Sepsis after Administering an Intramuscular Iron Injection in the Arm of an Elder Patient with Multiple Comorbidities: A Case Report

Background: Skin reactions and musculoskeletal injuries caused by intramuscular injection are an increased risk for skin and soft tissue infections and may culminate in sepsis. Objective: To describe the history of an elderly patient who developed a lesion in her right arm after home administration of intramuscular iron injection in this arm and its outcome. Methods: This is a case report of a patient who participated in a study in an intensive care unit. Results: This study describes a 65-year-old patient with multiple comorbidities who was admitted to the hospital complaining of pain for two months and difficulty in moving her right shoulder associated with skin and soft tissue infections in her right arm after intramuscular iron administration by a relative for the treatment of multifactorial anemia and refractory melena. The patient worsened her general condition and was transferred to the intensive care unit. Despite the therapy instituted, the patient developed sepsis of cutaneous origin, multiple organ dysfunction, and death. Conclusions: The administration of intramuscular medication requires the participation of qualified professionals, such as nurses, mainly within the scope of the Brazilian public health system which commonly comprises patients in a context of vulnerability.

Parotid gland adenocarcinoma is commonly a tumor of low malignancy and low incidence worldwide. The reported case shows the rapid progression of this tumor in an elderly patient and infrequent effects, such as a presentation of facial edema not commonly described in the medical literature. Patient was admitted to hospital in November 2019 with secretion and partial hearing loss in the right ear and infiltrative and stone lesion with initial skin ulceration in the right cervical region. After 42 days, he returned and was admitted to the intensive care unit with significant swelling of the face, hardened and hyperemic neck, difficulty in speech and inability to open the eye. He presented changes in the mobility of the speech and hearing organs, reduced laryngeal mobility, vocal changes, speech with altered articulation and severe oropharyngeal dysphagia with risk of bronchoaspiration. The patient was diagnosed in September 2019 with a parotid tumor (salivary gland adenocarcinoma T4). The medical team requested computed tomography, computed tomography angiography of the chest and cervical vessels and computed tomography of the neck, in addition to evaluation by the head and neck surgery service and general surgery. After analyzing the results, the medical team suggested a hypothesis of tumor invasion that could result in obstruction of local lymphatic drainage, something unusual in the evolution of this type of tumor. In addition, it was not possible to adhere to radiotherapy treatment due to the extent of the lesion and there was also no confirmation of metastases. The reported case shows us that parotid gland adenocarcinoma, when diagnosed in an advanced stage, can limit the approach to treatment. It was chosen in agreement with the family to proceed with palliative care without invasive measures. Palliative care may be the best option for cases like this, bringing some comfort to the patient and his family.

Learning to combine multiple string similarity metrics for effective toponym matching

Several tasks related to geographical information retrieval and to the geographical information sciences involve toponym matching,that is,the problem of matching place names that share a common *** this article,we present the results of a wide-ranging evaluation on the performance of different string similarity metrics over the toponym matching *** also report on experiments involving the usage of supervised machine learning for combining multiple similarity metrics,which has the natural advantage of avoiding the manual tuning of similarity *** with a very large dataset show that the performance differences for the individual similarity metrics are relatively small,and that carefully tuning the similarity threshold is important for achieving good *** methods based on supervised machine learning,particularly when considering ensembles of decision trees,can achieve good results on this task,significantly outperforming the individual similarity metrics.

High-pressure metamorphic rocks in the Borborema Province, Northeast Brazil: Reworking of Archean oceanic crust during proterozoic orogenies

We present the first evidence of Archean oceanic crust submitted to Proterozoic high-pressure(HP)metamorphism in the South American ***-Nd and Lu-Hf isotopic data combined with U-Pb geochronological data from the Campo Grande area,Rio Grande do Norte domain,in the Northern Borborema Province,reflect a complex Archean(2.9 Ga and 2.6 Ga)and Paleoproterozoic(2.0 Ga)evolution,culminating in the Neoproterozoic Brasiliano/Pan-African orogeny(ca.600 Ma).The preserved mafic rocks contain massive poikiloblastic garnet and granoblastic amphibole with variable proportions of plagioclase+diopside in symplectitic texture,typical of high-pressure *** clinopyroxene-garnet amphibolites and the more common garnet amphibolites from the Campo Grande area are exposed as rare lenses within an Archean migmatite *** amphibolite lenses represent 2.65 Ga juvenile tholeiitic magmatism derived from depleted mantle sources(positive values of+3.81 to+30.66)later enriched by mantle metasomatism(negative εNd(t)values of-7.97).Chondrite and Primitive Mantle-normalized REE of analyzed samples and discriminant diagrams define two different oceanic affinities,with E-MORB and OIB *** Eu anomalies(Eu/Eu*=0.75-0.95)indicate depletion of plagioclase in the *** zircon cores of 3.0-2.9 Ga in analyzed samples indicate that the Neoarchean tholeiitic magmatism was emplaced into 2923±14 Ma old Mesoarchean crust(εNd(t)--2.58 and Nd TDM=3.2 Ga)of the Rio Grande do Norte *** age of retro-eclogite facies metamorphism is not yet completely *** suggest that two high-grade metamorphic events are recognized in the mafic rocks:the first at 2.0 Ga,recorded in some samples,and the second,at ca.600 Ma,stronger and more pervasive and recorded in several of the mafic rock *** Neoproterozoic zircon grains are found in symplectite texture as inclusions in the garnet grains and represent the age of HP conditions in the *** zircon grains show a

