Tumor characteristics of hepatocellular carcinoma after direct-acting antiviral treatment for hepatitis C: Comparative analysis with antiviral therapy-naive patients

BACKGROUND Insufficient and contradictory data are available about the relation between directacting antivirals(DAAs)and hepatocellular carcinoma(HCC)development in patients with hepatitis C virus(HCV).AIM To analyze differences in basic clinical,radiological,and laboratory characteristics in addition to tumor behavior upon HCC diagnosis between patients with and without a previous history of DAAs exposure.METHODS This multicenter case-control study included 497 patients with chronic HCVrelated HCC,allocated into one of two groups according to their history of antiviral treatment for their HCV.RESULTS Group I included 151 HCC patients with a history of DAAs,while 346 patients who had never been treated with DAAs were assigned to group II.A significant difference was observed between both groups regarding basic assessment scores(Child,MelD,and BCLC),which tended to have more advanced liver disease and HCC stage upon diagnosis in group I.However,serum albumin was significantly affected,and serumα-fetoprotein was significantly higher in group II(P<0.001).In addition,group I showed significant HCC multicentricity than group II,while the incidence of portal vein thrombosis was significantly higher in group I(P<0.001).CONCLUSION The basic clinical scores and laboratory characteristics of HCC patients are advanced in patients who are naïve to DAAs treatment;however,HCC behavior is more aggressive in DAA-treated patients.

This article reports the effect of ageing on the microstructure, martensitic transformation, magnetic properties, and mechanical properties of Ni51FelsGa27Ti4 shape memory alloy. There are five specimens of this alloy aged at 573 up to 973 K for 3 h per each. This range of ageing temperature greatly affects the microstructure of the alloy. As the ageing temperature increased from 573 up to 973 K, the microstructure of Ni51FelsGa27Ti4 alloy gradually changed from the entirely martensitic matrix at 573 K to the fully austenitic microstructure at 973 K. The volume fraction of precipi- tated Ni3Ti particles increased with the ageing temperature increasing from 573 to 773 K. Further increasing the ageing temperature to 973 K decreased the content of Ni3Ti in the microstructure. The martensitic transformation tempera- ture was decreased steadily by increasing the ageing temperature. The magnetization saturation, remnant magnetization, and coercivity increased with the ageing temperature increasing up to 773 K. A further increase in ageing temperature decreased these raagnetic properties. Moreover, the hardness values were gradually increased at first by increasing the ageing temperature to 773 K, and then dramatically decreased to the lowest value at 973 K.

Drought stress has become more common in recent years as a result of climate change impacts on the production of banana crops and other fruit trees.The growth and productivity of Musa spp are severely impacted by the gradual degradation of water resources and the erratic distribution pattern of annual precipitation amount.The aim of the work includes increased drought tolerance in light of water scarcity in the world as a result of the bananas’being gluttonous for water needs.This investigation was carried out from 2019 to 2020 to study the effect of potassium silicate on morphological growth and biochemical parameters of Musa acuminata L under drought stress by PEG.As a result,drought stress reduced the morphological characteristics such as shoots number,shoot length,roots number,and survival percentage and biochemical characteristics such as chlorophyll a,b,carotenoids,stomatal status,and RWC.While proline content increased in the leaf of M.acuminata L.Media complemented with K2SiO3(2 to 6 mM)either individually or in combination with PEG led to an improvement in all morphological and biochemical characteristics.The activities of CAT,POD,and PPO enzymes increased significantly compared to control.Furthermore,the lowest PPO,CAT,and POD activity were achieved.Additionally,K2SiO3 treatments under drought stress successfully enhanced the leaf stomatal behavior.Our results suggest that K2SiO3 can help to maintain plant integrity in the tested cultivar under drought stress.

