AIM To assess the performance of BALAD, BALAD-2 and their component biomarkers in predicting outcome of hepatocellular carcinoma(HCC) patients after liver *** BALAD score and BALAD-2 class are derived from bilirubin, albumin, alpha-fetoprotein(AFP), Lens culinaris agglutinin-reactive AFP(AFP-L3), and des-gammacarboxyprothrombin(DCP). Pre-transplant AFP, AFP-L3 and DCP were measured in 113 patients transplanted for HCC from 2000 to 2008. Hazard ratios(HR) for recurrence and death were calculated. Univariate and multivariate regression analyses were conducted. C-statistics were used to compare biomarker-based to predictive models. REsULTs During a median follow-up of 12.2 years, 38 patients recurred and 87 died. The HRs for recurrence in patients with elevated AFP, AFP-L3, and DCP defined by BALAD cut-off values were 2.42(1.18-5.00), 1.86(0.98-3.52), and 2.83(1.42-5.61), respectively. For BALAD, the HRs for recurrence and death per unit increased score were 1.48(1.15-1.91) and 1.59(1.28-1.97). For BALAD-2, the HRs for recurrence and death per unit increased class were 1.45(1.06-1.98) and 1.38(1.09-1.76). For recurrence prediction, the combination of three biomarkers had the highest c-statistic of 0.66 vs. 0.64, 0.61, 0.53, and 0.53 for BALAD, BALAD-2, Milan, and UCsF, respectively. similarly, for death prediction, the combination of three biomarkers had the highest c-statistic of 0.66 vs 0.65,0.61, 0.52, and 0.50 for BALAD, BALAD-2, Milan, and UCsF. A new model combining biomarkers with tumor size at the time of transplant(s-LAD) demonstrated the highest predictive capability with c-statistics of 0.71 and 0.69 for recurrence and death. CONCLUsION BALAD and BALAD-2 are valid in transplant HCC patients, but less predictive than the three biomarkers in combination or the three biomarkers in combination with maximal tumor diameter(s-LAD).

本研究旨在研究山羊IDO1和MPO基因受到正选择的单核苷酸多态性位点(single Nucleotide Polymorphism, sNP)。在研究中,我们通过测定脱氧核糖核酸序列的方法得到了106只山羊中IDO1基因的多态性,最终在IDO1序列的外显子2-3和外显子10区域中共得到了5个突变位点。随后对IDO1基因外显子2-3(G290A;T309C)和外显子10(G290A;T309C)的进行基因分型,并通过聚合酶链式反应(Polymerase Chain Reaction, PCR)检测多态性位点。我们在MPO基因的外显子3-4中发现了一些突变,但是并未导致氨基酸变化。由于密码子的简并性,这些突变并未改变编码蛋白的氨基酸序列,最终成为对机体没有影响的沉默突变。系统发生分析表明,山羊IDO1和MPO基因与绵羊具有最高的相似性,高达98%和99%,与此同时山羊MPO基因与斑马鱼和小鼠仅有72%和83%的相似性。通过似然比检验,在使用朴素贝叶斯方法(Naive Empirical Bayes, NEB)分析正选择M8位点时候发现,IDO1和MPO表现出了较为丰富的参数信息。结果显示IDO1基因中有10个位点受到正选择(IDO1的LRTs分别为3.184和18.858,MPO为33.124)。这些位点包括350R、371N、398L、400T、302E、435T、231K、213I、210Y、18T (P>0.95);MPO有五个位点51E、652E(P>0.95)、325s、742s、765G(P>0.99)。由于只有大约15%的褪黑素是通过这条通路被分解的并且在这个通路中的相同位点上,这两个基因承担的任务相同。因此,我们推测IDO1和MPO基因在进化过程中受到正选择驱动。

Role of frailty and sarcopenia in predicting outcomes among patients undergoing gastrointestinal surgery

