AIM:To compare the safety and effectiveness of eyelid treatment with the ILux®-MGD Treatment System in one session versus five sessions of mechanical meibomian gland expression(MMGE)in patients with moderate to severe meibomian gland dysfunction(MGD).METHODS:A prospective,randomized,open-label,and controlled clinical trial that compared one session of the ILux®MGD Treatment System versus five sessions of MMGE in both eyes of 130 patients aged≥18y with Ocular Surface Disease Index(OSDI)scores≥13,total meibomian gland scores(MGS)of<15 in the lower eyelid of each eye,and non-invasive tear break-up time(NI-TBUT)<10s,who were randomized 1:1 to ILux®or MMGE.RESULTS:The mean age was 58±17.49y.Baseline total MGS scores in both treatment groups were comparable.During follow-up,there were significant differences in total MGS and per sector with P<0.001.Multivariate analysis was performed using generalized estimating equations corresponding to the generalized linear model for repeated means to determine the treatment relationship with total MGS,NIBUT,and OSDI.There was a significant difference between ILux®and MMGE(P<0.001)at follow-up from the first to the twelfth month in MGS,NI-BUT,and OSDI scores.No adverse events were reported.CONCLUSION:ILux®treatment compared to MMGE significantly improves symptoms and signs in patients with moderate to severe MGD for one year without adverse events.

BACKGROUND Pancreatic neuroendocrine tumors(PNETs)are relatively rare but rank as the second most common pancreatic neoplasm.They can be functional,causing early metabolic disturbances due to hormone secretion,or non-functional and diagnosed later based on tumor size-related symptoms.Recent diagnoses of PNETs under 2 cm in size have sparked debates about their management;some practitioners advocate for surgical removal and others suggest observation due to the tumors’lower potential for malignancy.However,it is unclear whether managing these small tumors expectantly is truly safe.AIM To evaluate poor prognostic factors in PNETs based on tumor size(>2 cm or<2 cm)in surgically treated patients.METHODS This cohort study included 64 patients with PNETs who underwent surgical resection between 2006 and 2019 at a high-complexity reference hospital in Medellín,Colombia.To assess patient survival,quarterly follow-ups were conducted during the first year after surgery,followed by semi-annual con-sultations at the hospital's hepatobiliary surgery department.Qualitative variables were described using absolute and relative frequencies,and quantitative variables were expressed using measures of central tendency and their corresponding measures of dispersion.RESULTS The presence of lymph node involvement,neural involvement,and lymphovascular invasion were all associated with an increased risk of mortality,with hazard ratios of 5.68(95%CI:1.26–25.61,P=0.024),6.44(95%CI:1.43–28.93,P=0.015),and 24.87(95%CI:2.98–207.19,P=0.003),respectively.Neural involvement and lymphovascular invasion were present in tumors smaller than 2 cm in diameter and those larger than 2 cm in diameter.The recurrence rates between the two tumor groups were furthermore similar:18.2%for tumors smaller than 2 cm and 21.4%for tumors larger than 2 cm.Patient survival was additionally comparable between the two tumor groups.CONCLUSION Tumor size does not dictate prognosis;lymph node and lymphovascular involvement affect mortality,which high-lights that histopath

Nitrate is the primary water-soluble macronutrient essential for plant growth that is converted from excess fish feeds,fish effluents,and degrading biomaterials on the aquaponic pond floor,and when aquacultural malpractices occur,large amounts of it retain in the water system causing increase rate in eutrophication and toxifies fish and aquaculture plants.Recent nitrate sensor prototypes still require performing the additional steps of water sample deionization and dilution and were constructed with expensive materials.In response to the challenge of sensor enhancement and aquaponic water quality monitoring,this study developed sensitive,repeatable,and reproducible screen-printed graphite electrodes on polyvinyl chloride and parchment paper substrates with silver as electrode material and 60:40 graphite powder:nail polish formulated conductive ink for electrical traces,integrated with 9-gene genetic expression model as a function of peak anodic current and electrochemical test time for nitrate concentration prediction that is embedded into low-power Arduino ESP32 for in situ nitrate sensing in aquaponic pond water.Five SPE electrical traces were designed on the two types of substrates.Scanning electron microscopy with energy dispersive X-ray confirmed the electrode surface morphology.Electrochemical cyclic voltammetry using 10 to 100 mg/L KNO3 and water from three-depth regions of the actual pond established the electrochemical test time(10.5 s)and electrode potential(0.135 V)protocol necessary to produce peak current that corresponds to the strength of nitrate ions during redox.The findings from in situ testing revealed that the proposed sensors have strong linear predictions(R2=0.968 MSE=1.659 for nSPEv and R2=0.966 MSE=4.697 for nSPEp)in the range of 10 to 100 mg/L and best detection limit of 3.15μg/L,which are comparable to other sensors of more complex construction.The developed three-electrode electrochemical nitrate sensor confirms that it is reliable for both biosensing in controlled solutions and in situ

