Improvement in renal functions with transcatheter aortic valve implantation

BACKGROUND & OBJECTIVES:In recent years, emerging transcatheter aortic valve implantation (TAVI) has become an alternative for surgery。 However, with advanced age, several co-morbid factors together with contrast agent usage can cause deterioration in renal function and increase in the risk of acute kidney injury (AKI) with poor prognosis in patients with AKI。 Therefore, many patients cannot benefit from this treatment。 In this study, we aim to examine the effects of TAVI on renal functions。 mETHODS AND RESULTS: Seventy patients, mean age of 77.6 years, underwent TAVI between July 2011 and December 2012。 Estimated glomerular filtration rate (eGFR) was calculated by using the Cockcroft and Gault Formula。 Patients were monitored for 48 h for urine output。 Stage 1 AKI, according to the VARC-2 AKIN system, developed in only five (7.1%) of the patients after the procedure。 There was a statistically significant increase between the mean 1(st) month eGFRs before (68。2 vs。 61.0, P mL/min, P month (52.1 mL/min, P mortality rate was higher in the group which developed AKI (P month eGFR showed a more statistically significant increase than pre-TAVI eGFR (62.8 and 69.8, P month。 CONCLUSIONS: In this study, we showed that the treatment of aortic stenosis through TAVI allows improvement of renal functions, and that AKI rates will be lower with careful patient selection, proper pre-procedural hydration, and careful use of contrast agent。

This study discusses the effects of local sites and hazard amplification on the seismic vulnerability assessment of existing masonry *** this context,a rapid seismic evaluation procedure was implemented on an old masonry building stock in the historical center Galata,located inİstanbul,to determine the seismic risk priority of the built *** scenarios were generated for all soil classes,different moment magnitudes,and source-to-site distances to obtain more accurate results for the seismic vulnerability assessment of the studied building ***,damage distributions estimated under nine different scenarios with/without site effects were compared and illustrated in maps to discuss changes in vulnerability owing to amplification *** this study,by re-examining the rapid seismic evaluation procedure by including geo-hazard-based assessment,the importance of site effects on the vulnerability and risk assessment of built heritage was *** proposed framework integrating field data and local site effects is believed to advance the current applications for vulnerability assessment of masonry buildings and provide an improvement in the application of rapid seismic assessment procedures with more reliable results.

A New Fluorescent Chemosensor for Selective and Sensitive Detection of mn²⁺ in Acidic medium

Recently, fluorescent sensors have attracted considerable attention in their sensitive and selective determination of heavy metal ions in the aqueous acidic medium due to their advantages such as low cost and easy handling. In this study, the bathocuproine (BCP) compound was used as a fluorescent chemosensor. The selectivity and sensitivity of BCP have been investigated against some metal ions of biological and environmental importance. The results obtained from the ultraviolet-visible region (UV-vis.) and the fluorescence spectroscopy experiments revealed that the BCP sensor showed selectivity and sensitivity only to manganese (II) ions in the experimental conditions studied. In addition, the binding stoichiometry of BCP and mn2+ was determined to be 1:1 by the Benesi-Hildebrand method.

Three Dimensional Simulation of Upset Forging by Using Variational Upper Bound method

The variation principle is discussed and Rayleigh-Ritz method is proposed for construction of veloci ty field. A kinematically admissible velocity field based on polynomials was appIied to the determina tion of forging load and deformed buIge profile during upset forging of blocks. Simulation of upsetforging of rectangular blocks under various friction condjtions was performed. Comparison of the computed results with experiments and FEm shows good agreement. It is shown that this techniquecan be used for 3D simulation of metal forming process.

Characterization of flgK gene and FlgK protein required for H pylori Colonization-from cloning to clinical relevance

AIm： To characterize the role of flgK and its protein product in Hpylori colonization. mETHODS： The PCR cloning method identified the flgK gene. An isogenic flgK mutant was constructed by gene replacement and confirmed by Southern blot analysis and PCR analysis. The recombinant FlgK protein （r-FlgK） was purified. Electron microscopy （Em） was applied to demonstrate the flagella of H pylori. An in vitro motility test was assessed in semisolid medium. The densities of H pylori colonization with either the wild-type strain or its flgK mutant were compared among BALB/c mice with or without pre-immunization with r-FlgK. The serological responses to r-FlgK were analyzed for 70 clinical patients with different densities of H pylori colonization. RESULTS： From a duodenal ulcer strain, the flgK gene was cloned and it contained 1821 bp, with a 95.7% identity to the published sequences. No flagella were observed under Em for the mutant strain, which had a loss of motility. Hpylori density was lower in the BALB/c mice inoculated by the mutant or with pre-immunization with r-FlgK compared to unimmunized mice or mice inoculated by the wild-type strain （P 〈 0.05）. In the H pylori-infected patients, the serological responses to r-FlgK were uniformly low in titer.CONCLUSION： FlgK encoded by flgK is important for flagella formation and H pylori motility. Deficiency in FlgK or an enhanced serological response to r-FlgK can interfere with Hpylori colonization. FlgK of Hpylori could be a novel target for vaccination.

