Infections Following Kidney Transplant in Children: A Single-Center Study

Introduction: Infections are a major cause of morbidity and mortality in pediatric patients undergoing kidney transplantation (KT). Aim and Methods: To determine the patterns of infectious complications during the first 6 months post transplantation, we report our single center experience with data from pediatric kidney recipients transplanted between 2006 and 2011. Results: Thirty-two children (20 males) were submitted to KT. The most common cause of end-stage renal disease (ESRD) was congenital anomalies of the kidney and urinary tract (CAKUT) accounting for 62%. Over the first 6 months post-transplant period, twenty-eight (87.5%) children developed a total of 77 infections, mainly urinary tract infections (UTI) (64.9%). CAKUT etiology of ESRD and UTI before KT increased the risk to develop more than one episode of UTI [71.4% vs. 14.3% and 81.8% vs. 18.2%, respectively;p < 0.05]. Twenty-three (29.9%) viral infections occurred. Cytomegalovirus (CMV) was the most common opportunistic pathogen, occurred in 11 patients and was more frequently in those with a donor (D)+/recipient (R)- CMV sero-status [74.5% vs. 25.5% (p < 0.05)]. A polyomaviruses (BKV) disease with nephropathy and meningitis was registered. The majority of infectious episodes had mild or moderate severity. No deaths occurred. Conclusion: A significant number of patients presented infectious complications during the first 6 months post transplantation. UTI are the most common type of infection, followed by viral infections, particularly CMV. Recognition, prevention and early treatment of infections are of major importance.

Importance:The 3-min step test is a simple option to monitor submaximal exercise capacity,although its use via remote video monitoring has not been investigated in children with cystic fibrosis(CF).Objective:This study aimed to assess the feasibility and reproducibility of performing the 3-min step test with remote ***:A cross-sectional study including CF patients(6-18 years)from two CF services were ***,anthropometric,clinical,and lung function data were collected and two 3-min step tests were performed:(i)in-person supervision,and(ii)remotely supervised by video *** and after the tests,heart rate(HR),oxygen saturation(SpO_(2)),and the Borg score for dyspnea and lower limb fatigue were ***:Twenty-three patients(10.7±3.7 years)with a mean FEV1 of 89.5%±23.2%were *** were no significant differences between tests,with mean differences(95%confidence intervals)in final HR of-3.3(-8.9,2.4),change in HR of-1.9(-6.1,2.1),final SpO_(2) of 0.3(-0.4,1.0),and final dyspnea of 0.1(-0.8,0.9).The intraclass correlation coefficient was 0.852(final HR),0.762(final SpO_(2)),and 0.775(final lower limb fatigue).Significant and moderate correlations were found between tests for final HR(r=0.75),change in HR(r=0.61),and final SpO_(2)(r=0.61).The Bland-Altman analysis showed a mean difference in final SpO_(2) between tests of 0.3%(limit of agreement-3.0%,3.5%).Interpretation:Physiological responses between tests were similar,indicating it was feasible to perform the 3-min step test with remote supervision in CF children.

A sensitive electrochemical detection of metronidazole in synthetic serum and urine samples using low-cost screen-printed electrodes modified with reduced graphene oxide and C60

Monitoring the concentration of antibiotics in body fluids is essential to optimizing the therapy and minimizing the risk of bacteria resistance,which can be made with electrochemical sensors tailored with appropriate *** this paper,we report on sensors made with screen-printed electrodes(SPE)coated with fullerene(C60),reduced graphene oxide(rGO)and Nafion(NF)(C60-rGO-NF/SPE)to determine the antibiotic metronidazole(MTZ).Under optimized conditions,the C60-rGO-NF/SPE sensor exhibited a linear response in square wave voltammetry for MTZ concentrations from 2.5×10^(-7) to 34×10^(-6) mol/L,with a detection limit of 2.1×10^(-7) mol/*** sensor was also capable of detecting MTZ in serum and urine,with recovery between 94%and 100%,which are similar to those of the standard chromatographic method(HPLC-UV).Because the C60-rGO-NF/SPE sensor is amenable to mass production and allows for MTZ determination with simple principles of detection,it fulfills the requirements of therapeutic drug monitoring programs.

