Unfavourable TB Outcomes in PLHIV with Delayed TB Treatment Initiation—A retrospective Analysis

rong>Background & Objectives:rong> Timely initiation of anti-tubercular treatment (ATT) is key to obtain favourable outcomes among people living with HIV (PLHIV) co-infected with tuberculosis (TB). There is little evidence to predict treatment outcome and the delay in initiation of ATT among co-infected. The analysis explores the relationship between different ATT initiation delay times and TB outcomes. rong>Methods:rong> retrospective analysis of secondary data of HIV-TB co-infected patients attending two antiretroviral therapy (ArT) clinics in Pune district of Maharashtra, India. Logistic regression models were used to calculate risk. rong>results:rong> Of the total 457 co-infected PLHIV initiated on ATT from December 2016 to June 2018, there were 298 who had completed TB treatment. Outcomes of ATT were available for 243 patients. Of the 243 patient’s records analysed, 61% were males and ≤11% were less than 18 years. 20% were previously treated. The risk of unfavourable outcomes increased monotonically from Or = 1.1, to Or = 1.73 for >2 days to >7 days delay in ATT initiation respectively. Males (Or = 2.11), newly initiated patients (Or = 1.54) are exposed to higher risk of unfavourable TB outcomes when ATT initiation is delayed beyond seven days. rong>Conclusion:rong> risk of unfavourable outcomes increase in delay time for ATT initiation. Patient characters should be considered for prioritised referral and availability of test results on the same day of diagnosis to foster same day ATT initiation. More analysis on larger data sets with richer clinical information will generate confirmatory evidence.

Effect of Scale-up on Heat Transfer Characteristics in Upper Splash region of Circulating Fluidized Bed risers

In the present work, steady state heat transfer experiments were conducted in the upper splash region of three cold circulating fluidized beds （CFB）, B 1, B2 and B3, with height of each 2.85 m and bed cross sections of 0.15 m × 0.15 m, 0.20 m × 0.20 m and 0.25 m × 0.25 m, respectively. Experiments were conducted under similar operating conditions on all the three CFt3 setups for two different non-dimensional air velocities （U＊ = 5 and 8） and two different sand inventories with average particles size of 460 μm. Effect of cross section of riser on heat transfer characteristics was studied. Bed temperature distribution across the heater placed in the upper splash region of riser was measured at two sections, at a height of 1.96 m and 2.24 m above the distributor plate. Axial distribution of local heat transfer coefficients along the height of heater were evaluated and compared for different bed cross sections. results obtained were compared with the available literatures.

The human striatum is essential for both lowand high-level functions and has been implicated in the pathophysiology of various prevalent disorders,including Parkinson's disease(PD)and schizophrenia(SCZ).It is known to consist of structurally and functionally divergent ***,previous parcellations are based on a single neuroimaging modality,leaving the extent of the multi-modal organization of the striatum ***,we investigated the organization of the striatum across three modalities—resting-state functional connectivity,probabilistic diffusion tractography,and structural covariance—to provide a holistic convergent view of its structure and *** found convergent clusters in the dorsal,dorsolateral,rostral,ventral,and caudal *** characterization revealed the anterior striatum to be mainly associated with cognitive and emotional functions,while the caudal striatum was related to action ***,significant structural atrophy in the rostral and ventral striatum was common to both PD and SCZ,but atrophy in the dorsolateral striatum was specifically attributable to *** study revealed a cross-modal convergent organization of the striatum,representing a fundamental topographical model that can be useful for investigating structural and functional variability in aging and in clinical conditions.

Activator protein-1(AP-1)transcription factor is a key component of many signal transduction pathways involved in the regulation of cellular processes and controls rapid responses of mammalian cells when exposed to the variety of *** phorbol 12-myristate 13-acetate and Forskolin(Fo)are well-known kinase activators/stimulators of Protein Kinase C(PKC)and Protein Kinase A(PKA)***,these kinases are found to be present in transitional points of many cell signaling pathways,especially those involved in *** stimulating effect of PKC and PKA on the expression of AP-1 factors in MCF-7 breast cell proliferation is not well ***,the role of PKC by PMA treatment and the role of PKA by using Fo in MCF-7 cells is ***,cells treated with PMA showed increased cell proliferation,while Fo had no effect,but inhibited the PMA induced *** rT-PCr results showed the PMA induced c-Jun,c-Fos and Fra-1 expressions compared to control and ***,Fo in combination with PMA,inhibit the PMA induced above mrNA expressions where Fo alone has no *** blot studies validated the c-Jun expressions in PMA treated MCF-7 ***,PMA increases the mrNA expression of Cyclin-E1,Cyclin-D1,and CDK-4,whereas Fo decreases their ***,mitogenic effect of PMA and inhibitory action of Fo on MCF-7 cells is probably enhanced via activation of AP-1 factors and concomitant action of cell cycle regulators in the downstream singling cascade.

