Better Selection Model for EML4-ALK Fusion Gene Test in Patients with Non-Small-Cell Lung Cancer

Background: In the last decade, the search for gene mutations in lung cancer has been constantly growing. EGFR, KRAS mutations and, recently, the EML4-ALK fusion can guide the selection of treatment for patients who carry a specific mutation. Methods: During 2010-2011, EML4-ALK fusion test has been performed in Israel, mostly for wild type EGFR non-squamous NSCLC patients based on fluorescent in-situ hybridization (FISH) technique to detect EML4-ALK rearrangements. Results: Between January 2010 and December 2011, 3341 patients were diagnosed with lung cancer in Israel. Of the 2997 patients with NSCLC 687 had squamous cell carcinoma and 2310 had non-squamous NSCLC. This study focused on available 125 non-squamous NSCLC cases in which analysis for EML4-ALK rearrangement was available. All were negative for EGFR mutation. Nineteen (15.2%) were found positive for the fusion, a figure 2 - 10 times higher compared with previously reported findings. The EML4-ALK fusion was significantly more prevalent in younger male patients (52.1 vs. 61.3 years, p = 0.049), in whom every additional year reduced the chance to find the fusion by 7% [CI = 0.93 (0.88 - 0.99), p = 0.03]. Conclusions: A stepwise approach based on histology and prior EGFR analysis to detect EML4-ALK fusion is highly efficient with a related increased yield of detection. We recommend testing patients with non-squamous cell lung carcinoma after ruling out an EGFR mutation. The chance to find the ALK fusion is significantly greater in younger men.

A multiscale analysis of herbaceous species richness in a Mediterranean ecosystem

Aims studies of species distribution patterns traditionally have been conducted at a single scale,often overlooking species-environment relationships operating at finer or coarser *** diversity-related hypotheses at multiple scales requires a robust sampling design that is nested across *** chief motivation in this study was to quantify the contributions of different predictors of herbaceous species richness at a range of local *** Here,we develop a hierarchically nested sampling design that is balanced across scales,in order to study the role of several envi-ronmental factors in determining herbaceous species distribution at various scales *** focus on the impact of woody vegetation,a relatively unexplored factor,as well as that of soil and *** detection and ranging(liDAR)imaging enabled precise characterization of the 3D structure of the woody vegetation,while acoustic spectrophotometry allowed a particularly high-resolution mapping of soil CaCo3 and organic matter *** Findings We found that woody vegetation was the dominant explanatory variable at all three scales(10,100 and 1000 m2),accounting for more than 60%of the total explained *** addition,we found that the species richness-environment relationship was scale *** studies that explicitly address the issue of scale do so by comparing local and regional *** results show that efforts to conserve plant communities should take into account scale dependence when analyzing species richness-environment relationships,even at much finer resolutions than local *** addition,conserving heterogeneity in woody vegetation structure at multiple scales is a key to conserving diverse herbaceous communities.

The plant-specific GIBBERELLIN （GA）-STIMULATED TRANSCRIPT 1 （GAST1）-like genes have been identified in numerous plant species and many of them are regulated by the phytohormone GA. All encode small proteins with a conserved cysteine-rich C-terminus, containing 12 cysteines in conserved positions. The fact that the number and positions of all 12 cysteines have not changed during the course of evolution strongly implies fundamental and important biochemical and bioloaical functions.

ConfDTree: A Statistical Method for Improving Decision Trees

Decision trees have three main disadvantages： reduced performance when the training set is small; rigid decision criteria; and the fact that a single ＂uncharacteristic＂ attribute might ＂derail＂ the classification process. In this paper we present ConfDTree （Confidence-Based Decision Tree） -- a post-processing method that enables decision trees to better classify outlier instances. This method, which can be applied to any decision tree algorithm, uses easy-to-implement statistical methods （confidence intervals and two-proportion tests） in order to identify hard-to-classify instances and to propose alternative routes. The experimental study indicates that the proposed post-processing method consistently and significantly improves the predictive performance of decision trees, particularly for small, imbalanced or multi-class datasets in which an average improvement of 5%-9% in the AUC performance is reported.

Continuous,real-time monitoring and identification of bacteria through detection of microbially emitted volatile molecules are highly sought albeit elusive *** introduce an artificial nose for sensing and distinguishing vapor molecules,based upon recording the capacitance of interdigitated electrodes(IDEs)coated with carbon dots(C-dots)exhibiting different *** of the C-dot-IDEs to volatile molecules induced rapid capacitance changes that were intimately dependent upon the polarities of both gas molecules and the electrode-deposited *** deciphered the mechanism of capacitance transformations,specifically substitution of electrode-adsorbed water by gas molecules,with concomitant changes in capacitance related to both the polarity and dielectric constants of the vapor molecules *** C-dot-IDE gas sensor exhibited excellent selectivity,aided by application of machine learning *** capacitive C-dot-IDE sensor was employed to continuously monitor microbial proliferation,discriminating among bacteria through detection of distinctive“volatile compound fingerprint”for each bacterial *** C-dot-IDE platform is robust,reusable,readily assembled from inexpensive building blocks and constitutes a versatile and powerful vehicle for gas sensing in general,bacterial monitoring in particular.

Preventive treatment of alveolar pulmonary edema of cardiogenic origin

Objective To evaluate the efficacy of preventive treatment (PT) on alveolar pulmonary edema (APE) of cardiogenic origin using a monitor based on principles of internal thoracic impedance (ITI) *** We conducted blinded clinical trials on patients with ST-elevation myocardial infarction (STEMI) and monitored whether the condition would progress to APE. ITI was measured noninvasively by the Edema Guard Monitor (EGM, model RS-207) every 30 min. The measurement threshold for the diagnosis of APE was fixed at > 12% decrease in ITI from baseline as described in our methodology. The patients were divided into one group that received standard treatment after the appearance of clinical signs of APE without considering the prediction of APE by EGM devise (Group 1), and another group of asymptomatic patients in whom development of APE was predicted by using only EGM measurements (Group 2). The latter participants’PT consisted of furosemide, intravenous nitroglycerine and supplemental oxygen. Results One-hundred and fifty patients with acute STEMI were enrolled into this study. Group 1 included 100 patients (53% males, age 64.1 ±12.6 years). Treatment was started after the clinical appearance of overt signs of APE. Group 2 included 50 patients (54% males, age 65.2 ±11.9 years) who received PT based on EGMmeasurements. Group 2 had significantly fewer cases of APE (n = 4, 8%) than Group 1 (n = 100, 100%) (P > 0.001). While APE was lethal in six (6%) Group 1 patients, PT resulted in prompt resolution of APE in all four (8%) Group 2 patients. Conclusion ITI is a usefulmodality for early diagnosis and PT of pulmonary edema of cardiogenic origin.

Ultra-processed food targets bone quality via endochondral ossification

Ultra-processed foods have known negative implications for health;however,their effect on skeletal development has never been ***,we show that young rats fed ultra-processed food rich in fat and sugar suffer from growth retardation due to lesions in their tibial growth *** bone mineral density decreases significantly,and the structural parameters of the bone deteriorate,presenting a sieve-like appearance in the cortices and poor trabecular parameters in long bones and *** results in inferior mechanical performance of the entire bone with a high fracture *** sequence analysis of the growth plates demonstrated an imbalance in extracellular matrix formation and degradation and impairment of proliferation,differentiation and mineralization *** findings highlight,for the first time,the severe impact of consuming ultra-processed foods on the growing *** pathology extends far beyond that explained by the known metabolic effects,highlighting bone as a new target for studies of modern diets.