Feature reduction with PCA/ KPCA for gait classification with different assistive devices

Purpose–The purpose of this paper is to propose a gait analysis technique that aims to identify differences and similarities in gait performance between three different assistive devices(ADs).Design/methodology/approach–Two feature reduction techniques,linear principal component analysis(PCA)and nonlinear kernel-PCA(KPCA),are expanded to provide a comparison of the spatio-temporal,symmetrical indexes and postural control parameters among the three different ***,a multiclass support vector machine(MSVM)with different approaches is designed to evaluate the potential of PCA and KPCA to extract relevant gait features that can differentiate between ***–Results demonstrated that symmetrical indexes and postural control parameters are better suited to provide useful information about the different gait patterns that total knee arthroplasty(TKA)patients present when walking with different *** combination of KPCA and MSVM with discriminant functions(MSVM DF)resulted in a noticeably improved *** combination demonstrated that,with symmetric indexes and postural control parameters,it is possible to extract with high-accuracy nonlinear gait features for automatic classification of gait patterns with ***/value–The information obtained with the proposed technique could be used to identify benefits and limitations of ADs on the rehabilitation process and to evaluate the benefit of their use in TKA patients.

Rock slope hazard assessment is an important part of risk analysis for open pit *** main parameters that can lead to rock slope failures are the parameters traditionally used in geomechanical classifications,the slope geometrical parameters and external factors like rainfall and *** paper presents a methodology for a hazard assessment system for open pit mine slopes based on 88 cases collated around the world using principal components analysis,discriminant analysis and confidence *** historical cases used in this study included copper,gold,iron,diamond,lead and zinc,platinum and claystone *** variables used in the assessment methodology are uniaxial compressive strength of intact rock;spacing,persistence,opening,roughness,infilling and orientation of the main discontinuity set;weathering of the rock mass;groundwater;blasting method;and height and inclination of the *** principal component analysis was used to quantify the data,the discriminant analysis was used to establish a rule to classify new slopes about its stability *** provide a practical hazard assessment system,confidence ellipses were used to propose a hazard graph and generate the *** discriminant rule developed in this research has a high discrimination capacity with an error rate of 11.36%.

Trauma to the peripheral nervous system often results in loss of motor and sensory functions of the affected area of the body,leading to a series of functional impairments(Allodi et al.,2012).Injuries to peripheral nerves initiate a series of complex events,known as Wallerian degeneration,which allows the injured axons to regenerate and reinnervate their targets(Allodi et al.,2012).Damage to a nerve induces multiple alterations,which include:axonal degeneration,breakdown of myelin sheath,Schwann cells(SC)proliferation and conversion to a repair phenotype which express cytokines and chemokines that allow the infiltration and activation of macrophages(Mietto et al.,2015;Jessen and Mirsky,2016).

α,β-amyrin, a natural triterpenoid ameliorates L-arginine-induced acute pancreatitis in rats

AIM: To study the benef icial effects of triterpene α,β-amyrin and the underlying mechanisms in an experimental pancreatitis model. METHODS: Acute pancreatitis was induced in five groups of rats (n = 8) by L-arginine (2 × 2.5 g/kg, intraperitoneal, 1 h apart) and 1 h later, they received a single oral dose of α,β-amyrin (10, 30 and 100 mg/kg),methylprednisolone (30 mg/kg) and vehicle (3% Tween 80). A saline (0.9% NaCl) treated group served as a normal control. Efficacy was assessed at 24 h by determination of serum levels of amylase, lipase and proinflammatory cytokines [tumor necrosis factor (TNF)-α and interleukin (IL)-6], pancreatic myeloperoxidase (MPO) activity, lipid peroxidation [thiobarbituric acid reactive substances (TBARS)], nitrate/nitrite levels, and the wet weight/body weight ratio. Tissue histology and the immunoreactivity for TNF-α and inducible nitric oxide synthetase (iNOS) were performed. RESULTS: α,β-amyrin and methylprednisolone treatments significantly (P < 0.05) attenuated the L-arginine-induced increases in pancreatic wet weight/body weight ratio, and decreased the serum levels of amylase and lipase, and TNF-α and IL-6, as compared to the vehicle control. Also, pancreatic levels of MPO activity, TBARS, and nitrate/nitrite were signifi cantly lower. Histological f indings and TNF-α and iNOS immunostaining further confirmed the amelioration of pancreatic injury by α,β-amyrin. CONCLUSION: α,β-amyrin has the potential to combat acute pancreatitis by acting as an anti-in? ammatory and antioxidant agent.
