Does the scleral encircling band provide a protective effect against the progression of diabetic retinopathy?

AIM: To evaluate the effect of scleral encircling bands on the development and progression of diabetic retinopathy(DR) in diabetic ***: The medical records of diabetic patients who underwent unilateral retinal detachment(RD) surgery using scleral buckle and encircling band were reviewed retrospectively. Both eyes of patients were included in the study: one eye in each patient had a scleral buckle with encircling band(the operated eye) and the other one is the non-operated eye. The demographic characters, duration of diabetes and period between surgery and the last recall visit were retrieved from each patient. All the cases underwent fundus photo and fluorescein angiography(when indicated) to confirm the DR ***: Totally 25 patients fulfilled the inclusion and the exclusion criteria were become eligible for the study. A total of 50 eyes of 25 patients were enrolled in this analysis. The mean period of time passed from surgery with encircling band and the last reassessment visit was 12.5±2 y. Even though DR could develop in the operated eyes, it was at a less degree of severity compared to the non-operated eyes of same patients(P=0.027).CONCLUSION: Scleral encircling bands have protective effects against the development and progression of DR.

Background Medical clowning has been proven effective in reducing pain,anxiety,and stress in many sporadic,usually small-scale *** meta-analysis aims to evaluate the efficiency of medical clowns in reducing pain and anxiety in hospitalized pediatric patients and their parents in different medical *** A thorough literature search was conducted from different databases,and only randomized controlled trials(RCTs)were included with children aged 0 to 18 years old.A total of 18 studies were included,and statistical analysis was performed on the combined *** A total of 912 children(14 studies)showed significantly reduced anxiety when procedures were performed with a medical clown compared with the controls(−0.76 on anxiety score,P<0.001).Preoperative anxiety was lower in 512 children(nine studies)with clown interventions than in the controls(−0.78,P<0.001).The pain scale was completed by 338 participants(six studies),indicating a trend toward reduced pain during procedures performed while the clown was acting compared to controls(−0.49,P=0.06).In addition,medical clown significantly(−0.52,P=0.001)reduced parental anxiety in 489 participants in ten studies;in six of the ten studies,with a total of 380 participants,medical clown significantly reduced parental preoperative anxiety(P=0.02).Conclusion Medical clowns have substantial positive and beneficial effects on reducing stress and anxiety in children and their families in various circumstances in pediatrics.

Hepatocellular carcinoma (HCC) is a major cause of cancer death worldwide, accounting for over half a million deaths per year. The geographic pattern of HCC incidence is parallel to exposure to viral etiologic factors. Its incidence is increasing, ranging between 3% and 9% annually depending on the geographical location, and variability in the incidence rates correspond closely to the prevalence and pattern of the primary etiologic factors. Chronic infections with hepatitis B viruses or hepatitis C viruses have both been recognized as human liver carcinogens with a combined attributable fraction of at least 75% of all HCC cases. Multiple non-viral factors have been implicated in the development of HCC. Increased body mass index and diabetes with subsequent development of non-alcoholic steatohepatitis represent significant risk factors for HCC. Other non-viral causes of HCC include iron overload syndromes, alcohol use, tobacco, oral contraceptive, aflatoxin, pesticides exposure and betel quid chewing, a prevalent habit in the developing world. Wilson disease, α-1 antitrypsin deficiency, Porphyrias, autoimmune hepatitis, Schistosoma japonicum associated with positive hepatitis B surface antigen, and thorotrastray are also contributing hepatocellualar carcinoma. In addition, primary biliary cirrhosis, congestive liver disease and family history of liver cancer increase the risk of HCC incident. In conclusion,clarification of relevant non-viral causes of HCC will help to focus clinicians on those risk factors that are modifiable. The multilevel preventative approach will hopefully lead to a reduction in incidence of non-viral HCC, and a decrease in the patient morbidity and mortality as well as the societal economic burden associated with HCC.

An information system is a type of knowledge representation,and attribute reduction is crucial in big data,machine learning,data mining,and intelligent *** are several ways for solving attribute reduction problems,but they all require a common *** selection of features in most scientific studies is a challenge for the *** working with huge datasets,selecting all available attributes is not an option because it frequently complicates the study and decreases *** the other side,neglecting some attributes might jeopardize data *** this case,rough set theory provides a useful approach for identifying superfluous attributes that may be ignored without sacrificing any significant information;nonetheless,investigating all available combinations of attributes will result in some ***,because attribute reduction is primarily a mathematical issue,technical progress in reduction is dependent on the advancement of mathematical *** the focus of this study is on the mathematical side of attribute reduction,we propose some methods to make a reduction for information systems according to classical rough set theory,the strength of rules and similarity matrix,we applied our proposed methods to several examples and calculate the reduction for each *** methods expand the options of attribute reductions for researchers.

