An Integrative Collaborative Project Approach to Climate-Change Resilience and Urban/Regional Sustainability for the Mexico-Lerma-Cutzamala Hydrological Region

In a rapidly urbanizing world, the social, economic, and ecological complexities of cities require conceptual and operational innovations to enhance climate resilience and sustainability. We describe our Integrative Collaborative Project (ICP) approach to co-create climate resilience in the Mexico-Lerma-Cutzamala Hydrological Region (MLCHR). In recent years, it has suffered from frequent natural disasters, and under climate change scenarios, the intensity and frequency of extreme events, including severe floods, droughts, heat waves and landslides are expected to increase. ICPs are framed as socio-technical capacity building enterprises, with networks operating at multiple scales. The approach differs from other integrative efforts, which tend to be top-down with scant civil society co-ownership, and focus on limited aspects like indicators/assessment, or institutional capacity building. We reimagine all operational stages, from creative thinking, through ethos and concept, assessment, planning, project design, implementation and management, and monitoring and evaluation. The design of ICPs is informed by six integrative domains: 1) project ethos, concept, and framing;2) sectors, topics, and issues;3) spatial and temporal scales;4) stakeholder interests, relationships and capacities;5) knowledge types, models and methods;and 6) socio-technical capacities and networks. Empirically, the approach is based on participatory development practices, pilot project work tackling sustainable water and sanitation in Mexico, and a synthesis of rich experiential knowledge spanning 20 years. The theoretical basis considers a pragmatic knowledge frame, socio-technical transitions literature, and education for social transformation. We describe forward-looking operational details of the Pilot ICP for the Mexico-Lerma-Cutzamala Hydrological Region, with our three-university partnership as catalyst, and a new breed of socio-technical enterprise organization as a key partner, engaging

Extreme Weather Disruptions and Emergency Preparedness Among Older Adults in Ohio: An Eight-County Assessment

The disproportionate risks and impacts of climate change and extreme weather on older adults are increasingly evident. While especially true in disaster-prone areas, human-caused climate change introduces an element of uncertainty even in previously identified “safe” regions such as the Midwestern United States. Using a cumulative disadvantage and vulnerability-informed framework and descriptive statistics from multiple data sources, this article provides an overview of climate impacts, vulnerabilities, and county-level characteristics, focusing on older adults living in Central Ohio. A comparative multiple-case study methodology was used to triangulate regionally representative primary and secondary data sources to examine state and county-level measures of vulnerability, emergency preparedness, and disruptions caused by extreme weather among older adults across eight counties in Central Ohio. Seventy-eight percent of older adults in the sample reported being prepared for emergencies per Federal Emergency Management Agency guidelines. Older adults in Union County reported the highest rates of preparedness, while those in Fayette County reported the lowest. County-level rates of disruption of life activities by extreme weather ranged widely. Among the most rural in the region, Fayette County emerged as uniquely disadvantaged, with the lowest median income, the most vulnerable across multiple social vulnerability dimensions, and the most reported disruptions to life activities from extreme weather. County profiles offer a snapshot of existing vulnerabilities, socioeconomic conditions, special needs, preparedness, and current disruptions among older adults in the region and can inform resource mobilization across community and policy contexts.

Utility of Preoperative Positron Emission Tomography (PET) in Localizing Perforator Vessels of Anterolateral Thigh Free Flap

Objectives: The anterolateral thigh (ALT) flap is often considered the workhorse in soft tissue reconstruction of head and neck defects secondary to trauma, infection, or tumor resection. Despite its many advantages, ALT flaps have been criticized due to variability in vasculature, which may result in inadequate or non-existent perforators. This retrospective study aims to investigate the utility and validity of positron emission tomography (PET) scan to identify the location and characteristics of perforators to the ALT flap. Methods: We performed a 10-year retrospective review of ALT flaps at our institution to identify patients with preoperative PET scans available for analysis. Three reviewers (attending physician, fellow, and resident) were asked to identify the number, location, and characteristics (myocutaneous versus septocutaneous) of ALT perforators on imaging, and reviewer agreement was assessed. Results were then compared to available operative data. Results: One hundred twenty-one patients were identified who underwent ALT free flap surgery. Thirty-eight preoperative PET scans were identified for review. At least one perforator was identified in 92.1% of scans. Agreement percentages regarding the number of perforators ranged from 53% - 61% whereas agreement regarding the location of a single perforator ranged from 79% - 90%. However, reviewers did not agree regarding the type of perforator, with agreement ranging from 34% - 53%. Poor agreement was observed when compared to intraoperative data, with the number of perforators ranging from 26% - 34% and the type of perforator 11% - 24%. These findings are likely due to insufficient data available in operative reports. Conclusion: Although initial studies suggest that PET scan shows promising evidence to support the capacity to preoperatively identify ALT perforators, future prospective studies are warranted to fully validate these findings.

