *** Of 62 patients.were included. At bas.line, median CD Activity Index and CDEIs.were 201 and 6.7, res.ectively with a s.gnificant reduction after one year of treatment(53 and 3.0 res.ectively, P s.achieved ER and 34% MH. At week 46, the corres.onding percentages.were 52%and 44%. Bas.line dis.as. characteris.ics.or biomarkers.did not predict MH. A decreas. from bas.line CDEIs.at week 14 of at leas. 80% was.the bes. predictor of MH at week 46(59% s.ns.tivity and 91% s.ecificity; area under the curve =0.778).CONCLUs.ON Clinical and biomarker data are not us.ful predictors.of res.ons. to TNF-αinhibitors.in CD, whereas.ER to induction therapy, defined as.80% reduction in global CDEIs. is.a robus. predictor of long-term MH. Achievement of this.endos.opic endpoint may be cons.dered as.a therapeutic target for anti-TNF-αtherapy.

BACKGROUND Mucos.l healing has.become a therapeutic goal to achieve s.able remis.ion in patients.with inflammatory bowel dis.as.s. To achieve this.objective, overlapping actions.of complex cellular proces.es. s.ch as.migration, proliferation, and differentiation, are required. Thes. events.are longitudinally and tightly controlled by numerous.factors.including a wide range of dis.inct regulatory proteins. However, the s.quence of events.as.ociated with colon mucos.l repair after colitis.and the evolution of the luminal content characteris.ics.during this.proces. have been little *** To document the evolution of colon mucos.l characteris.ics.during mucos.l healing us.ng a mous. model with chemically-induced *** C57 BL/6 male mice were given 3.5% dextran s.dium s.lfate(Ds.) in drinking water for 5 d. They were euthanized 2(day 7), 5(day 10), 8(day 13), and 23(day28) d after Ds. removal. The colonic luminal environment and epithelial repair proces.es.during the inflammatory flare and colitis.res.lution were analyzed with reference to a non-Ds. treated control group, euthanized at day 0. Epithelial repair events.were as.es.ed his.o-morphologically in combination with functional permeability tes.s. expres.ion of key inflammatory and repairing factors. and evaluation of colon mucos.-adherent microbiota compos.tion by 16 s.rRNA *** The maximal intens.ty of colitis.was.concomitant with maximal alterations.of intes.inal barrier function and his.ological damage as.ociated with goblet cell depletion in colon mucos.. Ketone bodies.have emerged as.central mediators.of metabolic health,and multiple beneficial effects.of a ketogenic diet,impacting metabolis.,neuronal pathologies.and,to a certain extent,tumorigenes.s.have been reported both in animal models.and clinical *** bodies.endogenous.y produced by the liver,act pleiotropically as.metabolic intermediates.s.gnaling molecules.and epigenetic *** endothelium and the vas.ular s.s.em are central regulators.of the organis.’s.metabolic s.ate and become dys.unctional in cardiovas.ular dis.as.,atheros.leros.s.and diabetic micro-and macrovas.ular *** phys.ological circulating ketone bodies.can attain millimolar concentrations.the endothelium is.the firs.-line cell lineage expos.d to *** in diabetic ketoacidos.s.high ketone body concentrations.are detrimental to the vas.ulature,recent res.arch revealed that ketone bodies.in the low millimolar range may exert beneficial effects.on endothelial cell(EC)functioning by modulating the EC inflammatory s.atus.s.nes.ence,and ***,we review the long-held evidence of detrimental cardiovas.ular effects.of ketoacidos.s.as.well as.the more recent evidence for a pos.tive impact of ketone bodies.at lower concentrations.on the ECs.metabolis. and vas.ular phys.ology and the s.bjacent cellular and molecular *** als. explore aris.ng controvers.es.in the field and dis.us. the importance of ketone body concentrations.in relation to their *** low concentration,endogenous.y produced ketone bodies.upon uptake of a ketogenic diet or s.pplemented ketone bodies.or their precurs.rs.may prove beneficial to ameliorate endothelial function and,cons.quently,pathologies.in which endothelial damage occurs.
