Adjusting CA19-9 values to predict malignancy in obstructive jaundice:Influence of bilirubin and C-reactive protein

AIM:To find a possible relationship between inflammation and CA19-9 tumor marker by analyzing data from patients with benign jaundice(BJ) and malignant jaundice(MJ).METHODS:All patients admitted for obstructive jaundice,in the period 2005-2009,were prospectively enrolled in the study,obtaining a total of 102 *** admission,all patients underwent complete standard blood test examinations including C-reactive protein(CRP),bilirubin,*** were considered eligible for the study when they presented obstructive jaundice confirmed by instrumental examinations and increased serum bilirubin levels(total bilirubin > 2.0 mg/dL).The standard cut-off level for CA19-9 was 32 U/mL,whereas for CRP this was 1.5 mg/*** CA19-9 level was adjusted by dividing it by the value of serum bilirubin or by the CRP *** patients were divided into 2 groups,MJ and BJ,and after the adjustment a comparison between the 2 groups of patients was ***,specificity and positive predictive values were calculated before and after the ***:Of the 102 patients,51 were affected by BJ and 51 by *** CA19-9 levels were found in 71.7% of the *** the group of 51 BJ patients there were 29(56.9%) males and 22(43.1%) females with a median age of 66 years(range 24-96 years),whereas in the MJ group there were 24(47%) males and 27(53%) females,with a mean age of 70 years(range 30-92 years).Pathologic CA19-9 serum level was found in 82.3% of *** levels were pathologic in 66.6% of the patients with BJ and in 49% with *** and CA19-9 average levels were significantly higher in MJ compared with BJ(P = 0.000 and P = 0.02),while the CRP level was significantly higher in BJ(P = 0.000).Considering a CA19-9 cut-off level of 32 U/mL,82.3% in the MJ group and 54.9% in the BJ group were positive for CA19-9(P = 0.002).A CA19-9 cut-off of 100 U/mL increases the difference between the two groups:35.3% in BJ and 68.6% in MJ(P = 0.0007).Adjusting the CA1

电子商务入口网站是数字化背景下跨境商务得以实现的要素并且逐渐成为了关键性的销售渠道,供全球各地的公司借以招徕在线客户。公司,特别是零售商的这些触点研究所产生的影响,推动了作为技巧的范式的变革,因为用户界面和用户体验对于电子商务平台的建设、定制和持续促进都对十分必要的。虽然电子商务平台使得公司能够进行跨国售卖。文化会对市场的诸多特异性产生影响,也会对用户在线购物的方式产生影响。在此背景下,本文对现有的对中国电子商务进行的研究实施分析,这些研究大部分都与此类服务的应用有关。同时本文还将对消费者行为的文化层面探究,从不同的角度对这样的应用进行讨论。为对消费者行为进行解释,一项调查对中国和欧洲的电子商务应用状况进行了研究,尝试判定消费者消费过程中最为重要的消费者体验要素。笔者对三个不同群体的观点进行了对比:第一种,生活于中国的中国学生与年轻职员;第二种,两所意大利大学的东方研究学院毕业生;第三种,生活于中国但来自其他不同国家的年轻学生与职员。这些研究的主要观点是社会互动是在中国市场中建立电子商务网站的关键要素。这一趋势在新一代群体中尤为明显,因此对这一发现进行应用能够确保针对中国客户的(?)户网站的成功建立。调查结果与该课题资深人员的观点为我们提供了必要的信息,使我们能够针对设计框架实施可用性测试,从而以社会互动为用户体验核心进行考量,并最终将结果纳入对于西方电子商务向中国市场提供的体验的分析。可用性实验在伦敦针对一组亚洲学生进行,由于实验对象对两种内户网站范式(西方与中国)都很熟悉,因此我们得到了非常有趣的结果。这一研究具有较好的实用性并能够很方便地获得分析工具从而实施可用性分析,以确保满足文化本土化和该市场下未来用户的需求,从而为未来此方面的研究提供参考。用户体验人员、网页设计师、业务分析师、产品经理/所有者都能够以本文研究为参考,进行以用户为中心的研究。因此,本研究突出了针对中国用户的(?)户网站中集体取向的影响,为针对中国市场进行电子商务开发提供了全面的特征或行为以供参考。无论其可行性如何,其优化应用将不仅依赖于创新设计的使用,同时还会受到社会互动对于进行更高可能性会话推送的推荐算法的影响的左右。

Aortic Graft Complicated by a Corynebacterium Striatum Infection Due to Previous Type IV Thoraco Abdominal Aortic Aneurysm Repair

We report successful management of aortic graft infection without graft explanation or extra-anatomic bypass. A 61 year-old male who had undergone surgical repair of a type IV thoraco-abdominal aortic aneurysm presented with left flank pain and raised inflammatory markers following graft implantation. CT scanning revealed a left psoas fluid collection. Graft infection was diagnosed. A radio-guided drainage and conservative management, with irrigation drain placement was undertaken with preservation of the aortic graft. There was no evidence of recurrent infection after follow-up at 34 months. Aortic endograft infection may be managed by surgical or radio-guided drainage, antibiotic irrigation of the graft and systemic antibiotic therapy without graft removal.

