Background:Existing reporting guidelines pay insufficient attention to the detail and comprehensiveness reporting of surgical *** Surgical techniqUe rePorting chEcklist and standaRds(SUPER)aims to address this gap by defining reporting standards for surgical *** SUPER guideline intends to apply to articles that encompass surgical technique in any study design,surgical discipline,and stage of surgical ***:following the EQUATOR(Enhancing the QUAlity and Transparency Of health Research)Network approach,16 surgeons,journal editors,and methodologists reviewed existing reporting guidelines relating to surgical technique,reviewed papers from 15 top journals,and brainstormed to draft initial items for the *** initial items were revised through a three-round Delphi survey from 21 multidisciplinary Delphi panel experts from 13 countries and *** final SUPER items were formed after an online consensus meeting to resolve disagreements and a three-round wording refinement by all 16 SUPER working group members and five SUPER ***:The SUPER reporting guideline includes 22 items that are considered essential for good and informative surgical technique *** items are divided into six sections:background,rationale,and objectives(items 1 to 5);preoperative preparations and requirements(items 6 to 9);surgical technique details(items 10 to 15);postoperative considerations and tasks(items 16 to 19);summary and prospect(items 20 and 21);and other information(item 22).Conclusions:The SUPER reporting guideline has the potential to guide detailed,comprehensive,and transparent surgical technique reporting for *** may also assist journal editors,peer reviewers,systematic reviewers,and guideline developers in the evaluation of surgical technique papers and help practitioners to better understand and reproduce surgical technique.

Does progesterone show neuroprotective effects on traumatic brain injury through increasing phosphorylation of Akt in the hippocampus?

There are currently no federally approved neuroprotective agents to treat traumatic brain injury. Progesterone, a hydrophobic steroid hormone, has been shown in recent studies to exhibit neu-roprotective effects in controlled cortical impact rat models. Akt is a protein kinase known to play a role in cell signaling pathways that reduce edema, inlfammation, apoptosis, and promote cell growth in the brain. This study aims to determine if progesterone modulates the phosphor-ylation of Aktvia its threonine 308 phosphorylation site. Phosphorylation at the threonine 308 site is one of several sites responsible for activating Akt and enabling the protein kinase to carry out its neuroprotective effects. To assess the effects of progesterone on Akt phosphorylation, C57BL/6 mice were treated with progesterone （8 mg/kg） at 1 （intraperitonally）, 6, 24, and 48 hours （subcutaneously） post closed-skull traumatic brain injury. The hippocampus was harvest-ed at 72 hours post injury and prepared for western blot analysis. Traumatic brain injury caused a signiifcant decrease in Akt phosphorylation compared to sham operation. However, mice treat-ed with progesterone following traumatic brain injury had an increase in phosphorylation of Akt compared to traumatic brain injury vehicle. Our ifndings suggest that progesterone is a viable treatment option for activating neuroprotective pathways after traumatic brain injury.

Amyotrophic lateral sclerosis（ALS） is a fatal neurodegenerative disease that causes progressive muscular atrophy and death within 3–5 years after its *** the significant advances in knowledge of ALS pathology,no effective treatment is ***,it is imperative to search for new alternatives to treat *** therapy,especially using bone-marrow cells,has showed to be very useful to protect the neural tissue in different brain disease or traumatic *** ALS,most published results show beneficial effects of the use bone marrow cells,especially mesenchymal stromal ***,until now,the best outcome extends animal＇s lifespan by only a few *** is essential to continue the search for a really effective therapy,testing different cells,routes and time-windows of *** the mechanisms that initiate and spread the degenerative process is also important to find out an effective ***,we discussed here some progresses that have been made using bone-marrow cell therapy as a therapeutic tool for ALS.

149Pm, 166Ho, 161Tb and 177Lu conjugated to chemical agents (monoclonal antibodies, polypeptide, etc.) have the appropriate decay characteristics for imaging and therapeutic studies and consequently the potential to be useful in radiotherapy and diagnosis. These carrier-free radioisotopes can be produced by neutron irradiation of a lanthanide target followed by β-?decay, and a posterior radiochemical separation of the daughter radionuclide from macro-amounts of the parent target. In order to produce carrier free 149Pm, 161Tb, 166Ho and 177Lu for radiotherapy, with a radionuclide purity of more than 99.9%, a device production was developed based on separation of Nd/Pm, Gb/Tb, Dy/Ho and Yb/Lu by extraction chromatography.

The clinical decision to control risk factors for cardiovascular disease （CVD） in the elderly takes the followings into consideration：（1） the elderly life expectancy; （2） the elderly biological age and functional capacity; （3） the role of cardiovascular disease in the elderly group; （4） the prevalence of risk factors in the elderly; and （5） The effectiveness of treatment of risk factors in the elderly. A large number of studies showed the efficacy of secondary and primary prevention of dyslipidemia in the elderly. However, the only trial that included patients over 80 years was the Heart Protection Study （HPS）. Statins are considered the first line therapy for lowering low-density lipoprotein cholesterol （LDL-C）. Because lifestyle changes are very difficult to achieve, doctors in general tend to prescribe many drugs to control cardiovascular risk factors. However, healthy food consumption remains a cornerstone in primary and secondary cardiovascular prevention and should be implemented by everyone.

Mirizzi syndrome:History,current knowledge and proposal of a simplified classification

Chronic complications of symptomatic gallstone disease, such as Mirizzi syndrome, are rare in Western developed countries with an incidence of less than 1% a year. The importance and implications of this condition are related to their associated and potentially serious surgical complications such as bile duct injury, and to its modern management when encountered during laparoscopic cholecystectomy. The pathophysiological process leading to the subtypes of Mirizzi syndrome has been explained by means of a pressure ulcer caused by an impacted gallstone at the gallbladder infundibulum, leading to an inflammatory response causing first external obstruction of the bile duct, and eventually eroding into the bile duct and evolving to a cholecystocholedochal or cholecystohepatic fistula. This article reviews the life of Pablo Luis Mirizzi, describes the earlier and later descriptions of Mirizzi syndrome, discusses the pathophysiological process leading to the development of these uncommon fistulas, reviews the current diagnostic modalities and surgical approaches and finally proposes a simplified classification for Mirizzi syndrome intended to standardize the reports on this condition and to eventually develop a consensual surgical approach to this unexpected and seriously dangerous condition.

Tree structure and diversity of lowland Atlantic forest fragments：comparison of disturbed and undisturbed remnants

This study describes the tree community structure of three moist lowland Atlantic forest fragments in Rio de Janeiro State, southeastern Brazil. Two fragments were disturbed and an undisturbed one was used as reference. Our hypothesis was that disturbed fragments show distinct structural patterns in comparison with undisturbed stands due to past disturbance practices and forest fragmentation. four 100 ×5 m sampling plots were demarcated in each fragment and all live and dead trees with DBH C 5 cm were located, measured and identified. The results supported our hypothesis, due to the high values found for standing dead trees, an increase of dominance of a few pioneer species, lower values of large trees and species richness in disturbed fragments in comparison with the undisturbed one. The advanced fragmentation process in the Southern Brazilian lowland areas and the high species richness in undisturbed areas highlight these forest fragments as priority areas for conservation and management.
