Investigation on reversible pump turbine flow structures and associated pressure field characteristics under different guide vane openings

The use of reversible pump turbines(RPT) within pumped storage power plants goes with prolonged periods of off-design operating conditions, which leads to the onset of operating mode-dependent instabilities. In order to decrease the gravity of RPT flow instabilities and associated damages or even completely eliminate them, a deep understanding of its onset and development mechanism is needed. In line with this, the present study seeks to numerically investigate the onset and development mechanism of RPT unsteady flow structures as well as the evolutional characteristics of associated pressure pulsations throughout the RPT complete flow passage, under off-design conditions for three GVOs namely 17, 21, and 25 mm. The study results showed that low torque operating conditions and associated vaneless space back flow structures were the trigger of flow unsteadiness onset within the RPT vaneless space, the instabilities which grew to cause the S-shape characteristics appearance. Moreover, the runner flow unsteadiness was found to decrease with the GVO increase. On the other hand, the GVO increase worsened the pressure pulsation levels within RPT flow zones, where pressure pulsations within the vaneless space and flow zones in its vicinities were found to be the most sensitive to GVO changes.

Do ankle braces provide similar effects on ankle biomechanical variables in subjects with and without chronic ankle instability during landing?

Purpose:The purpose of this study was to examine effects of a sport version of a semi-rigid ankle brace(ElementTM) and a soft ankle brace （ASO） on ankle biomechanics and ground reaction forces（GRFs） during a drop landing activity in subjects with chronic ankle instability(CAD compared to healthy subjects with no history of CAI. Methods:Ten healthy subjects and 10 subjects who had multiple ankle sprains participated in the study as the control and unstable subjects, *** CAI subjects were age,body mass index and gender matched with the control *** arch index and ankle functions of the subjects were measured in a subject screening *** the biomechanical test session,participants performed five trials of drop landing from 0.6 m,wearing no brace（NB）.Element? brace and ASO *** recording of three-dimensional kinematic（240 Hz） and GRF（1200 Hz） data were performed. results:The CAI subjects had lower ankle functional survey *** arch index and deformity results showed greater arch deformity of ElementTM against a static load than in NB and ASO due to greater initial arch position held by the *** participants had greater eversion velocity than healthy *** ASO brace reduced the first peak vertical GRF whereas ElementTM increased 2nd peak vertical GRF. ElementTMbrace reduced eversion range of motion（ROM） and peak eversion velocity compared to NB and *** addition,ElementTM reduced dorsiflexion ROM and increased peak plantarflexion moment compared to NB and ASO. Conclusion:results of static arch measurements and dynamic ankle motion suggest that the restrictions offered by both braces are in part due to more dorsiflexed ankle positions at contact,and higher initial arch position and stiffer ankle for ElementTM.

Epidemiological, Clinical and Therapeutic Aspects of Childhood Eye Diseases at the Prefectural Hospital of Macenta in Guinea

Introduction: Eye health in children is a real public health problem in developing countries. The aim of this work was to determine the frequency of ocular pathologies in children aged 0 to 16 at the prefectural hospital of Macenta in Guinea. Patients and Method: This was a descriptive prospective study. It covered a period of 6 months from October 1 to March 31, 2022 at the prefectural hospital of Macenta in Guinea. Children aged 0 to 16 received during the study period were included. Were excluded, those with an incomplete file and whose parents did not consent. results: Out of 1816 consultations, 224 children aged 0 to 16 were collected 7.95%;mean age 7.35 years ± 4.6;sex ratio 1.38. The age group of 6 to 11 years was more represented. More than half of the patients were educated 50.4% and came from rural areas 52.7%. Pathologies of the orbit were dominated by orbital cellulitis 13% and those of the annexes by conjunctivitis 40.6%. Keratitis and cataracts were pathologies dominating the cornea and the lens, respectively 5.4% and 8.5%. Atrophy of the eyeball was the most frequent among those of the eyeball, at 3.6%. The most performed surgical procedure was phacophagy in 6.7%. The most common reason for consultation was eye pain, at 33.5%. Both eyes were affected at the same time in the majority 49.5%, and the most represented antecedent was malaria 22.3% of cases. Visual acuity without correction ≥ 3/10 was the most represented 37.5% in the right eye and 34.8% in the left eye. The therapeutic classes used were dominated by antibiotic-corticoid combinations 88.8%. Conclusion: Ocular pathologies in children from 0 to 16 years old constitute a real public health problem. They are dominated by pathologies of the conjunctiva and the lens. Popularization for early management of ophthalmological pathologies would be beneficial. .

Hepatocellular carcinoma recurrence after acute liver allograft rejection treatment: A multicenter European experience

Background:During the last decades,several risk factors for the recurrence of hepatocellular carcinoma(HCC)after liver transplantation(LT)have been ***,the impact of two important drivers of oncogenesis,namely the immunosuppression and the treatment of acute cellular rejection(ACR)have been marginally *** study aimed at investigating the impact of ACR treatment on the incidence of tumor recurrence in a large European HCC-LT ***:Seven hundred and eighty-one adult patients transplanted between February 1,1985 and June 30,2016 were retrospectively *** propensity score match,116 patients treated for ACR using steroid boluses were compared with 115 patients who did not present any ACR or a histologic but clinical irrelevant ***:Steroid boluses treated patients had a 18-fold higher overall incidence of HCC recurrence than those non-treated patients(16.4%vs.0.9%;Pregression analysis,steroid boluses used to treat ACR were an independent risk factor for HCC recurrence(HR=14.2;95%CI:1.8–110.4;P=0.010).Conclusions:The decision to treat ACR as well as to reinforce immunosuppression load should be cautiously taken in view of the presented *** studies are needed to further elucidate the clinical impact of immunosuppression on HCC recurrence after transplantation.