According to the United states census bureau 20% of Americans will be older than 65 years in 2030 and half of them will need an operation- equating to about 36 million older surgical patients. Older adults are prone to complications during gastrointestinal cancer treatment and therefore may need to undergo special pretreatment assessments that incorporate frailty and sarcopenia assessments. A focused, structured literature review on Pub Med and Google scholar was performed to identify primary research articles, review articles, as well as practice guidelines on frailty and sarcopenia among patients undergoing gastrointestinal surgery. The initial search identified 450 articles; after eliminating duplicates, reports that did not include surgical patients, case series, as well as case reports, 42 publications on the impact of frailty and/or sarcopenia on outcome of patients undergoing gastrointestinal surgery were included. Frailty is defined as a clinically recognizable state of increased vulnerability to physiologic stressors resulting from aging. Frailty is associated with a decline in physiologic reserve and function across multiple physiologic systems. sarcopenia is a syndrome characterized by progressive and generalized loss of skeletal muscle mass and strength. Unlike cachexia, which is typically associated with weight loss due to chemotherapy or a general malignancy-related cachexia syndrome, sarcopenia relates to muscle mass rather than simply weight. As such, while weight reflects nutritional status, sarcopenia- the loss of muscle mass- is a more accurate and quantitative global marker of frailty. While chronologic age is an important element in assessing a patient's peri-operative risk, physiologic age is a more important determinant of outcomes. Which strategy after first-line therapy in advanced colorectal cancer?

second-line therapy for advanced colorectal cancer is an integral part of the treatment strategy that needs to be set from the beginning for each patient, bearing in mind the expected toxicities of chosen treatments, the patient's clinical condition, comorbidities, preferences, the aims of the treatment and the molecular status. Furthermore, the distinction between lines of therapy is no longer absolute. The perspective of "continuum of care" includes switching chemotherapy prior to disease progression, maintenance therapy, drug "holidays" if needed, surgical resection of metastases in selected patients, and seems to allow a tailored treatment, in which patients are more likely to benefit from exposure to all active agents, which is known to correlate with overall survival. The scenario of second-line treatment has changed dramatically over the years and could currently benefit from several options including chemotherapy with a single agent or in combination and the addition of molecular-targeted agents developed in the last decade, such as epidermal growth factor receptor antibodies(cetuximab, panitumumab) and vascular endothelial growth factor-targeting agents(bevacizumab, aflibercept), with the possibility of bevacizumab use even beyond first progression. The purpose of this review is to summarize the most important scientific data supporting the use of chemotherapy and the new biologic agents in the second-line setting in advanced colorectal cancer.

Cytoprotective role of heme oxygenase-1 and heme degradation derived end products in liver injury

The activation of heme oxygenase-1(HO-1) appears to be an endogenous defensive mechanism used by cells to reduce inflammation and tissue damage in a number of injury models. HO-1, a stress-responsive enzyme that catabolizes heme into carbon monoxide(CO), biliverdin and iron, has previously been shown to protect grafts from ischemia/reperfusion and *** addition, the products of the HO-catalyzed reaction, particularly CO and biliverdin/bilirubin, have been shown to exert protective effects in the liver against a number of stimuli, as in chronic hepatitis C and in transplanted liver grafts. Furthermore, the induction of HO-1 expression can protect the liver against damage caused by a number of chemical compounds. More specifically, the CO derived from HO-1-mediated heme catabolism has been shown to be involved in the regulation of inflammation; furthermore, administration of low concentrations of exogenous CO has a protective effect against inflammation. Both murine and human HO-1 deficiencies have systemic manifestations associated with iron metabolism, such as hepatic overload(with signs of a chronic hepatitis) and iron deficiency anemia(with paradoxical increased levels of ferritin).Hypoxia induces HO-1 expression in multiple rodent,bovine and monkey cell lines, but interestingly, hypoxia represses expression of the human HO-1 gene in a variety of human cell types(endothelial cells, epithelial cells, T cells). These data suggest that HO-1 and CO are promising novel therapeutic molecules for patients with inflammatory diseases. In this review, we present what is currently known regarding the role of HO-1 in liver injuries and in particular, we focus on the implications of targeted induction of HO-1 as a potential therapeutic strategy to protect the liver against chemically induced injury.

