Cadaveric liver transplantation for non-acetaminophen fulminant hepatic failure:A 20-year experience

AIM: To investigate the long-term results of liver transplantation (LT) for non-acetaminophen fulminant hepatic failure (FHF). METHODS: Over a 20-year period, 29 FHF patients underwent cadaveric whole LT. Most frequent causes of FHF were hepatitis B virus and drug-related (not acetaminophen) liver failure. All surviving patients were regularly controlled at the out-patient clinic and none was lost to follow-up. Mean follow-up was 101 mo. RESULTS: One month, one-, five- and ten-year patient survival was 79%, 72%, 68% and 68%, respectively. One month, one-, five- and ten-year graft survival was 69%, 65%, 51% and 38%, respectively. Six patients needed early (< 2 mo) retransplantation, four for primary non-function, one for early acute refractory rejection because of ABO blood group incompatibility, and one for a malignant tumor found in the donor. Two patients with hepatitis B FHF developed cerebral lesions peri-transplantion: One developed irreversible and extensive brain damage leading to death, and one suffered from deep deficits leading to continuous medical care in a specialized institution. CONCLUSION: Long-term outcome of patients transplanted for non-acetaminophen FHF may be excellent. As the quality of life of these patients is also particularly good, LT for FHF is clearly justified, despite lower graft survival compared with LT for other liver diseases.

Early local intracoronary platelet activation after drug-eluting stent placement

Background Early local platelet activation after coronary intervention identifies patients at increased risk of acute stent thrombosis （AST）. However, early changes in platelet activation in coronary circulation following drug-eluting stent （DES） implantation have never been reported. Methods In a prospective study of 26 consecutive elective stable angina patients, platelet activation was analyzed by measuring soluble glycoprotein V （sGPV） and P-selectin （CD62P） before and after implantation of either DES or bare metal stent （BMS）. All patients were pretreated with clopidogrel （300 mg loading dose） and aspirin （75 mg orally） the day before the procedure. Blood samples were drawn from the coronary ostium and 10 - 20 mm distal to the lesion site. Results Consistent with the lower baseline clinical risk, the levels of CD62P and sGPV were within normal reference range, both in the coronary ostium and distal to the lesion before percutaneous coronary intervention （PCI） procedure. The levels of CD62P and sGPV did not change significantly （CD62P：（31.1 ± 9.86） ng/ml vs （29.5 ± 9.02） ng/ml, P=0.319 and sGPV：（52.4 ± 13.5） ng/ml vs （51.8 ± 11.7） ng/ml, P=0.674, respectively） after stent implantation when compared with baseline. Changes in these platelet activation markers did not differ between stent types. Conclusions Intracoronary local platelet activation does not occur in stable angina patients before and immediately followina DES implantation when dual anti-Dlatelet is administered.

Value of intravascular ultrasound imaging in following up patients with replacement of the ascending aorta for acute type A aortic dissection

Background The value of intravascular ultrasound （IVUS） imaging in patients with replacement of the ascending aorta for acute type A aortic dissection （AD） is unknown. The purpose of this study was to assess the potential use of IVUS imaging in this setting. Methods From September 2002 to July 2005, IVUS imaging with a 9 MHz probe was performed in a series of 16 consecutive patients with suspected or established AD. This study focused on 5 of them with replacement of the ascending aorta for acute type A AD. Among these 5 patients, other imaging modalities including aortography, spiral computed tomography, magnetic resonance imaging and transesophageal echocardiography were performed in 5, 3, 3 and 1 patients, respectively. Results There were no complications related to IVUS imaging. For the replaced graft, as other imaging modalities, IVUS could identify all 5 grafts, the proximal and the distal anastomoses, and the ostia of the reimplanted coronary arteries. In 2 cases, IVUS detected 2 peri-graft pseudo-aneurysms （1 per case）, which were also detected by magnetic resonance imaging but omitted by aortography. For the residual dissection, IVUS had similar findings as other imaging modalities in detecting the patency （5/5）, the longitudinal and the circumferential extent, the thrombus （4/5）, the recurrent dissection （1/5） and an aneurysm distal to the graft （5 in 4 patients）. However, it detected more intimal tears and side branch involvements than other imaging modalities （15 vs 10 and 3 vs 1, respectively）. Conclusions In following-up patients with replacement of the ascending aorta for acute type A AD, IVUS imaging can provide complete information of the replaced graft and the residual dissection. So, IVUS imaging may be considered when the four current frequently used imaging modalities can not supply sufficient information or there are some discrepancies between them.

