Background Erythema nodosum can be associated with a number of systemic diseases. There is, however, a paucity of information in the pediatric literature on this condition. The purpose of this article is to familiarize pediatricians with the evaluation, diagnosis, and treatment of erythema nodosum. Data sources A Pubmed search was completed in Clinical Queries using the key terms 'erythema nodosum'. Results Clinically, erythema nodosum presents with a sudden onset of painful, erythematous, subcutaneous nodules mainly localized to the pretibial areas. Lesions are usually bilateral and symmetrical, ranging from 1 to 5 cm in diameter. Erythema nodosum may be associated with a variety of conditions such as infection, medications, sarcoidosis, pregnancy, inflamma-tory bowel disease, vaccination, autoimmune disease, malignancy, and miscellaneous causes. The condition is idiopathic in approximately 50% of cases. The diagnosis is mainly clinical with biopsy reserved for atypical cases. To evaluate for the underlying cause, some basic laboratory screening studies are worthwhile in most cases and include a complete blood cell count, erythrocyte sedimentation rate and/or C-reactive protein, throat swab culture, antistreptococcal O titers, and a chest radiograph. Other tests should be individualized, guided by the history and physical examination results. most cases of ery-thema nodosum are self-limited and require no treatment. Bed rest and leg elevation are generally recommended to reduce the discomfort. Nonsteroidal anti-inflammatory drugs are the first-line treatment for pain management. Conclusions As erythema nodosum is often a cutaneous manifestation of a systemic disease, a thorough search should be performed to reveal the underlying cause.

A mathematical model for the dynamical transmission of polio is considered,with the aim of investigating the impact of environment *** model captures two infection pathways through both direct human-to-human transmission and indirect human-to-environment-to-human transmission by incorporating the environment as a transition and/or reservoir of *** derive the basic reproduction number R0^env We show that the disease free equilibrium is globally asymptotically stable(GAS)if R0^env1,there exists a unique endemic equilibrium which is locally asymptotically stable(LAS).Similar results hold for environmental contamination free sub-model(without the incorporation of the indirect transmission).At the endemic level,we show that the number of infected individuals for the model with the environ mental-related contagion is greater than the corresponding number for the environmental contamination free *** conjunction with the inequality R0mber for the environmental contamination free sub-model,our finding suggests that the contaminated environment plays a detrimental role on the transmission dynamics of polio disease by increasing the endemic level and the severity of the ***,it is natural to implement control strategies to reduce the severity of the disease by providing adequate hygienic living conditions,educate populations at risk to follow rigorously those basic hygienic rules in order to avoid adequate contacts with suspected contaminated ***,we perform numerical simulations to support the theory.

已经证明阿片类物质如吗啡能够刺激猴免疫缺陷病毒 ( SIV)的复制 ,并最终加速细胞死亡 ,但是其机理却研究很少 .为探讨吗啡和甲硫氨酸脑啡肽对细胞内 p53合成的作用 ,用 SIV感染CEm× 1 74细胞 ,同时分析 SIV感染时病理过程的机理 .在 CEm× 1 74细胞感染 SIV后不同时间 ,p53含量逐渐增加 ,但 1 0 -7mol/L的吗啡仅在起始阶段对其有促进作用 .在 SIV感染组加入吗啡或甲硫氨酸脑啡肽进行时间曲线实验时 ,p53含量较低 .加入 1 0 -8～ 1 0 -6mol/L吗啡 8h,正常细胞 p53含量仅有轻微改变 .但在 SIV感染情况下 ,则呈现剂量依赖性的大量增加 .相反 ,1 0 -8- 1 0 -6mol/L甲硫氨酸脑啡肽在 8h时能增加正常细胞 p53合成达 60 % .在 SIV感染时 ,SIV本身能够促进 p53的含量 .尽管各组 p53仍然高于对照组 ,但甲硫氨酸脑啡肽对其不再起作用 .结果提示甲硫氨酸脑啡肽对正常细胞 p53含量有明显影响 ,而吗啡 8h增加 SIV感染细胞的 p53含量可能是其加速爱滋病病理过程的机理之一 .

Validation and reproducibility of the Heidelberg Edge Perimeter in the detection of glaucomatous visual field defects

AIm: To validate the ability of the Heidelberg Edge Perimeter(HEP) in detecting glaucomatous visual field(VF) defects compared to the Octopus Visual Field(OVF) Analyzer and to determine the test-retest repeatability of both ***: This prospective, cross-sectional study was conducted at Wills Eye Hospital, Philadelphia, PA. Glaucoma subjects and unaffected controls underwent VF testing using HEP standard automated perimetry(SAP) Ⅲ 30-2 Adaptive Staircase Thresholding Algorithm(ASTA) FAST protocol and OVF G-TOP white-on-white strategy. Testing order was randomized. Pearson's correlation coefficient was calculated for both mean deviation(mD) and pattern standard deviation/square root of loss of variance(PSD/s LV). Receiver operating characteristic(ROC) curves were used to determine the diagnostic abilities of both modalities. Glaucoma subjects returned for repeat testing and intraclass correlation coefficients(ICCs) were calculated to assess test-retest repeatability. RESULTS: Seventy-nine subjects with glaucoma and 36 unaffected controls were enrolled. HEP and OVF were significantly correlated for both mD(r=-0.84; Pmparable between HEP and OVF for both mD(0.74 and 0.79, respectively; P=0.26) and PSD/sL V(0.74 and 0.82; P=0.08). ICC was high for both HEP (0.96 for mD, 0.95 for PSD/s LV) and OVF(0.82 and 0.88, respectively). mean test duration(min) was significantly shorter for OVF(2.63) compared to HEP(5.15; PmD and PSD/s LV, and have similar validity for detecting glaucoma. The test-retest repeatability is high for both HEP and OVF, however the average testing duration of HEP is significantly longer than that of OVF.

Neurogenesis dynamics in the olfactory bulb:deciphering circuitry organization, function, and adaptive plasticity

Adult neurogenesis persists after birth in the subventricular zone, with new neurons migrating to the granule cell layer and glomerular layers of the olfactory bulb, where they integrate into existing circuitry as inhibitory interneurons. The generation of these new neurons in the olfactory bulb supports both structural and functional plasticity, aiding in circuit remodeling triggered by memory and learning processes. However, the presence of these neurons, coupled with the cellular diversity within the olfactory bulb, presents an ongoing challenge in understanding its network organization and function. moreover,the continuous integration of new neurons in the olfactory bulb plays a pivotal role in regulating olfactory information processing. This adaptive process responds to changes in epithelial composition and contributes to the formation of olfactory memories by modulating cellular connectivity within the olfactory bulb and interacting intricately with higher-order brain regions. The role of adult neurogenesis in olfactory bulb functions remains a topic of debate. Nevertheless, the functionality of the olfactory bulb is intricately linked to the organization of granule cells around mitral and tufted cells. This organizational pattern significantly impacts output, network behavior, and synaptic plasticity, which are crucial for olfactory perception and memory. Additionally, this organization is further shaped by axon terminals originating from cortical and subcortical regions. Despite the crucial role of olfactory bulb in brain functions and behaviors related to olfaction, these complex and highly interconnected processes have not been comprehensively studied as a whole. Therefore, this manuscript aims to discuss our current understanding and explore how neural plasticity and olfactory neurogenesis contribute to enhancing the adaptability of the olfactory system.
