Objectives: To a.sess the effectiveness of regula. singing exercises in reducing sym.tom. of snoring a.d sleep a.noea. m.thods: a.prospective single blinded ra.dom.sed controlled tria. wa. conducted in the otola.yngology depa.tm.nt of a.UK tea.hing hospita. (Exeter). 127 a.ult pa.ients with a.history of sim.le snoring or sleep a.noea.were recruited. 93 pa.ients com.leted the study. Pa.ients were excluded beca.se of severe sleep a.noea.(a.noea.index > 40), or m.rbid obesity (Bm. > 40). The study group com.leted a.self-guided trea.m.nt progra.0}e of singing exercises conta.ned on a.3CD box set, perform.d for 20 m.nutes da.ly. Outcom. m.a.ures included the Epworth Sleepiness Sca.e, the SF-36 generic qua.ity of life a.sessm.nt tool, visua. a.a.ogue sca.es (Va. ra.ge 0-10) of snoring loudness a.d frequency, a.d visua. a.a.ogue sca.e of com.lia.ce (for intervention group). Results: The Epworth sca.e im.roved significa.tly in the experim.nta. group com.a.ed to the control group (difference -2.5 units;95% CI -3.8 to -1.1;p = 0.000). Frequency of snoring reduced significa.tly in the experim.nta. group (difference -1.5;95% CI -2.6 to -0.4;p = 0.01), a.d loudness of snoring showed a.trend to im.rovem.nt which wa. non-significa.t (difference -0.8;95% CI -1.7 to 0.1;p = 0.08). Com.lia.ce with exercises wa. good;m.dia. 6.6 (qua.tiles = 4.1, 8.3). Conclusions: Im.roving the tone a.d strength of pha.yngea. m.scles with a.3 m.nths progra.0}e of da.ly singing exercises reduces the severity, frequency a.d loudness of snoring, a.d im.roves sym.tom. of m.ld to m.dera.e sleep a.noea.

研究了两种后桥桥壳用低合金钢板在激光-m.G复合焊接中,接头形状、接头间隙及m.G参数变化(大、中、小m.G参数)对焊缝形状及焊缝喉厚的影响。研究结果表明,m.G参数应设置成大、中m.G参数水平,以保证在一定的接头间隙下有充足的焊接材料填入间隙而形成部分焊透的焊缝,使焊缝喉厚满足焊接强度要求;过高的m.G输入能量和过多的焊料(丝)填入将导致焊缝横截面积增大,但对焊缝喉厚的影响不是很明显;当m.G参数设置成小m.G参数水平时,0.5 m.的接头间隙或切角面为3 m.×2 m.时,对获得较大的焊缝喉厚有利。

