Urethral pressure profilometry in artificial urinary sphincter implantation: A case report

BACKGROUND Artificial urethral sphincter(AUS)implantation is currently the gold standard for treating moderate and severe urinary incontinence.Currently,cuffs are chosen based on the surgeon’s experience,and adjusting cuff tightness is crucial.The TDOC air-charged catheter has not been proven to be inferior to traditional catheters.We report how intraoperative urethral pressure profilometry is performed using a T-DOC air-charged catheter with ambulatory urodynamic equipment,to guide cuff selection and adjustment.CASE SUMMARY A 67-year-old man presented to our hospital with complete urinary incontinence following transurethral prostatectomy,using five pads/d to maintain local dryness.Preoperatively,the maximum urethral pressure(MUP)and maximum urethral closure pressure(MUCP)were 52 cmH2O and 17 cmH2O,respectively.An AUS was implanted.Intraoperatively,in the inactivated state,the MUP and MUCP were 53 cmH2O and 50 cmH2O,respectively;in the activated state,they were 112 cmH2O and 109 cmH2O,respectively.The pump was activated 6 wk postoperatively.Re-measurement of the urethral pressure on the same day showed that in the inactivated state,MUP and MUCP were 89 cmH2O and 51 cmH2O,respectively,and in the activated state,120 cmH2O and 92 cmH2O,respectively.One month after device activation,telephonic follow-up revealed that pad use had decreased from five pads/d to one pad/d,which met the standard for social continence(0-1 pad per day).There were no complications.CONCLUSION The relationship between intraoperative urethral pressure and urinary continence post-surgery can provide data for standardizing AUS implantation and evaluating efficacy.

Increased activation of the caudate nucleus and parahippocampal gyrus in Parkinson's disease patients with dysphagia after repetitive transcranial magnetic stimulation:a case-control study

Repetitive transcranial magnetic stimulation(r TMS)has been shown to effectively improve impaired swallowing in Parkinson's disease(PD)patients with dysphagia.However,little is known about how r TMS affects the corresponding brain regions in this patient group.In this casecontrol study,we examined data from 38 PD patients with dysphagia who received treatment at Beijing Rehabilitation Medicine Academy,Capital Medical University.The patients received high-frequency r TMS of the motor cortex once per day for 10 successive days.Changes in brain activation were compared via functional magnetic resonance imaging in PD patients with dysphagia and healthy controls.The results revealed that before treatment,PD patients with dysphagia showed greater activation in the precentral gyrus,supplementary motor area,and cerebellum compared with healthy controls,and this enhanced activation was weakened after treatment.Furthermore,before treatment,PD patients with dysphagia exhibited decreased activation in the parahippocampal gyrus,caudate nucleus,and left thalamus compared with healthy controls,and this activation increased after treatment.In addition,PD patients with dysphagia reported improved subjective swallowing sensations after r TMS.These findings suggest that swallowing function in PD patients with dysphagia improved after r TMS of the motor cortex.This may have been due to enhanced activation of the caudate nucleus and parahippocampal gyrus.The study protocol was approved by the Ethics Committee of Beijing Rehabilitation Hospital of Capital Medical University(approval No.2018 bkky017)on March 6,2018 and was registered with Chinese Clinical Trial Registry(registration No.Chi CTR 1800017207)on July 18,2018.

