Long-term outcomes of endoscopic submucosal dissection and surgery for undifferentiated intramucosal gastric cancer regardless of size

BACKGROUND The clinical outcomes of endoscopic submucosal dissection(ESD)for undifferentiated(UD)intramucosal early gastric cancer(EGC)compared with those of surgery,regardless of lesion size,are not well ***,there is a concern regarding the treatment plan before and after ESD in cases of UD intramucosal EGC within expanded *** To evaluate clinical outcomes of ESD compared with those of surgery in UD intramucosal EGC patients regardless of tumor *** We enrolled patients with UD intramucosal EGC after ESD with complete resection or surgery from January 2005 to August 2020 who met the within or beyond expanded indications with lesion size>2 cm(the only non-curative factor).Overall,123 and 562 patients underwent ESD and surgery,*** propensity-score matching,clinical and long-term outcomes,i.e.,recurrencefree survival(RFS)and overall survival(OS),were *** multivariable Cox proportional hazard model with treatment modality and ESD indication was used to evaluate the recurrence *** After matching,119 patients each were finally enrolled in the ESD and surgery *** median length of hospital stay was shorter in the ESD group than surgery group(4.0 vs 9.0 days,P<0.001).Four cases of recurrence after ESD were local recurrences,all of which occurred within 1 *** recurrence was seven(5.9%)and two(1.7%)in the ESD and surgery groups,*** difference was observed between the two groups with respect to OS(P=0.948).However,the ESD group had inferior RFS compared with the surgery group(P=0.031).ESD was associated with the risk of recurrence after initial treatment in all enrolled patients(hazard ratio,5.2;95%confidence interval:1.0-25.8,P=0.045).CONCLUSION Although OS was similar between the two groups,surveillance endoscopy was important for the ESD than for the surgery group because RFS was inferior and local recurrence was an issue.

Prediction of Nitrogen Diffusivity in α-ferrite Based on Thermodynamics

A thermodynamic based equation to predict the diffusivity of nitrogen in α-ferrite was investigated in consideration of the equilibrium nitrogen concentration. The temperature-dependent jump distance calculated from the lattice parameter of ferrite was used to derive the frequency factor as a function of temperature. The calculation accuracy for nitrogen diffusivity using the proposed thermodynamic based equation was improved by comparing the calculation results using previous empirical equations based on Arrhenius type relationship with measured diffusivity of nitrogen for α-ferrite at different temperatures.

Phosphatidylserine improves axonal transport by inhibition of HDAC and has potential in treatment of neurodegenerative diseases

Familial dysautonomia （FD） is a rare children neurodegenerative disease caused due to a point mutation in the IKBKAP gene that results in decreased IKK complex-associated protein （IKAP） protein production. The disease affects mostly the dorsal root ganglion （DRG） and the sympathetic ganglion. Recently, we found that the molecular mechanisms underlying neurodegeneration in FD patients are defects in axonal transport of nerve growth factors and microtubule stability in the DRG. Neurons are highly polarized cells with very long axons. In order to survive and maintain proper function, neurons depend on transport of proteins and other cellular components from the neuronal body along the axons. We further demonstrated that IKAP is necessary for axon maintenance and showed that phosphatidylserine acts as an HDAC6 inhib- itor to rescue neuronal function in FD cells. In this review, we will highlight our latest research findings.

Climatic and anthropogenic impacts on the decrease of sediment discharge to the Mediterranean coast from the largest river of Maghreb

The current study focuses on the issue of the decrease in sediment discharge to the Mediterranean Sea by the largest river in Algeria, the Wadi Cheliff(i.e. Cheliff River). This study clarifies the effect of climatic and anthropogenic factors on the changes in the sedimentary dynamics of the Cheliff River discharging to the sea. The data used(rainfall, water discharge, and sediment discharge) concern the Sidi Bel Attar gaging station on the Cheliff River, only 18 km from discharge to the Mediterranean Sea. A power-type statistical regression model was used to fill the 74.2% gap in Suspended Solids(SS) measurements in the establishment of a SS database for the period 1951 e2012. The study results show that the transport of suspended sediment discharged to the sea is about 487 t/km2/yr. Statistical tests of breaks highlight that rainfall decreased by 26% and that water and sediment input to the sea declined from 74% to 63% for the period of 1981 e2012. The correlation analysis shows a decrease in the contribution of rainfall on sediment and water input at the outlet equal to 77.9% and 77.8%, respectively, during the period of 1980 e2012, compared to the period of 1968 e1980. However, the double mass method reflects the contribution of factors other than rainfall to the decrease of sediment input to the sea. These other factors mainly include large dams, which intercepted about 71% of the total volume of sediment discharged to the sea during the period of 1968 e2010. In addition the contribution of large dams to the reduction of sedimentary input to the sea is more important than that of the decrease in rainfall. The management of large dams also contributes to the increase in the sediment deficit to the sea through the prioritization of interception of sediment at the expense of releases, for socio-economic purposes, 68.4% of the Cheliff River discharge is diverted for human use. Attitude toward preventive counseling and healthy practices among medical students at a Colombian university

