Efficacy and sustainability of dialectical behaviour therapy for inpatient adolescents:a follow-up study

Background Dialectical behaviour therapy(DBT)is an evidence-based treatment for adolescents targeting suicidal and non-suicidal self-injurious behaviours.Research supports DBT’s efficacy in inpatient settings,but implementation and sustainability are understudied.Aims This study is a follow-up of a previous study by Tebbett-Mock et al and examines the efficacy and sustainability of an adolescent DBT inpatient unit within a psychiatric hospital in the Northeast.We hypothesised that adolescents who received DBT in our follow-up group(DBT Group 2)would not have statistical difference(ie,greater or fewer)of the following compared with the first group of patients who received DBT on the unit the year prior(DBT Group 1)and would have significantly fewer of the following compared with the treatment as usual(TAU)group:(1)constant observation hours for suicidal ideation,self-injury and aggression;(2)incidents of suicide attempts,self-injury and aggression;(3)restraints;(4)seclusions;(5)days hospitalised;(6)times readmitted to the unit within 30 days of discharge.Methods We conducted a retrospective chart review for adolescents receiving inpatient DBT(DBT Group 1,n=425;DBT Group 2,n=393)and a historical control group(TAU,n=376).The χ^(2) tests and one-way analysis of variance were conducted as preliminary analyses to examine group differences on diagnosis,gender and age.Kruskal-Wallis H tests were conducted to examine group differences on outcomes.Mann-Whitney U tests were used as post hoc analyses.Results Patients in DBT Group 2 were comparable to DBT Group 1 for the number of constant observation hours for self-injury(U=83432.50,p=0.901),restraints(U=82109,p=0.171)and days hospitalised(U=83438.5,p=0.956).Patients in DBT Group 2 had a significantly greater number of incidents of suicide attempts compared with DBT Group 1(U=82662.5,p=0.037)and of self-injury compared with patients in DBT Group 1(U=71724.5,p<0.001)and TAU(U=65649.0,p<0.001).Conclusions Results provide support for adolescent inpatient DBT compared with TAU a

Background:Arthrofibrosis is a joint disorder characterized by excessive scar formation in the joint tissues.Vitamin E is an antioxidant with potential anti-fibroblastic effect.The aim of this study was to establish an arthrofibrosis rat model after joint replacement and assess the effects of vitamin E supplementation on joint fibrosis.Methods:We simulated knee replacement in 16 male Sprague–Dawley rats.We immobilized the surgical leg with a suture in full flexion.The control groups were killed at 2 and 12 weeks(n=5 per group),and the test group was supplemented daily with vitamin E(0.2 mg/mL)in their drinking water for 12 weeks(n=6).We performed histological staining to investigate the presence and severity of arthrofibrosis.Immunofluorescent staining andα2-macroglobulin(α2M)enzyme-linked immunosorbent assay(ELISA)were used to assess local and systemic inflammation.Static weight bearing(total internal reflection)and range of motion(ROM)were collected for functional assessment.Results:The ROM and weight-bearing symmetry decreased after the procedure and recovered slowly with still significant deficit at the end of the study for both groups.Histological analysis confirmed fibrosis in both lateral and posterior periarticular tissue.Vitamin E supplementation showed a moderate anti-inflammatory effect on the local and systemic levels.The vitamin E group exhibited significant improvement in ROM and weight-bearing symmetry at day 84 compared to the control group.Conclusions:This model is viable for simulating arthrofibrosis after joint replacement.Vitamin E may benefit postsurgical arthrofibrosis,and further studies are needed for dosing requirements.

Long-term physical,mental and social health effects of COVID-19 in the pediatric population:a scoping review

Background The majority of coronavirus disease 2019(COVID-19)symptom presentations in adults and children appear to run their course within a couple of weeks.However,a subgroup of adults has started to emerge with effects lasting several months or more after initial infection,which raises questions about the long-term physical,mental and social health effects of COVID-19 in the pediatric population.The purpose of this review was to determine these impacts well into the second year of the pandemic.Methods A search was conducted using PubMed,Web of Science,Science Direct,and Cochrane between 11/1/2019 and 9/1/2021.Search inclusion criteria were as follows:(1)COVID-19 illness and symptoms in children;(2)severe acute respiratory syndrome coronavirus 2 in children;(3)English language;and(4)human studies only.Results The few studies that have documented long-term physical symptoms in children show that fatigue,difficulty in concentrating(brain fog),sleep disturbances,and sensory problems are the most reported outcomes.Most studies examining the impact of COVID-19 in pediatric populations have focused on initial clinical presentation,and symptoms,which are similar to those in adult populations.In addition,COVID-19 has had a moderate impact on children and adolescents’social environment,which may exacerbate current and future physiological,psychological,behavioral,and academic outcomes.Conclusions There are limited studies reporting long physical symptoms of COVID-19 in the pediatric population.However,pediatric COVID-19 cases are underreported due to low rates of testing and symptomatic infection,which calls for more longitudinal studies.Children who have experienced COVID-19 illness should be monitored for long physiological,psychological,behavioral,and academic outcomes.

Anemia in pregnancy is a significant event that often requires treatment to prevent long term effects in the mother and the fetus.Many factors determine which iron preparation to use,including route and ease of administration,patient tolerability,safety,and cost.This case report summarizes two separate patient cases where sodium ferric gluconate complex was administered intravenously to pregnant women,resulting in fetal bradycardia and emergent cesarean procedures.

