Outcomes after natural orifice extraction vs conventional specimen extraction surgery for colorectal cancer: A propensity scorematched analysis

BACKGROUND Natural orifice specimen extraction(NOSE)via the anus or vagina replaces conventional transabdominal specimen retrieval via the transabdominal route through a limited mid-line laparotomy or Pfannenstiel incision.Reducing the number of laparoscopic ports further decreases operative abdominal wall trauma.These techniques reduce the surgical wound size as well as the risk of incisionrelated morbidity.AIM To compare short-term outcomes following 3-port NOSE surgery with a matched cohort of conventional non-NOSE colorectal cancer surgery.METHODS Patients who underwent elective 3-port laparoscopic colorectal NOSE surgery between February to October 2021 were identified.Selection criteria for NOSE surgery was adapted from the 2019 International Consensus on Natural Orifice Specimen Extraction Surgery for colorectal cancer.Patients with clinical T4 or N2 tumors on staging computed tomography were also excluded.The propensity score-matched cohort was identified amongst patients who underwent conventional laparoscopic colorectal surgery from January 2019 to December 2020.Matching was performed in the ratio of 1:4 based on age,gender,type of resection,and p-tumor node metastasis staging.RESULTS Over the eight-month study duration,14 consecutive cases(nine female,five male)of elective 3-port laparoscopic surgery with NOSE were performed for colorectal cancer.Median age and body mass index were 70(range 43-82)years and 24.1(range 20.0-31.7)kg/m2 respectively.Six patients underwent transanal NOSE and eight had transvaginal NOSE.Median operative time,intraoperative blood loss and postoperative length of stay were 208(range 165-365)min,30(range 10-150)mL and 3(range 2-6)d respectively.Two(14%)suffered minor postoperative compilations not attributable to the NOSE procedure.Median follow-up duration was 12(range 8-15)mo.No instances of mortality,local or distant disease recurrence were recorded in this cohort.Compared to the conventional surgery cohort of 56 patients,the 3-port NOSE cohort had significantly quicker mean

Elderly patients over 80 years undergoing colorectal cancer resection: Development and validation of a predictive nomogram for survival

BACKGROUND Surgery remains the primary treatment for localized colorectal cancer(CRC).Improving surgical decision-making for elderly CRC patients necessitates an accurate predictive tool.AIM To build a nomogram to predict the overall survival of elderly patients over 80 years undergoing CRC resection.METHODS Two hundred and ninety-five elderly CRC patients over 80 years undergoing surgery at Singapore General Hospital between 2018 and 2021 were identified from the American College of Surgeons–National Surgical Quality Improvement Program(ACS-NSQIP)database.Prognostic variables were selected using univariate Cox regression,and clinical feature selection was performed by the least absolute shrinkage and selection operator regression.A nomogram for 1-and 3-year overall survival was constructed based on 60%of the study cohort and tested on the remaining 40%.The performance of the nomogram was evaluated using the concordance index(C-index),area under the receiver operating characteristic curve(AUC),and calibration plots.Risk groups were stratified using the total risk points derived from the nomogram and the optimal cut-off point.Survival curves were compared between the high-and low-risk groups.RESULTS Eight predictors:Age,Charlson comorbidity index,body mass index,serum albumin level,distant metastasis,emergency surgery,postoperative pneumonia,and postoperative myocardial infarction,were included in the nomogram.The AUC values for the 1-year survival were 0.843 and 0.826 for the training and validation cohorts,respectively.The AUC values for the 3-year survival were 0.788 and 0.750 for the training and validation cohorts,respectively.C-index values of the training cohort(0.845)and validation cohort(0.793)suggested the excellent discriminative ability of the nomogram.Calibration curves demonstrated a good consistency between the predictions and actual observations of overall survival in both training and validation cohorts.A significant difference in overall survival was seen between elderly patients stratified into

Artificial bone grafting materials such as collagen are gaining interest due to the ease of production and implantation.However,collagen must be supplemented with additional coating materials for improved osteointegration.Here,we report room-temperature atomic layer deposition(ALD)of MgO,a novel method to coat collagen membranes with MgO.Characterization techniques such as X-ray photoelectron spectroscopy,Raman spectroscopy,and electron beam dispersion mapping confirm the chemical nature of the film.Scanning electron and atomic force microscopies show the surface topography and morphology of the collagen fibers were not altered during the ALD of MgO.Slow release of magnesium ions promotes bone growth,and we show the deposited MgO film leaches trace amounts of Mg when incubated in phosphate-buffered saline at 37◦C.The coated collagen membrane had a superhydrophilic surface immediately after the deposition of MgO.The film was not toxic to human cells and demonstrated antibacterial properties against bacterial biofilms.Furthermore,in vivo studies performed on calvaria rats showed MgO-coated membranes(200 and 500 ALD)elicit a higher inflammatory response,leading to an increase in angiogenesis and a greater bone formation,mainly for Col-MgO500,compared to uncoated collagen.Based on the characterization of the MgO film and in vitro and in vivo data,the MgO-coated collagen membranes are excellent candidates for guided bone regeneration.