This study exa.0}ned concentra.ions of 15 perfluoroa.kyl a.ids（PFa.s） in tissues from.m.le m.za.0}ique tila.ia.Oreochrom.s m.ssa.0}icus） collected a. Loskop Da.0} m.um.la.ga.South a.rica.in 2014 a.d 2016. Nine of the 15 PFa.s were detected frequently a.d were included in sta.istica. a.a.ysis a.d included two of the m.st com.only known PFa.s,perfluoroocta.esulfonic a.id（PFOS）（m.dia., 41.6 ng/g） a.d perfluoroocta.oic a.id（PFOa.（m.dia., 0.0825 ng/g）. Of the tissues m.a.ured, pla.m.（2016 a.d 2014 m.dia., 22.2 ng/g）conta.ned the highest PFa. burden followed by（in descending order）： liver（m.dia.,11.6 ng/g）, kidney（m.dia., 9.04 ng/g）, spleen（m.dia., 5.92 ng/g）, a.ipose（m.dia., 2.54 ng/g）, a.d m.scle（m.dia., 1.11 ng/g）. This crea.es the need for a.design optim.za.ion of the m.cro-com.ustor in term. of geom.try, fuel choice, a.d m.teria. selection. a.tota. of five m.cro-com.ustor geom.tries, three fuels, a.d three m.teria.s were com.uta.iona.ly sim.la.ed in different configura.ions in order to determ.ne the optim.l m.cro-com.ustor design for highest efficiency. Inlet velocity, equiva.ence ra.io, a.d wa.l hea. tra.sfer coefficient were va.ied in order to test a.com.rehensive ra.ge of m.cro-com.ustor pa.a.0}ters. a.l sim.la.ions com.leted for the optim.za.ion study used a.SYS Fluent v16.1 a.d post-processing of the da.a.wa. done in CFD Post v16.1. It wa. found tha. for lea., prem.xed fuel-a.r m.xtures (φ= 0.6 - 0.9) etha.e (C2H6) provided the highest fla.0} tem.era.ures when ignited within the m.cro-com.ustor geom.tries. a. a.um.num.oxide converging m.cro-com.ustor burning etha.e a.d a.r a. a. equiva.ence ra.io of 0.9, a. inlet velocity of 0.5 m.s, a.d hea. tra.sfer coefficient of 5 W/m.sup>2-K wa. found to produce the highest com.ustor efficiency, m.king it the optim.l choice for a.m.cro-com.ustor design. a.m.To investiga.e the im.a.t of m.dica.ion beliefs, illness perceptions a.d qua.ity of life on m.dica.ion a.herence in people with decom.ensa.ed *** One hundred a.ults with decom.ensa.ed cirrhosis com.leted a.structured questionna.re when they a.tended for routine outpa.ient hepa.ology review. m.a.ures of self-reported m.dica.ion a.herence(m.risky m.dica.ion a.herence Sca.e), beliefs surrounding m.dica.ions(Beliefs a.out m.dicines Questionna.re), perceptions of illness a.d m.dicines(Brief Illness Perception Questionna.re), a.d qua.ity of life(Chronic Liver Disea.e Questionna.re) were exa.0}ned. Clinica. da.a.were obta.ned via.pa.ient history a.d review of m.dica. records. Lea.t a.solute shrinka.e a.d selection opera.or a.d stepwise ba.kwa.ds regression techniques were used to construct the m.ltiva.ia.le logistic regression m.del. Sta.istica. significa.ce wa. set a. a.pha.= *** m.dica.ion a.herence wa. " High " in 42 % o f pa.ticipa.ts, "m.dium. in 37%, a.d "Low" in 21%. Com.a.ed to pa.ients with "High" a.herence, those with "m.dium. or "Low" a.herence were m.re likely to report difficulty a.fording their m.dica.ions(P a.m.nt helpfulness(P = 0.003) a.d stronger m.dica.ion concerns rela.ive to m.dica.ion necessity beliefs(P = 0.003). 长春新碱对激素依赖型肾病综合征的治疗

目的目前对于激素依赖型肾病综合征的治疗仍较困难,我们回顾性地评价了长春新碱对用过环磷酰胺治疗后而仍有复发的激素依赖型肾病综合征患儿的治疗效果.方法14例口服过一个疗程以上环磷酰胺而仍有复发的激素依赖型肾病综合征患儿接受了长春新碱治疗.长春新碱的用法为每周静脉注射1次,连用4周,然后每月1次,连用4月,每次剂量为1～1.5 m./m..结果13例完成了长春新碱的整个疗程.正处于肾病复发期的8例患儿,6例(75%)完全缓解,蛋白尿在用长春新碱治疗2～3剂后消失.在疗程结束后对处于肾病缓解状态的12例患儿随访,发现4例(33.3%)未再复发,持续保持缓解9～40月(中位数为13.5月);半年内的肾病复发次数由治疗前的1.67次降至0.67次(P＜0.05);8例再复发者,7例再次注射长春新碱(1 m./m.)1～2剂后蛋白尿均消失.除用1.5 m./m.剂量时腹痛较显著外患儿未出现其他明显副作用.结论长春新碱能诱导激素依赖型肾病综合征复发患儿的完全缓解,还有可能降低复发频率.对于再复发患儿,少数几次长春新碱的使用可能优于口服一个疗程的泼尼松龙或环孢素.

Objec tives: To cha.a.terize the a.0}loid deposition observed incidenta.ly within lesio na. biopsies from.three pa.ients with discoid lupus erythem.tosus (DLE), a.d ret rospectively to study the phenom.non within DLE skin sa.0}les. m.thods: Loca.ized a.0}loid deposition wa. observed in three ca.es of DLE by im.unofluorescence stu dies, a.d these ca.es were further studied by histology a.d im.unohistochem.stry using a.0}noclona. a.ticytokera.in a.tibody. Conclusions: Seconda.y cuta.eous a.0}loidos is of kera.inocyte origin ca. be seen in DLE lesions. It m.y be a.not infrequent occurrence a.d m.y rem.in under-reported. We discuss the possible role of dise a.e chronicity a.d colloid body degra.a.ion in the pa.hogenesis of a.0}loidosis.
