Parathyroid ultrasonography and bone metabolic profile of patients on dialysis with hyperparathyroidism

AIM To evaluate the parathyroid ultrasonography and define parameters that can predict poor response to treatment in patients with secondary hyperparathyroidism due to renal *** This cohort study evaluated 85 patients with chronic kidney disease stage V with parathyroid hormone levels above 800 pg/mL. All patients underwent ultrasonography of the parathyroids and the following parameters were analyzed: demographic characteristics(etiology of chronic kidney disease, gender, age, dialysis vintage, vascular access, use of vitamin D), laboratory(calcium, phosphorus, parathyroid hormone, alkaline phosphatase, bone alkaline phosphatase), and the occurrence of bone changes, cardiovascular events and death. The χ~2 test were used to compare proportions or the Fisher exact test for small sample frequencies. Student t-test was used to detect differences between the two groups regarding continuous *** Fifty-three patients(66.4%) had parathyroid nodules with higher levels of parathyroid hormone, calcium and phosphorus. Sixteen patients underwent parathyroidectomy and had higher levels of phosphorus and calcium × phosphorus product(P = 0.03 and P = 0.006, respectively). They also had lower mortality(32% vs 68%, P = 0.01) and lower incidence of cardiovascular or cerebrovascular events(27% vs 73%, P = 0.02). Calcium × phosphorus product above 55 mg^2/dL^2 [RR 1.48(1.06, 2.08), P = 0.03], presence of vascular calcification [1.33(1.01, 1.76), P = 0.015] and previous occurrence of vascular events [RR 2.25(1.27, 3.98), P dentification of nodules at ultrasonography strengthens the indication for parathyroidectomy in patients with secondary hyperparathyroidism due to renal failure.

In the last decade, Brazil was the scene of two of the world’s biggest disasters related to mining tailings dams. The collapse of the Mariana and Brumadinho dams was catastrophic event that brought to light major environmental and humanitarian crises. Facts like this incite debate on the safety of dam construction methods and highlight the need for a broad discussion of dam safety legislation in the country. From the above, this paper promotes a review of various forms of construction of tailings dams and their associated problems, as well as discusses the techniques for soil reinforcement and their construction methods. The work also brings an overview of the Brazilian dams from April to July 2019, tracing the context of the Brazilian tailings dams months after the tragedy of Brumadinho. The methodology includes a robust literature review and discourse analysis. In Brazil, the number of ruptured dams is alarming, especially in the state of Minas Gerais, in which more than 7 have broken in the last two decades. Among the damages, the deaths impact society abruptly and were cataloged 455 deaths, of which 216 correspond to the rupture of the dam of the Córrego do Feijão mine in Brumadinho. To evaluate and diagnose a dam, one should consider ordinance N ° 70,389, 2017 revoked by Resolution ANM n ° 95, of February 7, 2022. In this context, in March 2019, we experienced several disturbing situations regarding dam instability in the state of Minas Gerais, being categorized as risky structures. To exemplify, one can cite dams classified as level 2 risk that became level 3. Due to the above facts, several studies are being conducted to improve soil aspects. For example, we highlight the increase in soil resistance that can be done using commercial and alternative materials available in the environment.

Context and objectives :The unexpected pandemic caused by the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) affected several activities and exposed workers, students and the large mass of unemployed people to an economic and psychological pressure that could lead to various problems, such as anxiety, fear, depression and insomnia, consequently causing impairment of quality of life. Method: A cross-sectional study was conducted using the COVID-19 Peri-Traumatic Distress Index (CPDI) and the Fear of COVID-19 Scale (FCV-19S) to measure peri-traumatic stress and fear, such as psychological reactions during the 2019 coronavirus disease (COVID-19) pandemic. An online spreadsheet was used as a data collection tool to send the questionnaire and scales to a sample of 1,844 participants. Data were analyzed in groups of health professionals, students and teachers, and an analysis was performed between unemployed and employed people in order to evaluate the impact of employment on the population’s mental health. Results: All groups presented from moderate to high CPDI and FCV-19S scores, and students and unemployed people had higher levels of stress and fear. Conclusion: COVID-19 pandemic had a significant impact on the Brazilian population, with students and the unemployed showing the highest rates of stress and fear.

