BACKGROUNd We previously reported national 30-d readmission rates of 27% in patients with decompensated cirrhosis(dC).AIM To study prospective interventions to reduce early readmissions in dC at our tertiary *** Adults with dC admitted July 2019 to december 2020 were enrolled and randomized into the intervention(INT) or standard of care(SOC) arms. Weekly phone calls for a month were completed. In the INT arm, case managers ensured outpatient follow-up, paracentesis, and medication compliance. Thirty-day readmission rates and reasons were *** Calculated sample size was not achieved due to coronavirus disease 2019;240 patients were randomized into INT and SOC arms. 30-d readmission rate was 33.75%, 35.83% in the INT vs 31.67% in the SOC arm(P = 0.59). The top reason for 30-d readmission was hepatic encephalopathy(HE, 32.10%). There was a lower rate of 30-d readmissions for HE in the INT(21%) vs SOC arm(45%, P = 0.03). There were fewer 30-d readmissions in patients who attended early outpatient follow-up(n = 17, 23.61% vs n = 55, 76.39%, P = 0.04).CONCLUSION Our 30-d readmission rate was higher than the national rate but reduced by interventions in patients with dC with HE and early outpatient follow-up. development of interventions to reduce early readmission in patients with dC is needed.

Metal foam and fins are two popular structures that are employed to enhance the heat transfer of phase change materials in shell-and-tube heat storage ***,it remains unclear which structure is better in terms of energy storage *** this study,the heat transfer enhancement performances of metal foam and fins are compared to provide guidance on the optimal structure to be chosen for practical *** fin structures(four fins,two vertical fins,and two horizontal fins)are *** the full configuration(volume fraction of metal=3%),the unit with four fins was found to have a faster melting rate than those with vertical or horizontal *** other words,increasing the number of fins helps to accelerate the melting ***,the unit with metal foam enhancement has the highest melting *** the half configuration(volume fraction of metal=1.5%),the melting rate of the unit enhanced by metal foam is significantly decreased,whereas there is no remarkable changes in the units enhanced by ***,metal foam is still shown to be the best thermal *** energy storage rate of the unit enhanced by metal foam can be up to 10 times higher than that of the unit enhanced by fins.

The foundations of proteomics are to study gene products and their regulatory roles within cells. Paradoxically, the only evidence that sperm cells make new proteins is through mitochondrial protein synthesis. Yet despite this, spermatozoa are the perfect candidates for mass spectrometry and hence, proteomic analysis. These enterprising cells use a plethora of post-translational modifications in order to gain functionality following their production within the testis. By using a combination of two-dimensional polyacrylamide gel electrophoresis （2d-PAGE）, and more recently liquid chromatography-mass spectrometry （LC-MS）/MS, recent advances in sperm cell biology, through the use of proteomics, is making unparalleled progress. The protein inventory lists being generated have shed light on transmembrane proteins, kinases and chaperones never previously recognized. In addition, the ability to isolate either phosphopeptides or glycopeptides and quantify the differences between cells of two different populations make proteomic analysis of spermatozoa a real chance to finally answer some age old questions.

Background and Aims:Pharmaceutical therapy for NASH is associated with lipid modulation,but the consensus on drug treatment is limited and lacks comparative analysis of effectiveness.A network meta-analysis was conducted to compare NASH drug classes in lipid ***:Online databases were searched for randomized controlled trails(RCTs)evaluating NASH treatments in biopsy-proven NASH *** were classified into four groups:(1)inflammation,(2)energy,(3)bile acids,and(4)fibro-sis based on the mechanism of action.A Bayesian network analysis was conducted with outcome measured by mean difference(Md)with credible intervals(Crl)and surface un-der the cumulative ranking curve(SUCRA).Results:Forty-four RCTs were included in the *** acid modulat-ing treatments(Md:0.05,Crl:0.03-0.07)were the best treatment for improvement in high-density lipid(HdL)cho-lesterol,followed by treatments modulating energy(Md:0.03,Crl:0.02-0.04)and fibrosis(Md:0.01,Crl:−0.12 to 0.14)compared with *** top three treatments for reduction in triglycerides were treatments modulating energy(Md:−0.46,Crl:−0.49 to−0.43),bile acids(Md:−0.22,Crl:−0.35 to−0.09),and inflammation(Md:−0.08,Crl:−0.13 to−0.03)compared with *** found treatment modulating fibrosis(Md:−1.27,Crl:−1.76 to−0.79)was the best treatment for reduction in low-density lipid(LdL)cholesterol followed by treatment modulating in-flammation(Md:−1.03,Crl:−1.09 to−0.97)and energy(Md:−0.37,Crl:−0.39 to−0.34)compared with placebo,but LdL cholesterol was worsened by treatments modulat-ing bile ***:Network analysis comparing the class effects of dyslipidemia modulation in NASH found that treatment targets can include optimization of athero-genic *** studies are required to evaluate the cardiovascular outcomes.

The Mt. Kitanglad Range is one of the country’s important key biodiversity sites;however, information about anuran diversity in this protected area remains depauperate. Herein we provided accounts of anuran species from high-elevation forests, in three sites of the western slope of Mt. Kitanglad range. The combined belt-transect sampling and microha bitat searches accounted for 13 species representing five families. The most represented family was Rhacophoridae with five representative species of the genus Philautus. Twelve out of the 13 species documented in the current survey are endemic. Four previously unaccounted species(Pelophr yne brevipes, Pulchrana grandocula, Sanguirana mearnsi, and Philautus surrufus) were added and brought the total anurans known from Mt. Kitanglad to 26 species. Most of the species were also recorded in forested sites, suggestive of their lesser affinity to non-forested ecosystems. The additional species detected during our survey may also imply that full understanding of anuran diversity of Mt. Kitanglad remains far from complete.

The diagnosis and management of cirrhosis and portal hypertension(PH)with its complications including variceal hemorrhage,ascites,and hepatic encephalopathy continues to *** there are established“standards of care”in liver biopsy and measurement of PH,gastric varices remain an area without a universally accepted therapeutic *** concept of“Endo Hepatology”has been used to describe of the applications of endoscopic ultrasound(EUS)to these ***-liver biopsy(EUS-LB)offers an alternative to percutaneous and transjuglar liver biopsy without compromising safety or efficacy,and with added advantages including the potential to reduce sampling error by allowing biopsies in both hepatic ***,EUS-LB can be performed during the same procedure as EUS-guided portal pressure gradient(PPG)measurements,allowing for the collection of valuable diagnostic and prognostic ***-guided PPG measurements provide an appealing alternative to the transjugular approach,with proposed advantages including the ability to directly measure portal vein *** addition,EUS-guided treatment of gastric varices(GV)offers several possible advantages to current ***-guided treatment of GV allows detailed assessment of the vascular anatomy,similar efficacy and safety to current therapies,and allows the evaluation of treatment effect through doppler ultrasound *** appropriate selection of patients for these procedures is paramount to ensuring generation of useful clinical data and patient safety.
