Introduction: At the beginning of the COVID-19 pandemic, a drop in the number of patients treated for cardiac emergencies raised concern about cardiovascular mortality in that period. An increase in care delay for patients with ST-segment elevation myocardial infarction (STEMI) may have affected clinical outcomes. Objectives: To analyze delay times and clinical outcomes of patients with STEMI undergoing primary percutaneous coronary intervention (PPCI), before and during the COVID-19 pandemic. Methods: Retrospective observational study that included patients with STEMI undergoing PPCI from December 2018 to July 2021. The COVID-19 pandemic cases were divided into two groups: pandemic I—from March to August 2020;and pandemic II—from September 2020 to July 2021. Patients were compared according to the period of hospitalization. Primary outcomes were delay times in assistance and clinical outcomes (acute kidney injury [AKI], post-procedural vascular complications and in-hospital mortality). Results: 108 patients were included, 39 (36.1%) in the pre-pandemic period, 13 (12.1%) in pandemic I and 56 (51.8%) in pandemic II. Time from onset of symptoms to arrival at the service and door-to-balloon time did not differ significantly among groups. Vascular complications were more frequent during the pandemic (I and II) than in the pre-pandemic period (2.5% pre-pandemic vs 15.4% pandemic vs 12.5% pandemic II;p = 0.03). AKI incidence was similar in all three periods. There was a non-significant increase in in-hospital mortality during the COVID-19 pandemic. Conclusion: In patients with STEMI, there was an increase in vascular complications and a trend toward increased mortality during the COVID-19 pandemic. Delay times to admission and reperfusion did not differ significantly between before and during the pandemic.

Correlation between the Six-Minute Walk Test and Subjective Functional Class in Patients with Heart Failure

Introduction: The Six-Minute Walk Test (6MWT) is an inexpensive method to objectively evaluate physical capacity or limitation and stratify prognosis in patients with Heart Failure (HF). Since the clinical perception of symptoms may be adapted or compromised, regular evaluation from medical interviews often fails to determine functional classification. This study aimed to assess the correlation between New York Heart Association Functional Class (NYHA-FC) and the distance walked in the 6MWT. Methods: We conducted a cross-sectional observational study that included patients with HF with reduced ejection fraction followed up at an outpatient service of a teaching hospital, from August 2018 to April 2019. Patients in NYHA-FC I, II, or III were included. We compared NYHA-FC subjectively obtained during the consultation with the 6MWT performed after medical consultation, and the correlation between these two parameters was assessed. Results: The study included 70 patients with HF, 41 (58.6%) of whom were female. The mean age was 61.2 ± 12.7 years. The most prevalent etiologies were dilated idiopathic cardiomyopathy (35.7%) followed by ischemic cardiomyopathy (25.7%). The mean ejection fraction was 34.1% ± 9.8%. The average distance walked in the 6MWT by NYHA-FC I patients was 437.8 ± 95.8 meters, NYHA-FC II 360.1 ± 96.4, and NYHA-FC III 248.4 ± 98.3. Functional class measured by the 6MWT was different than that estimated by NYHA-FC in 34 patients (48.6%), 23 (32.9%) for a higher functional class and 11 (15.7%) for a lower one (p = 0.07). Pearson’s correlation coefficient between NYHA-FC and the 6MWT was -0.55. Conclusion: There was a moderate correlation between the subjective NYHA-FC and the 6MWT. The 6MWT revealed a different classification from NYHA-FC in almost half of the patients. Among those who presented discrepancies between methods, 6MWT reclassification towards a higher functional class was more common.

