BACKGROUND The incidence of hypertriglyceridemic acute pancreatitis(HTG-AP)has increased yearly,but updated population-based estimates on the incidence of HTG-AP are lacking.Reducing serum triglyceride(TG)levels quickly is crucial in the early treatment of HTG-AP.Decreased serum TG levels are treated by non-invasive methods,which include anti-lipidemic agents,heparin,low-molecular weight heparin,and insulin,and invasive methods,such as blood purification including hemoperfusion(HP),plasmapheresis,and continuous renal replacement therapy.However,authoritative guidelines have not been established.Early selection of appropriate treatment is important and beneficial in controlling the development of HTG-AP.AIM To evaluate the effect between patients treated with intravenous insulin(INS)and HP to guide clinical treatment.METHODS We retrospectively reviewed 371 patients with HTG-AP enrolled in the Department of Fujian Provincial Hospital form April 2012 to March 2021.The inpatient medical and radiologic records were reviewed to determine clinical features,severity,complications,mortality,recurrence rate,and treatment.Multivariate logistic regression analyses were used to analyze risk factors for severe HTG-AP.Propensity score matching was used to compare the clinical outcomes of INS and HP.RESULTS A total of 371 patients met the HTG-AP criteria.The incidence of HTG-AP was increased by approximately 2.6 times during the 10 years(8.4%in April 2012-March 2013 and 22.3%in April 2020-March 2021).The highest incidence rate of acute pancreatitis was observed for men in the age group of 30-39 years.The amylase level was elevated in 80.1%of patients but was only three times the normal value in 46.9%of patients.The frequency of severe acute pancreatitis(26.9%),organ failure(31.5%),rate of recurrence(32.9%),and mortality(3.0%)of HTG-AP was high.Improved Marshall score,modified computed tomography severity index score,baseline TG,baseline amylase,C-reactive protein(CRP),albumin,aspartate aminotransferase,low-density lipoprotein ch

AIM To assess whether elevated serum carcinoembryonic antigen(CEA) is in the inferior prognosis for pathological lymph node-negative(p N_0) gastric cancer(GC) patients who underwent D_2 gastrectomy.METHODS About 469 p N0 GC patients,who received D^2 radical gastrectomy were retrospectively analyzed. The X-tile plots cut-off point for CEA were 30.02 ng/m L using minimum P-value from log-rank χ~2 statistics,and p N_0 GC patients were assigned to two groups: those more than 30.02 ng/m L(n = 48;CEA-high group) and those less than 30.02 ng/m L(n = 421;CEA-low group). Clinicopathologic characteristics were compared usingPearson's χ2 or Fisher's exact tests,and survival curves were so manufactured using the Kaplan-Meier method. Univariate and multivariate analysis were carried out using the logistic regression method.RESULTS The percentage of vessel carcinoma embolus(31.35% vs 17.1%) and advanced GC(T_(2-4b))(81.25% vs 65.32%) were higher in CEA-high group than CEA-low group. The CEA-positive patients had a significantly poorer prognosis than the CEA-nagetive patients in terms of overall survival(57.74% vs 90.69%,P lus(all P > 0.05). Multivariate survival analysis showed that CEA(OR = 4.924),and T category(OR = 2.214) were significant prognostic factors for stage p N0 GC(all P luSION Those pretreatment serum CEA levels over 30.02 ng/m L on behalf of worse characteristics and unfavourable tumor behavior,and a poor prognosis for a nearly doubled risk of mortality in GC patients.

