BACKGROUND Chilaiditi syndrome is a rare disorder characterized by the hepatodiaphragmatic interposition of the *** SUMMARY Here we report a case of a 12-year-old male who was admitted to the pediatric intensive care unit secondary to abdominal pain and severe respiratory *** was treated conservatively but the symptoms persisted requiring a surgical *** there have been several cases of Chilaiditi syndrome reported in adults,there is a scarcity of cases reported in the pediatric *** review of the literature found only 30 pediatric cases,including our reported case,with Chilaiditi syndrome,19(63%)of which were *** median age of diagnosis was 4.5 years old with an interquartile range of 2.0-10.0 *** our review,we found that the most common predisposing factors in children are aerophagia(12/30 cases)and constipation(13/30 cases).Ninety percent of the cases presented with complete intestinal interposition,in 100%of which,the colon was *** of the 30 cases were associated with *** In the pediatric population,conservative(21/30 cases)and surgical(8/30 cases)treatment approaches have produced satisfactory outcomes for all the patients,regardless of approach.

Fatty liver,carotid disease and gallstones:A study of age-related associations

AIM: To evaluate carotid intima-media thickening (IMT) and plaques, gallstone disease (GD) and fatty liver (fl) as a function of age. METHODS: In 449 subjects, fl and carotid disease were assessed ultrasonographically. In a subgroup of 65/449 patients with non-alcoholic fatty liver disease (NAflD), carotid disease, GD and associated factors were determined. RESULTS: fl of unspecified etiology was more common in younger and GD in older individuals. fl subjects had an increased prevalence of IMT and a decreased prevalence of plaques and manifested carotid disease earlier. Plaques were more common in subjects with GD. Age was an independent predictor of carotid disease outcome and fl was a protective factor for plaques. In NAflD, there was an inverse correlation between body weight and age and the latter independently predicted carotid findings. CONCLUSION: Cardiovascular risk in patients with fl and NAflD needs to be assessed as a function of age and body weight.

Culture Circle as a Teaching Approach in the Education of Teenager Health Multipliers on Leprosy Awareness

Objective: The study aims to evaluate the use of a teaching method proposed by Paulo Freire, Culture Circles, in the education of teenagers multipliers on leprosy awareness. Methods: It is an action-research study with a qualitative approach developed in a public school in Pernambuco, Brazil. Five Culture Circles were conducted involving the participation of 26 teenagers. The followings were used as data collection tools: observation, field notes, photography and filming. Results: The educational intervention on health addressed the following topics: 1) Definition and transmission of leprosy;2) Characteristics and diagnosis of leprosy;3) Treatment of leprosy;4) Aesthetics, prejudice and mental health related to leprosy;and 5) Planning of educational activities for teenagers as health multipliers on leprosy awareness. The educational action on health provided this age group to perceive themselves, act as political subjects in the development of Culture Circles, and act as protagonists in the dissemination of knowledge on leprosy. Conclusions: This study highlights that the application of active methodologies, such as Culture Circles, is able to encourage the engagement of young people in community empowerment and bring together health professionals and the school community in an intersectoral work in order to develop action strategies involving the promotion of health in the context of neglected diseases such as leprosy.

Upscaling human mesenchymal stromal cell production in a novel vertical-wheel bioreactor enhances extracellular vesicle secretion and cargo profile

Human mesenchymal stromal cells(hMSCs)are mechanically sensitive undergoing phenotypic alterations when subjected to shear stress,cell aggregation,and substrate changes encountered in 3D dynamic bioreactor ***,little is known about how bioreactor microenvironment affects the secretion and cargo profiles of hMSC-derived extracellular vesicles(EVs)including the subset,“exosomes”,which contain therapeutic proteins,nucleic acids,and lipids from the parent *** this study,bone marrow-derived hMSCs were expanded on 3D Synthemax II microcarriers in the PBS mini 0.1L Vertical-Wheel bioreactor system under variable shear stress levels at 25,40,and 64 RPM(0.1-0.3 dyn/cm^(2)).The bioreactor system promotes EV secretion from hMSCs by 2.5-fold and upregulates the expression of EV biogenesis markers and glycolysis genes compared to the static 2D *** microRNA cargo was also altered in the EVs from bioreactor culture including the upregulation of miR-10,19a,19b,21,132,and *** protein cargo was characterized by proteomics analysis,showing upregulation of metabolic,autophagy and ROS-related proteins comparing with 2D cultured *** addition,the scalability of the Vertical-Wheel bioreactor system was demonstrated in a 0.5L bioreactor,showing similar or better hMSC-EV secretion and cargo content compared to the 0.1L *** study advances our understanding of bio-manufacturing of stem cell-derived EVs for applications in cell-free therapy towards treating neurological disorders such as ischemic stroke,Alzheimer’s disease,and multiple sclerosis.