BACKGROUND Acid-base imbalance has been poorly described in patients with coronavirus disease 2019(COVID-19).Study by the quantitative acid-base approach may be able to account for minor changes in ion distribution that may have been over-looked using traditional acid-base analysis techniques.In a cohort of critically ill COVID-19 patients,we looked for an association between metabolic acidosis surrogates and worse clinical outcomes,such as mortality,renal dialysis,and length of hospital stay.AIM To describe the acid-base disorders of critically ill COVID-19 patients using Stewart’s approach,associating its variables with poor outcomes.METHODS This study pertained to a retrospective cohort comprised of adult patients who experienced an intensive care unit stay exceeding 4 days and who were diagnosed with severe acute respiratory syndrome coronavirus 2 infection through a positive polymerase chain reaction analysis of a nasal swab and typical pulmonary involvement observed in chest computed tomography scan.Laboratory and clinical data were obtained from electronic records.Categorical variables were compared using Fisher’s exact test.Continuous data were presented as median and interquartile range.The Mann-Whitney U test was used for comparisons.RESULTS In total,211 patients were analyzed.The mortality rate was 13.7%.Overall,149 patients(70.6%)presented with alkalosis,28 patients(13.3%)had acidosis,and the remaining 34 patients(16.2%)had a normal arterial pondus hydrogenii.Of those presenting with acidosis,most had a low apparent strong ion difference(SID)(20 patients,9.5%).Within the group with alkalosis,128 patients(61.0%)had respiratory origin.The non-survivors were older,had more comorbidities,and had higher Charlson’s and simplified acute physiology score 3.We did not find severe acid-base imbalance in this population.The analyzed Stewart’s variables(effective SID,apparent SID,and strong ion gap and the effect of albumin,lactate,phosphorus,and chloride)were not different between the groups.CONCLUSION Alkalemia

The intestinal microbiota has gained increased attention from researchers within the swine industry due to its role in promoting intestinal maturation,immune system modulation,and consequently the enhancement of the health and growth performance of the host.This review aimed to provide updated scientific information on the interaction among intestinal microbiota,dietary components,and intestinal health of pigs.The small intestine is a key site to evaluate the interaction of the microbiota,diet,and host because it is the main site for digestion and absorption of nutrients and plays an important role within the immune system.The diet and its associated components such as feed additives are the main factors affecting the microbial composition and is central in stimulating a beneficial population of microbiota.The microbiotaehost interaction modulates the immune system,and,concurrently,the immune system helps to modulate the microbiota composition.The direct interaction between the microbiota and the host is an indication that the mucosa-associated microbiota can be more effective in evaluating its effect on health parameters.It was demonstrated that the mucosa-associated microbiota should be evaluated when analyzing the interaction among diets,microbiota,and health.In addition,supplementation of feed additives aimed to promote the intestinal health of pigs should consider their roles in the modulation of mucosa-associated microbiota as biomarkers to predict the response of growth performance to dietary interventions.

This study investigated the effects of using soy protein concentrate (SPC) to replace animal protein supplements on mucosa-associated microbiota,intestinal health,and growth performance of nursery pigs.Fifty-six newly weaned pigs (BW=6.4±0.6 kg) were allotted to 5 treatments in a randomized complete block design.Pigs were fed for 35 d in 3 phases (P;1,2,3) for 10,12,13 d,respectively.Dietary treatments were:(1) basal diet with fish meal (P1:4%,P2:2%,and P3:1%),poultry meal (P1:10%,P2:8%,and P3:4%),and blood plasma (P1:4%,P2:2%,and P3:1%),where SPC replacing none (NC);(2) basal diet with SPC replacing fish meal (RFM);(3) basal diet with SPC replacing poultry meal (RPM);(4) basal diet with SPC replacing blood plasma (RBP);and (5) basal diet with SPC replacing all animal protein supplements (PC).Growth performance was recorded for each phase.Pigs were euthanized on d 35 to collect jejunal mucosa and tissue to evaluate intestinal health and microbiota,and ileal digesta to measure apparent ileal digestibility (AID) of nutrients.Data were analyzed using the MIXED procedure of SAS.Overall,RFM,RPM,and RBP did not affect growth performance,whereas PC decreased (P<0.05) ADG and ADFI.The RPM increased (P<0.05) Prevotella stercorea and decreased (P<0.05) Helicobacter rappini.The PC decreased (P<0.05) H.rappini,whilst increasing (P<0.05) Prevotella copri,Propionibacterium acnes,and Pelomonas aquatica.The RFM tended to increase (P=0.096) immunoglobulin A in the jejunum.The PC tended to decrease (P=0.078) jejunal crypt cell proliferation.There were no differences in the villus height,AID of nutrients,intestinal inflammation,and intestinal oxidative stress among treatments.In conclusion,SPC can replace fish meal,poultry meal,or blood plasma individually without affecting growth performance and intestinal health,and AID of nutrients of nursery pigs.Particularly SPC replacing poultry meal benefitted intestinal health by reducing H.rappini and increasing P.stercorea.However,SPC replacing all three animal protein supplements reduced

