Advanced gastric cancer usually presents with symptoms due to direct extension into adjacent viscera, distant metastases from lymphatic or hematogenic dissemination and peritoneal seeding. However, portal hypertension as a presentation of metastatic gastric cancer is rare and usually seen in association with other malignancies, e.g. hepatocellular and pancreatic carcinoma. We report a case of signet ring adenocarcinoma of the stomach that presented with esophageal and duodenal varices and bleeding due to portal hypertensive gastropathy. Pagetoid spread of cancer cells likely caused early metastasis and the unusual presentation. We also discussed the pathophysiology of development of portal hypertension in association with malignancies.

This paper interprets Hegel’s engagement with tragedy and especially tragic action as an interpretive model for understanding ethical life in complex societies in which independent value spheres collide. Tragic recognition, in contrast to the kind of recognition introduced in the master and slave dialectic, is not based on desire, but arises from the suffering deriving from clashing value spheres. As a coming to terms with one’s finitude, tragic recognition presents an important corrective to the account of mutual recognition that has been the reference point of contemporary interpretations of Hegel’s social and political philosophy. The paper concludes by pointing to some of the limits of tragedy as a universal interpretive framework for modern societies.

The objective of this report is to give an overall view of the epidemiological, clinical, diagnostic and therapeutic features of Cytomegalovirus （CMV） infection in inflammaton/ bowel disease （IBD）. A review of published reports on this topic was carried out, with particular attention paid to the selection of patients included in studies and the diagnostic methods employed. CMV is frequently associated with IBD. In some cases, CMV infection is as- sociated with a poor outcome but it is not clear which patients are more likely to be affected and in which stage of the disease. The use of anti-viral therapy in IBD is controversial and an empirical study with controls is needed. The natural history of CMV infection related to the development and treatment of IBD has not been clarified but it is important to take it in consideration because of the possibility of viral persistence in the immunocompromised host and viral interaction with the immune system.

The increasing awareness of the general society toward the seismic safety of structures has led to more restrictive performance requirements hence, many times, to the need of using new and more accurate methods of analysis of structures. Among these, nonlinear static procedures are becoming, evermore, the preferred choice of the majority of design codes, as an alternative to complete nonlinear time-history analysis for seismic design and assessment of structures. The many available software tools should therefore be evaluated and well understood, in order to be easily and soundly employed by the practitioners. The study presented herein intends to contribute to this need by providing further insight with respect to the use of commonly employed structural analysis software tools in nonlinear analysis of bridge structures. A comparison between different nonlinear modeling assumptions is presented, together with the comparison with real experimental results. Furthermore, alternative adaptive pushover procedures are proposed and applied to a case study bridge, based on a generic plastic hinge model. The adopted structural analysis program proved to be accurate, yielding reliable estimates, both in terms of local plastic hinge behavior and global structural behavior.

Ammonium nitrate and fuel oil(ANFO)based explosive is a classic example of non-ideal high *** detonation is characterized by a strong dependence of detonation parameters on explosive charge diameter,presence and characteristics of confinement,as well as incomplete consumption of explosive at the sonic *** this work we propose a detonation model based on the Wood-Kirkwood(WK)theory coupled with the thermochemical code EXPLO5 and supplemented with reaction rate *** objective is to analyze the validity of the model for highly non-ideal ANFO explosives,with emphasis on effect of reaction rate *** was found that both single-step and two-step pressure-based models can be calibrated to reproduce experimental detonation velocity-charge radius data of ANFO at radii significantly above the failure radius(*** D/D_(id)>~0.6).Single-step pressure-based model,with the pressure exponent equal to 1.4,proved to be the most accurate,even in the vicinity of the failure *** impact of the rate models is most evident on temporal(and spatial)distribution of flow parameters in detonation driving zone,especially when it comes to the conversion and width of detonation driving zone.

Plant pest and disease outbreaks,which occur with increasing frequency and intensity,cause catastrophic losses and threaten food security in many areas around the *** impacts are expected to be exacerbated by climate *** this challenge requires mechanisms that ensure the financial security of farmers while incentivizing private biosecurity efforts to prevent future *** study explored crop producers’ preferences for a subsidized insurance scheme as an instrument to manage novel biotic *** ally,we developed a choice experiment to evaluate Spanish growers’ willingness to pay for a crop insurance product that promotes compliance with best biosecurity management *** results show that while growers are willing to pay more for high coverage products that increase the resilience of crops to potential catastrophic outbreaks,there is neither a strong demand nor widespread availability of such *** required reductions in premiums before undertaking risk prevention measures;they are more willing to pay for schemes that link their eligibility to access to ad hoc funds in the eventuality of a catastrophic outbreak than they are to purchase *** findings also suggest that Spanish growers prefer expanding the eligible risks covered by insurance and envisage a role for insurance in offering biosecurity protection.

Background:The side effects of the FIFA 11+program on performance have not been generally *** objective of this study was to synthesize the literature on the effects of the 11+on players’***:Five online databases(PubMed,Scopus,ScienceDirect,Springer,and Google Scholar)were searched(from April 2006 to March 2022)using predefined keywords and *** potential references were primarily recorded through Endnote and imported to *** of the 123 references screened by 2 blinded researchers through the software,59 full texts were assessed for eligibility,33 of which were ultimately *** quality of the studies and the risk of bias were then *** ID,title,place,aim,design,start/end dates,population description,study criteria,statistical analysis,and outcomes were ***:Studies were conducted on male and female players aged 10-32 years *** quality of the studies was moderate to high,and except for unclear bias for blinding outcome assessment,the risk of bias for all domains was ***-term application of the 11+improved most biomechanical measures and physiological responses except for lower extremity stability,ankle evertors time latency,ankle dorsiflexion,and ***,the 11+showed acute negative effects on physical performance compared to dynamic warm-ups and non-significant effects on technical ***:Mid-to-long-term implementation of the 11+improved the maj ority of biomechanical and a couple of physical measure s but showed no effects on technical *** must be observed for using the 11+before competitions,as it could acutely decrease physical/technical *** the contradictory nature of the literature,further studies should evaluate the short-to-mid-term effects of the 11+.Further studies are required to address ankle responses to the 11+intervention.

AIM： To describe the morphological changes of the lamina cribrosa （LC） in patients with optic nerve compression. METHODS： Cross-sectional study. Twenty eyes with optic nerve compression, affected by Graves＇ ophthalmopathy （GO） were compared with 18 refractive error-matched healthy eyes. The following examinations were performed： best-corrected visual acuity （BCVA）, intraocular pressure, optic nerve echography, visual field, SD-OCT including the retinal nerve fiber layer （rNFL）, ganglion cell complex （GCC）, and LC thickness and extent. rESULTS： A-scan revealed significant differences in the subarachnoid space （SAS） between the affected and control groups. LC thickness and LC area were 233 pm （SD 23） and 0.41 mm2 （SD 0.19）, respectively. Average GCC thickness （P=-0.0005）, LC thickness （P=-0.001）, MD （P=-0.001） and PSD （P=-0.001） differed significantly between the two groups; whereas LC area （P=-0.2） and average rFNL （P=-0.1） did not. CONCLUSION： Optic nerve compression reduces the SAS thereby altering the morphology of LC thickness and causing GCC damage.
