Acupoint catgut embedding versus acupuncture for simple obesity:a systematic review and Meta-analysis of randomized controlled trials

OBJECTIVE: To evaluate the clinical effectiveness and safety of acupoint catgut embedding and acupuncture on simple obesity by Meta-analysis. METHODS: Studies on clinical randomized controlled trials of acupoint catgut embedding for simple obesity which were published from January 2015 to November 2020 were searched in Cochrane Central Register of Control Trials(Central), Pub Med, China Science and Technology Journal Database, China National Knowledge Infrastructure Database and Wanfang databases. And those that met the inclusion criteria were screened. Rev Man5.3 was used for Meta-analysis. The “Risk of Bias” tool was used to evaluate the quality of included studies. R studio software was used for the measurement of publication bias. RESULTS: A total of 33 studies were included for Metaanalysis, including 2685 patients with simple obesity. Meta-analysis results showed the comparison of effectiveness rate was relative risk(RR) = 1.12, 95%CI(1.08, 1.16), body mass index(BMI) was mean difference(MD) =-1.12, 95%CI(-2.09,-0.14), waist circumference was MD =-2.14, 95%CI(-4.22,-0.06), and body mass was MD =-2.36, 95%CI(-3.99,-0.73). On the basis of diet and exercise intervention, the effectiveness rate [RR = 1.12, 95%CI(1.05, 1.19)], BMI [MD =-0.88, 95%CI(-1.35,-0.40)], waist circumference [MD =-1.10, 95%CI(-4.27, 2.07)], and body mass [MD =-0.68, 95%CI(-2.90, 1.54)]. The risk of bias of included literatures was low. CONCLUSIONS: Acupoint catgut embedding therapy was slightly better than acupuncture therapy in most of the outcomes. Moreover, the treatment frequency of acupoint catgut embedding is less with larger stimulation intensity, which is more conducive to clinical promotion.

Tuina for treatment of atopic eczema in children under 14 years: A systematic review of randomized controlled trials

Objective:Due to a lack of available effective treatments for atopic eczema (AE),non-pharmaceutical therapy such as Tuina has been frequently sought after as an alternative treatment.We evaluated the benefits and harms of Tuina for children with AE under 14 years of age.Methods:We searched for randomized controlled trials (RCTs) on Tuina in seven Chinese and English databases from their inception to June 2018.We included children under 14 years of age with AE received Tuina alone or combined with conventional medicine.Two authors independently extracted data and used the Cochrane 'Risk of bias' tool to assess the methodological quality.Effects were presented as relative risk with 95% confidence interval using RevMan 5.3.Data not suitable for statistical pooling were synthesized qualitatively.Results:Nine RCTs involving 1246 children with AE were included.The trials were at unclear or high risk of bias.Tuina manipulation in each trial was different.Compared to usual care,three trials showed greater effects of Tuina alone or combined with usual care for global symptoms and signs improvement,such as itching,and skin lesions.Four trials showed that Tuina had >50% improvement in symptoms and signs.Due to clinical heterogeneity,meta-analysis was not possible.At follow-up of between 4 weeks and 6 months,five trials of Tuina alone or Tuina combined with usual care showed lower rates of relapse compared to usual care alone.Four trials reported no occurrence of severe adverse events.Conclusions:Limited evidence demonstrates that Tuina may improve symptoms and signs of AE and reduce relapse rate in children <14 yrs with AE.Tuina is generally acceptable in these trials.Further well-designed RCTs are warranted to confirm these benefits and to explore safety further.

Fire needling for herpes zoster: A systematic review and meta-analysis of randomized clinical trials

Objective:To evaluate the effectiveness and safety of fire needling for herpes zoster from randomized clinical trials (RCTs).Methods:We searched Cochrane Central Register of Controlled Trials,Pubmed,Sino-Med,CNKI,VIP,WanFang databases,and conference proceedings to November,2017.RCTs were eligible if they tested fire needling for treating herpes zoster more than 3 times.Two authors screened all references,assessed the risk of bias,extracted data,independently,and analyzed data using Trial Sequential Analysis (TSA).Treatment effects were presented as risk ratio (RR)for binary data and standardized mean difference (SMD) for continuous data with 95% confidence interval (CI).Results:We included 27 RCTs with a total of 1933 participants.Only one RCT had low risk of bias,and the others were of high or moderate risk of bias.For total effectiveness rate (proportion of total number of people who were cured or significant symptom improved),there was no significant difference between Western medicine (acyclovir,valacyclovir,adenosine cobalamin) and fire needling (risk ratio 1.05,95% CI 0.98 to 1.12;n =5).For pain relief (VAS scale):fire needling used alone showed lower scores than Western medicine (SMD-1.37,95% CI-1.77 to-0.97;n =2) or external medicine (diclofenac) (SMD-2.23,95% CI-2.81 to-1.64;n =1).Combination of fire needling and Western medicine was better than Western medicine alone in relieving pain (VAS scale) (SMD-2.19,95% CI-3.40 to-0.97,I2 =94%;n =4).Patients receiving fire needling had lower incidence of neuralgia than those receiving Western medicine (3.3% vs 26.7%,RR 0.09,95% Cl 0.01 to 0.82;n =1) at follow up for 30 days.No serious adverse events such as infection were reported.Conclusion:Fire needling appears to offer relief for alleviating pain in herpes zoster.As the sample size of included trials was small and the quality of studies was generally low,rigorous clinical trials with robust reporting and appropriate outcome measures are still needed.