Oxidative stress in retinal pigment epithelium degeneration:from pathogenesis to therapeutic targets in dry age-related macular degeneration

Age-related macular degeneration is a primary cause of blindness in the older adult population. Past decades of research in the pathophysiology of the disease have resulted in breakthroughs in the form of anti-vascular endothelial growth factor therapies against neovascular age-related macular degeneration;however, effective treatment is not yet available for geographical atrophy in dry agerelated macular degeneration or for preventing the progression from early or mid to the late stage of age-related macular degeneration. Both clinical and experimental investigations involving human agerelated macular degeneration retinas and animal models point towards the atrophic alterations in retinal pigment epithelium as a key feature in age-related macular degeneration progression. Retinal pigment epithelium cells are primarily responsible for cellular-structural maintenance and nutrition supply to keep photoreceptors healthy and functional. The retinal pigment epithelium constantly endures a highly oxidative environment that is balanced with a cascade of antioxidant enzyme systems regulated by nuclear factor erythroid-2-related factor 2 as a main redox sensing transcription factor. Aging and accumulated oxidative stress triggers retinal pigment epithelium dysfunction and eventually death. Exposure to both environmental and genetic factors aggravates oxidative stress damage in aging retinal pigment epithelium and accelerates retinal pigment epithelium degeneration in age-related macular degeneration pathophysiology. The present review summarizes the role of oxidative stress in retinal pigment epithelium degeneration, with potential impacts from both genetic and environmental factors in age-related macular degeneration development and progression. Potential strategies to counter retinal pigment epithelium damage and protect the retinal pigment epithelium through enhancing its antioxidant capacity are also discussed, focusing on existing antioxidant nutritional supplementation, and exploring nuclear factor erythroid-2-relat

Facile synthesis of Pd concave nanocubes： From kinetics to mechanistic understanding and rationally designed protocol

We report a rationally designed one-pot method for the facile synthesis of Pd concave nanocubes in an aqueous solution at room temperature by manipulating the reduction kinetics through the selection of a proper combination of a salt precursor （PdBr4^2-） and reductant （sodium ascorbate）. Our kinetic analysis demonstrates that, through this selection, the nucleation and growth of Pd nanocrystals could be effectively separated into two kinetic regimes involving distinctive reduction pathways： i） solution reduction for the initial formation of single-crystal seeds and ii） surface reduction for the formation of concave nanocrystals via autocatalytic growth from the single-crystal seeds. The suppressed surface diffusion at room temperature, when coupled with the capping effect of bromide ions, ultimately leads to the formation of concave nanocubes with an asymmetric shape and high-index facets, whose synthesis would otherwise require multiple steps and the use of elevated temperatures.

Introduction:Multicentric reticulohistiocytosis(MRH)is a rare disease that is known to affect the skin and joints,primarily.It is considered a rare form of non-Langerhans cell histiocytosis(Group C)that can cause destructive inflammatory arthritis involving both the small and large joints.Cutaneous eruptions of periungual,“coral beads”and nodules appearing over the distal fingers are considered pathognomonic clues for identifying this disorder.Histology evaluation of the cutaneous papules typically shows infiltrative histiocytes and multinucleated giant cells.Although no well-established therapies exist to date,a variety of immunosuppressants have been used with varying degrees of success.Case Description:A 53-year-old Caucasian female patient with a family history of rheumatoid arthritis and a personal history of Sjogren's syndrome presented to the rheumatology clinic complaining of pain in her bilateral hands and fingers.There were several small,papulo-nodular lesions ranging from 1 to 2mm in size noted at the base of her nails.A 4mm punch biopsy of one of the papules from the neck showed dermal infiltration of eosinophilic mononucleated and multinucleated giant cells with“ground glass”appearing cytoplasm consistent with MRH.X-ray of her hands showed periarticular demineralization and erosions surrounding several bilateral proximal and distal interphalangeal joints,and thus tofacitinib in addition to methotrexate,hydroxychloroquine,and dexamethasone(dosed weekly)was started to help control any further articular damage.Conclusion:Our aim is to further support the relation of MRH with autoimmune diseases,including Sjogren's syndrome.Autoimmune diseases have been reported in association with MRH,although a clear association has yet to be made.

The relationship between chronic PTSD,cortical volumetry and white matter microstructure among Australian combat veterans

Background:Posttraumatic stress disorder(PTSD)has been associated with volumetric and white matter microstructural changes among general and veteran populations.However,regions implicated have greatly varied and often conflict between studies,potentially due to confounding comorbidities within samples.This study compared grey matter volume and white matter microstructure among Australian combat veterans with and without a lifetime diagnosis of PTSD,in a homogenous sample assessed for known confounding comorbidities.Methods:Sixty-eight male trauma-exposed veterans(16 PTSD-diagnosed;mean age 69 years)completed a battery of psychometric assessments and underwent magnetic resonance and diffusion tensor imaging.Analyses included tractbased spatial statistics,voxel-wise analyses,diffusion connectome-based group-wise analysis,and volumetric analysis.Results:Significantly smaller grey matter volumes were observed in the left prefrontal cortex(P=0.026),bilateral middle frontal gyrus(P=0.021),and left anterior insula(P=0.048)in the PTSD group compared to controls.Significant negative correlations were found between PTSD symptom severity and fractional anisotropy values in the left corticospinal tract(R^(2)=0.34,P=0.024)and left inferior cerebellar peduncle(R^(2)=0.62,P=0.016).No connectome-based differences in white matter properties were observed.Conclusions:Findings from this study reinforce reports of white matter alterations,as indicated by reduced fractional anisotropy values,in relation to PTSD symptom severity,as well as patterns of reduced volume in the prefrontal cortex.These results contribute to the developing profile of neuroanatomical differences uniquely attributable to veterans who suffer from chronic PTSD.
