b>Introduction:b>b> b>Giant hernias induce changes which reduce the quality of life of patients and make their surgical management complex. Adequate preoperative preparation of the patient guarantees good postoperative progress. It is necessary to avoid resorting to a technique of separation of the compartments during the cure. Here we report the case of a patient who benefited a successful cure using the Ramirez technique. b>Observation:b>b> b>We report the case of a 60-year-old patient admitted to an outpatient clinic for abdominal swelling evolving for 30 years without the notion of trauma gradually increasing in volume. The interrogation and physical examination led to the diagnosis of a giant white line hernia with incarceration. A preoperative assessment and a preanesthetic consultation were carried out. Intraoperatively, the cecum, transverse colon, sigmoid, jejunum and greater omentum were incarcerated in the bag. After adhesiolysis we proceeded to resect the bag and cure it using the Ramirez technique. The consequences were simple and the patient was discharged on day 4 after her dressing and was seen on day 15, 1 month, 3 months and 1 year. The patient benefited from the placement of an abdominal compression sheath for 3 months. b>Conclusion:b>b> b>Success in the management of prosthetic material in the absence of prosthetic material depends on good preoperative preparation and the mastery of certain surgical techniques.

＜正＞background and Aims: The treatment of prostate cancer （PCa） depends on stage. To improve treatment strategies, it would be helpful to have reliable molecular markers able to predict PCa progression. However, to date, such markers have not yet been included in therapeutic decisions. Recently, several studies from our laboratory described the prognostic potential of molecular markers that include Erbb3, Akt and Nuclear Factor kappab （NF- κb） . Erbb3 is a member of the growth factor receptor family Erbb. Akt is a signaling component of the Erbb receptor family. Phos-phoinositol 3 - kinase （PI3K） through Akt regulates downstream signaling proteins such as NF - κb. Most of these molecules are often overexpressed and/or activated in cancer cells. Furthermore, they can be detected in the cytoplasm and/or nucleus of PCa cells. They are known to be involved in cell survival, cell growth, cell cycle progression and apoptotic response. The aim of our study was to determine whether these molecules could be used as markers to predict PCa progression.

Analysis of Reactivity Induced Accident for Control Rods Ejection with Loss of Cooling

Understanding of the time-dependent behavior of the neutron population in nuclear reactor in response to either a planned or unplanned change in the reactor conditions, is a great importance to the safe and reliable operation of the reactor. In the present work, the point kinetics equations are solved numerically using stiffness confinement method (SCM). The solution is applied to the kinetics equations in the presence of different types of reactivities and is compared with different analytical solutions. This method is also used to analyze reactivity induced accidents in two reactors. The first reactor is fueled by uranium and the second is fueled by plutonium. This analysis presents the effect of negative temperature feedback with the addition positive reactivity of control rods to overcome the occurrence of control rod ejection accident and damaging of the reactor. both power and temperature pulse following the reactivity- initiated accidents are calculated. The results are compared with previous works and satisfactory agreement is found.

Maternal TMPRSS6 Gene Polymorphism rs855791SNP in Women with Preeclampsia

Introduction: Preeclampsia can lead to several maternal and perinatal adverse effects. There are few published data on the association between transmembrane serine protease 6 (TMPRSS6) gene polymorphism and preeclampsia. Objective: To assess the association between TMPRSS6 gene polymorphism rs855791SNP in women with preeclampsia compared with healthy pregnant women. Method: A case-control study (60 women in each arm) was conducted at saad Abuaela Maternity Hospital in Khartoum, Sudan. Sociodemographic and clinical data were gathered through a questionnaire. The participant was genotype for TMPRSS6 gene rs855791SNP using Polymerase Chain Reaction and Restriction Fragment Length Polymorphism (PCR-RFLP). The results were confirmed by DNA sequencing. Result: There was no significant difference in the median of age, parity, and body mass index. The distribution of the genotypes and alleles of TMPRSS6 rs855791 was consistent with the HWE. The overall TMPRSS6 rs855791 polymorphism was not significantly associated with preeclampsia. However, the proportion of heterozygotes (TC) was considerably higher in the women with preeclampsia (46.7%) than in the control group (23.3%) (p = 0.001;OR = 2.71;95% CI = 1.21 - 6.07). The proportion of homozygotes (TT) and T alleles was not significantly different between women with preeclampsia and the control group. Conclusion: The overall TMPRSS6 rs855791 polymorphism was not significantly associated with preeclampsia and healthy control.

