Selection of patients with hepatocellular carcinoma for livertransplantation:Past and future

The aim of liver transplantation(LT) for hepatocellular carcinoma(HCC) is to ensure a rate of disease-free survival similar to that of patients transplanted due to benign disease. Therefore, we are forced to adopt strict criteria when selecting candidates for LT and prioritizing patients on the waiting list(WL), to have clarified indications for bridging therapy for groups at risk for progression or recurrence, and to establish certain limits for downstaging therapies. Although the Milan criteria(MC) remain the standard and most employed criteria for indication of HCC patients for LT by far, in the coming years, criteria will be consolidated that take into account not only data regarding the size/volume and number of tumors but also their biology. This criteria will mainly include the alpha fetoprotein(AFP) values and, in view of their wide variability, any of the published logarithmic models for the selection of candidates for LT. Bridging therapy is necessary for HCC patients on the WL who meet the MC and have the possibility of experiencing a delay for LT greater than 6 mo or any of the known risk factors for recurrence. It is difficult to define single AFP values that would indicate bridging therapy(200, 300 or 400 ng/m L); therefore, it is preferable to rely on the criteria of a French AFP model score > 2. Other single indications for bridging therapy include a tumor diameter greater than 3 cm, more than one tumor, and having an AFP slope greater than 15 ng/m L per month or > 50 ng/m L for three months during strict monitoring while on the WL. When considering the inclusion of patients on the WL who do not meet the MC, it is mandatory to determine their eligibility for downstaging therapy prior to inclusion. The upper limit for this therapy could be one lesion up to 8 cm, 2-3 lesions with a total tumor diameter up to 8 cm, or a total tumor volume of 115 cm^3. Optimal Positioning Strategy for Multi-Camera Zooming Drones

In the context of multiple-target tracking and surveillance applications,this paper investigates the challenge of determining the optimal positioning of a single autonomous aerial vehicle or agent equipped with multiple independently-steerable zooming cameras to effectively monitor a set of targets of *** camera is dedicated to tracking a specific target or cluster of *** key innovation of this study,in comparison to existing approaches,lies in incorporating the zooming factor for the onboard cameras into the optimization *** enhancement offers greater flexibility during mission execution by allowing the autonomous agent to adjust the focal lengths of the onboard cameras,in exchange for varying real-world distances to the corresponding targets,thereby providing additional degrees of freedom to the optimization *** proposed optimization framework aims to strike a balance among various factors,including distance to the targets,verticality of viewpoints,and the required focal length for each *** primary focus of this paper is to establish the theoretical groundwork for addressing the non-convex nature of the optimization problem arising from these *** this end,we develop an original convex approximation *** paper also includes simulations of diverse scenarios,featuring varying numbers of onboard tracking cameras and target motion profiles,to validate the effectiveness of the proposed approach.

Colorectal cancer(CRC)screening is a fundamental tool in the prevention and early detection of one of the most prevalent and lethal *** the years,screening,particularly in those settings where it is well organized,has succeeded in reducing the incidence of colon and rectal cancer and improving the prognosis related to *** considerable advancements in screening technologies and strategies,the effectiveness of CRC screening programs remains less than *** paper examined the multifaceted reasons behind the persistent lack of effect-iveness in CRC screening *** a critical analysis of current methodologies,technological limitations,patient-related factors,and systemic challenges,we elucidated the complex interplay that hampers the successful reduction of CRC morbidity and mortality *** acknowledging the ad-vancements that have improved aspects of screening,we emphasized the necessity of addressing the identified barriers *** study aimed to raise awareness of how important CRC screening is in reducing costs for this *** and early diagnosis are not only important in improving the prognosis of patients with CRC but can lead to an important reduction in the cost of treating a disease that is often diagnosed at an advanced *** more sooner can mean saving money later.

BACKGROUND Oxidative damage of DNA and RNA has been associated with mortality of patients with different ***,there is no published data on the potential use of DNA and RNA oxidative damage to predict the prognosis of patients with hepatocellular carcinoma(HCC)undergoing liver transplantation(LT).AIM To determine whether patients with increased DNA and RNA oxidative damage prior to LT for HCC have a poor LT *** Patients with HCC who underwent LT were included in this observational and retrospective *** levels of all three oxidized guanine species(OGS)were measured prior to LT since guanine is the nucleobase that forms DNA and RNA most prone to *** mortality at 1 year was the end-point *** Surviving patients(n=101)showed lower serum OGS levels(P=0.01)and lower age of the liver donor(P=0.03)than non-surviving patients(n=13).An association between serum OGS levels prior to LT and 1-year LT(odds ratio=2.079;95%confidence interval=1.356-3.189;P=0.001)was found in the logistic regression *** The main new finding was that high serum OGS concentration prior to LT was associated with the mortality 1 year after LT in HCC patients.