Hemodynamic monitoring and management of patients undergoing high-risk surgery:a survey among Chinese anesthesiologists

Why Do Pregnant Women Choose a Planned Caesarean Section in Burkina Faso? A Qualitative Study

Despite the complications related to the caesarean section, some couples choose it by desirability. This study aimed to understand the reasons for maternal requests for a planned caesarean section in Burkina Faso, in the absence of obstetric indications. A qualitative descriptive study was conducted in March 2019, consisting of twelve semi-structured interviews with women requesting caesarean section at Tingandogo Teaching Hospital in Burkina Faso. The operative reports were also examined. Thematic analysis based on Braun and Clarke approach was performed with QDA Miner software. Twelve patients were involved in the study. The mean age was 33 years. The majority had a higher level of education. Half were employees. The mean stay duration was 72 hours, with an estimated cost of CFA 300.000 (457,347 EUR). The choice of a planned caesarean was motivated by fear of birth (tokophobia), the positive feeling associated to the surgical technology, the doctor-patient relationship and the confidence in the quality of healthcare services such as reception and hygiene. Maternal reasons for requesting a planned caesarean section in Burkina Faso are multifactorial. Information, education and communication and shared decision-making strategies concerning the different modes of delivery and complications at the end of pregnancy, may contribute to reducing this problem.

Meta-Analysis Study of the Effects of Yeast Probiotic Supplementation on Milk Production and Energy Corrected Milk of Lactating Dairy Cows

The aim of this study was to use a meta-analytic approach to evaluate the effect of commercially available yeast probiotic “Actisaf® Sc 47” (Saccharomyces cerevisiae CNCM I-4407) produced and marketed by Phileo by Lesaffre on milk performance in dairy cows. Data from 22 trials including 17 with parallel designs and 5 with cross-over designs were collected, and only data with parallel designs were analyzed. From those trials, 4 are published and 13 are from technical reports. In total, 34 comparisons and 1074 dairy cows met the criteria for inclusion in the final analysis of milk yield (MY). For energy corrected milk (ECM), six trials with 12 comparisons and 476 dairy cows met the criteria for inclusion in the final analysis. Because the data are from different trials with different conditions, the statistical model defined includes the fixed effect of the treatment (with vs. without Actisaf® Sc 47) and the random effect of the trial. The meta-analysis showed a moderate heterogeneity for MY and ECM. The random effect meta-analysis showed an estimated mean difference +1.72 kg/d [95% confidence interval (CI): 1.01 to 2.44] and +2.45 kg/d (95% CI: 1.73 to 3.17) for MY and ECM respectively, in favour of Actisaf® Sc 47. The analysis of data without trials conducted under heat stress conditions showed positive effect of Actisaf® Sc 47. The random effect meta-analysis showed an estimated mean difference of +1.69 kg/d [95% CI: 1.24 to 2.14] and +2.92 kg/d (95% CI: 2.45 to 3.40) for MY and ECM respectively, in favour of Actisaf® Sc 47. These observations provide strong evidence that this commercially available yeast probiotic can significantly improve milk performances of dairy cows under different conditions.

Guillain-Barrésyndrome(GBS)is often triggered by a preceding bacterial or viral ***,it has been observed in association with acute hepatitis A,B and C,and three cases have been previously described in India in which GBS was associated with acute hepatitis E.A molecular mimicry mechanism is supposed to be involved in the pathogenesis of GBS triggered by infectious agents,although the nature of the shared epitopes has not been characterized in most instances,including that in the case of hepatotropic *** report a case of GBS following acute hepatitis E in a European *** presence of antiganglioside GM2 antibodies in this patient suggested molecular mimicry involving ganglioside GM2 in the pathogenesis of GBS associated with hepatitis E.

Bipolar Leads for Prevention of Phrenic Nerve Stimulation: results from the ORPHEE Observational Study

Background: Up to one in three patients implanted with a cardiac resynchronization therapy-defibrillator (CRT-D) device experience phrenic nerve stimulation (PNS). Quadripolar leads are effective at reducing PNS, but compared to standard bipolar leads they have limitations related to maneuverability and high pacing thresholds. The ability of standard bipolar leads to overcome PNS is explored here. Methods: The French multicenter, observational study ORPHEE enrolled 90 CRT-D-eligible patients. Detection of PNS took place after satisfactory positioning of the LV bipolar lead (stable pacing threshold - LV ring, LV tip - RV ring and LV ring - RV coil) could prevent PNS from occurring in at least 90% of patients. results: In 80 evaluable patients, PNS was reported in 12 patients (15%). reprogramming overcame PNS in 10 patients: LV ring - RV coil in 8 patients;LV tip - LV ring in 1;and LV tip- RV ring in 1. As PNS was avoided in 78 of 80 patients (97.5%), the primary endpoint was significant (97.5% vs. 90%, p = 0.01). Conclusion: During CRT-D implantation, PNS occurred in 15% of patients. In most (97.5%) implanted patients, PNS could be avoided by vector reprogramming using a bipolar LV lead. For patients whose coronary sinus anatomy precludes the implantation of multi-electrode leads, bipolar leads are a suitable, reliable alternative.
