Optimization of Exenatide plus Detemir Treatment for Morbid Obese Patients with Insulin Dependent Diabetes Resistant to Conventional Treatment. Effect of Exenatide on Patients with Mellitus Diabetes Type 2

The purpose of this study was to analyze the effects of the treatment with Exenatide joint to insulin detemir. The study was carried out with obese patients with type 2 diabetes mellitus (DM2) and deficient control, despite intensive insulin therapy combined with oral antidiabetics. A longitudinal study was made with 15 patients controlled by the Endocrinology and Nutrition Section of the Reina Sofía University Hospital, Murcia. These patients were treated with Exenatide joint to insulin during 3, 6 and 12 months. Statistically meaningful differences (p < 0.05) were observed in the reduction of HbA1c in the 3 and 6 months treatment groups (initial HbA1c: 9.55% ± 1.086%, weight: 119.94 kg, insulin requirement: 0.71 IU/kg/day;after 3 months: HbA1c: 7.77% ± 1.164%, weight: 98.13 kg, insulin requirement: 0.59 IU/kg/day;after 6 months: HbA1c: 7.21%, weight: 97.93 kg, insulin requirement: 0.55 IU/kg/day). It was observed that the association of Exenatide and insulin promotes an improvement in several indicators such as weight, body mass index (BMI), systolic pressure (SP), diastolic pressure (DP), triglycerides (TG), cLDL, cHDL and total cholesterol (TC), in patients with diabetes resistant to conventional therapy. However, these improvements were statistically no significant (p < 0.05). It is possible to talk of a reduction of cardiovascular risk factors in patients with Exenatide, being thus a suitable option for this kind of individuals. It is important to emphasize the fact that our study showed bigger body-weight reductions results after 6 and 12 months, as compared to the researches quoted.

Objectives： This study aimed to evaluate the in vitro antioxidant capacity, to determine the anti-inflammatory effect due to lipoxygenase inhibition and to test the antimicrobial activity of ethanolic extracts from leaves of seven climbing species belonging to the Bignoniaceae family. These species are Adenocalyrnrna rnarginatum （Cham.） DC., Amphilophium vauthieri DC., Cuspidaria convoluta （VEIL） A. H. Gentry, Dolichandra dentata （K. Schum.） L. G. Lohmann, Fridericia caudigera （S. Moore） L. G. Lohmann, Fridericia chica （Bonpl.） L. G. Lohmann and Tanaecium selloi （Spreng.） L. G. Lohmann. Methods： The antioxidant activity was evaluated using three methods, 2,2＇-azino-bis（3-ethylbenzothiazo line-6-sulfonic acid）（ABTS）, 2,2-diphenyl-l-picrylhydrazyl （DPPH）, and ferric reducing antioxidant power. Lipoxygenase-inhibiting activity was assayed spectrophotometrically; the result was expressed as percent inhibition. The antimicrobial activity was assessed using the agar disk diffusion method. Minimal inhibitory concentration （MIC） and minimal bactericidal]fungicidal concentration were also determined for each extract against 12 pathogenic bacterial strains of Staphylococcus aureus and seven fungal strains of the Candida genus. The identification of the major compounds present in the most promising extract was established by high-performance liquid chromatography-tandem mass spectrometry. Results： C convoluta, F. caudigera, and F. chica exhibited the best antioxidant activity by scavenging DPPH and ABTS~ radicals and reducing Fe3＋ ion. These extracts showed a notable inhibition of lipnxygenase. F. caudigera was found to have the lower MIC value against S. aureus strains and six Candida species. The extracts of F. caudigera and C. convoluta were active even against methicillin-resistant S. aureus. C convoluta had higher total phenol content, better antioxidant activity and superior anti- inflammatory and antimicrobial activity. The main phenolic compounds found in this extract

Hepatic lipid metabolism changes in short-and long-term prehepatic portal hypertensive rats