The sphingosine-1-phosphate/RhoA/Rho associated kinases/myosin light chain pathway in detrusor of female rats is down-regulated in response to ovariectomy

Background:Dysuria is one of the main symptoms of genitourinary syndrome of menopause,which causes serious disruption to the normal life of peri-menopausal women.Studies have shown that it is related to decrease of detrusor contractile function,but the exact mechanism is still poorly understood.Previous results have suggested that the sphingosine-1-phosphate(S1P)pathway can regulate detrusor contraction,and this pathway is affected by estrogen in various tissues.However,how estrogen affects this pathway in the detrusor has not been investigated.In this study,we detected changes of the S1P/RhoA/Rho associated kinases(ROCK)/myosin light chain(MLC)pathway in the detrusor of ovariectomized rats in order to explore the underlying mechanism of dysuria during peri-menopause.Methods::Thirty-six female Sprague-Dawley rats were randomly divided into SHAM(sham operation),OVX(ovariectomy),and E groups(ovariectomy+estrogen),with 12 rats in each group.We obtained bladder detrusor tissues from each group and examined the mRNA and protein levels of the major components of the S1P/RhoA/ROCK/MLC pathway using quantitative real-time polymerase chain reaction and Western blotting,respectively.We also quantified the content of S1P in the detrusor using an enzyme linked immunosorbent assay.Finally,we compared results between the groups with one-way analysis of variance.Results::The components of the S1P pathway and the RhoA/ROCK/MLC pathway of the OVX group were significantly decreased,as compared with SHAM group.The percent decreases of the components in the S1P pathway were as follows:sphingosine kinase 1(mRNA:39%,protein:45%)(both P0.05).The percent decreases of the components in the RhoA/ROCK/MLC pathway were as follows:ROCK2(protein:41%,mRNA:36%)(both P<0.0

Combustion crack-network reaction evolution model for highly-confined explosives

The evolution behavior of combustion crack reaction of highly confined solid explosives after non-shock ignition is governed by multiple dynamic processes,including intrinsic combustion of explosives,crack propagation,and rapid growth of combustion surface area.Here,the pressure increase can accelerate the combustion rate of explosives,and the crack propagation can enlarge the combustion surface area.The coupling between these two effects leads to the self-enhanced combustion of explosive charge system,which is the key mechanism for the reaction development after ignition.In this study,combustion cracknetwork(CCN) model is established to describe the evolution of combustion crack reaction of highly confined solid explosives after non-shock ignition and quantify the reaction violence.The feasibility of the model is verified by comparing the computational and experimental results.The results reveal that an increase in charge structure size causes an increase in the time of crack pressurization and extension of cracks due to the high temperature-generated gas flow and surface combustion during the initial stage of explosive reaction,but when the casing is fractured,the larger the charge structure,the more violent the late reaction and the larger the charge reaction degree.The input pressure has no obvious influence on the final reaction violence.Further,a larger venting hole area leads to better pressure relief effect,which causes slower pressure growth inside casing.Larger reserved ullage volume causes longer lowpressure induction stage,which further restrains the internal pressure growth.Furthermore,the stronger the casing constraint,the more rapid the self-enhanced combustion of the high temperaturegenerated gas,which results in more violent charge reaction and larger charge reaction degree during casing break.Overall,the proposed model can clarify the effects of intrinsic combustion rate of explosives,charge structure size,input pressure,relief area,ullage volume,and constraint strength on the reaction evolution

Experimental study of polyurea-coated fiber-reinforced cement boards under gas explosions

Five types of polyurea elastomers were synthesized by changing the isocyanate component and the mechanical properties of polyurea materials were measured. Fiber-reinforced cement boards(FRCB)strengthened by polyurea with different formulations were processed, and a series of experiments were carried out on the specimens with gas explosion devices. The results showed that the conventional mechanical properties of different types of polyureas had their own advantages. Based on the gas explosion overpressure criterion, the blast resistances of reinforced plates were quantitatively evaluated,and the best polyurea was selected to guide the formulation design. The three typical failure modes of polyurea-reinforced FRCBs were flexural, shear, and flexural-shear failure. Dynamic thermodynamics and shock wave spectral analysis revealed that the polyurea did not undergo a glass transition in the gas explosion tests but retained its elastic properties, allowing it to effectively wrap the fragments formed by the brittle substrates.