Is there a relationship between length of resection and lymph-node ratio in colorectal cancer?

Background:The prognosis of colorectal cancer depends on the number of positive lymph nodes(LNþ)and the total number of lymph nodes resected(rLN).This represents the lymph-node ratio(LNR).The aim of our study is to assess how the length of the resected specimen(RL)influences the prognostic values of the ***:We conducted a retrospective study of all the patients operated on for colorectal cancer from 2000 to 2015 at our *** details were *** total number of rLN,the number of LNþ,and the LNR were calculated and measured against the *** receiver-operating characteristic(ROC)curve of patients with LNþwas ***:Of the 670 patients included in our study,337 were men(50.3%)and the mean age was 69.2 *** correlation with prognosis of the LNR is greater than that of the LNR adjusted to RL(LNR/RL),both in subjects with positive nodes(n=312)and in all cases(n=670).The LNR presents a higher prognostic value than LNR/RL and RL in patients with LNþexcept for metastatic recurrence,for which the predictive value appears slightly higher for LNR/*** statistical significance of the maximal divergence in Kaplan–Meier survival plots was demonstrated for the LNR(P=0.043),not for LNR/RL(P=0.373)and RL alone(P=0.314).Conclusion:An increase in RL causes an increase in the number of harvested lymph nodes without affecting the number of LNþ,thus representing a confounding factor that could alter the prognostic value of the *** larger-scale studies are needed to confirm these findings.

Endoscopic management of adolescent closed Cowper’s gland syringocele with holmium:YAG laser

Dear editor,The term“syringocele”originated from Greek words“syringo”meaning tube and“cele”meaning swelling and was first coined by Maizels et al.[1]in 1983 to represent a spectrum of dilatation of the normal Cowper’s gland *** classified syringocele into four morphological types:Simple,perforate,imperforate,and *** et al.[2]simplified the classification by dividing syringocele into two groups:Closed and open *** diagnosed in the pediatric population,only 10 cases of adult syringocele were reported in literature till 2007[3].Endoscopic management is preferred in most symptomatic adolescent patients and various energy sources employed for endoscopic marsupialization include cold knife urethrotomy,electrocautery,or holmium laser[4,5].Use of holmium laser is reported only in three cases with maximum follow-up of up to 1 year[6e8].We herein report three additional cases of adolescent closed syringocele,provide detailed technique of laser marsupialization,and report its long-term postoperative *** study was approved by the University of Miami Ethics Committee(study ID:20180511).

Increased virulence of the oral microbiome in oral squamous cell carcinoma revealed by metatranscriptome analyses

Oral squamous cell carcinoma(OSCC) is the most prevalent and most commonly studied oral cancer. However, there is a void regarding the role that the oral microbiome may play in OSCC. Although the relationship between microbial community composition and OSCC has been thoroughly investigated, microbial profiles of the human microbiome in cancer are understudied. Here we performed a small pilot study of community-wide metatranscriptome analysis to profile mRNA expression in the entire oral microbiome in OSCC to reveal molecular functions associated with this disease. Fusobacteria showed a statistically significantly higher number of transcripts at tumour sites and tumour-adjacent sites of cancer patients compared to the healthy controls analysed. Regardless of the community composition, specific metabolic signatures were consistently found in disease. Activities such as iron ion transport, tryptophanase activity, peptidase activities and superoxide dismutase were over-represented in tumour and tumour-adjacent samples when compared to the healthy controls. The expression of putative virulence factors in the oral communities associated with OSCC showed that activities related to capsule biosynthesis, flagellum synthesis and assembly, chemotaxis, iron transport, haemolysins and adhesins were upregulated at tumour sites. Moreover, activities associated with protection against reactive nitrogen intermediates, chemotaxis, flagellar and capsule biosynthesis were also upregulated in non-tumour sites of cancer patients. Although they are preliminary, our results further suggest that Fusobacteria may be the leading phylogenetic group responsible for the increase in expression of virulence factors in the oral microbiome of OSCC patients.