Background and Aims: Patients and the general public are under insurmountable psychological pressure which may lead to various psychological problems, such as anxiety, fear, depression, and insomnia, causing, consequently, the impaired quality of life. Psychological crisis intervention plays a pivotal role in the overall deployment of health-related quality of life and disease control. A novel Severe Acute Respiratory Syndrome Coronavirus-2 (SARS- CoV-2), a pathogen of the new coronavirus disease (COVID-19), has affected several sector activities, including people’s health. To enhance infection control methods, appropriate interventions, and public health policies, the present study aims to assess the fear and peri-traumatic stress during the COVID-19 inBrazil. Method: A cross-sectional survey has been conducted from April 12th to 18th from 2020, using the Peri-Traumatic Distress Scale (CPDI) and the Fear Scale (FCV-19S) aiming to measure the peri-traumatic stress and fear as psychological reactions during the COVID-19 pandemic. For that purpose, an online spreadsheet was used to send the questionnaire and scales to a sample of 1844 participants as a collecting information tool. COVID-19 pandemic continues to spread exponentially worldwide, especially in America. At the end of August 2020, Brazil is one of the most affected countries with more than three and a half million cases and up to 114,250 deaths. This study aims to assess the fear and peri-traumatic stress during the COVID-19 pandemics in Brazil, to enhance infection control methods, appropriate interventions, and public health policies. A cross-sectional survey has been conducted from April 12th to 18th using the Peri-Traumatic Distress Scale (CPDI) and the Fear Scale (FCV-19S) aiming to measure the peri-traumatic stress and fear as psychological reactions during the COVID-19 pandemic. For that purpose, an online spreadsheet was used to send the questionnaire and scales to a sample of 1844 participants as a collecting information tool. Significant positive relationship (r = 0.660, p der differences as in both scales women’s mean scores are higher showing that it is paramount that women’s voices were represented in policy spaces as socially constructed gender roles place them in a strategic position to enhance multi-level interventions (primary and secondary effects of COVID-19), equitable policies, and new approaches to control the pandemic.

Background: Pain perception may result from orthodontic treatment and should be considered in clinical management. Its assessment should not be limited to the intensity of perceived pain but should also consider the psychosocial and behavioral aspects involved. Objective: The aims of this study were to adapt and validate the Multidimensional Pain Inventory (MPI) for orthodontic patients, and present a proposal to calculate an overall score of the pain-related aspects and compare these scores with the usual methodology used. Methods: Face validity of the MPI was estimated and orthodontic version (MPI-Orthodontic) was developed. Factorial validity was evaluated by confirmatory analysis. Convergent and discriminant validity and reliability were estimated. The fit of Second-Order Hierarchical Models was estimated. Concurrent validity of MPI-Orthodontic was evaluated against the Visual Analogue Scale. Invariance of the factorial models was evaluated for independent samples and according to sex and age. Overall score was calculated using the matrix of regression weights and compared to simple arithmetic mean. Results: 507 individuals (63.3% women;age: 26.32 (SD = 11.70) years) participated. For the fit of Part I (psychosocial aspects) of the MPI-Orthodontic, it added correlation between two items and excluded one item;for Part II (behavioral aspects) two items were excluded. The models presented adequate fit to the sample. Reliability was adequate. MPI-Orthodontic presented invariance for independent samples and adequate concurrent and divergent validity. Score obtained with the simple arithmetic mean was overestimated for Part I and underestimated for Part II. Conclusion: MPI-Orthodontic was valid, reliable and invariant for the evaluation of the orthodontic pain. It is recommended to calculate overall weighted scores for pain assessment.

Aim: Hepatitis C virus (HCV) cirrhosis is an important cause of hepatocellular carcinoma (HCC). This study aimed to identify factors of HCC presence among HCV cirrhotic patients with and without small diameter HCC (≤ 3 cm). Methods: A case control transversal study between 1998 and 2003 including 93 patients: 31 with small diameter HCC and 62 without HCC. Groups were matched by age and gender. Multiple logistic regression analysis using Akaike Information Criteria to estimate the probability of HCC was performed. A model score was generated and bootstrap analysis was performed for internal validation. Results: Three significant laboratorial variables for HCC presence were found: alanine aminotransferase > 37 U/L [odds ratio (OR): 7.43 (1.61-34.19),P = 0.01], alpha-fetoprotein > 20 ng/mL [OR: 16.2 (4.17-63.01),P del score with an area under curve of 0.79 (95% CI: 0.7-0.89) was built based on these variables. The negative predictive value of those classified as at low risk of HCC was 99.1%. Conclusion: An easy and practical model score was generated. It may be an auxiliary tool for identification of HCV patients with low probability of small diameter HCC at initial evaluation composed of three serum examinations used in routine outpatient clinical practice.

Petroleum and Natural Gas still represent a considerable share in terms of energy consumption in the current global matrix, so that its exploration/exploitation is present in the market and driving activities in locations of specific complexities, as the ones along unconventional hydrocarbon resources from the Brazilian pre-salt. The daily cost of well drilling under harsh conditions can exceed US $1 million a day, turning any type of downtime or necessary maintenance during the activities to be very costly, moment in which processes optimization starts to be a key factor in costs reduction. Thus, new technologies and methods in terms of automating and optimizing the processes may be of great advantages, having its impact in total related project costs. In this context, the goal of this research is to allow a computation tool supporting achieving a more efficient drilling process, by means of drilling mechanics parameters choosiness aiming rate of penetration (ROP) maximization and mechanic specific energy (MSE) minimization. Conceptually, driven by the pre-operational drilling test curve trends, the proposed system allows it to be performed with less human influences and being updateable automatically, allowing more precision and time reduction by selecting optimum parameters. A Web Operating System (Web OS) was designed and implemented, running in online servers, granting accessibility to it with any device that has a browser and internet connection. It allows processing the drilling parameters supplied and feed into it, issuing outcomes with optimum values in a faster and precise way, allowing reducing operating time.
