Inflammatory bowel disease: An evaluation of health information on the internet

To evaluate the quality and accuracy of websites written to the public on inflammatory bowel disease (IBD) (Crohn’s disease and ulcerative colitis) and assess their readability ***™, Bing™, and Yahoo™ search engines were searched independently by three researchers in December 2014. Only English-language websites were selected on the basis of predetermined inclusion and exclusion criteria. Researchers independently evaluated the quality of each website by using the DISCERN and the HONcode instruments. The readability levels were calculated using two formulas; the Flesch-Kincaid Grade Level Index, and the Coleman-Liau Readability Index. The agreement between the evaluators was calculated using Cohen kappa ***-four websites were finally identified. Scores varied from a minimum DISCERN score of 18 to a maximum of 68 [mean ± SD, 42.2 ± 10.7; median = 41.5, interquartile range, interquartile range (IQR) = 15.8] and a minimum score of HONcode of 0.14 and a maximum of 0.95 (mean ± SD, 0.16 ± 0.19; median = 0.45, IQR = 0.29). Most of these websites were reviewed in 2014 and 2015 (n = 51). The creators of these websites were: universities and research centers (n = 25, 30%), foundations and associations (n = 15, 18%), commercial and pharmaceutical companies (n = 25, 30%), charities and volunteer work (n = 9, 10%), and non-university educational bodies (n = 10, 12%). The Flesch-Kincaid Grade Level readability score (mean ± SD) was 11.9 ± 2.4 and the Coleman-Liau Readability Index score was 12.6 ± 1.5. Significant correlation was found between the two readability scores (R2 = 0.509, P = 0.001). The overall agreement between evaluators measured by Cohen kappa coefficient was in the range of 0.804-0.876; rated as 'Good'.CONCLUSIONThe DISCERN and the HONcode scores of websites varied and the readability levels of most websites were above the public readability level. Exploring ferromagnetic half-metallic nature of Cs2NpBr6 via spin polarized density functional theory

By employing the spin resolved density functional theory, half-metallic character is investigated in Cs2NpBr6 having a K2PtCl6-type structure. The results precisely predict the half-metallic behavior of Cs2NpBr6. In spin-down state it presents an indirect band gap, while in spin-up channel it turns metallic. The structure optimization confirms the half-metallic nature in ferromagnetic configuration. The calculated magnetic moment is 3 μB toward which the main contributor is the Np ***, all the computed results are compared with the available experimental and theoretical values. According to the present analysis, we recommend Cs2NpBr6 for spintronic applications.

Colorectal cancer screening: An updated review of the available options

Colorectal cancer (CRC) is a significant cause of morbidity and mortality worldwide. However, colon cancer incidence and mortality is declining over the past decade owing to adoption of effective screening programs. Nevertheless, in some parts of the world, CRC incidence and mortality remain on the rise, likely due to factors including “westernized” diet, lifestyle, and lack of health-care infrastructure and resources. Participation and adherence to different national screening programs remain obstacles limiting the achievement of screening goals. Different modalities are available ranging from stool based tests to radiology and endoscopy with varying sensitivity and specificity. However, the availability of these tests is limited to areas with high economic resources. Recently, FDA approved a blood-based test (Epi procolon®) for CRC screening. This blood based test may serve to increase the participation and adherence rates. Hence, leading to increase in colon cancer detection and prevention. This article will discuss various CRC screening tests with a particular focus on the data regarding the new approved blood test. Finally, we will propose an algorithm for a simple cost-effective CRC screening program.

Bacille Calmette-Gu6rin(BCG) vaccine is one of the most widely used vaccines in *** Egypt,it is a part of the national compulsory childhood immunization *** most controversial aspect of BCG is the variable efficacy found in different *** study was to evaluate the efficacy status of the available BCG vaccine in Egypt within the last 10 years(BCG-Copenhagen).The pilot cross sectional study included 597 Egyptian children randomly *** ages ranged from 6 months to 10 years old(mean_ 5 years,median: 3 years).All were assessed for history of BCG vaccine intake(primary at infancy and/or secondary at school age) and examined for the presence BCG scar.A group of the vaccinated children(62 children with BCG scar and 69 children without BCG scar) were further assessed with tuberculin skin test(TST).Prevalence of BCG vaccine intake in the studied children was 86.9%(519/597).Efficacy in term of BCG scar after vaccination was 66.6%(346/519).However,efficacy in term of post BCG vaccination tuberculin sensitization was only 3.8%(5/131).BCG vaccination program in Egypt seems to be widely prevalent;however, the immunological efficacy of the available strain is questionable.

Extracorporeal membrane oxygenation(ECMO)has emerged as a vital circulatory life support measure for patients with critical cardiac or pulmonary conditions unresponsive to conventional *** allows blood to be extracted from a patient and introduced to a machine that oxygenates blood and removes carbon *** blood is then reintroduced into the patient’s circulatory *** process makes ECMO essential for treating various medical conditions,both as a standalone therapy and as adjuvant ***-venous(VV)ECMO primarily supports respiratory function and indicates respiratory ***,veno-arterial(VA)ECMO provides hemodynamic and respiratory support and is suitable for cardiac-related *** study reviews recent literature to elucidate the evolving role of ECMO in trauma care,considering its procedural intricacies,indications,contraindications,and associated ***,the use of ECMO in trauma patients,particularly for acute respiratory distress syndrome and cardiogenic shock,has demonstrated promising outcomes despite challenges such as anticoagulation management and complications such as acute kidney injury,bleeding,thrombosis,and *** studies have shown that VV ECMO was associated with significantly higher survival rates than conventional mechanical ventilation,whereas other studies have reported that VA ECMO was associated with lower survival rates than VV *** plays a critical role in managing trauma patients,particularly those with acute respiratory *** research is necessary to explore the full potential of ECMO in trauma *** should have a clear understanding of the indications and contraindications for the use of ECMO to maximize its benefits in treating trauma patients.

Mainstreaming Decentralization and Collaboration in Disaster Risk Management:Insights from Coastal Bangladesh

Bangladesh is renowned in disaster risk reduction(DRR)for active involvement of community people and local disaster management institutions in DRR *** study aimed to describe the disaster risk management(DRM)institutions and assess their functioning in six coastal unions across the three coastal zones of *** qualitative and quantitative research approaches were *** study focused on two key local institutions—the Union Disaster Management Committees(UDMCs)and the Cyclone Preparedness Program(CPP)—functioning at the union level in *** institutions have both horizontal and vertical collaborations with other ***,we argue that the UDMCs'external dependencies in their functioning indicate their limited financial and administrative autonomy,which is a barrier to successfully institutionalizing disaster *** results show that the CPP is the most successful program,markedly increasing the trust of the people in warning dissemination and evacuation efforts in the event of a *** the adoption of decentralized risk management systems has resulted in significant progress in increased rate of evacuation and reduced death rate and damage,lack of funding and equipment,limited coordination between institutions,lack of skilled and knowledgeable workforce,and inappropriate power structures may reduce the effectiveness of DRR activities prior to,during,and following disasters.