The Intertropical Convergence Zone(ITCZ)is a key atmospheric system on a global scale,primarily driven by trade wind convergence near the *** ITCZ plays a crucial role in modulating the climate of the borders of tropical continental *** instance,Northeastern Brazil experiences a climate influenced by the ITCZ over the Atlantic *** some periods,the ITCZ exhibits double bands,known as the double *** the features of the ITCZ have been described using various approaches and atmospheric variables,there is still a lack of regional studies focusing on the ITCZ and double ITCZ in the Atlantic *** this context,the main goals of this study are(1)to describe a simple algorithm based on precipitation to identify the ITCZ and double ITCZ,(2)to present a climatology(1997-2022)of the position,width,and intensity of these two convective bands,and(3)to investigate variabilities in the ITCZ characteristics associated with anomalies of sea surface temperature(SST)in the tropical Pacific and Atlantic *** double ITCZ typically occurs southward of the main cloud band,and between February and April,both bands are more distant(~4.5°).In the western sector of the Atlantic Ocean,the ITCZ and its double band extend to more southerly latitudes in austral *** the entire Atlantic basin,the annual mean of the latitudinal position,width,and intensity of the ITCZ is 4.9°N,4.2°,and 11 mm/day,respectively,while for the double ITCZ,it is 0.4°N,2.6°,10.3 mm/day,*** the SST anomalies in the Pacific Ocean(El Niño and La Niña episodes)affect more the ITCZ width,the SST anomalies in the Tropical South Atlantic affect both its position and width.

Antimicrobial activity of alexidine alone and associated with N-acetylcysteine against Enterococcus faecalis biofilm

The purpose of this study was to assess the efficacy of alexidine（ALX）,alone and combined with N-acetylcysteine（NAC）,in eradicating two Enterococcus faecalis strain *** biofilms of *** ATCC 29212 and the clinical isolate *** D1 were grown in the MBEC-high-throughput device for 24 h and were exposed to five twofold dilutions of ALX（2%–0.007 8%）alone and combined with100 mg？mL21NAC,for 1 and 5 *** was defined as 100%kill of biofilm *** Student’s t-test was used to compare the efficacy of the associations of the two *** 1-min contact time,ALX eradicated the biofilms at all concentrations except for 0.007 8%and 0.015 6%–0.007 8%with *** ATCC 29212 and *** D1,*** results for eradication and concentration were obtained when it was combined with 100 mg？*** 5 min of contact time,ALX alone and combined with NAC eradicated all enterococci *** showed antimicrobial properties against the two *** strain biofilms tested at very low concentrations,and its combined use with NAC was not seen to enhance its activity.

Mean Kinematic Characteristics of Synoptic Easterly Disturbances over the Atlantic

This study investigates the mean kinematic characteristics of the tropical Atlantic easterly disturbances in January-March （JFM）, April-June （AM J）, July-September （JAS） and October-December （OND） from 1968-1998. For each season, the preferential tracks of these disturbances in the 3--10-day band periods were computed and spatialized, as well as their associated wavelength, velocity and main period, which lies between 3-5 days and between 6-9 days depending on the track and the season. Two main tracks are highlighted over the Atlantic Ocean. During OND and JFM these two tracks are well separated and located in each hemisphere around 15°S and 12.5°N. From AMJ to JAS these tracks migrate northward; in JAS, they merge into one over the northern tropical Atlantic along 17.5°N. The associated wavelength fields exhibit a meridional gradient, with large wavelengths （greater than 4000 km） around the equator, between 5°N and 5°S, and smaller wavelengths outside of this latitude band （between 2500-3500 kin）. The phase speed is also found to exhibit poleward decreasing values from 12-6 m s^-1. Over the north Atlantic track, 6-9-day disturbances were found to occur from January to May and approximately from October to December. From June to September, the 3-5-day waves dominate the synoptic activity. Over the south Atlantic track, between May and August the synoptic variability is mainly explained by the 3-5-day disturbances but from January to April and from September to December both 3-5-day waves and 6-9-day waves can occur.