Due to their intense electrical activity,neurons have high energy demands.This requirement makes them particularly sensitive to mitochondrial dysfunction.Like all eukaryotic cells,neurons have intrinsic mechanisms to mitigate the impact of mitochondrial dysfunction and its consequent production of toxic substances.Among such(neuro)protective mechanisms,mitochondrial autophagy(mitophagy)is responsible for the removal of dysfunctional mitochondria.Pathological inhibition of mitophagy,together with insufficient mitochondrial activity,results in a shortage of adenosine triphosphate(ATP)and the accumulation of reactive oxygen species(ROS)(Simmons et al.,2020).

Axonal growth inhibitors are released during traumatic injuries to the adult mammalian central nervous system, including after spinal cord injury. These molecules accumulate at the injury site and form a highly inhibitory environment for axonal regeneration. Among these inhibitory molecules, myelinassociated inhibitors, including neurite outgrowth inhibitor A, oligodendrocyte myelin glycoprotein, myelin-associated glycoprotein, chondroitin sulfate proteoglycans and repulsive guidance molecule A are of particular importance. Due to their inhibitory nature, they represent exciting molecular targets to study axonal inhibition and regeneration after central injuries. These molecules are mainly produced by neurons, oligodendrocytes, and astrocytes within the scar and in its immediate vicinity. They exert their effects by binding to specific receptors, localized in the membranes of neurons. Receptors for these inhibitory cues include Nogo receptor 1, leucine-rich repeat, and Ig domain containing 1 and p75 neurotrophin receptor/tumor necrosis factor receptor superfamily member 19(that form a receptor complex that binds all myelin-associated inhibitors), and also paired immunoglobulin-like receptor B. Chondroitin sulfate proteoglycans and repulsive guidance molecule A bind to Nogo receptor 1, Nogo receptor 3, receptor protein tyrosine phosphatase σ and leucocyte common antigen related phosphatase, and neogenin, respectively. Once activated, these receptors initiate downstream signaling pathways, the most common amongst them being the Rho A/ROCK signaling pathway. These signaling cascades result in actin depolymerization, neurite outgrowth inhibition, and failure to regenerate after spinal cord injury. Currently, there are no approved pharmacological treatments to overcome spinal cord injuries other than physical rehabilitation and management of the array of symptoms brought on by spinal cord injuries. Background Soy protein supplements,with high crude protein and less antinutritional factors,are produced from soybean meal by different processes.This study evaluated the comparative effects of various soy protein supplements replacing animal protein supplements in feeds on the intestinal immune status,intestinal oxidative stress,mucosaassociated microbiota,and growth performance of nursery pigs.Methods Sixty nursery pigs(6.6±0.5 kg BW)were allotted to five treatments in a randomized complete block design with initial BW and sex as blocks.Pigs were fed for 39 d in 3 phases(P1,P2,and P3).Treatments were:Control(CON),basal diet with fish meal 4%,2%,and 1%,poultry meal 10%,8%,and 4%,and blood plasma 4%,2%,and 1%for P1,P2,and P3,respectively;basal diet with soy protein concentrate(SPC),enzyme-treated soybean meal(ESB),fermented soybean meal with Lactobacillus(FSBL),and fermented soybean meal with Bacillus(FSBB),replacing 1/3,2/3,and 3/3 of animal protein supplements for P1,P2,and P3,respectively.Data were analyzed using the MIXED procedure in SAS 9.4.Results The SPC did not affect the BW,ADG,and G:F,whereas it tended to reduce(P=0.094)the ADFI and tended to increase(P=0.091)crypt cell proliferation.The ESM did not affect BW,ADG,ADFI,and G:F,whereas tended to decrease(P=0.098)protein carbonyl in jejunal mucosa.The FSBL decreased(P<0.05)BW and ADG,increased(P<0.05)TNF-α,and Klebsiella and tended to increase MDA(P=0.065)and IgG(P=0.089)in jejunal mucosa.The FSBB tended to increase(P=0.073)TNF-α,increased(P<0.05)Clostridium and decreased(P<0.05)Achromobacter and alpha diversity of microbiota in jejunal mucosa.Conclusions Soy protein concentrate,enzyme-treated soybean meal,and fermented soybean meal with Bacillus could reduce the use of animal protein supplements up to 33%until 7 kg body weight,up to 67%from 7 to 11 kg body weight,and entirely from 11 kg body weight without affecting the intestinal health and the growth performance of nursery pigs.Fermented soybean meal with Lactobacillus,however,increased the immune reac
