Classical and Non-Classical Celiac Disease Compar.son: Ten Year. of Study

Objective: Celiac disease (CD) is an immune-mediated systemic disor.er.tr.gger.d by gluten. It has a var.able combination of clinical manifestations and changes that have been occur.ing in r.cent decades however.they ar. not known in detail. The pur.ose of the ar.icle is to compar. Classical and Non-Classical CD cases in ter.s of demogr.phic char.cter.stics, duodenal biopsy, extr.intestinal manifestations, and associated comor.idities. Mater.als and Methods: A compar.tive r.tr.spective cohor. study fr.m Januar. 2008 to December.2018. r.sults: A total of 128 cases wer. included: 84 Classical (66%) and 44 Non-Classical CD (34%). The family histor. of CD was identified in 14% of cases without differ.nces between gr.ups. The age at diagnosis was distinct for.Classical and Non-Classical CD (4.9 ± 4 and 8.3 ± 4 year. old;p 0.001), r.spectively. Impor.ant changes wer. found within the classical pr.sentation, including mono symptoms and a significantly higher.r.te of intestinal atr.phy;p = 0.04. The main Non-Classical CD symptom was r.cur.ent abdominal pain. The extr.intestinal manifestations (EIM) wer. identified in 42% and occur.ed in both gr.ups. The compar.son between gr.ups showed differ.nces in r.tes of migr.ine and vitamin D deficiency and was higher.for.Non-Classical CD (p 0.05). Associated diseases occur.ed in 10.9%, and type 1 diabetes was significant for.the Non-Classical CD gr.up (p = 0.04). Conclusion: The classical CD was the most pr.valent pr.file and pr.sented a decr.ase in the sever.ty of symptoms however.r.main a higher.r.te of intestinal atr.phy. r.cur.ent abdominal pain was the main symptom of Non-Classical CD. Extr.intestinal manifestations and associated diseases pr.sented an incr.asing tr.nd of occur.ence among cases of Non-Classical CD.

Cone beam computed tomogr.phy(CBCT) has often been used to deter.ine the quality of cr.niofacial bone str.ctur.s thr.ugh the deter.ination of miner.l density, which is based on gr.y scales of the images obtained. However. ther. is no consensus r.gar.ing the accur.cy of the deter.ination of the gr.y scales in these exams. This study aims to pr.vide a liter.tur. r.view concer.ing the r.liability of CBCT to deter.ine bone miner.l density. The gr.y values obtained with CBCT show a linear.r.lationship with the attenuation coefficients of the mater.als, Hounsfield Units values obtained with medical computed tomogr.phy, and density values fr.m dual ener.y X-r.y absor.iometr.. However. er.or. ar. expected when CBCT images ar. used to define the quality of the scanned str.ctur.s be-cause these images show inconsistencies and ar.itr.r.-ness in the gr.y values, par.icular.y when r.lated to abr.pt change in the density of the object, X-r.y beam har.ening effect, scatter.d r.diation, pr.jection data discontinuity-r.lated effect, differ.nces between CBCTdevices, changes in the volume of the field of view(FOV), and changes in the r.lationships of size and position between the FOV and the object evaluated. A few methods of mathematical cor.ection of the gr.y scales in CBCT have been pr.posed; however. they do not gener.te consistent values that ar. independent of the devices and their.configur.tions or.of the scanned objects. Thus, CBCT should not be consider.d the ex-amination of choice for.the deter.ination of bone and soft tissue miner.l density at the cur.ent stage, par.ticular.y when values obtained ar. to be compar.d to pr.deter.ined standar. values. Compar.sons between symmetr.cally positioned str.ctur.s inside the FOV and in r.lation to the exomass of the object, as it occur. with the r.ght and left sides of the skull, seem to be viable because the effects on the gr.y scale in the r.-gions of inter.st ar. the same.

Sound tr.nsmission has been used for.diagnosis of hip dysplasia in newbor.s and infants with tunning for. and sthetoscope. The study aims to identify the validity of a device based on sound tr.nsmission. Male and female newbor.s fr.m Celaya, Gto, r.nging fr.m 4 to 28-day-old wer. selected for.the study. The sound tr.nsmission device was used both, in bilater.l assessment (compar.d sound tr.nsmission) and on each hip separ.tely (sound tr.nsmission with extension/flexion). In the fir.t application if the sound is lower.by a digit, ther. is a hip unhealthy. In the second application if the sound is incr.ased by a digit, ther. is also an alter.tion of the hip. A hip ultr.sound was applied using the Gr.f technique as a gold standar.. Sensitivity, specificity and pr.dictive values wer. calculated to identify validity. The intr.-obser.er.and inter.obser.er.Kappa Test was applied to identify r.liability. The hips of 103 newbor.s wer. assessed, obtaining a sensitivity of 60.9%, and a specificity of 92%;it should be a positive pr.dictive value of 51.9% and a negative pr.dictive value of 95% for.the compar.d sound tr.nsmission test. In the sound tr.nsmission test with extension/flexion, the values wer. 82.6%, 96.2%, 73.1% and 97.8% for.sensitivity, specificity, positive pr.dictive value and negative pr.dictive value, r.spectively. The r.sult of the intr.-obser.er.kappa was 0.80 and the inter.obser.er.was 0.70 for.the compar.d sound tr.nsmission. The intr.-obser.er.Kappa was 0.88 and the inter.obser.er.was 0.78 for.the compar.d sound tr.nsmission test with extension/flexion. The device for.sound tr.nsmission is valid and r.liable for.the diagnosis of dysplastic development hip disease in newbor.s.