AIM: To verify the impairment of the hepatic lipid metabolism in prehepatic portal hypertension. METHODS: The concentrations of free fatty acids, diacylglycerol, triglycerides, and phospholipids were assayed by using D-[U-14C] glucose incorporation in the different lipid fractions and thin-layer chromatography and cholesterol was measured by spectrophotometry, in liver samples of Wistar rats with partial portal vein ligation at short- (1 mo) and long-term (1 year) (i.e. portal hypertensive rats) and the control rats. RESULTS: In the portal hypertensive rats, liver phospholipid synthesis significantly decreased (7.42 ± 0.50 vs 4.70 ± 0.44 nCi/g protein; P < 0.01) and was associated with an increased synthesis of free fatty acids (2.08 ± 0.14 vs 3.36 ± 0.33 nCi/g protein; P < 0.05), diacylglycerol (1.93 ± 0.2 vs 2.26 ± 0.28 nCi/g protein), triglycerides (2.40 ± 0.30 vs 4.49 ± 0.15 nCi/g protein) and cholesterol (24.28 ± 2.12 vs 57.66 ± 3.26 mg/g protein; P < 0.01). CONCLUSION: Prehepatic portal hypertension in rats impairs the liver lipid metabolism. This impairment consists in an increase in lipid deposits (triglycerides,diacylglycerol and cholesterol) in the liver, accompanied by a decrease in phospholipid synthesis.

Theater play that shows how textiles, make up, hair dyes and music players has changed along the last six decades, especially due to polymers. It is a play that has a historic, chemical and musical side that introduces the use of ICT （information and communications technology）, such as Faeebook, Moodle platform, Web sites, E-mail, Word, Publisher, PowerPoint, Videos and Digital music. Results in the conduct of the students that participated： assertiveness, self-esteem, tolerance, cooperation, responsibility and teamwork, all of them significantly contributed a meaningful learning about the polymers.

Thermal analysis of olive tree pruning and the by-products obtained by its gasification and pyrolysis: The effect of some heavy metals on their devolatilization behavior

In this work, the effect of the presence of nickel and lead in thermal decomposition of olive tree pruning (OTP), OTP-char and OTP-ashes was studied by thermogravimetry. Experiments were conducted in two kinds of atmosphere (nitrogen atmosphere and oxidizing atmosphere with 20% of O2) at a heating rate of 10 ℃/min. This investigation describes the chemical, physical and fuel properties of the OTP, which shows a similar composition to other lignocellulosic materials such as hazelnut husk and wood sawdust. In addition, SEM analysis indicated that OTP-char surface is higher than OTP surface with plenty of holes and channels. It makes the char an ideal support for metal retention specially for Pb metal (OTP retained 8.55 mg/g whereas OTP char retained 11.57 mg/g). On the other hand, metal retention occurred by adsorption or ion exchange, according to the IR spectrum of the samples. The results of thermogravimetric tests proved that the presence of lead did not have a strong effect on the decomposition of the samples, since TG and DTG curves were very similar. However, nickel increased the mass loss rate, accelerating the decomposition process, showing higher peaks in DTG curves. Additionally, for temperatures higher than 360 ℃, the volatilization of the samples was improved in the nickel-polluted sample, achieving a higher mass loss, getting more energy from the biomass and reducing the quantity of residues left after the process.

In the general problem of Parametric Point Estimation the Mean Squared Error often appears as a useful measure of goodness or closeness of estimates. Nevertheless, in very rare cases an estimator with smallest Mean Squared Error exists, but Statistical Inference provides a variety of methods to find estimates. that are usually characterized by a small Mean Squared *** the observation of outcomes from the probabilistic information system or experiment concerning the estimation problem involves fuzzy imprecision, so that the observable events are described by means of fuzzy events on the sample space, the use of Zadeh’s probabilistic definition allows us to immediately extend the Mean Squared *** the present paper we are going to verify that the presence of fuzziness in experimental data entails a variation in that measure of goodnesss of estimation. On the basis of the last assertion the problem of selecting the suitable sample size, in order to remove the variation in the Mean Squared Error due to fuzziness or to estimate the parameter with a specified degree of precision, will be then discussed.

Exenatide Response in an Adolescent’s Morbid Obesity

Adolescent obesity is a health problem worldwide. Drug therapy for obesity in children is only acceptable if done within the framework of a comprehensive weight loss, prevention of comorbidities, including health education, changes in lifestyles, diet and exercise. These pharmacological treatments indicate that mainly young patients did not respond to standard measures and developing complications. The role of bariatric surgery, with or without the use of drugs is subject to further research to elucidate its role among the various treatment options for these patients. Currently being studied a wide range of drugs to those included gastrointestinal hormones. Scientific information regarding the use of exenatide in obese patients without diabetes is still limited. The use of exenatide in adolescents with morbid obesity may represent a therapeutic complement invaluable, effective and free of complications, and prevents future bariatric surgery.