Evidence base of randomized controlled trials and guideline recommendations of patent Traditional Chinese Medicines for uncomplicated acute lower respiratory tract infections in adults

OBJECTIVE:To compare clinical practice guideline recommendations on the use of oral patent Traditional Chinese Medicines(PTCMs) for uncomplicated acute lower respiratory tract infections(ALRTIs)in adults with the existing evidence using results of a systematic review of randomized controlled trials(RCTs).METHODS:A systematic review on RCTs and a systematic review of current guidelines on orally taken PTCMs for uncomplicated ALRTIs were performed.Pub Med,Cochrane Library,EMBASE and four Chinese databases were searched from inception to September 2016 for RCTs testing orally taken PTCMs for uncomplicated ALRTIs(excluding pneumonia).Two reviewers independently screened each study,extracted study data,and assessed risk of bias.Disagreements were resolved through discussion or by consultation with a third reviewer.Clinical practice guidelines for uncomplicated ALRTIs containing PTCM recommendations were identified and quality appraised.The quality of pooled evidence of the RCTs and the guidelines was assessed with GRADE and AGREE Ⅱ respectively.The consistency of the evidence base in RCTs and the guideline recommendations were then compared.RESULTS:For the systematic review of RCTs,4810 papers were identified,among which 29 RCTs(5093 patients) were included in the review.PTCMs compared to placebo increased the effective treatment rate of cough(3 trials,949 patients,risk ratio(RR) 2.50,1.16 to 5.43;low certainty);improved assessment of global health(3 trials,948 patients,RR1.70,1.44 to 2.01;low certainty);and increased the effective rate of specific symptom relief(1 trial,478 patients,RR 4.01,2.76 to 5.81;moderate certainty).21 trials(3432 patients) compared effects of different PTCMs.For the guideline evaluation,29 PTCMs were recommended for the use of uncomplicated ALRTIs,of which27 had no supportive evidence from RCTs.CONCLUSION:The evidence base of PTCMs for uncomplicated ALRTIs is weak and the guideline recommendations were based on almost no clinical trial evidence.Rigorous clinical research is urgently needed to

Chinese herbal medicine for hand-foot-and-mouth disease in children: An overview of systematic reviews

Objective:To summarize the use of Chinese herbal medicines (CHM) for the treatment of hand-foot-and-mouth disease (HFMD) in children and to provide high-level evidence for clinical decision-making.Methods:We conducted an overview of systematic reviews (SRs).Two English-language and four Chinese-language electronic databases were searched from inception to March 31,2018.Published SRs and meta-analyses evaluating CHM use in children with HFMD and reporting clinically-relevant outcomes such as time to fever resolution were eligible for inclusion in this overview.Reviews were accepted if the intervention featured CHM with or without other treatment.Two authors evaluated the methodological quality of the included SRs by using ASMTAR and ROBIS.Results:Thirty SRs comprising 90 244 children with HFMD were included in this overview of SRs.All SRs were published in Chinese between 2011 and 2017.All intervention arms received CHM with or without conventional treatment and were compared against control arms receiving no treatment,conventional treatment alone,or placebo.All 30 reviews reported the time to fever resolution and results demonstrated that CHM had a greater benefit in reducing fever compared with controls.Twenty-three reviews reported the time to rash resolution which presented similar results.Fourteen SRs reported the time to oral ulcer healing and CHM demonstrated a significantly reduced time to resolution compared to controls.The quality of the included SRs was low to moderate as assessed by the AMSTAR tool.Conclusion:Published reviews demonstrated potential benefits of CHM in children with HFMD.Overall,the methodological quality of reviews included in this overview of SRs was low and our findings should be interpreted with caution.We would strongly recommend that future SRs be designed and reported rigorously following PRISMA in order to provide more robust evidence on which to base clinical guidance.