We studied effects of nutrient quercetin on cytochromes＇ Р450 1А （CYP1A） activities （measured spectrofluorimetrically using 7-ethoxy-resorufin for CYP1A1 and 7-methoxy-resorufin for CYP1A2 as substrates）, on m RNA levels （measured by RT-PCR）, and on DNA-binding activities （evaluated by an electrophoretic mobility shift assay） of proteins regulating CYP1A expression in untreated and benzo（α）pyrene （Ba P）-treated rats. Wistar rats received quercetin, Ba P, or both once daily for 1-3 days. Quercetin did not influence CYP1A1 in untreated rats but inhibited Ba P-mediated CYP1A induction on the transcriptional level decreasing positive input （Ah R functional activity） and increasing negative input （Ah RR/ARNT expression and Oct-1 and C/EBP functional activities）.

Clinical predictors of severe gallbladder complications in acute acalculous cholecystitis

AIm: To evaluate the relationship between clinical information (including age, laboratory data, and sonographic findings) and severe complications, such as gangrene,perforation, or abscess, in patients with acute acalculous cholecystitis (AAC).mETHODS: The medical records of patients hospitalized from January1997 to December 2002 with a diagnosis of acute cholecystitis were retrospectively reviewed to find those with AAC, confirmed at operation or by histologic examination. Data collected included age, sex, white blood cell count, AST, total bilirubin, alkaline phosphatase,bacteriology, mortality, and sonographic findings. The sonographic findings were recorded on a 3-point scale with 1 point each for gallbladder distention, gallbladder wall thickness ＞3.5 mm, and sludge. The patients were divided into 2 groups based on the presence (group A) or absence (group B) of severe gallbladder complications, defined as perforation, gangrene, or ***: There were 52 cases of AAC, accounting for 3.7% of all cases of acute cholecystitis. males *** patients were diagnosed by ultrasonography (48 of 52) or computed tomography (17 of 52). Severe gallbladder complications were present in 27 patients (52 %, group A) and absent in 25(group B).Six patients died with a mortality of 12 %.Four of the 6 who died were in group *** in group A were significantly older than those in group A were significantly older than those in group B (mean 60.88 y vs. 54.12 y, P=0.04) and had a significantly higher white blood cell count (mean 15 885.19 vs. 9 948.40,P=0.0005). Vascular Z-shaped ligation technique in surgical treatment of haemorrhoid

AIm: To present the effectiveness of minimal invasive vascular zet ligation in the surgical treatment of haemorrhoidal disease(HD).mETHODS: Among 138 patients with 2nd-4th grade internal HD having several complaints and operated at our hospital between 2003-2013; 116 patients who regularly attended 1-year control were included in the study. Operation times, postoperative early period pain, satisfaction score, complications and relapse details were obtained from computer records retrospectively. Visual Analogous Scale(VAS) scores were used for patient satisfaction on the 3rd, 7th and 21 st days. Technique; fixed suture which is constituted by the first leg of the Z-shaped suture(to pass by the mucosa and muscular layer) was put in the pile root in order to ensure vascular ligation and fixation. The second leg of the Z-shaped suture is constituted by mobile suture and it passes by the pile mucosa and submucosa which prolapses 5-10 mm below the first suture. RESULTS: Seventy-five of the patients(65%) were male, 41 of them(35%) were female and their age average was 41. The mean operation time was 12 ± 4.8 min. VAS/satisfaction score was found as 2.2/4.3, 1.8/4.0, 1.2/4.4 respectively on the 3rd, 7th, and 21 st days. The Comparison of the Forms of Land Capability Classification of Atalay and USA in Eskişehir Province (Turkey)

Turkey is an area where climate changes immediately, vegetation, or land gets different in a short distance. Geological and lithological features show diversity. Also, our country’s territorial existence and diversity also bring about different land use conditions. Therefore, land capability also differs from each other. Nevertheless, the classification of land capability used in Turkey is the classification of land capability for agricultural lands prepared by the United States (USA) in 1961. Due to this, [1] have made suggestion on a new classification of land capability considering our country’s geographical conditions. In this study, comparing the land capability with the classification carrying out in our country, the classification which Atalay and Gündüzoğlu suggested, has been aimed. Working method has been established according to regional approach and field observations have been done. In preparing the cartographical material, ArcGIS 10.3 has been used. The map of this study as a material topography, physical map, slope, aspect, the usage of the land, ground, geology, land capability, geomorphology, temperature, and precipitation has been examined, meteorological data have been appreciated. According to the findings attained, Eskişehir’s map of land capability has been done through the criteria of the suggestions of Atalay and Gündüzoğlu. As a result, it has been understood that there is a difference between the USA land capability that applied in Eskişehir and Atalay and Gündüzoğlu’s criteria. In the study, it is suggested to determine the land capabilities again considering the ecological conditions of Turkey.