Energy Analysis of a Counter-Flow Plate Heat Exchanger Working in Stationary Mode and Using Geothermal Energy for Drying

This paper presents an energy analysis of a counter-flow plate heat exchanger operating in stationary mode. The exchanger comprises a rectangular plate of which we vary the heat transfer surface (by fixing its width and varying its length) and analyze the evolution of some parameters as a function of this heat transfer surface. The parameters to be analyzed are the NTU (number of transfer units), the effectiveness, the temperatures of both primary and secondary fluids and the heat flux. The analysis carried out will help understand the behavior of the counter-flow plate heat exchanger operating in stationary mode. Then, the heat transfer surface of the exchanger is determined for the sizing of a counter-flow plate heat exchanger intended to produce hot air for drying. That plate heat exchanger uses water vapor heated by geothermal energy as a primary fluid and atmospheric air as a secondary fluid. The products to be dried are onions with a mass flow of water to be evacuated of 100 kg/h.

Monitoring the metabolism and function of the central nervous system not only is an old idea but also is a topic that is of increasing interest to the technological evolution. Beside the optimization of cerebral and spinal cord perfusion and the preservation of vasoreactivity to ensure the viability of cerebral tissues and structures, we want to know more and more about the real intimate situation of these organs in real time at the patient’s bedside. To this end, several tracks have been explored during the two last decades, leading to the development of numerous concepts and the conception of various monitoring systems. One of the main problems is to characterize the respective strong points and weaknesses of those ones and to conclude regarding their individual relevance and value in current clinical practice. It is more and more clear that the combination of different categories of monitoring is a way to try to find the most valuable technological compromise, to increase the chance of prediction or of early detection of intercurrent deleterious events corresponding to the concept of multimodality. The intraoperative period and the intensive care goals and targets are appreciably different. This is the reason for the attempt to define different and distinct sets of goals and targets for the intraoperative anesthetic setting and for the intensive care unit.

Impact of ribavirin dose on retreatment of chronic hepatitis C patients

AIM: To study the efficacy and factors associated with a sustained virological response (SVR) in chronic hepatitis C (CHC) relapsing patients. METHODS: Out of 1228 CHC patients treated with pegylated interferon (PEG-IFN) and ribavirin (RBV), 165 (13%) had a relapse. Among these, 62 patients were retreated with PEG-IFN-2a or-2b and RBV. Clinical, biological, virological and histological data were collected. Initial doses and treatment modifications were recorded. The efficacy of retreatment and predictive factors for SVR were analyzed. RESULTS: An SVR was achieved in 42% of patients. SVR was higher in young ( 15.2 mg/kg per day (70% vs 35%, P = 0.04). In logistic regression, predictors of a response were age (P = 0.018), genotype (P = 0.048) and initial RBV dose/body weight (P = 0.022). None of the patients without a complete early virological response achieved an SVR (negative predictive value = 100%). CONCLUSION: Retreatment with PEG-IFN/RBV is effective in genotype 2 or 3 relapsers, especially in young patients. A high dose of RBV seems to be important for the retreatment response.

Phenotypic, Cytological and Molecular (AFLP) Analyses of the Cotton Synthetic Allohexaploid Hybrid (G. hirsutum×G. longicalyx)²

The wild cotton diploid species (2n = 2x = 26) are important sources of useful traits such as high fiber quality, resistance to biotic and abiotic stresses etc., which can be introgressed into the cultivated tetraploid cotton Gossypium hirsutum L (2n = 4x = 52), for its genetic improvement. The African wild diploid species G. longicalyx Hutchinson and Lee could be used as donor of the desirable traits of fiber fineness and resistance to reniform nematode. However, hybridization of wild diploid species and cultivated tetraploid cotton encounters a sterility problem of the triploid (2n = 3x = 59), mainly due to ploidy. The restoration of the fertility can be done by creating an allohexaploid (2n = 6x = 78) through the doubling with colchicine of the sterile triploid chromosomes. With this method, a synthetic allohexaploid hybrid (G. hirsutum × G. longicalyx)2 has been obtained. This genotype was studied using phenotypic, cytological and molecular (AFLP) analyses in order to confirm its hybridity and its caryotype, and also to verify the expression of the desirable traits coming from G. longicalyx. The studied genotype showed a quite good level of pollen fertility (83%), and apart from larger seeds and some minor seedling anomalies, most of its morphological characteristics were intermediate between the two parental species. It had 78 chromosomes, proving its hexaploid status. Molecular analysis revealed 136 AFLP loci in this hexaploid, all from G. hirsutum and G. longicalyx, demonstrating its hybrid status. In addition, the hexaploid exhibited the useful traits of G. longicalyx with regard to its remarkable fiber fineness and its high resistance to the reniform nematode. This allohexaploid hybrid constitutes an interesting agronomic material, which can be used as a bridge for the transfer of useful agronomic traits from wild species to varieties of G. hirsutum.

SnRK2 kinases, PP2C phosphatases and the PYR/PYL/RCAR receptors constitute the core abscisic acid （ABA） signaling module that is thought to contain all of the intrinsic properties to self-regulate the hormone signal output. Here we identify Casein Kinase （CK）2 as a novel negative regulator of SnRK2. CK2 phosphor- ylates a cluster of conserved serines at the ABA box of SnRK2, increasing its binding to PP2C and triggering protein degradation. Consequently, CK2 action has implications on SnRK2 protein levels, as well as kinase activity and its response to abiotic stimuli.