Unhealthy behaviors of medical students influence their attitudes toward preventive counseling. The burden of chronic diseases is continually growing in developing countries, emphasizing the need for the increased role of general practitioners in preventive counseling. The objective of this study was to describe the effect of medical training on the risk profiles and attitudes of medical students toward preventive counseling in a Colombian university. Students in their first and fifth years of training were surveyed using the ＂Healthy Doctor = Healthy Patient＂ questionnaire to assess counseling attitudes; personal practices, such as risky drinking, smoking, inadequate nutrition, and non-compliance with physical activity recommendations; and the university environment. The association among these components was estimated, as well as the effect of the university environment. Risky drinking and smoking decreased from first to fifth year （59.3% vs. 37.1%, P = 0.021 and 31.5~ vs. 25.9%, P = 0.51, respectively）, whereas inadequate nutrition and non-compliance with physical activity recommendations increased. Physical activity （PA） was associated with positive counseling attitude （ORs： nutrition 7.6; alcohol 5.2; PA 10.6）. Areas governed by institutional policies that are emphasized in the curriculum positively affected student practices. PA promoted preventive counseling and healthy lifestyles most effectively. Universities should therefore strengthen their preventive medicine curricula and modify social determinants.

We prove the Hyers-Ulam stability of linear N-isometries in linear N-normed Banach mod- ules over a unital C^＊-algebra. The main purpose of this paper is to investigate N-isometric C^＊-algebra isomorphisms between linear N-normed C^＊-algebras, N-isometric Poisson C^＊-algebra isomorphisms between linear N-normed Poisson C^＊-algebras, N-isometric Lie C^＊-algebra isomorphisms between linear N-normed Lie C^＊-algebras, N-isometric Poisson JC^＊-algebra isomorphisms between linear N-normed Poisson JC^＊-algebras, and N-isometric Lie JC^＊-algebra isomorphisms between linear N-normed Lie JC^＊-algebras. Moreover, we prove the Hyers- Ulam stability of t：heir N-isometric homomorphisms.

Synthesis and electrochemical properties of dual doped spinels LiNi_xAl_yMn_(2-x-y)O_4 via facile novel chelated sol–gel method as possible cathode material for lithium rechargeable batteries

LiMnOand LiNiAlyMnO（x= 0.50;y = 0.05-0.50） powders have been synthesized via facile solgel method using Behenic acid as active cheiating *** synthesized samples are subjected to physical characterizations such as thermo gravimetric analysis（TG/DTA）,X-ray diffraction（XRD）,Fourier transform infrared spectroscopy（FT-IR）,field-emission scanning electron microscopy（FESEM）,transmission electron microscopy（TEM） and electrochemical studies viz.,galvanostatic cycling properties,electrochemical impedance spectroscopy（EIS） and differential capacity curves（dQ/dE）.Finger print XRD patterns of LiMnOand LiNiAlMnOfortify the high degree of crystallinity with better phase *** images of the undoped pristine spinel illustrate uniform spherical grains surface morphology with an average particle size of 0.5 μm while Ni doped particles depict the spherical grains growth（50nm） with ice-cube surface *** images of the spinel LiMnOshows the uniform spherical morphology with particle size of（100 nm） while low level of Al-doping spinel（LiNio.5Alo.05Mn1.45O4） displaying cloudy particles with agglomerated particles of（50nm）.The LiMnOsamples calcined at 850℃ deliver the discharge capacity of 130 mAh/g in the first cycle corresponds to 94%coiumbic efficiency with capacity fade of 1.5 mAh/g/cycle over the investigated 10 *** all four dopant compositions investigated,LiNiAlMnOdelivers the maximum discharge capacity of 126 mAh/g during the first cycle and shows the stable cycling performance with low capacity fade of 1 mAh/g/cycle（capacity retention of 92%） over the investigated 10 *** impedance studies of spinel LiMnOand LiNiAlMnOdepict the high and low real polarization of 1562 and 1100 Ω.

Changes in coronary bifurcations after stent placement in the main vessel and balloon opening of stent cells:theory and practical verification on a bench-test model

Objective To describe changes that occur in stent morphology and structure after its implantation in coronary *** branch (SB) compromise after stenting of main vessel in coronary bifurcation is a major intraprocedural problem and for the long term,as a place of *** We created an elastic wall model (parent vessel diameter 3.5mm,daughter branches 3.5mm and 2.75mm)with 30,45 and 60 degree distal angulation between *** stent implantation,struts to the side branch were opened with 2.0mm and consequently 3.0mm diameter *** balloon redilatations and kissing balloon inflations (KBI) were *** stages of the procedure were photographed with magnification up to 100 *** We found that the leading mechanism for side branch compromise was carina displacement,and discovered theoretical description for expected ostial stenosis *** on our model we found that displacement of bifurcation flow divider cause SB stenosis with almost perfect coincidence with our theoretical *** of stent cells through the proximal and distal stent struts always increased interslrut distance,but never achieved good apposition to the *** diameter increase didn't give proportional enlargement in stent cell *** leads to some small better stent positioning,correcting main vessel strut dislodgment from wall,but never gave full strut-wall *** between struts and wall was minimal only when the stent cell perfectly faced ostium of *** was also our observation that the shape of ostium of SB becomed eUiptically-bean shaped after stent implantation and generally kept that shape during consequent stages of *** diameter and area stenosis were perfectly fitted and theoretically predicted from our concept Conclusion We have described stent-wall deformations in stent-balloon technique for treatment of coronary bifurcation demonstrating carina displacement as possibly main me