BACKGROUND Alternative splicing(AS)increases the diversity of mRNA during transcription;it might play a role in alteration of the immune microenvironment,which could influence the development of immunotherapeutic strategies against cancer.AIM To obtain the transcriptomic and clinical features and AS events in stomach adenocarcinoma(STAD)from the database.The overall survival data associated with AS events were used to construct a signature prognostic model for STAD.METHODS Differentially expressed immune-related genes were identified between subtypes on the basis of the prognostic model.In STAD,2042 overall-survival-related AS events were significantly enriched in various pathways and influenced several cellular functions.Furthermore,the network of splicing factors and overallsurvival-associated AS events indicated potential regulatory mechanisms underlying the AS events in STAD.RESULTS An eleven-AS-signature prognostic model(CD44|14986|ES,PPHLN1|21214|AT,RASSF4|11351|ES,KIAA1147|82046|AP,PPP2R5D|76200|ES,LOH12CR1|20507|ES,CDKN3|27569|AP,UBA52|48486|AD,CADPS|65499|AT,SRSF7|53276|RI,and WEE1|14328|AP)was constructed and significantly related to STAD overall survival,immune cells,and cancer-related pathways.The differentially expressed immune-related genes between the high-and low-risk score groups were significantly enriched in cancer-related pathways.CONCluSION This study provided an AS-related prognostic model,potential mechanisms for AS,and alterations in the immune microenvironment(immune cells,genes,and pathways)for future research in STAD.

Coupling and coordinated evolution characteristics of regional economy-energy-carbon emission multiple systems:A case study of main China's Basin

Comprehensive analysis of the Economy-Energy-Carbon Emission(EECE)system is beneficial for promoting sustainable social development.This study analyzes the system development of major watersheds in China from 2010 to 2019.The research fully considers the system’s internal and external inputs and outputs and proposes an evaluation index system for regional EECE coupling and coordinated development.Then,using the difference in system weight allocation to improve the coupling and coordination model,the study explores the dynamic system’s coupling and coordination.The results show that(1)The development of the system structure is relatively stable,but the overall development status is not ideal;(2)The downstream of China’s main river basins has obvious economic advantages,while the energy system fluctuates greatly.The efficiency of the carbon emission system will decrease in areas with rapid economic development.The coupling and coordination level of the EECE system is better in the Yangtze River Basin than in the Yellow River Basin;(3)From the perspective of dynamic coordinated development,the main river basins have been divided into two states since 2012,but it is relatively stable overall.Regional dynamic coordination is often at a disadvantage in regions with rapid economic and energy development;(4)The coupling coordination degree of the two river basins has significant positive spatial autocorrelation.Most provinces’significant spatial clustering characteristics of the coupling coordination degree are High-High type.Low-Low type provinces are mainly concentrated downstream.The research process has certain reference significance for the collaborative governance of complex regional systems.

G protein-coupled receptors(GPCRs),the largest family of human membrane proteins and an important class of drug targets,play a role in maintaining numerous physiological processes.Agonist or antagonist,orthosteric effects or allosteric effects,and biased signaling or balanced signaling,characterize the complexity of GPCR dynamic features.In this study,we first review the structural advancements,activation mechanisms,and functional diversity of GPCRs.We then focus on GPCR drug discovery by revealing the detailed drug-target interactions and the underlying mechanisms of orthosteric drugs approved by the US Food and Drug Administration in the past five years.Particularly,an up-to-date analysis is performed on available GPCR structures complexed with synthetic small-molecule allosteric modulators to elucidate key receptor-ligand interactions and allosteric mechanisms.Finally,we highlight how the widespread GPCR-druggable allosteric sites can guide structure-or mechanism-based drug design and propose prospects of designing bitopic ligands for the future therapeutic potential of targeting this receptor family.

This study aimed to explore the chemical looping gasification(CLG)reaction characteristics of the metal-supported composite phosphogypsum(PG)oxygen carriers(OCs)and the thermodynamic mechanism.The FactSage 7.1 thermodynamic simulation was used to explore the oxygen release and H_(2)S removal mechanisms.The experimental results showed that the syngas yield of CLG with PG-CuFe_(2)O_(4)was more than that with PG-Fe_(2)O_(3)20/CuO40 or PG-Fe_(2)O_(3)30/CuO30 OC at 1023 K when the water vapor content was 0.3.Furthermore,the maximum syngas yield of the CO selectivity was 70.3% and of the CO_(2)selectivity was 23.8%.The H_(2)/CO value was 0.78,and the highest carbon conversion efficiency was 91.9% in PG-CuFe_(2)O_(4)at the gasification temperature of 1073 K.The metal-supported PG composite oxygen carrier was proved not only as an oxygen carrier to participate in the preparation of syngas but also as a catalyst to catalyze coal gasification reactions.Furthermore,both the experimental results and FactSage 7.1 thermodynamic analysis revealed that the trapping mechanism of H_(2)S by composite OCs was as follows:CuO first lost lattice oxygen as an oxygen carrier to generate Cu_(2)O,which,in turn,reacted with H_(2)S to generate Cu_(2)S.This study provided efficient guidance and reference for OC design in CLG.

