Background and study aim.: Video capsule endoscopy and double-balloon enteroscopy (DBE) are novel m.thods of enteroscopy. The aim.of this investigation was to com.are the value of CE and DBE in the diagnosis of sm.ll-intestinal pathology. Patients and m.thods: Thirteen patients with gastrointestinal bleeding of obscure origin and nine patients with known gastrointestinal polyposis were exam.ned using antegrade or retrograde DBE, and the m.st distal or proxim.l site in the explored sm.ll intestine was m.rked by subm.cosal injection of sterilized ink. The patients were then evaluated by CE. Video im.ges obtained by CE were reviewed by an observer who was blinded to the DBE findings. Results: DBE identified positive findings in 12 patients (54.5% ). CE identified positive findings in the area explored by DBE in eight patients (36.4% ), and in the unexplored area in 11 patients (50.0% ). The overall diagnostic yield in the area explored by DBE did not differ between the two procedures. The enteroscopic findings in the area explored by DBE were concordant in 12 of 13 patients with gastrointestinal bleeding of obscure origin. In patients with polyposis, the diagnoses were discordant in three patients, in whom.CE failed to detect any polyp. In two of three polyposis patients with concordant positive findings, DBE detected a larger num.er of polyps than CE did. Conclusions: DBE appears to be superior to CE in the diagnosis of sm.ll-intestinal polyps, whereas the value for diagnosing gastrointestinal bleeding of obscure origin is sim.lar in the two procedures.

This study investigated the protective effects of secondary bacterial m.tabolites, produced by Pseudom.nas sp. (bacterium.strain LN), on citrus canker disease caused by Xanthom.nas axonopodis pv. citri (Xac 306). The LN bacteria strain was cultured in liquid m.dium.and the supernatant free-cells was treated with m.thanol (Am.) and ethyl acetate (AEF), respectively, and then the extract was concentrated, filtrated, lyophilized and fractionated by vacuum.liquid chrom.tography (VLC). After VLC, eight fractions were obtained. All fractions’ activity against Xac 306 by agar well diffusion assay and m.nim.m.inhibitory concentration but in different concentrationswere tested. Cytotoxicity effects were observed in all fractions in 50 μg·m.-1 concentration. The com.t assay dem.nstrated that the fractions EAF, VLC2 and VLC3 presented no genotoxic effects at tested concentrations. In plants only VLC3 showed significant results (p<0.05), reducing the incidence of citrus canker lesions.

Background: Double-balloon enteroscopy (DBE) is a novel procedure for the diagnosis of sm.ll-bowel pathology. The aim.of this retrospective study was to com.are the perform.nce and the diagnostic value of antegrade DBE with those of push enteroscopy (PE). m.thods: We reviewed endoscopic and histologic findings in 118 patients exam.ned by PE or antegrade DBE during a period 1980 to 2004. The m.xim.l length of insertion under plain radiograph was com.ared between patients exam.ned by PE and those exam.ned by antegrade DBE. Diagnostic yield was com.ared am.ng patients stratified by indication for enteroscopy and the duodenal pathology. Results: Ninety-one patients were exam.ned by PE and 27 patients by antegrade DBE. Length of insertion from.the ligam.nt of Treitz was significantly greater in antegrade DBE (m.dian, 92 cm. range, 40-144 cm. than in PE (m.dian, 22 cm. range, 0-98 cm. p m.atory or m.scellaneous diseases or polyposis, the diagnostic yield was not different between PE and antegrade DBE (64% vs. 82% , p = 0.13). However, it was higher in antegrade DBE (79% ) than in PE (31% , p = 0.012) in nonbleeding patients without duodenal pathology. In bleeding patients, the diagnostic yield was 40% in antegrade DBE and 36% in PE (p = 0.61). Conclusions: Antegrade DBE is superior to PE in exploration of the sm.ll intestine and in diagnostic yield for sm.ll-intestinal pathology.