Irritable bowel syndrome(IBS)is a common gastrointestinal disorder that is generally considered to be functional because there appears to be no associated anatomical defect.Stress and psychological factors are thought to play an important role in IBS.The gut neuroendocrine system(NES),which regulates all functions of the gastrointestinal tract,consists of endocrine cells that are scattered among the epithelial cells of the mucosa,and the enteric nervous system.Although it is capable of operating independently from the centra nervous system(CNS),the gut NES is connected to and modulated by the CNS.This review presents evidence for the presence of an anatomical defect in IBS patients,namely in the gastrointestinal endocrine cells.These cells have specialized microvilli that project into the lumen and function as sensors for the luminal content and respond to luminal stimuli by releasing hormones into the lamina propria,which starts a chain reaction that progresses throughout the entire NES.The changes in the gastrointestinal endocrine cells observed in IBS patients are highly consistent with the other abnormalities reported in IBS patients,such as visceral hypersensitivity,dysmotility,and abnormal secretion.

BACKGROUND Faecal microbiota transplantation(FMT)seems to be a promising treatment for irritable bowel syndrome(IBS)patients.In Western countries(United States and Europe),there is a female predominance in IBS.A sex difference in the response to FMT has been reported recently in IBS patients.AIM To investigate whether there was a sex difference in the response to FMT in the IBS patients who were included in our previous randomized controlled trial of the efficacy of FMT.METHODS The study included 164 IBS patients who participated in our previous randomized controlled trial.These patients had moderate-to-severe IBS symptoms belonging to the IBS-D(diarrhoea-predominant),IBS-C(constipation-predominant)and IBSM(mixed)subtypes,and had not responded to the National Institute for Health and Care Excellence(NICE)-modified diet.They belonged in three groups:placebo(own faeces),and active treated group(30-g or 60-g superdonor faeces).The patients completed the IBS severity scoring system(IBS-SSS),Fatigue Assessment Scale(FAS)and the IBS quality of life scale(IBS-QoL)questionnaires at the baseline and 2 wk,1 mo and 3 mo after FMT.They also provided faecal samples at the baseline and 1 mo after FMT.The faecal bacteria profile and dysbiosis were determined using the 16S rRNA gene polymerase chain reaction DNA amplification covering V3-V9;probe labelling by single nucleotide extension and signal detection.The levels of short-chain fatty acids(SCFAs)were determined by gas chromatography and flame ionization.RESULTS There was no sex difference in the response to FMT either in the placebo group or active treated group.There was no difference between females and males in either the placebo group or actively treated groups in the total score on the IBS-SSS,FAS or IBS-QoL,in dysbiosis,or in the faecal bacteria or SCFA level.However,the response rate was significantly higher in females with diarrhoea-predominant(IBS-D)than that of males at 1 mo,and 3 mo after FMT.Moreover,IBS-SSS total score was significantly lower in female patients

Inflammatory bowel disease(IBD) is a chronic recurrent condition whose etiology is unknown,and it includes ulcerative colitis,Crohn's disease,and microscopic colitis. These three diseases differ in clinical manifestations,courses,and prognoses. IBD reduces the patients' quality of life and is an economic burden to both the patients and society. Interactions between the gastrointestinal(GI) neuroendocrine peptides/amines(NEPA) and the immune system are believed to play an important role in the pathophysiology of IBD. Moreover,the interaction between GI NEPA and intestinal microbiota appears to play also a pivotal role in the pathophysiology of IBD. This review summarizes the available data on GI NEPA in IBD,and speculates on their possible role in the pathophysiology and the potential use of this information when developing treatments. GI NEPA serotonin,the neuropeptide Y family,and substance P are proinflammatory,while the chromogranin/secretogranin family,vasoactive intestinal peptide,somatostatin,and ghrelin are antiinflammatory. Several innate and adaptive immune cells express these NEPA and/or have receptors to them. The GI NEPA are affected in patients with IBD and in animal models of human IBD. The GI NEPA are potentially useful for the diagnosis and follow-up of the activity of IBD,and are candidate targets for treatments of this disease.