soluble sugar is a key quality trait of soybean *** developed rapid and economic extraction and quantification methods for seed soluble sugars using an ultra-high performance liquid chromatography system with a refractive index *** evaluated the soluble sugar compositions of 1164 soybean accessions collected from diverse ecoregions and grown in multiple locations and *** soluble sugar(Tss)content was influenced by accession type,year of cultivation,and *** mean contents of fructose,glucose,sucrose,raffinose,stachyose and Tss were 3.31,5.21,55.60,6.60,35.47,and 106.19 mg g^(-1),*** highest mean Tss content(108.71 mg g^(-1)) was observed in accessions from Northern Region of *** contained higher contents of sucrose,raffinose,and Tss,whereas landraces had a higher content of *** accessions with mean Tss contents>130 mg g^(-1) were identified as elite soybean *** was correlated with sucrose,raffinose,stachyose,protein,oil and total *** main soluble sugar components were correlated with latitude and longitude,indicating that the geographical origin of the accessions affected their seed soluble sugar *** developed methods and elite identified accessions can be used in the food and feed industry and in soybean breeding programs aimed at improving soybean seed nutrition.

目的：1976年，Marbach将术语咬合幻觉定义为患者自我感知到不规律的咬合，但临床医师却不能发现任何咬合差异的情况。通常情况下，此类患者以前有过改变咬合的治疗经历，对咬合超敏，以及长久抱怨咬合的不适，但往往并不伴有疼痛。存在咬合幻觉的患者往往会经过漫长的、昂贵的、不可逆的、侵入性的以及不必要的治疗手段以试图解决这些症状。本研究的目标是：①了解正畸医师对咬合幻觉及其相关症状和体征的了解：②确定治疗该症状的最常见的方法；③确定地区差异或执医经验的长短是否影响上述因素：④确定咬合幻觉患者的性别特征。方法：该研究采用survey Monkey软件电子管理进行了14项调查。根据美国正畸学会（AAO）的医师信息目录，我们通过电子邮件招募了4124名正畸医师参加该项调查，其中337人完成这项调查。结果：在回应的正畸医师中，大约有50％的医师不熟悉“咬合幻觉”这项术语。然而，有许多正畸医师报道说见到过抱怨咬合幻觉的患者。人口地区差异，例如执业的区域和执业的时间长短并不影响对此类症状及其治疗方法的熟悉程度。结论：正畸医师需要提高对咬合幻觉这种症状的认识，并为该类患者提供适当的治疗。

Cerebrospinal fluid amyloid beta and tau proteins in atypical Parkinsonism: a review

Assessment of the Quality and shelf-Life in Enriched n3 PUFA Raw Beef Patties Using Dry soybean sprouts as Antioxidant

The appearance of fresh meat is a major determinant of its appeal to consumers. Ground beef in retail display oxidizes at a faster rate than whole muscle products;the use of antioxidants to break the radical chain reaction in the oxidation process is an effective method for slowing oxidation. soybean is a complex matrix with several bioactive compounds, including peptides and proteins, isoflavones, saponins, and other compounds with antioxidant properties. This study was conducted for 1) to evaluate the antioxidant properties of dry soybeans sprouts (Dss) from Glycine max (L.) Merr., MUNAsQA?;2) to evaluate quality characteristics of raw beef patties added with dried soybean sprouts at different levels (0.5%, 1% and 2%) and soybean oil as a replacer of fat;and 3) to estimate color changes and lipid oxidation of n3 fortified raw beef under frozen storage. Results showed that dry soybean sprouts had a high level of superoxide dismutase enzyme, high total phenolics and flavonoids content and appreciable vitamin c content. Although reduction power was low, 1,1-diphenyl-2-picrylhydrazyl (DPPH) and 2,2’-azino-bis(3-ethylbenzthiazoline-6-ulphonic acid) (ABTs) radicals scavenging activity were elevated. When different concentrations of Dss were incorporated to enriched beef patties with soybean oil, the nutritional value was slightly affected, while that n3 and n6 fatty acid content were increased around three twice. The beef patties formulated with 1% of natural antioxidant showed significantly (p stability than those without antioxidants. The lipid oxidation was inhibited in all cases, and was strongly effective at 1% concentration of Dss being the reduced percent of thiobarbituric acid reactive substances (TBARs) comparable to butilated hydroxianisole (BHA). The results shown in this research suggest that dry soybeans sprouts could also be used as a cheap natural antioxidant source for meat products.
