Objectives: The finding of new prognostic factors in human cervix cancer is necessary to improve present conventional treatments. The aim of the present study was to determine the expression and evaluate the prognostic value of hypoxia-inducible factor-1(HIF-1α), vascular endothelial growth factor (VEGF) and eritropoyetin receptor (EpoR) in cervix cancer stages Iia-IIB and in preinvasive high grade squamous intraepithelial lesions (HSIL) Methods: The study included 70 patients with cervix cancer, FIGO stages Iia-IIB, 28 patients with HSIL and normal cervix (n = 28). HIF-1α, VEGF and EpoR expression were analyzed in tissue samples by immunohistochemistry using commercial antibodies. Expression and overexpression of the tumor markers were quantified according to German Immunoreactive Score. Results: HIF-1α, EpoR and VEGF overexpression was detected in 30%, 37% and 51% of cancer patients respectively. Patients with HSIL showed enhanced expression only of EpoR and VEGF (39.2% and 71.4%) while VEGF was overexpressed in 21% of the specimen. No correlation was found between VEGF and EpoR with disease-free overall survival (OS), tumor recurrences or prognostic factors. Only overexpression of HIF-1 was associated with less median survival measured up to 24 months, unless it was not maintained a long time. Conclusion: Although any of the markers could be considered as independent prognostic factor for cervix cancer patients, our data showed a significant increase in their expression from the premalignant lesion up to the invasive stages of tumor progression.

Evaluation of Deformable Image Registration and Dose Accumulation Using Histogram matching Algorithm between kVCT and MVCT with Helical Tomotherapy

Purpose: To evaluate the accuracy of deformable image registration (DIR) between the planning kVCT (pCT) and the daily MVCT combined with the histogram matching (HM) algorithm, and evaluate the deformable dose accumulation using a suggested method for adaptive radiotherapy with Helical Tomotharapy (HT). Methods: For five prostate cancer patients (76 Gy/38 Fr) treated with HT in our institution, seven MVCT series (a total of 35 series) acquired weekly were investigated. First, to minimize the effect of different HU values between pCT and MVCT, this image-processing method adjusts HU values between pCT and MVCT images by using image cumulative histograms of HU values, generating an HM-MVCT. Then, the DIR of the pCT to the HM-MVCT was performed, generating a deformed pCT. Finally, deformable dose accumulation was performed toward the pCT image. Results: The accuracy of DIR was significantly improved by using the HM algorithm, compared with non-HM method for several structures (p ±0.05, 0.83 ±0.06, and 0.90 ± 0.04 for the CTV, rectum, and bladder, respectively, while that of the HM method was 0.81 ±0.06, 0.81 ±0.04, and 0.92 ±0.06, respectively. For the deformable dose accumulation, some difference was observed between the two methods, particularly for the small calculated regions, such as rectum V60 and V70. Conclusion: Adapting the HM method can improve the accuracy of DIR. Furthermore, dose calculation using the deformed pCT using HM methods can be an effective tool for adaptive radiotherapy.

作为标准熔炉设计的一部分,使用矿物燃料并且燃烧气氛可控制的辐射管式熔炉技术已经发展了至少60年.而Hauck制造公司自20世纪50年代起就已经研究并生产辐射管烧嘴(见图1、图2).目前,对于使用矿物燃料的辐射管,常见问题有:燃烧产物的热量利用率低,由于辐射管的破裂,导致管内燃烧产物进入炉腔破坏炉内气氛,所以使辐射管表面热分布均匀性不好;就使用的烧嘴特点看,快速混合(燃料与助燃空气)烧嘴易使辐射管热梯度过高而缩短烧嘴使用寿命;而慢速混合(燃料与助燃空气)烧嘴则易导致燃烧尾气中CO浓度过高,当要求燃烧完全时就不得不使用大量的过量空气.

Human Embryo Neuronal Culture <i>in Vitro</i>: A Model to Study Cellular Physiology, Receptors, Power and Toxicity of Cytostatic Drugs for Human Use

Neural cells cultures from human embryo brain of 9° - 11°W gestational age have been used to study ERα (Estrogens Receptor α) and to perform toxicity test for Mitomycin C and Methotrexate. Histochemical confirmation of cellular neuronal phenotype was based on histochemical evidence of NSE (Neuron Specific Enolase).The detection of ERα in neuronal cells was performed with a rabbit Monoclonal Antibody. ERα was absent both on neurons grown in vitro and on tissue brain specimens. This finding is apparently in contrast with the positive immunoreactivity of ERα and ERβ reported by other Authors on foetal and adult CNS (Central Nervous System). The absence of nuclear ERα on neurons in culture and in brain tissue specimens in our experiment is not in contrast with the relevant physiologic role of estrogens on nervous central system, but it could be correlated to the embryonic period of life and could represent a protection of male brain from an undue estrogens imprinting. The mitomycin C, alkylation agent, has shown in our experiment a major neurotoxic and cytostatic power in comparison with methotrexate. Our conclusion is that human embryo neuronal culture in vitro is a powerful instrument for physiology and human therapy for cancer and neurodegenerative diseases.

Clinical Predictors of Appropriate Implantable Cardioverter-Defibrillator Therapies in Primary Prevention: A Retrospective Study

Implantable cardioverter-defibrillator (ICD) in heart failure with reduced ejection fraction (EF) patients reduces risk for sudden cardiac death (SCD). Previous data suggest that the benefit of ICD therapy in real life may be lower than expected from the results of controlled studies and only about one-third of ICD patients receive appropriate therapies. Nevertheless, all ICD patients are at risk of perioperative complications and inappropriate shocks. We retrospectively studied 613 patients undergoing ICD for primary prevention in 2002-2015;we excluded inherited arrhythmogenic syndromes. Patients underwent 12-leads ECG, echocardiography, laboratory tests and quality of life questionnaire. We evaluated comorbidities, appropriate therapies, complications and all-cause mortality. Consecutive patients (age 67 ± 10 years, 81% males, 59% ischaemic aetiology) were followed for 51 ± 31 months. 198 patients (32%) received appropriate ICD therapy, 93 (15%) had inappropriate shocks, 53 (8%) had at least one complication (electrode dysfunction, infection and pocket related) and 191 (33%) died. Multivariate analysis showed atrial fibrillation (OR = 1.8, CI = 1.27 - 2.53;p iabetes (OR = 1.8, CI = 1.27 - 2.53;p = 0.041) and vasculopathy (OR = 1.8, CI = 1.27 - 2.53;p = 0.031) as predictors of appropriate therapy. Logistic regression, considering atrial fibrillation, diabetes, vasculopathy, EF, NYHA class, left atrial diameter and natremia, identified SCD low risk group (probability ias necessitating ICD therapy are common, but complications and inappropriate therapies are frequent. many parameters should be considered for a better selection of ICD candidates, to reduce ineffective implants. Our multifactorial score may eventually reduce about 10% ICD implantation.