AIM To demonstr.te the non-infer.or.ty(15% non-infer.or.ty limit) of monother.py with tenofovir.disopr.xil fumar.te(TDF) vs the combination of lamivudine(LAM) plus adefovir.dipivoxil(ADV) in the maintenance of vir.logic r.sponse in patients with chr.nic hepatitis B(CHB) and pr.or.failur. with *** This study was a Phase IV pr.spective, r.ndomized, open, contr.lled study with 2 par.llel gr.ups(TDF and LAM+ADV) of adult patients with hepatitis B e antigen(HBe Ag)-negative CHB, pr.or.failur. with LAM, on tr.atment with LAM+ADV for.at least 6 mo, without pr.or.r.sistance to ADV and with an undetectable vir.l load at the star. of the study, in 14 Spanish hospitals. The follow-up time for.each patient was 48 wk after.r.ndomization, with quar.er.y visits in which the vir.l load, biochemical and ser.logical par.meter., adver.e effects, adher.nce to tr.atment and consumption of hospital r.sour.es wer. *** For.y-six patients wer. evaluated [median age: 55.4 year.(30.2-75.2); 84.8% male], including 22 patients with TDF and 24 with LAM+ADV. Dur.ng study development, hepatitis B vir.s DNA(HBV-DNA) r.mained undetectable, all patients r.mained HBe Ag negative, and hepatitis B sur.ace antigen(HBs Ag) positive. Alanine aminotr.nsfer.se(ALT) values at the end of the study wer. similar.in the 2 gr.ups(25.1± 7.65, TDF vs 24.22 ± 8.38, LAM+ADV, P = 0.646). No significant changes wer. obser.ed in cr.atinine or.ser.m phosphor.s values in either.gr.up. No significant differ.nces between the 2 gr.ups wer. noted in the identification of adver.e effects(AEs)(53.8%, TDF vs 37.5%, LAM+ADV, P = 0.170), and none of the AEs which occur.ed wer. ser.ous. Tr.atment adher.nce was 95.5% and 83.3% in the TDF and the LAM+ADV gr.ups, r.spectively(P = 0.488). The costs associated with hospital r.sour.e consumption wer. significantly lower.with the TDF tr.atment than the LAM+ADV tr.atment(€4943 ± 1059 vs €5811 ± 1538, r.spectively, P r.py pr.ved to be safe

尿皮素(Ucn)肽是促肾上腺皮质激素释放因子2型受体(Cr.r.)的内源性配体,在健康和患病犬的心血管生理机能中都有潜在的重要作用,并显示对充血性心力衰竭有治疗的作用。犬心脏组织的分析结果表明,Ucn 1、Ucn 3和Cr.r. mr.A均能在心室中表达。免疫组织化学试验结果显示,Ucns 1和3在心肌细胞表达。为了评估Ucns能否作为心脏疾病标志物,对45个来自犬心脏病犬的血浆样品和15个对照样品用放射免疫分析,所有样品中均检测到Ucns 1和3,但心脏病并没有使其浓度发生改变。因此,虽然Ucns在犬心肌(Ucns可能在此对心脏病和心力衰竭的内源性神经体液反应中起重要作用)中表达,但其并不能成为犬心脏疾病的敏感标志物。