AIM:To compare the site,age and gender of cases of colorectal cancer(CRC) and polyps in a single referral center in Rome,Italy,during two ***:CRC data were collected from surgery/pathology registers,and polyp data from colonoscopy *** who met the criteria for familial adenomatous polyposis,hereditary non-polyposis colorectal cancer syndrome or inflammatory bowel disease were excluded from the *** of patients between the two groups(cancers and polyps) was carefully ***χ 2 statistical test and a regression analysis were ***:Data from a total of 768 patients(352 and 416 patients,respectively,in periods A and B) who underwent surgery for cancer were *** the same time periods,a total of 1693 polyps were analyzed from 978 patients with complete colonoscopies(428 polyps from 273 patients during period A and 1265 polyps from 705 patients during period B).A proximal shift in cancer occurred during the latter years for both sexes,but particularly in *** cancer increased > 3-fold in period B compared to period A in males [odds ratio(OR) 3.31,95%CI:2.00-5.47;P < 0.0001).A similar proximal shift was observed for polyps,particularly in males(OR 1.87,95%CI:1.23-2.87;P < 0.0038),but also in females(OR 1.62,95%CI:0.96-2.73;P < 0.07).CONCLUSION:The prevalence of proximal proliferative colonic lesions seems to have increased over the last decade,particularly in males.

Causes and consequences of microsatellite instability in gastric carcinogenesis

Loss of DNA mismatch repair(MMR) function, due to somatic or germline epi/genetic alterations of MMR genes leads to the accumulation of numerous mutations across the genome, creating a molecular phenotype known as microsatellite instability(MSI). In gastric cancer(g C), MSI occurs in about 15% to 30% of the cases. This review summarizes the current knowledge on the molecular mechanisms underlying the acquisition of MSI in g C as well as on the clinic, pathologic and molecular consequences of the MSI phenotype. Additionally, current therapeutic strategies for g C and their applicability in the MSI subset are also discussed.

Towards a Tolerance of Ambiguity Model in Greek Tourism Industry

The present paper examines the influence of locus of control, involvement, job satisfaction, and organizational commitment on hotel chief executive officers＇（CEOs） tolerance of ambiguity. The research sample consists of 82 Greek hotels＇ CEOs. The first part of this paper analyses their level of locus of control, involvement, job satisfaction, and organizational commitment, in front of the frequent, uncertain, and ambiguous changes in their business environment. In the second part, results of principal component analysis indicate that two factors characterize CEOs＇ involvement, namely, importance and interest. Further, regression results reveal the significant influence of interest, locus of control, and job satisfaction on CEOs＇ tolerance of ambiguity. Finally, this paper discusses the research findings and proposes certain practical implications for enhancing Greek hotel CEOs＇ level of tolerance of ambiguity and thus, their efficiency during change.

Background:Parkinson’s Disease(PD)with mild cognitive impairment(MCI)(PD-MCI)represents one of the most dreaded complications for patients with PD and is associated with a higher risk of developing *** transcranial direct current stimulation(tDCS)has been demonstrated to improve motor and non-motor symptoms in PD,to date,no study has investigated the effects of tDCS on Theory of Mind(ToM),i.e.,the ability to understand and predict other people’s behaviours,in ***:In this randomized,double-blind,sham-controlled study,we applied active tDCS over the medial frontal cortex(MFC)to modulate ToM performance in twenty patients with *** matched healthy controls(HC)were also enrolled and were asked to perform the ToM task without receiving ***:In the patients with PD-MCI,i)ToM performance was worse than that in the HC,ii)ToM abilities were poorer in those with fronto-executive difficulties,and iii)tDCS over the MFC led to significant shortening of latency for ToM ***:We show for the first time that active tDCS over the MFC enhances ToM in patients with PD-MCI,and suggest that non-invasive brain stimulation could be used to ameliorate ToM deficits observed in these patients.