Beyond network centrality: individual-level behavioral traits for predicting information superspreaders in social media

Understanding the heterogeneous role of individuals in large-scale information spreading is essential to manage online behavior as well as its potential offline consequences. To this end, most existing studies from diverse research domains focus on the disproportionate role played by highly connected ‘hub’ ***, we demonstrate here that information superspreaders in online social media are best understood and predicted by simultaneously considering two individual-level behavioral traits: influence and susceptibility. Specifically, we derive a nonlinear network-based algorithm to quantify individuals' influence and susceptibility from multiple spreading event data. By applying the algorithm to large-scale data from Twitter and Weibo, we demonstrate that individuals' estimated influence and susceptibility scores enable predictions of future superspreaders above and beyond network centrality, and reveal new insights into the network positions of the superspreaders.

Tracking the change in Spanish greenhouse gas emissions through an LMDI decomposition model:A global and sectoral approach

The reduction of GHG emissions to reverse the greenhouse effect is one of the main challenges in this *** this paper we pursue two ***,we analyze the evolution of GHG emissions in Spain in 2008–2018,at both the global and sectoral levels,with the variation in emissions decomposed into a set of determining ***,we propose several actions specifically oriented to more tightly controlling the level of *** results showed a remarkable reduction(18.44%)in GHG emissions,mainly due to the intensity effect,but also to the production-per-capita *** detected somewhat different patterns among the various sectors *** the intensity effect was the most influential one in the agricultural,transport,and others sectors,the production-per-capita effect was predominant in the case of *** carbonization effect was revealed as crucial in the commerce *** above findings highlight the importance of the energy efficiency measures taken in recent years in the Spanish economy,also pointing to the need to deepen those strategies and to propose new measures that entail greater efficiency in *** efforts in areas like innovation,R&D,diffusion of more eco-friendly technologies,and a greater use of greener energies all prove to be essential reduction actions to fight the greenhouse effect.

Primary Hyperparathyroidism in a Twin Pregnancy: A Case Report

The primary hyperparathyroidism during pregnancy is an infrequent disorder but it is rarer in a twin gestation. Often it is not suspected because the symptomatology is overlapped with the normal course of pregnancy and it is usually discovered after a routine blood test. Ultrasound is the best option to identify the adenoma, while the gold-standard is technetium-99 m sestamibi but is contraindicated in this situation. The treatment is controversial and depends of the time of the gestation: the medical therapy is accepted in the first and third trimester, and the surgery is reserved for the second trimester, where the side effects are low. We present a case of primary hyperparathyroidism in a twin pregnancy treated by a minimally invasive parathyroidectomy in the second trimester without consequences for both, mother and newborns.

Critical evaluation of transit policies in Lima,Peru;resilience of rail rapid transit(Metro)in a developing country

This paper evaluates rail transit within the context of the transit policies implemented in Lima,*** it reviews the implementation of rapid transit,and bus ***,it evaluates the outcomes of such policies by using Total Factor Productivity for policy effectiveness,Data Envelopment Analysis for rapid transit performance,and Generalized Cost of Travel for *** paper finds that implementation failed in enforcing key requirements for rail transit regarding penetration of CBD and short transfers to bus transit;and that the basic assumptions of bus reform did not hold regarding bus oversupply,bus congestion or bus *** paper also finds that outcomes of policies failed dramatically in achieving the planning goals;however,rail transit(Metro)shows high level of resilience in serving large ridership at high *** the other hand,bus reform was associated with a disproportionate increase of motorization,well over the effect of income growth or car attractiveness,and more related to the excessive reduction of bus transit capacity ill-advised from unproved bus reform *** paper recommends expanding rail rapid transit due to its intensive use of green renewable energy and its potential of demand growth if combined with modern Intelligent Transportation services,but this opportunity can be wasted without the proposed policy constraint to achieve lower Generalized Cost of Travel at any governmental intervention for bus reform,instead of just reducing bus transit capacity as ***,this paper recommends government to government contracts to build rail transit and to enforce proper planning.
