Interpreting siliciclastic sedimentation in the upper Paleozoic Mulargia-Escalaplano Basin(Sardinia, Italy):influence of tectonics on provenance

Late to post-Variscan molassic basins of Late Pennsylvanian-Permian age are exposed in Sardinia(Italy). Here, the compositional and stratigraphic evolution of the Mulargia-Escalaplano sedimentary basin(central Sardinia) has been investigated to highlight how the tectono-magmatic processes have influenced the sedimentation. Ruditic and arenitic samples were collected along well-characterized stratigraphic sections to provide a new insight into the impact of the tectono-magmatic processes on siliciclastic sedimentation. As a result, the conglomerates are mainly clast-supported, petromictic, and thus immature, with no defined maturity trend upwards. Nevertheless, pebble composition changes in times from Variscan basement pebble-rich to volcanic rock-rich, as a consequence of the basin widening and the dismantling and reworking of the coeval volcanic activity. The sandstone composition clearly changes from quartzolithic to feldspatholithic upwards, as a response to the same change of feeding and reworking of the volcanic rocks. Occasionally, interbedded quartzolithic arenites suggest exceptional floods carrying debris from the far borders of the basin. Also, the immature sandstone composition has been interpreted as being controlled by a continuous supply of fresh debris and to a rapid burial rate. In addition, the disappearance of metaradiolarite(lydite AA) Paleozoic grains in the sandstone mineral suite could represent a distinctive marker of a progressive unroofing of the Variscan chain and a clastic supply from deeper tectonic units.

Background and study aims: The current visualization of small- bowel strictures using traditional radiological methods is associated with high radiation doses and false- negative results. These methods do not always reveal small- bowel patency for solids. The aim is to assess the safety of the given patency system and its ability to detect intestinal strictures in patients with strictures that are known or suspected radiologically. Materials and methods: The given patency capsule is composed of lactose, remains intact in the gastrointestinal tract for 40- 100 hours post ingestion, and disintegrates thereafter. A total of 34 patients with small- bowel stricture were prospectively enrolled; 30 had a previous diagnosis of Crohn’ s disease, three had adhesion syndrome and in one ischemic enteritis was suspected. Of the patients, 15 (44.1% ) had previously undergone surgery. Following ingestion, the capsule was monitored for integrity and transit time, using a specially designed given scanner and also radiologically. Seventeen patients had been enrolled with the intent of using the patency capsule as a preliminary test in patients with small- bowel strictures before undergoing video capsule endoscopy. Results: 30 patients (88.2% ) retrieved the capsule in the stool; it was intact in 20 (median transit time 22 hours), and disintegrated in 10 patients (median transit time 53 hours). Six patients complained of abdominal pain which disappeared within 24 hours. The scanner successfully indicated the presence of the capsule in 94% of cases. Ten patients underwent video capsule endoscopy following the patency capsule examination; in all of these the video capsule passed through the small- bowel stricture. Conclusions: This feasibility study has shown that the given patency capsule is a safe, effective, and convenient tool for assessment of functional patency of the small bowel. It can indicate functional patency even in cases where traditional radiology indicates stricture.

Background: Endoscopic removal of malfunctioning self-expandable metallic biliary stents (SEMS) is difficult and not well described. The aim of this study is to review the indications, the techniques, and the results of SEMS removal in a cohort of patients with malfunctioning stents. Methods: All patients who underw ent an attempt at endoscopic removal of biliary SEMS over a 5-year period were retrospectively identified. The main indications for SEMSremovalwere the followi ng: distal migration of the stent or impaction to the duodenum, impaction into t he bile-duct wall, tissue ingrowth, and inappropriate length of the stent causi ng occlusion of intrahepatic ducts. SEMS were removed by using foreign-body for ceps or polypectomy snares. Results: Endoscopic removal of 39 SEMS (13 uncovered and 26 covered) was attempted in 29 patients (17 men; mean age, 66 years). SEMS extraction was attempted after a mean of 7.5 months (8.75 months standard devia tion) post-SEMS insertion. Removal was successful in 20 patients (68.9%) and i n 29 SEMS (74.3%). Covered SEMS were effectively removed more frequently than u ncovered ones: 24 of 26 (92.3%) and 5 of 13 (38.4%), respectively (p gth, stent-mesh design (zigzag vs. interlaced), and indication for removal were no t predictive of success at stent removal. Conclusions: Endoscopic removal of bil iary SEMS is feasible and safe in more than 70%of cases. Because only 38%of un covered SEMS were removable, the presence of a stent covering is the only factor predictive of successful stent extraction. The presence of diffuse and severe i ngrowth was the main feature limiting SEMS removal.