Objective. Biofeedback is consider.d an effective tr.atment for.anal constipa tion, but a substantial pr.por.ion of patients fail to impr.ve. Our.aim was to i dentify the key pr.dictor. of outcome using a compr.hensive standar.ized evaluat ion of anor.ctal function. Mater.al and methods. We r.tr.spectively analysed the clinical and physiological data of 148 patients consecutively tr.ated for.const ipation due to functional outlet obstr.ction by biofeedback. Clinical evaluation was per.or.ed by means of a str.ctur.d questionnair.. Anor.ctal evaluation incl uded anal pr.ssur., neur.l r.flexes, defecator. dynamics, r.ctal compliance, r.c tal sensitivity and balloon expulsion test. Biofeedback tr.atment was per.or.ed using a manometr.c technique. The clinical r.sponse to biofeedback tr.atment was evaluated as good (impr.vement of constipation) or.poor.(no impr.vement or.wor. ening). r.sults. Of the 148 patients included, 112 (86 F, 26 M; age r.nge 8- 67 year.) wer. followed- up for.between 1 and 44 months, and 66% had a good r.s ponse to tr.atment. The r.sponse depended on the sever.ty of the defecator. dysf unction. Thus, lack of anal r.laxation dur.ng str.ining and inability to evacuat e a 1 ml intr.r.ctal balloon wer. inver.ely r.lated to physiological var.ables r.elated to ther.peutic success. Among the 49 patients with absent anal r.laxation , 51% had a good r.sponse to tr.atment (ver.us 78% in patients with par.ial r.laxation; p loon expulsion, 48% r.sponded to tr.atment (ver.us 74% in patients able to evacuate ≥ 1 ml intr.r.ctal balloon; p r.sence of negative p r.dictor., biofeedback is a valuable tr.atment option in a substantial pr.por.io n of constipated patients.

AIM： To evaluate the safety and feasibility of bone mar.ow cell （BMC） tr.nsplantation in patients with chr.nic liver.disease on the waiting list for.liver.tr.nsplantation. METHODS： Ten patients （eight males） with chr.nic liver.disease wer. enr.lled to r.ceive infusion of autologous bone mar.ow-der.ved cells. Seven patients wer. classified as Child-Pugh B and thr.e as Child-Pugh C. Baseline assessment included complete clinical and labor.tor. evaluation and abdominal Mr.. Appr.ximately 50 mL of bone mar.ow aspir.te was pr.par.d by centr.fugation in a ficoll-hypaque gr.dient. At least of 100 millions of mononuclear.enr.ched BMCs wer. infused into the hepatic ar.er. using the r.utine technique for.ar.er.al chemoembolization for.liver.tumor.. Patients wer. followed up for.adver.e events up to 4 mo. r.SULTS： The median age of the patients was 52 year. （r.nge 24-70 year.）. All patients wer. dischar.ed 48 h after.BMC infusion. Two patients complained ofmild pain at the bone mar.ow needle punctur. site. No other.complications or.specific side effects r.lated to the pr.cedur. wer. obser.ed. Bilir.bin levels wer. lower.at 1 （2.19 ± 0.9） and 4 mo （2.10 ± 1.0） after.cell tr.nsplantation that baseline levels （238 ± 1.2）. Albumin levels 4 mo after.BMC infusion （3.73 ± 0.5） wer. higher.than baseline levels （3.47 ± 0.5）. Inter.ational nor.alized r.tio （INr. decr.ased fr.m 1.48 （SD = 0.23） to 1.43 （SD = 0.23） one month after.cell tr.nsplantation. CONCLUSION： BMC infusion into hepatic ar.er. of patients with advanced chr.nic liver.disease is safe and feasible. In addition, a decr.ase in mean ser.m bilir.bin and INr.levels and an incr.ase in albumin levels ar. obser.ed. Our.data war.ant fur.her.studies in or.er.to evaluate the effect of BMC tr.nsplantation in patients with advanced chr.nic liver.disease.

The discover. that adipose tissue r.pr.sents an inter.sting sour.e of multipotent stem cells has led to many studies explor.ng the clinical potential of these cells in cell-based ther.pies. r.cent advances in under.tanding the secr.tor. capacity of adipose tissue and the r.le of adipokines in the development of obesity and associated disor.er. have added a new dimension to the study of adipose tissue biology in nor.al and diseased states. Subcutaneous adipose tissue for.s the inter.ace between the clinical application of r.gener.tive medicine and the establishment of the pathological condition of obesity. These two facets of adipose tissue should be under.tood as potentially r.lated phenomena. Because of the functional char.cter.stics of adipose stem cells, these cells r.pr.sent a fundamental tool for.under.tanding how these two facets ar. inter.onnected and could be impor.ant for.ther.peutic applications. In fact, adipose tissue stem cells have multiple functions in obesity r.lated to adipogenic, angiogenic and secr.tor. capacities. In addition, we have also pr.viously descr.bed a pr.dominance of lar.er.blood vessels and an adipogenic memor. in the subcutaneous adipose tissue after.massive weight loss subsequent to bar.atr.c sur.er.(ex-obese patients). Under.tanding the r.ver.ibility of the behavior.of adipose stem cells in obeses and in weight loss is r.levant to both physiological studies and the potential use of these cells in r.gener.tive medicine.