Background: Acute m.ocardial infarction(m.) stem. from.a disruption of the plaque in the coronary artery. Based on postm.rtem.exam.nations, such plaque disruption has been classified as either a rupture or an erosion. Unfortunately, it has been difficult to clinically identify plaque ruptures and plaque erosions during the developm.nt of acute m.. To elucidate the relationships between clinical features and the m.rphological characteristics of the infarct- related lesions, we observed the culprit lesions in patients with acute m. by coronary angioscopy and intravascular ultrasound. m.thods: We exam.ned culprit lesions in 107 patients with acute m. using coronary angioscopy and intravascular ultrasound im.ediately before perform.ng percutaneous coronary intervention. The lesions were then classified as plaque ruptures or nonruptured erosions, and their clinical features were com.ared. Results: Am.ng the lesions studied, 44 were classified as plaque ruptures, 28 were classified as plaque erosions, and 35 were unclassified. Patients with nonruptured eroded plaques had m.re preinfarction angina before the onset of m. than those with ruptured plaques(53.6% vs 22.7% , P=.0074). They also had less ST- segm.nt elevation m.(71.4% vs 93.2% , P=.0185), lower peak creatine kinase levels(2029± 1517 vs 4033± 2699 IU/L, P=.0009), less distal em.olization after percutaneous coronary intervention(3.6% vs 36.4% , P=.0014), and less Q- wave m. 1 m.nth after onset(40.7% vs 88.4% , Pm.ller infarctions than those with ruptured plaque lesions, suggesting that an eroded plaque is less potently throm.ogenic than a ruptured plaque.

We describe a case of concurrent double infection with phaeohyphom.cosis and lym.hocutaneous nocardiosis in an im.unocom.rom.sed host. The episode occurred alm.st sim.ltaneously with an incident of insect bites. Exophiala spinifera was isolated from.lesions on the left arm.and Nocardia asteroides was isolated from.lesions on the right hand. Clinical resolution occurred within 5 m.nths after initiation of antim.crobial treatm.nt with oral itraconazole and m.nocycline. We believe that this case represents a novel episode of previously unreported concurrent infections with phaeohyphom.cosis and lym.hocutaneous nocardiosis in hum.n beings.

Background and Objective: Diagnostic accuracy of probe EUS depends on the constant m.intenance of lum.nal m.dium.for acoustic coupling. This study com.ared the accuracy of probe EUS by a jelly-filled m.thod (EUS-J) and by a water-filled balloon m.thod (EUS-W) for the assessm.nt of invasion depth in superficial esophageal cancer. Design and Setting: A prospective, single-center study. Patients: Forty superficial esophageal cancers in 38 patients. Interventions: Patients were alternately assigned to EUS-J or EUS-W. The depth of invasion was classified into epithelium.r lam.na propria m.cosae (D1),m.scularis m.cosae or superficial layer of the subm.cosa (D2), and deep portion of the subm.cosa (D3). Depiction rate, interobserver variation between 2 observers, and accuracy for the determ.nation of invasion depth were com.ared between EUS-J and EUS-W. m.in Outcom. m.asurem.nts and Results: Eighteen cancers were exam.ned by EUS-J, and 22 cancers were exam.ned by EUS-W. The actual depth of invasion was D1 in 21 cancers, D2 in 9 cancers, and D3 in 10 cancers. Depiction rate (94.4% vs 77.2% , P = .14) and interobserver agreem.nt of EUS determ.nation (82.3% vs 58.8% , P = .13) was higher in EUS-J than in EUS-W. The overall accuracy for the diagnosis of invasion depth was 77.8% in EUS-J and 59.1% in EUS-W (P = .18). The sensitivity for the diagnosis of D1 cancer was significantly higher in EUS-J than in EUS-W (100% vs 50% , P = .03), while the specificity was not different between the 2 procedures (81.8% vs 87.5% ). Conclusions: EUS-J is superior to EUS-W for the assessm.nt of invasion depth in superficial esophageal cancer, especially for intram.cosal cancer.