Pleuropulmonary Blastoma in Pediatric Age Group: A Very rare Case report and review of Literature

Pleuropulmonary blastoma (PPB) is a very rare and very aggressive malignant tumor that affects children and adults. Though its presentation in children has been reported in literature, incidence is very low as compared to adults. This neoplasm is characterized on histology by primitive blastema and a malignant mesenchymal stroma that often demonstrates multidirectional differentiation. Even though availability of multimodal therapy, the prognosis of patients with PPB remains poor.

Dielectric study of three homologous Schiff base ferroelectric liquid crystals with the variations of temperature and frequency

In this investigation,the dielectric behaviors of three ferroelectric liquid crystals(FLCs)belonging to a homologous series have been revealed in the frequency range of 10 Hz-10 *** used in this study are three-ring calamitic LCs,namely(S)-4-(((4-(octan-2-yloxy)phenyl)imino)methyl)phenyl 4-(n-octyloxy)benzoate,(S)-4-(((4-(octan-2-yloxy)phenyl)imino)methyl)phenyl 4-(n-decyloxy)benzoate and(S)-4-(((4-(octan-2-yloxy)phenyl)imino)methyl)phenyl 4-(n-undecyloxy)*** polarizing optical microscopic and differential scanning calorimetric results confirm that these compounds,synthesized as per the known synthetic steps,show not only a ferroelectrically switchable chiral smectic C(SmC*)phase over a wide thermal range,but also other mesophases such as blue phase-I/II(BP-I/II),chiral nematic(N*)and unknown smectic(SmX)*** essential dielectric parameters of the FLC phase have been measured at different *** relative permittivity has been measured with the variations of temperature and ***,the dielectric loss and tanδhave been *** dielectric relaxations have been calculated and explained at the molecular *** systematic measurements revealed a strong anomaly,and crossover of relative permittivity values for the FLCs has been determined at selective frequencies between 1 kHz and 20 *** dielectric anomaly/fall for the response function infers the involvement of collective response of dipolar assembly confined to ferroelectric Weiss domains.

Synthesis and characterization of organomodified Na-MMT using cation and anion surfactants

The Sodium Montmorillonite(Na-MMT)is not susceptible to polymer due to its organophilic character and low basal *** primary objective of this study was to improve Na-MMT platelets separation by organically modifying it with cation and anion exchanges using Cetyl trimethyl ammonium bromide(CTAB)and Sodium dodecyl sulfate(SDS)*** spacing,presence of functional groups,Zetapotential with particle size analysis and thermal stability of the Organomodified Na-MMT(OMMT)were characterized using XrD,FTIr,zeta-potential analyzer and TGA *** basal spacing of CTAB modified OMMTincreased to 19.5Åfrom 11.0Åwhich corresponds to the basal spacing of *** SDS modified OMMT did not show any increase in the basal *** spectra of CTAB modified Na-MMT illustrated the attachment of CTAB functional groups to Na-MMT,while the same was absent in the case of SDS modified *** zeta-potential of Na-MMT shifted from–24.88 mV to 15.66 mV in the case of CTAB modified Na-MMTand 12.49 mV for SDS modified Na-MMT,indicating a greater surface potential of the modified *** TGA showed greater weight loss for CTAB modified Na-MMT than that for Na-MMT,indicating the effective Na+ion exchange with alkyl amines.

Duplication of Urethra with Primary Vesico-Ureteric reflux in Solitary Kidney: A rare Case report

Uethral duplication is very rare congenital anomaly with ill defined etiology. Very few cases have been reported in literature till date. Patients may present with urinary incontinence, abnormal or dual urinary stream, recurrent urinary tract infections and sometimes associated penile deformity. Aim of presenting this case report is to limelight one of such rare presentations. Herewith reporting a case of 16 months old baby boy presented with history of recurrent urinary tract infection, solitary kidney with grade 4 primary vesico-ureteric reflux and duplication of urethra. Diagnosis and treatment plan in such cases require a multistage approach.
