Acute myeloid leukemia(AML)is a malignancy due to clonal expansion of hematopoietic stem and progenitor *** recent years,the factors,such as aging of population,increased exposure to toxic chemicals and prolonged survival period of patients receiving radiotherapy and chemotherapy for other malignancies,contribute to significantly increased incidence of AML in the elderly(60 years old),whereby the median age of AML approaches 70 years old.[1,2]Although the past decades have witnessed a significant increase in cure rate of young AML patients,the long-term survival rate of elderly AML patients is still very low,of which 5-year survival rate is only 5%-10%.[3-5]Compared with young AML patients,elderly AML patients have unique biological and clinical *** the one hand,most of them have complex chromosome karyotype,higher expression rate of multidrug resistance gene,and the disease is often secondary to some physical and chemical factors.

Bruton’s tyrosine kinase inhibitors(BTKis)have revolutionized the treatment of B-cell ***,safety issues related to the use of BTKis may hinder treatment continuity and further affect clinical efficacy.A comprehensive and systematic expert consensus from a pharmacological perspective is lacking for safety issues associated with BTKi treatment.A multidisciplinary consensus working group was established,comprising 35 members from the fields of hematology,cardiovascular disease,cardio-oncology,clinical pharmacy,and evidencebased *** evidence-based expert consensus was formulated using an evidence-based approach and the Delphi *** Joanna Briggs Institute Critical Appraisal(JBI)tool and Grading of Recommendations Assessment,Development,and Evaluation(GRADE)approach were used to rate the quality of evidence and grade the strength of recommendations,*** consensus provides practical recommendations for BTKis medication based on nine aspects within three domains,including the management of common adverse drug events such as bleeding,cardiovascular events,and hematological toxicity,as well as the management of drug-drug interactions and guidance for special *** multidisciplinary expert consensus could contribute to promoting a multi-dimensional,comprehensive and standardized management of BTKis.

1 概述肝纤维化是存在于大多数慢性肝脏疾病过程中的病理变化,主要表现为肝组织内细胞外基质(extracellular matrix,ECM)过度增生与沉积,从而导致肝脏组织结构异常改变,并影响肝脏正常生理功能。其本质是慢性肝病过程中一种可逆的肝组织损伤过度修复反应,肝纤维化的持续存在,伴随正常肝实质细胞的坏死和凋亡,而ECM不断累积,肝实质逐步被ECM形成的瘢痕组织取代,最终形成肝硬化,甚至引起门静脉高压或肝癌,导致肝功能衰竭[1]。

The disease burden of hepatocellular carcinoma (HCC) in China is heavy, and the prognosis is stillunfavourable. Therefore, early screening of high-risk groups of HCC through simple methods is the key toachieving early diagnosis and treatment and improving survival. At present, alpha-fetoprotein and otherhematological tests are still the main methods in the early screening of HCC, but the sensitivity andspecificity are limited, and the risk of missed diagnosis is high. In recent years, with the continuousdevelopment of science and technology, the improvement of traditional detection methods and theemergence of novel markers such as methylated deoxyribonucleic acid and microRNA have brought hopefor further improving the sensitivity and specificity of early HCC screening. This consensus summarizesthe research progress of traditional and new hematological test methods and puts forward expertguidance on the role of hematological markers in the early screening of HCC to provide a basis forimproving the prevention and control level in China.

This study aimed to investigate the treatment outcomes of esophageal variceal bleeding(EVB)in China.A total of 1087 cases were collected from 19 hospitals in 16 large and medium sized cities across China between January 1st,2005 and January 1st,*** were 313 cases(29.0%)of mild(1500 mL)*** hemostasis was achieved in 89.8%of *** hundred and eighty-five cases were treated by medication with a hemostasis rate of 91.8%.Seventy-one cases were treated using a Sengstaken-Blakemore tube with a hemostasis rate of 54.9%.Thirty-seven cases were treated with emergency endoscopic variceal ligation with a hemostasis rate of 83.8%.Seventy-seven cases were treated with endoscopic sclerotherapy with a hemostasis rate of 94.8%.Forty-three cases were treated with emergency surgical operation with a hemostasis rate of 95.3%.Sixty-six cases were treated with combined therapy with a hemostasis rate of 97.0%.There was a significant difference(Pliver dysfunction and activation of hepatitis were predictive factors for suc-cessful *** cases of EVB were mild and mod-erate in *** first-line treatment for EVB is *** endoscopic intervention has not been widely available *** overall management out-come of EVB has been improved.