SOX9 in biliary atresia: New insight for fibrosis progression

Background:Liver fibrosis is a hallmark determinant of morbidity in biliary atresia(BA)even in successfully operated cases.Responsible factors for this rapid progression of fibrosis are not completely defined.Aberrant expression of the transcription factor SOX9 and hepatic progenitor cells(HPCs)proliferation have roles in fibrogenesis in cholestatic disorders.However,they were not investigated sufficiently in BA.We aimed to delineate the relation of SOX9 and HPCs to fibrosis and its progression in BA.Methods:Forty-eight patients with BA who underwent an initial diagnostic liver biopsy(LB)and consequent intraoperative LB were recruited and compared to 28 cases with non-BA cholestasis that had an LB in their diagnostic workup.Liver fibrosis,tissue SOX9 and HPC expressions were studied in both BA and non-BA-cholestasis cases.Liver fibrosis,SOX9,and HPCs’dynamic changes in BA cases were assessed.Relation of fibrosis and its progression to SOX9 and HPCs in BA was assessed.Results:SOX9 and HPCs in ductular reaction(DR)form were expressed in 100%of BA and their grades increased significantly in the second biopsy.The rapidly progressive fibrosis in BA,represented by fibrosis grade of the intraoperative LB,correlated significantly to SOX9-DR and HPC-DR at the diagnostic(r=0.420,P=0.003 and r=0.405,P=0.004,respectively)and the intraoperative(r=0.460,P=0.001 and r=0.467,P=0.001,respectively)biopsy.On the other hand,fibrosis,SOX9-DR,and HPC-DR were significantly lower in non-BA cases at a comparable age(Pely).Conclusions:Fibrosis in BA is rapidly progressive within a short time and is significantly correlated to SOX9 and HPCs.Assessment of targeting SOX9 and HPCs on fibrosis progression is warranted.

Irritable bowel syndrome(IBS) is a common gastrointestinal disorder, the pathophysiology of which is not completely known, although it has been shown that genetic/social learning factors, diet, intestinal microbiota, intestinal low-grade inflammation, and abnormal gastrointestinal endocrine cells play a major role. Studies of familial aggregation and on twins have confirmed the heritability of IBS. However, the proposed IBS risk genes are thus far nonvalidated hits rather than true predisposing factors. There is no convincing evidence that IBS patients suffer from food allergy/intolerance, with the effect exerted by diet seemingly caused by intake of poorly absorbed carbohydrates and fiber. Obesity is a possible comorbidity of IBS. Differences in the microbiota between IBS patients and healthy controls have been reported, but the association between IBS symptoms and specific bacterial species is uncertain. Low-grade inflammation appears to play a role in the pathophysiology of a major subset of IBS, namely postinfectious IBS. The density of intestinal endocrine cells is reduced in patients with IBS, possibly as a result of genetic factors, diet, intestinal microbiota, and low-grade inflammation interfering with the regulatory signals controlling the intestinal stem-cell clonogenic and differentiation activities. Furthermore, there is speculation that this decreased number of endocrine cells is responsible for the visceral hypersensitivity, disturbed gastrointestinal motility, and abnormal gut secretion seen in IBS patients.

This article attempts to establish a mathematical epidemic model for the outbreak of the new COVID-19 coronavirus.A new consideration for evaluating and controlling the COVID-19 outbreak will be constructed based on the SEIQR Pandemic Model.In this paper,the real data of COVID-19 spread in Saudi Arabia has been used for the mathematical model and dynamic analyses.Including the new reproductive number and detailed stability analysis,the dynamics of the proposed SEIQR model have been applied.The local sensitivity of the reproduction number has been analyzed.The domain of solution and equilibrium based on the SEIQR model have been proved using a Jacobian linearization process.The state of equilibrium and its significance have been proved,and a study of the integrity of the disease-free equilibrium has been carried out.The Lyapunov stability theorem demonstrated the global stability of the current model equilibrium.The SEIQR model has been numerically validated and projected by contrasting the results from the SEIQR model with the actual COVID-19 spread data in Saudi Arabia.The result of this paper shows that the SEIQR model is a model that is effective in analyzing epidemic spread,such as COVID-19.At the end of the study,we have implemented the protocol which helped the Saudi population to stop the spread of COVID-19 rapidly.