Fluid-dynamics evaluation in a conical spouted bed and characterization of foxtail millet seeds

Foxtail millet （Setaria italica） is one of the most valuable species in economic terms in the genus Setaria and plays an important role in human nutrition, animal feed, and agriculture. The present study described chemical, physical, and quality aspects of seeds of foxtail millet. Furthermore, the fluid-dynamic behavior of the seeds was evaluated in a conical spouted bed, which has advantages in terms of promoting the cyclic and regular movement of the seed particles. Dynamic parameters of spouting （minimum spouting veloc- ity, stable and peak pressure drop） were determined and compared with those obtained from empirical correlations available in the literature. The results obtained from physical characterization showed that the seeds can be classified as belonging to Group D of Geldart, having a non-rough surface, mean diameter of 1.75 mm, and sphericity of 0.74. Fluid-dynamics analysis showed that the seeds are suitable for processing in a spouted bed, which is in agreement with the results of particle physical characterization.

Interactive Multimodal Narrative as an Approach to Developing Emergent Literacy in Early Childhood Education

At present, the Portuguese education system promotes the development of new literacies in all education levels in response to new requirements of the digital world which imply changes in children＇s education. In fact, reading and writing are no longer limited exclusively to books, rather they are associated with diversified digital media integrating text, sound, image, and video. In this context, we developed an exploratory study in a kindergarten with the children and their family members. Data were collected through participant observation, children conversational interviews, and digital narratives produced by children. Data analysis is based on content analysis with NVivo software support. The emergence of literacy is evident in verbal interactions in peer-group work, in contact with the written text, in ＂writing＂ in various media, and in sharing knowledge, discoveries, and digital narratives produced in an online community （http：//***）. This paper presents：（1） a systematic literature review in the field of digital narrative and emergent literacy; （2） the description of children＇s activities concerning interactive multimodal narrative; （3） the results of this work; and （4） the conclusion about contribution of multimodal narratives to the emergence of reading, writing, and digital skills

Hybrid combinations of Eucalyptus have increased due to expansion of plantations into unconventional areas and to the search for higher quality ***,most of these species have difficulties surviving in vitro *** chlorine and sealing systems are often used to reduce contamination and increase gas *** aim of the present study is to evaluate the establishment,multiplication,elongation and adventitious rooting of *** × *** clones(C1 and C2) and four active chlorine concentrations(0.000%,0.001%,0.003%,and 0.005%) were tested in the establishment and multiplication *** sealing forms(W/M,1/M and 3/M) and the same four active chlorine concentrations were applied to the elongation *** luminosities(dark and light)and three sealings(W/M,1/M and 3/M) were tested during adventitious *** chlorine concentration of0.005% led to the lowest fungal contamination rate and to the highest in vitro *** chlorine concentration of 0.003% resulted in the greatest length and highest number of shoots per explant in the multiplication *** were no phytotoxicity problems and the quality of plants grown in an environment with active chlorine was maintained in comparison with those grown in an *** increase in gas exchange in ventilation systems had a positive impact on the in vitro growth and development of plants.

Batch CGTase Production with Free and Immobilized <i>Bacillus firmus</i>Strain 37 in Bovine Bone Charcoal

The present study aimed to study the batch production of CGTase (cyclomaltodextrin-glucanotransferase) with Bacillus firmus strain 37 free and immobilized in bovine bone charcoal in batch mode and in a fluidized bed batch reactor, respectively. The bovine bone charcoal is an innovative support material for the immobilization of microorganisms’ producers of enzymes and the use of this microbial support allows its reuse to a significant cost reduction of the process. The batch fermentation with free cells was investigated for 96 h and reached a CGTase activity equal to 0.77 U/mL. When the microorganism was immobilized on bovine bone charcoal (7 g) and cultivated in fluidized bed batch reactor with air supplementation (1 volume of air/volume of medium * minute), the same activity could be achieved in 24 h. The results of enzymatic activity achieved show the potential of CGTase production in a short time with Bacillus firmus strain 37 immobilized in bovine bone charcoal matrix and using air supplementation in the production medium.