background: Renal artery pseudoaneurysms are rarely described sequelae of blunt abdominal trauma. Interventional radiological advances have allowed such lesions to be managed nonoperatively. Methods: The authors review the presentation, diagnostic evaluation, and hospital course of an 11- year-old girl who developed a right renal artery pseudoaneurysm 14 days after blunt abdominal trauma. Results: An 11- year-old girl fell off a horse onto her right flank. She sustainedmultiple right hepatic lobe lacerations and a complex fracture of the upper pole of the right kidney. Her initial hospital course was uncomplicated, and she was discharged after an uneventful 6- day course. The child did well for 2 weeks, until she developed right back pain and gross hematuria. A computed tomography scan revealed a right renal artery pseudoaneurysm. Angiography confirmed the presence of a pseudoaneurysm, which was fed by a single segmental branch originating from the renal artery. The artery was successfully occluded with a single platinum microcoil, which was demonstrated by the absence of contrast flow into the pseudoaneurysm. The patient recovered and was discharged shortly after the procedure. She initially had intermittent pain and hematuria, which resolved. Follow-up computed tomography scans have shown resolution of both the renal and hepatic lesions. Conclusions: Renal artery pseudoaneurysms that arise after blunt abdominal trauma in the pediatric population may be safely and effectively managed with arterial embolization, thereby avoiding extensive surgical interventions.

背景呼吸道合胞病毒(respiratory syncytial virus,RSV)是引起婴幼儿急性下呼吸道感染最常见的病原体。此前估计,2015年全球60月龄以下儿童共发生3 310万次RSV相关急性下呼吸道感染,导致118 200人死亡。此后,几项社区监测研究获得了对RSV相关社区死亡的更精确估计。本文旨在更新2019年全球、地区和国家层面60月龄以下儿童RSV相关急性下呼吸道感染的发病率和死亡率,重点关注总体死亡率和在研RSV免疫制剂针对的更窄的婴儿年龄组。方法检索获得MEDLINE、Embase、Global Health、CINAHL、Web of Science、LILACS、OpenGrey、中国知网、万方和维普数据库2017—2020年发表文献中的新数据,并纳入RSV GEN合作者提供的未发表数据,以扩展全球RSV疾病负担数据集。纳入的研究符合以下条件:报告了社区中或需要住院治疗的60月龄以下儿童RSV原发性急性下呼吸道感染的数据;报告了至少连续12个月的数据,但院内病死率或RSV季节性定义明确的地区除外;报告了RSV相关急性下呼吸道感染的发病率、住院率、院内病死率或急性下呼吸道感染住院的RSV阳性比例。如果病例定义不明或应用不一致、RSV感染未经实验室确认或仅基于血清学检测、报告的急性下呼吸道感染病例少于50例,则排除该研究。本研究应用广义线性混合效应模型(generalized linear mixed-effects model,GLMM)估计了2019年全球和地区(按国家发展状况和世界银行收入分类)RSV相关急性下呼吸道感染的发病率、住院率和院内病死率;通过基于危险因素的模型估计了国家层面RSV相关急性下呼吸道感染的发病率;通过GLMM开发了新模型,纳入最新的RSV社区死亡率数据,用于估计RSV的总体死亡率。本综述已在PROSPERO注册(CRD42021252400)。结果除了先前综述中纳入的317项研究外,还纳入了113项符合条件的新研究以及51项未发表的研究,共计481项研究。据估计,2019年全球60月龄以下儿童中,共发生3 300万(置信区间为2 540万~4 460万)次RSV相关急性下呼吸道感染,导致360万(290万~460万)例住院、26 300(15 100~49 100)例院内死亡以及101 400(84 500~125 200)例可归因于RSV的总死亡数。在0~6个月的婴儿中,共发生660万(460万~970万)次RSV相关急性下呼吸道感染,导致140万(100万~200万)例住院、13 300(6 800~28 100)例院内死亡以及45 700(38 400~55 900)例可归因于RSV的总死亡数。60月龄以下和28 d~6月龄儿童的死亡中分别有2.0%(置信区间为1.6%~2.4%)和3.6%(3.0%~4.4%)归因于RSV。在各年龄组中,95%以上的RSV相关急性下呼吸道感染事件和97%以上的可归因于RSV的死亡事件都发生在中低收入国家。结论 RSV显著增加了全球60月龄以下儿童的发病和死亡负担,尤其是在出生后前6个月和中低收入国家。本研究强调了全球RSV疾病的总体死亡率负担,60月龄以下儿童中,每50例死亡就有1例可归因于RSV;28 d至6个月大的婴儿中,每28例死亡就有1例可归因于RSV。每1例RSV相关急性下呼吸道感染院内死亡,对应社区中约3例可归因于RSV的死亡。对6月龄内婴儿提供保护的RSV被动免疫计划有望对减轻RSV疾病负担,但实施计划时,仍需要更多数据来了解RSV疾病负担高峰可能向大龄转移的影响。