Relationship between phase angle,steatosis,and liver fibrosis in patients coinfected with human immunodeficiency virus/hepatitis C virus

BACKGROUND Malnutrition,lipodystrophy,and dyslipidemia are prevalent characteristics in patients with human immunodeficiency virus(HIV)infection with or without previous *** a clinical condition can lead to the hypothesis of the presence of hepatic steatosis with possible progression to fibrosis and the risk of hepatocellular ***,a low phase angle(PA),evaluated by bioelectrical impedance analysis(BIA),is an independent prognostic marker of clinical progression and survival in HIV-infected *** To evaluate the relationship between PA and body composition with steatosis and hepatic fibrosis in HIV/hepatitis C virus(HCV)-coinfected *** A retrospective observational study by convenience sampling of coinfected HIV/HCV patients,in which all patients underwent transient elastography(Fibroscan)and BIA ***’s t test was used for group comparisons,and Spearman’s or Pearson’s correlation test was used when *** significance level was set at 5%,and analyses were performed using SPSS version *** Forty-three patients who received antiretroviral therapy met the inclusion criteria,and 23(53.5%)were under treatment with protease inhibitors(PIs).There was no difference in PA between those who used PIs and those who did not(P=0.635).There was no correlation between fibrosis grade and PA(P=0.355)or lean mass(P=0.378).There was a significant inverse correlation between the controlled attenuation parameter(CAP)and lean mass(P=0.378),positive correlation between PA and lean mass(P=0.378),and negative correlation between PA and fatty mass(P=0.378),although the CAP and PA were not *** evaluated by sex,no significant correlations were *** PA determines the muscle function of HIV/HCV-coinfected patients,and the CAP values reinforce the association with lean mass,suggesting that patients require early nutritional interventions.

Characterization of Cnidoscolus quercifolius Pohl bark root extract and evaluation of cytotoxic effect on human tumor cell lines

Objective: To evaluate the chemical components of active extract from Cnidoscolus quercifolius root bark and its cytotoxic potential against several tumor strains. Methods: The high-performance liquid chromatography with diode-array detection and ~1H and ^(13)C nuclear magnetic resonance spectroscopy of the extract were used to distinguish the existence of possible functional groups in the root bark extract. The in vitro cytotoxic activity of methanol extract on human colon cancer cell lines was evaluated using OVCAR-8, SF-295, HCT-116, HL-60 strains and the samples were assessed by 3-(4,5-dimethylthiazol2-yl)-2,5-diphenyltetrazolium bromide method. Results: The analysis of nuclear magnetic spectra of the active chloroform fraction revealed the presence of absorptions bands correspondent to a mixture of favelines such as neofavelanone, deoxofaveline or methyl-faveline, which structures were confirmed by ultraviolet spectra upon high-performance liquid chromatography with diode-array detection analysis. The active fraction showed cytotoxic effects in the tested strains, HCT-116, SF-295, OVCAR-8 and HL-60 cells with IC_(50) of 72 hours ranging from 4.95 to 15.23 μg/mL. Conclusions: The results suggest that the substances present in faveleira(Cnidoscolus quercifolius) root bark extract have a cytotoxic potential against several tumor lines, showing a broader antitumour potential, and in addition no adverse to healthy cells. Therefore, the root bark extract of Cnidoscolus quercifolius has a possibility of use for anticarcinogenic therapies.

Extramedullary plasmocytoma associated with a massive deposit of amyloid in the duodenum

We report a rare case of extramedullary plasmocytoma associated with a massive deposit of amyloid in the duodenum.A 72-year-old Japanese man was admitted to our hospital presenting with a 3-mo history of epigastric pain, vomiting and weight *** computed tomography(CT) a wall thickening of the fourth part of the duodenum was *** biopsies obtained from the lesion showed inflltration of plasma cells and lymphocytes, but they were not *** patient underwent resection of the lesion and, on histopathological examination, the lesion consisted of a dense and diffuse inflltrate of plasma cells and a few admixed lymphocytes with reactive follicles extending to the muscular *** extensive deposition of amyloid was also *** stains revealed that a few plasmacytoid cells showed λ light chain staining, though most were κ light chain *** cells also were positive for CD138 and CD56 but negative for CD20 and *** flndings were consistent with extramedullary plasmocytomaassociated with a massive deposit of amyloid in duodenum.A subsequent workup for multiple myeloma was completely *** patient showed no signs of local recurrence or dissemination of the disease after 12 mo *** of the association of plasmocytoma and amyloidosis, the patient must be followed up because of the possible systemic involvement of the neoplasm and amyloidosis in future.