Drought stress(DS)is one of the most critical environmental abiotic stresses for wheat production in the arid *** of high-yielding genotypes tolerant to DS can play a significant role in mitigation the negative impacts associated with *** the present study,generation means analysis(GMA)was used to study the performance of two crosses under well irrigation(WI)and deficit irrigation[cross I(Line 44×Shandweel-1)and cross II(Line 20×Sakha 93)].Significant differences were observed for days to heading(DH),days to maturity(DM),plant height(PH),spike length(SL),number of spikes per plant(NS/P),number of grains per spike(NG/S),thousand-grain weight(TGW),grain yield per plant(GY/P),and proline content(PC)in the six populations of the two crosses within each irrigation *** II had early maturity and the highest PC,NS/P,TGW,and GY/P regardless of the irrigation *** I showed positive significant relative heterosis and heterobeltiosis for GY/P under the two irrigation *** inheritance of characters of cross I revealed additive,dominant,and epistatic effects,which varied with trait and *** genetic effects predominated in DH,SL,and PC,while non-additive were found in DM,NS/P,NG/S,and GY/***-sense heritability estimates(h^(2) n)were high for DH and PC,moderate to high for PH and SL,moderate for DM,NG/S,NS/P,and TGW,and low for GY/*** on different drought indices the populations BC_(1),BC_(2),F_(1),and P1 of cross II and BC1 of cross I were more tolerant to drought ***,PC,TGW and DH can be used as selection indicators to improve wheat for drought tolerance in early generations and other yield components traits in late *** second cross(Line 20×Sakha 93)shows promise and is of interest to a drought tolerance breeding program,where wheat breeders can use recombinant breeding strategies to construct desirable drought stress *** and path coefficient revealed that TGW and PC were the main

Childhood glaucoma profile in Dakahelia, Egypt: a retrospective study

AIM： To analyze childhood glaucoma regarding its demographics, presentations, different causes and surgical modalities used among patients in Dakahelia and to apply the Childhood Glaucoma Research Network（CGRN） classification retrospectively to evaluate its ***： A retrospective study in which the medical files of all glaucoma patients 〈16 years old presented to Mansoura Ophthalmic Center, Mansoura University from 2014 to 2017, were retrieved and analyzed. Collected data included： age, gender, laterality, visual acuity（VA）, refraction, intraocular pressure（IOP）, corneal diameter, cup-disc ratio, types and number of surgeries and antiglaucomatous drugs（AGD） at the first and last visit. Prevalence of different subtypes was calculated and means of clinical features were ***： A total of 305 eyes of 207 patients were included classified into 6 groups： primary congenital glaucoma（PCG）, juvenile open angle glaucoma（JOAG）, glaucoma associated with systemic disease, glaucoma associated with ocular anomalies, acquired glaucoma and glaucoma following cataract surgery. PCG was the predominant type（55.1%） followed by acquired glaucoma（29.5%）. Males represented 63.8% of the whole studied patients. Glaucoma associated with ocular anomaly group showed the youngest age at diagnosis（21.9±30.0 mo）. The shortest corneal diameter was recorded in post cataract group（10.4±0.5 mm）. Highest cup-disc ratio was found in the PCG group（P〈0.0005）. Glaucoma associated with systemic disease presented with the highest baseline IOP（34.5±5.0 mm Hg）. All the cases with PCG were treated surgically with 21.8% required more than one surgery. The majority of the patients（74.2%） in the acquired group were treated medically. Combined trabeculotomy-trabeculectomy was the most frequent operation done, accounting for 34.5% of all primary surgeries. Ahmed valve implantation comprised 87% of the secondary surgeries. We sought to evaluate immediate and delayed micro-tensile bond strength of Panavia F2.0 and Multilink Sprint resin cement to superficial, deep and cervical dentin. Thirty-six freshly extracted non-carious human molars were sectioned in the mesiodistal direction to expose three different dentin regions including superficial dentin （1 mm below the dentine-enamel junction）, deep dentin （1 mm above the highest pulp horn） and cervical dentin （0.5 mm above the cemento-enamel junction and 0.5 mm below the dentine-enamel junction）. Resin cements were applied on dentin surfaces and composite blocks were luted under constant seating pressure. Each group was divided into three subgroups according to time intervals. Specimens were sectioned to obtain sticks of 1 mm2 in diameter and subjected to microtensile bond strength testing at a cross head speed of 1 mrn/min. Both resin cements showed higher micro-tensile bond strength to superficial dentin than that to deep or cervical dentin （P 〈 0.001）. Micro-ten- sile bond strengths of Panavia F2.0 were higher than those of Multilink Sprint at different dentin regions （P 〈 0.001）. Immediate ＂micro-tensile bond strengths were higher than those of delayed micro-tensile bond strengths for both resin cements （P 〈 0.001）. It was concluded that resin cements with different chemical formulations and applications yield significantly different micro-tensile bond strengths to different dentin regions.