Shaped charge warhead is important for enhancing the damage performance of underwater weapons.This paper used finite element analysis software and based on JPC water penetration experiments to examine the influence of liner parameters(wall thickness,material),charge aspect ratio,and stand-off distance on the movement characteristics of JPC in water.The findings reveal that the head diameter of the JPC increases and experiences significant erosion after entering the water,the effective length of the JPC in water undergoes two distinct phases:a growth phase and a decrease phase,with the velocity of the JPC decaying exponentially.Increasing the liner thickness,stand-off distance and the charge aspect ratio can improve the erosion resistance and the velocity retention capacity of the JPC.The optimal ranges for liner thickness and stand-off distance are 0.0363D_(k) to 0.0545D_(k)(D_(k) is the charge diameter),the stand-off distance should be within 1.0D_(k).After the charge aspect ratio higher than 1.25,the charge structure exerts minimal influence the movement characteristics of the JPC in water.Material density plays a crucial role in the velocity decay pattern of the JPC during penetration.JPC with higher densities exhibit superior velocity retention capabilities in water,with the velocity decay pattern converging if the densities are similar.Consequently,copper,tantalum and tungsten liners are deemed appropriate for underwater shaped charge warhead.Finally,the results will provide an important reference for the design of underwater shaped charge warhead.

Immune checkpoint inhibitors(ICIs)are used to relieve and refuel anti-tumor immunity by blocking the interaction,transcription,and translation of co-inhibitory immune checkpoints or degrading co-inhibitory immune checkpoints.Thousands of small molecule drugs or biological materials,especially antibody-based ICIs,are actively being studied and antibodies are currently widely used.Limitations,such as anti-tumor efficacy,poor membrane permeability,and unneglected tolerance issues of antibody-based ICIs,remain evident but are thought to be overcome by small molecule drugs.Recent structural studies have broadened the scope of candidate immune checkpoint molecules,as well as innovative chemical inhibitors.By way of comparison,small molecule drug-based ICIs represent superior oral bioavailability and favorable pharmacokinetic features.Several ongoing clinical trials are exploring the synergetic effect of ICIs and other therapeutic strategies based on multiple ICI functions,including immune regulation,anti-angiogenesis,and cell cycle regulation.In this review we summarized the current progression of small molecule ICIs and the mechanism underlying immune checkpoint proteins,which will lay the foundation for further exploration.

Liquid crystals are a vital component of modern photonics,and recent studies have demonstrated the exceptional sensing properties of stimuli-responsive cholesteric liquid crystals.However,existing cholesteric liquid crystal-based sensors often rely on the naked eye perceptibility of structural color or the measurement of wavelength changes by spectrometric tools,which limits their practical applications.Therefore,developing a platform that produces recognizable sensing signals is critical.In this study,we present a visual sensing platform based on geometric phase encoding of stimuli-responsive cholesteric liquid crystal polymers that generates real-time visual patterns,rather than frequency changes.To demonstrate this platform’s effectiveness,we used a humidity-responsive cholesteric liquid crystal polymer film encoded with a q-plate pattern,which revealed that humidity causes a shape change in the vortex beam reflected from the encoded cholesteric liquid crystal polymers.Moreover,we developed a prototype platform towards remote humidity monitoring benefiting from the high directionality and long-range transmission properties of laser beams carrying orbital angular momentum.Our approach provides a novel sensing platform for cholesteric liquid crystals-based sensors that offers promising practical applications.The ability to generate recognizable sensing signals through visual patterns offers a new level of practicality in the sensing field with stimuli-responsive cholesteric liquid crystals.This platform might have significant implications for a broad readership and will be of interest to researchers working in the field of photonics and sensing technology.
