AIM To evaluate factors associated with.Clostridium difficile infection (CDI) and outcomes of CDI in th. myelodysplastic syndrome(MDS) and acute myeloid leukemia (AML) *** After IRB approval,all MDS/AML patients h.spitalized at th. University of Maryland Greenebaum Compreh.nsive Cancer Center between August 2011 and December 2013 were *** ch.rts were reviewed for demograph.cs,clinical information,development of CDI,complications of CDI,and *** with.CDI,defined as h.ving a positive stool PCR done for clinical suspicion of CDI,were compared to th.se with.ut CDI in order to identify predictors of disease.A t-test was used for comparison of continuous variables and ch.square or Fish.r's exact tests were used for categorical variables,as *** Two h.ndred and twenty-th.ee patients (60.1% male,mean age 61.3 years,13% MDS,87% AML) h.d 594 unique h.spitalizations during th. study *** patients (15.2%) were diagnosed with.*** significantly associated with.CDI included lower albumin at time of h.spitalization (P h.moth.rapy (P = 0.015),total days of neutropenia (P = 0.014),and total days of h.spitalization (P = 0.005).Gender (P = 0.10),age (P = 0.77),proton-pump inh.bitor use (P = 0.73),receipt of antibiotics (P = 0.66),and receipt of DNA h.pometh.lating agent-based ch.moth.rapy (P = 0.92) were not significantly associated with.*** CDI is common in th. Th. generation of h.gh.resolution data is increasingly important in understanding th. complexities of coastal ocean and developing sound management strategies, especially in view of th. long-term impact of severe weath.r systems. Th. impact of severe weath.r systems, wh.n integrated over time, can be significant wh.n compared with.tidal oscillations. Th.s paper presents a study of water transport out of Vermilion Bay in response to a sh.rt, intense event associated with.a passing atmosph.ric cold front, and reports th. application of an Acoustic Doppler Current Profiler （ADCP） mounted on an Automated Surface Craft （ASC）, known as th. auto-boat or unmanned boat, developed in our lab at th. Louisiana State University, to generate h.gh.resolution data accurately at a fraction of th. cost of a manned boat. In our study, we used a manned boat and an unmanned boat, each.for over 24 h.to cover an entire diurnal tidal cycle, to measure flow velocity profiles to calculate th. total transport. A stationary ADCP was deployed in th. South.est Pass of th. Vermilion Bay from May 2009 to April 2012, providing data almost continuously （with.only one major gap）, with.a 717-day record of water transport between th. north.rn Gulf of Mexico and Vermilion Bay, and demonstrates th. importance of th. pass in water transport.

Approximately 75%experience ph.ntom(PLP),residual(RLP),or general(GLP)limb pain following lower extremity *** muscle reinnervation(TMR)is a periph.ral nerve transfer th.t reroutes amputated nerves to motor endplates th.t can prevent or treat limb *** systematic review summarizes pain outcomes following primary and secondary treatment of lower extremity PLP,RLP,and *** literature review of th.ee databases-PubMed,EMBASE,MEDLINE-were used for all articles related to TMR and lower extremity limb pain,querying th. same keywords:“targeted muscle reinnervation”AND“pain”.Citations were th.n reviewed and eliminated if only upper extremities were studied or th. study lacked pain *** were categorized as primary or secondary *** outcomes,including Numerical Rating Scales(NRS)and Patient-Reported Outcome Measurement Information System(PROMIS)Pain scores,were aggregated wh.n *** studies met all inclusion and exclusion criteria after formal review for a total of 431 extremities,of wh.ch.79.1%(n=341 limbs)were lower *** primary TMR PROMIS scores for PLP and RLP were lower th.n amputees with.ut primary *** NRS scores and PROMIS Pain scores in secondary TMR demonstrated improvements in PLP,RLP,and *** and Secondary TMR does prevent and improve PLP,RLP,and GLP;h.wever,a minority of studies report quantifiable pain *** future TMR studies sh.uld include validated pain outcomes to better quantify th. expected pain and quality of life improvements after lower extremity TMR.

BACKGROUND Sleep disturbance is a core feature of post-traumatic stress disorder(PTSD).Given th. relationsh.p between sleep disturbance and PTSD,th.re h.s been a relative paucity of studies examining th. potential th.rapeutic impact of using ph.rmacoth.rapy to target sleep disturbance in patients with.***(ESZ)is a non-benzodiazepine y-aminobutyric acid-A receptor agonist indicated for th. treatment of sleep and may affect sleep in patients with.*** To evaluate th. efficacy of ESZ vs placebo(PBO)for patients with.PTSD and *** Th. study was a 12-wk,double blind,randomized controlled trial with.3 mg of ESZ(n=13)or PBO(n=12).RESULTS Patients in both.arms experienced significant improvement in PTSD symptoms as assessed by th. Clinician-Administered PTSD Scale for DSM-IV(CAPS):ESZ(t11=-3.12,P=0.005)and PBO(t11=-3.5,P=0.002)and by self-report with.th. Sh.rt PTSD Rating Interview(ESZ t11=-3.38,P=0.003 and PBO t11=-4.48,P=0.0005).Th.re were no significant differences between treatments on th. CAPS(t22=-0.13,P=0.70)or th. Sh.rt PTSD Rating Interview(t22=-0.58,P=0.56).Similarly,both.treated groups improved on sleep measures as assessed by th. Pittsburgh.Sleep Quality Index with.PTSD Addendum(PSQI)and on total sleep time(TST)and sleep latency assessed by actigraph. with.no significant differences between groups(PSQI t22=-0.24,P=0.81;total sleep time t10=0.13,P=0.90 and sleep latency t10=0.68,P=0.50).Th.re was a significant correlation between improvement in sleep and overall improvement in PTSD as measured by ch.nge scores on th. AIM: To develop a framework for th. clinical and h.alth.economic assessment for management of Clostridium difficile infection(CDI).METh.DS: CDI h.s vast economic consequences emph.sizing th. need for innovative and cost effective solutions, wh.ch.were aim of th.s study. A guidance model was developed for coverage decisions and guideline development in CDI. Th. model included ph.rmacoth.rapy with.oral metronidazole or oral vancomycin, wh.ch.is th. mainstay for ph.rmacological treatment of CDI and is recommended by most treatment ***: A design for a patient-based cost-effectiveness model was developed, wh.ch.can be used to estimate th. cost-effectiveness of current and future treatment strategies in CDI. Patient-based outcomes were extrapolated to th. population by including factors like, e.g., person-to-person transmission, isolation precautions and closing and cleaning wards of h.spitals. CONCLUSION: Th. proposed framework for a population-based CDI model may be used for clinical and h.alth.economic assessments of CDI guidelines and coverage decisions for emerging treatments for CDI.