Structure-engineered platinum-based nanoframes(NFs)at the atomic level can effectively improve the catalytic performance for fuel cells and other heterogeneous catalytic fields.We report herein,a microwave-assisted wet-chemical method for the preparation of platinum-copper-cobalt NFs with tunable defect density and architecture,which exhibit enhanced activity and durability towards the electro-oxidation reactions of methanol(MOR)and formic acid(FAOR).By altering the reduction/capping agents and thus the nucleation/growth kinetics,trimetallic platinum-copper-cobalt hexapod NFs with different density high-index facets are achieved.Especially,the rough hexapod nanoframes(rh-NFs)exhibit excellent specific activities towards MOR and FAOR,7.25 and 5.20 times higher than those of benchmark Pt/C,respectively,along with prolonged durability.The excellent activities of the rh-NFs are assigned to a synergistic effect,including high density of defects and high-index facets,suitable d-band center,and open-framework structure.This synergistic working mechanism opens up a new way for enhancing their electrocatalytic performances by increasing defect density and high-index facets in open-framework platinum-based NFs.

The Antarctic,including the continent of Antarctica and the Southern Ocean,is a critically important part of the Earth system.Research in Antarctic meteorology and climate has always been a challenging endeavor.Studying and predicting weather patterns in the Antarctic are important for understanding their role in local-to-global processes and facilitating field studies and logistical operations in the Antarctic(e.g.,Walsh et al.,2018).Studies of climate change in the Antarctic are comparatively neglected compared to those of the Arctic.However,significant climate changes have occurred in the Antarctic in the past several decades,i.e.,a strong warming over the Antarctic Peninsula even with a recent minor cooling,a deepening of the Amundsen Sea low,a rapid warming of the upper ocean north of the circumpolar current,an increase of Antarctic sea ice since the late 1970s followed by a recent rapid decrease,and an accelerated ice loss from the Antarctic ice shelf/sheet since the late 1970s(e.g.,Turner et al.,2005;raphael et al.,2016;Sallée,2018;Parkinson,2019;Rignot et al.,2019).Investigating recent climate change in the Antarctic and the underlying mechanisms are important for predicting future climate change and providing information to policymakers.

以空间句法为方法论对考古遗址博物馆的遗址空间进行可达性与可见性的空间感知探索,以彼得·卒姆托的柯伦巴艺术博物馆、拉斐尔·莫内欧的梅里达国立古罗马艺术博物馆、莫伯治的南越王墓博物院等三座遗址博物馆为主要研究对象,探究其遗址空间的结构对空间感知的影响。结合建筑师对遗址空间的更新理念与空间句法分析成果,证明遗址空间的可达性与可见性对博物馆建筑具有一定连接意义,验证遗址空间对考古遗址博物馆的吸引程度与重要意义。

透水再生骨料混凝土(PRAC)是近年来新兴的一种环境友好型混凝土,利用建筑废弃物破碎所得再生骨料置换天然骨料,改善传统混凝土在城市路面建设中的弊端,实现废物资源再利用和建筑材料的可持续发展。总结了水灰比、孔隙率、再生骨料粒径和取代率、成型工艺等因素对透水再生骨料混凝土力学性能、透水性以及耐久性的影响,并提出了有待进一步研究的问题。已有研究成果表明再生骨料可以用于制备透水混凝土,应用于停车场、人行道及植生护坡等工程。

约翰·弥尔顿在书信《论教育》中对教育目的做了两种界定,在史诗《失乐园》里则通过两类天堂教师和三种话语主题、多幅启示性幻景对教育目的进行了哲学意义上的思考。拉斐尔借助天使堕落、创世和天体运行的话语主题将教育目的界定为上帝般理性力量的培养,米迦勒则借助人类未来的幻景将其界定为理智与情感的同步训练。两种界定实则相辅相成、互为补充:一种在将理性力量强化,另一种则是为善与恶的思考提供素材。二者之间又暗含某种一致:都是为了培养美德,强化意志,提升理智并使所有的官能都始终处于合适的关系之中。两类天堂老师都无意去压制感觉,而是要将感觉来培养,以便让人在造物主的形象中健康成长。这正是英国文艺复兴最后一位人文主义者的教育哲学。

人类的建筑活动可以一直追溯到文明诞牛之初2400年前,柏拉图曾明确指出,建筑师是"工匠的指挥者"。而职业建筑师工作室的出现,却是18世纪中叶之后的事情。直到19世纪,建筑才成为一种专门职业。几千年中,"建筑师"、"大师"一再被重新定义。在现代建筑教育出现之前,建筑师都是从其它行业"跨界"从事建筑,其工作室大都为一件具体的作品而存在。本文以"七日谈"的形式,试举现代建筑师工作室之先例七种。从古埃及大祭司印何阗到文艺复兴绘画巨匠拉斐尔,七位大师的故事或要引发当今建筑师对自身实践的思考。