Improving the health and treatment success rates of in vitro fertilization patients with traditional Chinese medicine:Need for more robust evidence and innovative approaches

Maximising access to and the success of fertility treatments should be a priority for global reproductive health,as should overall patient *** demand for in vitro fertilization(IVF)and other assisted fertility treatments has increased over the past decade and is likely to further increase in years to ***,there is still considerable unmet demand for infertility support ***,the high emotional,physical and financial burden experienced by individuals undergoing IVF cycles can be a risk for their mental and physical health,which in turn can influence treatment continuation and the likelihood of IVF *** from various parts of the world show that most individuals undergoing IVF also use adjunct alternative medicines and procedures,the most common being traditional Chinese medicine(TCM).The complementary and synergistic role of TCM for individuals undergoing IVF is an area that merits further attention and research,both for its potential positive effects on IVF success rates and for its broader physical and mental health ***,much of the existing evidence is not sufficiently robust or consistent for findings to be adopted with *** commentary argues that much work must be done to understand the efficacy and clinical best practices for these integrated *** can be achieved in part by developing more robust and clinically relevant randomized controlled trial protocols,collecting and triangulating evidence through a variety of study designs and methods,and strengthening the collection and pooling of clinic-level data.

An Overview of the Geoenvironmental Status of the Kullu Valley, Himachal Pradesh, India

The north-south trending Kullu valley between Rohtang in the north and Hansu in the south is a wide and open valley filled with Quaternary sediments along the main course of the Beas River. The valley in the middle is drained by the Beas River and numerous tributaries join it laterally. The tributary channels have deposited large alluvial fans at their mouths which form three distinct levels. The Beas River has deposited alluvial terraces, which are very distinct towards the lower reaches and form three to four levels. The upper slopes and high altitudinal areas are covered with periglacial and glacial deposits. The terrace, fan and hill slopes have provided an ideal geoenvironment for human activities including agriculture, horticulture, dense settlements and other civil establishments. The Kullu Valley is prone to various natural hazards, flash floods and cloudbursts that are very common in this valley due to its peculiar geomorphic condition, high relief of peripheral ridges and impact of monsoon winds. The studies carried out so far indicate that the losses caused by these phenomena both in terms of life and property are mainly due to unwise human interaction with the geoenvironment of the area. The paper gives an overview of the geoenvironmental status of the Kullu Valley and suggests the necessity of undertaking further detailed studies including resource mapping for balanced development of the area.

Analysis of Pioneering Computable Biomedical Knowledge Repositories and their Emerging Governance Structures

A growing interest in producing and sharing computable biomedical knowledge artifacts(CBKs) is increasing the demand for repositories that validate, catalog, and provide shared access to CBKs. However, there is a lack of evidence on how best to manage and sustain CBK repositories. In this paper, we present the results of interviews with several pioneering CBK repository owners. These interviews were informed by the Trusted Repositories Audit and Certification(TRAC) framework. Insights gained from these interviews suggest that the organizations operating CBK repositories are somewhat new, that their initial approaches to repository governance are informal, and that achieving economic sustainability for their CBK repositories is a major challenge. To enable a learning health system to make better use of its data intelligence, future approaches to CBK repository management will require enhanced governance and closer adherence to best practice frameworks to meet the needs of myriad biomedical science and health communities. More effort is needed to find sustainable funding models for accessible CBK artifact collections.

Prediction of pavement roughness using a hybrid gene expression programming-neural network technique

Effective prediction of pavement performance is essential for transportation agencies to appropriately strategize maintenance, rehabilitation, and reconstruction of roads. One of the primary performance indicators is the international roughness index （IRI） which rep- resents the pavement roughness. Correlating the pavement roughness to other perfor- mance measures has been under continuous development in the past decade. However, the drawback of existing correlations is that most of them are not practical yet reliable for prediction of roughness. In this study a novel approach was developed to predict the IRI, utilizing two data sets extracted from long term pavement performance （LTPP） database. The proposed methodology included the application of a hybrid technique which combines the gene expression programming （GEP） and artificial neural network （ANN）. The developed algorithm showed reasonable performance for prediction of IRI using traffic parameters and structural properties of pavement. Furthermore, estimation of present IRI from his- torical data was evaluated through another set of LTPP data. The second prediction model also depicted a reasonable performance power. Further extension of the proposed models including different pavement types, traffic and environmental conditions would be desir- ab]e in future studies.
