An examination of predictive markers for successful sperm extraction procedures:a linear model and systematic review

The authors performed a comprehensive review of current literature to create a model comparing commonly evaluated variables in male factor infertility,for example,follicle-stimulating hormone(FSH),testicular volume(TV),and testosterone(T),to better predict sperm retrieval rate(SRR).Twenty-nine studies were included,9 with data on conventional testicular sperm extraction(cTESE)for a total of 1227 patients and 20 studies including data on microdissection testicular sperm extraction(mTESE)for a total of 4760 patients.A weighted-means value of SRR,FSH,T,and TV was created,and a weighted linear regression was then used to describe associations among SRR,type of procedure,FSH,T,and *** this study,weighted-means values demonstrated mTESE to be superior to cTESE with an SRR of 51.9%vs 40.1%.Multiple weighted linear regressions were created to describe associations among SRR,procedure type,FSH,T,and *** models showed that for every 1.19 mIU ml^(−1)increase in FSH,there would be a significant decrease in SRR by 1.0%.Seeking to create a more clinically relevant model,FSH values were then divided into normal,moderate elevation,and significant elevation categories(FSH20 mIU ml^(−1),respectively).For an index patient undergoing cTESE,the retrieval rates would be 57.1%,44.3%,and 31.2%for values normal,moderately elevated,and significantly elevated,*** conclusion,in a large meta-analysis,mTESE was shown to be more successful than cTESE for sperm *** has an inverse relationship to SRR in retrieval techniques and can alone be predictive of cTESE SRR.

Recently more information has emerged on possible adverse health effects associ-ated with some building and furnishing materials,leading to or initiating legisla-tive changes towards their reduction or elimination in many parts of the ***,more general knowledge of the health risks associated with building and furnishing materials could make a significant contribution to improvements in indoor air quality.A study was set up to evaluate the level of knowledge in the rel-evant literature and the general population(from New Zealand,United kingdom and the United States),and New Zealand architectural *** results for vinyl and linoleum are presented,these being two flooring materials very similar in terms of appearance and application,but different in terms of chemical content and possible impact on *** article indicates significant issues with the level of reported knowledge,with participants from the general population struggling to differentiate between vinyl and linoleum,and professionals reporting more preva-lent use of vinyl,which they rate as less healthy.

Anal cancer represents less than 1% of all new cancers diagnosed annually in the United States. Yet, despite the relative paucity of cases, the incidence of anal cancer has seen a steady about 2% rise each year over the last decade. As such, all healthcare providers need to be cognizant of the evaluation and treatment of anal squamous cell carcinoma. While chemoradiation remains the mainstay of therapy for most patients with anal cancer, surgery may still be required in recurrent, recalcitrant and palliative disease. In this manuscript, we will explore the diagnosis and management of squamous cell carcinoma of the anus.

We have examined ten human subjects with a previously developed instrument for near-infrared diffuse spectral imaging of the female *** instrument is based on a tandem,planar scan of two collinear optical fibers(one for illumination and one for collection)to image a gently compressed breast in a transmission *** optical data collection features a spatial sampling of 25 points/cm2 over the whole breast,and a spectral sampling of 2 points/nm in the 650-900nm wavelength *** the ten human subjects examined,eight are healthy subjects and two are cancer patients with unilateral invasive ductal carcinoma and ductal carcinoma in situ,*** each subject,we generate second-derivative images that identify a network of highly absorbing structures in the breast that we assign to blood vessels.A previously developed paired-wavelength spectral method assigns oxygenation values to the absorbing structures displayed in the second-derivative *** resulting oxygenation images feature average values over the whole breast that are significantly lower in cancerous breasts(69±14%,n=2)than in healthy breasts(85±7%,n=18)(p<0.01).Furthermore,in the two patients with breast cancer,the average oxygenation values in the cancerous regions are also significantly lower than in the remainder of the breast(invasive ductal carcinoma:49±11%vs 61±16%,p<0.01;ductal carcinoma in situ:58±8%vs 77±11%,p<0.001).

An efficient method to compute the permeability of disordered fibrous arrays is pro- posed. A stabilized mixed finite element method is used with an immersed domain ap- proach to represent the porous material at its microscopic scale. Therefore, the Stokes e- quations are solved in the whole domain （including solid part） using a penalization method. The accuracy is controlled by refining the mesh around the fluid-solid interface defined by a level-set function. Using homogenization techniques, the permeability of an RVE is obtained. Furthermore, a new method to generate disordered fibers in function of the porosity, Ф, and other microstructural parameters is proposed and a study of the effect of inter-fiber spacing on k：, the permeability tensor, is performed using parallel computation and over 460 simulations were carried RVEs consisting of over 555 fibers. Background Patients undergoing transcatheter aortic valve replacement(TAVR) are likely to be discharged to a location other than home. We aimed to determine the association between preoperative risk factors and non-home discharge after TAVR. Methods Patients discharged alive after TAVR at three centers were identified from a prospectively maintained database randomly divided into 80% derivation and 20% validation cohorts. Logistic regression models were fit to identify preoperative factors associated with non-home discharge in the derivation cohort. Multivariable models were developed and a nomogram based risk-scoring system was developed for use in preoperative counseling. Results Between June 2012 and December 2018, a total of 1,163 patients had TAVR at three centers. Thirty-seven patients who died before discharge were excluded. Of the remaining 1,126 patients(97%) who were discharged alive, the incidence of non-home discharge was 25.6%(n = 289). The patient population was randomly divided into the 80%(n = 900) derivation cohort and 20%(n = 226) validation cohort. Mean ± SD age of the study population was 83 ± 8 years. In multivariable analysis, factors that were significantly associated with non-home discharge were extreme age, female sex, higher STS scores, use of general anesthesia, elective procedures, chronic liver disease, non-transfemoral approach and postoperative complications. The unbiased estimate of the C-index was 0.81 and the model had excellent calibration. Conclusions One out of every four patients undergoing TAVR is discharged to a location other than home. Identification of preoperative factors associated with non-home discharge can assist patient counseling and postoperative disposition planning.