Chemical investigation of Beouverio felina,a marine-derived entomopathogenic fungus,obtained two new destruxin hexadepsipeptides(1 and 2)containing an/.-Ala^(5)residue instead of the usual A/-Me-L-Ala^(5).Interestingly,the presence of the/.-Ala^(5)residue enabled two slowly-exchanging solution conformers with cis/trons(ILE^(3))-(A/-Me-Val^(4))amide bond for 1 and 2 with 1.5:1 and 1:1 cis/trans ratios,respectively,while only the conformer with c/s amide bond was found in the solid-state.The solution and solid-state conformers were studied by NMR,X-ray diffraction and solution and solid-state ECD measurements,as well as by TDDFT-ECD calculations.Structural(conformational)parameter-activity correlations suggested that the extended destruxin geometry with the preferred c/s(ILE^(3))-(A/-Me-Val^(4))bond probably resulted in improved bioactivity and reduced toxicity for the destruxin class.This was the first report on the interconversion phenomenon of cis/trans amide bonds of destruxin class,which could give insights in the development of bioactive destruxin agents with lower toxicity.

Depression,a common mental illness,seriously affects the health of individuals and has deleterious effects on society.The prevention and treatment of depression has drawn the attention of many researchers and has become an important social issue.The treatment strategies for depression include drugs,psychotherapy,and physiotherapy.Drug therapy is ineffective in some patients and psychotherapy has treatment limitations.As a reliable adjuvant therapy,physiotherapy compensates for the shortcomings of drug and psychotherapy and effectively reduces the disease recurrence rate.Physiotherapy is more scientific and rigorous,its methods are diverse,and to a certain extent,provides more choices for the treatment of depression.Physiotherapy can relieve symptoms in many ways,such as by improving the levels of neurobiochemical molecules,inhibiting the inflammatory response,regulating the neuroendocrine system,and increasing neuroplasticity.Physiotherapy has biological effects similar to those of antidepressants and may produce a superimposed impact when combined with other treatments.This article summarizes the findings on the use of physiotherapy to treat patients with depression over the past five years.It also discusses several methods of physiotherapy for treating depression from the aspects of clinical effect,mechanism of action,and disadvantages,thereby serving as a reference for the in-depth development of physiotherapy research.

Trauma kills more than four million people worldwide each year,with chest trauma accounting for 25%of these deaths.Rib fractures are the main manifestation of chest trauma.Biodegradable Zn alloys offer a new option to overcome clinical problems caused by permanent rib fracture internal fixation mate-rials,e.g.,long-term stress masking and secondary surgery.In this study,the fabrication procedure of biodegradable Zn-0.5Li alloy rib plates is successfully developed,which consists of casting,hot-warm rolling,cutting,and pressing sequentially.Biomechanical three-point bending performance of the Zn al-loy rib plates is comparable to that of commercial pure Ti rib plates,much higher than that of pure Zn rib plates.In addition,the Zn alloy exhibits the best antibacterial ability against E.coli and S.aureus among the three materials.Although the Zn alloy exhibits a weaker MC3T3 cytocompatibility than pure Ti,it is better than pure Zn.This study provides a foundation for the future development of various biodegrad-able Zn alloy rib plates.

Although antiretroviral therapy(ART)is effective at suppressing the human immunodeficiency virus type 1(HIV-1)replication,HIV-1 infection is still a global public health problem.HIV-1 accessory protein viral protein R(Vpr)is a multifunctional protein with a primary role in regulating cellular apoptosis.[1]The amino acid(aa)positions 52-96 in the C-terminus of Vpr were identified as the apoptosis-regulating domain.[2,3]In this study,we found a natural Vpr variant truncated at the 54 aa position(*54Vpr)from HIV patients in the acquired immune deficiency syndrome(AIDS)phase that mediated both pro-and antiapoptotic cellular effects by interacting with distinct adenine nucleotide translocator(ANT)isoforms.A novel apoptosis-regulating domain was further identified in the 23-37 aa position in the N-terminus of Vpr.