Chemokines are best known for their abilities of recruiting immune cells and forming lymphoid tissue.Through interactions between chemokines and their receptors,various immune cell subsets are recruited into the tumor microenvironment which is the primary location for tumor cells interacting with responding host cells.In recent decades,a large volume of studies have revealed chemokines'role in the tumor microenviron-ment in regulating tumor growth,invasion,and/or metastasis as well as tumor immune response;however,their molecular mechanisms are not well understood.Recently,increasing evidence has reported the import-ance of the CXCL13-CXCR5 axis in the tumor microenvironment of vanious human malignancies.Thus,in this review,we will focus on the CXCL13-CXCR5 axis and elaborate on the expression patterns,regulating and cor-responding regulatory mechanisms as well as clinical values in a wide range of human cancers.

Physiology Mechanism of Metabolisms in the Middle Scales of Lilium davidii var. unicolor Bulbs Stored at Low Temperature for Dormancy-Release

Changes in carbohydrate, phenols, free amino acids and endogenous hormones in middle scales of Lilium davidii var.unicolor bulbs during the 101 days storage at 2, 6 and 10°C in moist sawdust were studied. The results indicated that asthe storage time increased and the storage temperature delined, contents of starch and abscisic acid (ABA) decreasedmarkedly, and the level of total soluble sugar, phenols, gibberellic acid (GA3) and indole acetic acid (IAA) increasedsharply. In the first 34 days, the contents of the above substances changed most actively. Correlation analysis revealedthat the increase in content of total soluble sugar and phenols was respectively caused by activation of amylase andphenylalanine ammonia-lyase (PAL). During the storage, the content of free amino acids decreased, and the level ofarginine was the highest and changed most remarkably. According to the results of stepwise regression analysis and pathanalysis, we could primarily conclude that the metabolism of carbohydrates, phenols and free amino acids in the middlescales may be mainly regulated by endogenous GA3.

目的:探讨efa(2)对刀豆蛋白(con-A)诱导的自身免疫性肝炎小鼠模型的保护作用及其可能的作用机制。方法:将50只昆明种小鼠随机分为正常对照组、con-A自身免疫性肝炎模型组及样品efa(2)高(0. 04mL/20g)、中(0. 02mL/20g)、低(0. 01mL/20g)剂量组,分别给予0. 9%氯化钠注射液或相应浓度的efa(2)样品灌胃,每天1次,连续7d。本次灌胃结束禁食1晚后,尾静脉注射Con-A,断头取血测生化指标,并取肝右叶组织作病理切片,对肝脏组织中的CYP5A1、CYP2E1和CYP3A进行蛋白分析。结果:efa(2)能使自身免疫性肝炎模型小鼠血清中IL-10升高以及IFN-γ、TNF-α、PGE1活性降低。结论:efa(2)具有调节肝炎模型小鼠血清各细胞因子及ALT的水平,并显著改善肝脏组织病理变化的作用,其保护机制可能与CYP2E1相关。

使用三种亚格子应力模型,对长深比(L/D)为5的三维矩形开式空腔的可压缩流体进行大涡模拟计算。研究得到的空腔自激振荡频率与Rossiter公式计算结果和实验结果吻合良好,结果显示振荡能量主要集中在较低频率区域,压力幅值主要出现在前三阶模态。Dynamic Smagorinsky-Lilly(DSM)模型在空腔前后壁面附近区域的脉动强度分布比Smagorinsky-lily(SM)模型更为接近实验值,Wall Adapting Local Eddy Viscosity(WALE)模型的脉动强度分布与实验值最为接近。由空腔底部监测点声压级分布及声压频谱图可以看出:WALE模型性能最佳,DSM模型结果也与实验结果相符合,SM模型的预测性能略差。

采用SSR荧光标记技术对240份睡莲资源进行遗传多样性分析;利用Structure软件分析其群体结构,采用主成分分析和聚类分析进行验证;基于最小距离逐步取样法构建核心种质并进行t筛选验证。结果表明,16对SSR荧光引物共检测到205个等位基因,平均Shannon’s信息指数(I)为0.2036,Nei’s基因多样性指数(H)为0.1168,有效等位基因数(N_(e))为1.1697,表明睡莲的遗传多样性丰富。Structure软件分析结果显示最优的群体数K值为3,群体间有少量种质混杂;群体结构分析将240个睡莲品种(种)分为3个类群。采用最小距离逐步取样法,按70%、65%、60%、55%、50%、45%、40%、35%、30%、25%、20%、15%和10%的比例取样,各个核心子集遗传多样性指数总体上变化不明显;最终确立了15%的取样比例,包含36个睡莲品种(种)核心种质,其中Shannon’s信息指数(I)保留率为117%,Nei’基因多样性指数(H)保留率为118%,有效等位基因数(N_(e))保留率为102%。t检验结果表明,构建的包含36个睡莲品种(种)的核心种质与原始种质的遗传参数无显著差异,能充分代表原始种质的多样性。
