背景与目的单例病例报告和单例病例对照研究均已显示，乳房假体与间变性淋巴瘤激酶阴性的间变性大T细胞淋巴瘤（anaplastic large T—cell lymphoma，ALCL）之间有相关关系，但没有因果关系证据。ALCL是一种罕见的非霍奇金淋巴瘤。本文综述了已发表的证据，包括病例报告和流行病学研究。方法在PubMed数据库中，检索已报道的与乳房相关ALCL病例的文献，且文献语种为英文，检索词为“乳房假体”“淋巴瘤”“原发性T细胞乳腺淋巴瘤”“乳房假体和ALCL”。结果共纳入18篇文献，包括27例乳房置入假体后发生ALCL的病例。无论是否有乳房假体置入史，与乳房相关的ALCL均可能发生。在27例患者中，21例（78％）CD30间变性淋巴瘤激酶为阴性，表现为无痛的临床病程。报道中使用的乳房假体填充物包括盐水和硅胶；然而，多数报道没有报道假体类型和表面是光面还是毛面。肿瘤分期：Ⅰ期16例，Ⅱ期及以上7例，未报道4例。没有前瞻性流行病学研究报道乳房假体与ALCL之间具有相关性；然而，只有1项以荷兰女性为研究对象的单例病例对照研究报道了乳房置入假体患者发生ALCL的风险会增加，其估算的概率为百万分之一。结论许多研究已报道了乳房假体与ALCL之间具有相关关系，但没有因果关系的证据，还需要进一步地通过研究来证实这种相关关系。与乳房相关的ALCL发生率很低（无论是否有乳房假体置入史），其通常表现为无痛的临床病程，这一发现可能导致世界卫生组织修改淋巴瘤的命名；但是，也有研究报道了其临床表现具有侵袭性。ALCL的发生不限于某种特定类型的假体（临床问题／证据等级：风险，v.。

Background: Diabetic gastroparesis is a disabling condition with no consistently effectiv. treatment. In animals, ghrelin increases gastric emptying and rev.rses postoperativ. ileus. We present the results of a double blind, placebo controlled, crossov.r study of ghrelin in gastric emptying in patients with diabetic gastroparesis. Methods: Ten insulin requiring diabetic patients (fiv.men, six type I) referred with symptoms indicativ. of gastroparesis receiv.d a two hour infusion of either ghrelin (5 pmol/kg/min) or saline on two occasions. Blood glucose was control led by euglycaemic clamp. Gastric emptying rate (GER) was calculated by real tim e ultrasound following a test meal. Blood was sampled for ghrelin, growth hormon e (GH), and pancreatic polypeptide (PP) lev.ls. Cardiov.gal neuropathy was asses sed using the Mayo Clinic composite autonomic sev.rity score (range 0 (normal)- 3). Results: Baseline ghrelin lev.ls were mean 445 (SEM 36) pmol/l. Ghrelin infu sion achiev.d a peak plasma lev.l of 2786 (188) pmol/l at 90 minutes, correspond ing to a peak GH of 70.9 (19.8) pmol/l. Ghrelin increased gastric emptying in se v.n of 10 patients (30 (6)%to 43 (5)%; p = 0.04). Impaired cardiov.gal tone co rrelated inv.rsely with peak postprandial PP v.lues (p v.gal tone. We propose that analo gues of ghrelin may represent a new class of prokinetic agents.
