Long-term stroke rates after catheter ablation or antiarrhythmic drug therapy for atrial fibrillation： a meta-analysis of randomized trials

Background Atrial fibrillation （AF） is an independent risk factor for ischemic stroke and is associated with increased risk of death. Randomized studies suggest improved quality of life for patients with AF after successful catheter ablation compared to antiarrhythmic drug therapy. The value of ablation in long-term risk of ischemic stroke, however, has not been assessed. We conducted a meta-analysis to determine whether AF ablation reduces the long-term risk of stroke compared to antiarrhythmic drug therapy in randomized controlled trials. Methods ＆ Results PubMed and the Cochrane Central Register were searched for randomized trials from January 1990 to December 2014 comparing AF catheter ablation to drug therapy. The results are reported as risk differences （RDs） and 95% CI. Thirteen trials were analyzed with 1097 patients treated by catheter ablation and 855 patients received antiarrhythmic drug therapy. Overall, seven patients （0.64%） in the catheter ablation group had ischemic stroke or transient ischemic attacks vs. two patients （0.23%） in the drug therapy group. No difference was shown in the rate of stroke or transient ischemic attack between ablation and drug therapy （RD： 0.003, 95% CI： -0.006 to 0.012, P = 0.470）, and no evidence of heterogeneity was observed （I^2 = 0, P = 0.981）. No potential publication bias was found. There was also no difference in mortality between the two groups （RD： -0.004, 95% CI： -0.014 to 0.006, P = 0.472）. Conclusions This meta-analysis of randomized controlled trials showed similar rates of ischemic stroke or transient ischemic attack and death in AF patients undergoing catheter ablation compared to drug therapy. A larger prospective randomized trial to confirm this finding is warranted.

目的探讨三七总皂苷(PNS)对巨噬细胞分拣蛋白(sortilin)、腺苷三磷酸结合盒转运子A1(ABCA1)表达的影响和抑制泡沫细胞形成的作用及机制。方法将巨噬细胞分为A组(常规培养)、B组(转染空载慢病毒)、C组(转染过表达sortilin慢病毒)、D组(转染过表达sortilin慢病毒+60μg·mL^(-1)PNS)和E组(转染过表达sortilin慢病毒+10μmol·L^(-1)PD98059+60μg·mL^(-1)PNS)。用蛋白质印迹法检测各组巨噬细胞中sortilin、ABCA1、细胞外调节蛋白激酶(ERK)和磷酸化ERK(p-ERK)蛋白的表达水平;用50μg·mL^(-1)氧化低密度脂蛋白刺激巨噬细胞形成泡沫细胞,用油红O染色评估各组巨噬细胞的脂质含量。结果A、B、C、D、E组的sortilin蛋白相对表达水平分别为1.00±0.08、0.91±0.15、2.28±0.13、1.62±0.09和2.01±0.08,ABCA1蛋白相对表达水平分别为1.00±0.01、0.92±0.07、0.29±0.04、0.66±0.09和0.44±0.07,p-ERK/ERK比值分别为1.00±0.09、0.92±0.05、1.03±0.12、2.00±0.12和1.64±0.14,脂质含量分别为(4.82±2.19)%、(6.70±0.88)%、(44.56±4.15)%、(27.66±3.25)%和(41.67±5.45)%。除p-ERK/ERK外,C组的其他指标与A组相比,在统计学上差异均有统计学意义(均Plin表达和上调ABCA1表达,从而减少泡沫细胞的形成;ERK是参与PNS调节sortilin和ABCA1表达的主要信号通路之一。

Video harmonization is an important step in video editing to achieve visual consistency by adjusting foreground appear-ances in both spatial and temporal *** methods always only harmonize on a single scale or ignore the inaccuracy of flow estimation,which leads to limited harmonization *** this work,we propose a novel architecture for video harmoniza-tion by making full use of spatiotemporal features and yield temporally consistent harmonized *** introduce multiscale harmon-ization by using nonlocal similarity on each scale to make the foreground more consistent with the *** also propose a fore-ground temporal aggregator to dynamically aggregate neighboring frames at the feature level to alleviate the effect of inaccurate estim-ated flow and ensure temporal *** experimental results demonstrate the superiority of our method over other state-of-the-art methods in both quantitative and visual comparisons.

One of the challenges in global change research is the significant uncertainty in global historical land use and land cover(LUCC)datasets,which are widely used as foundational *** addition to the regional cropland area reconstructions,improving the grid allocation method is another feasible way to raise the reliability of historical LUCC *** this study,an integrated reconstruction of the national cropland areas over the past 200 years was developed for 36 European *** that,the allocation algorithm was built using physiogeographic variables and historical city sites for accounting for land suitability and cultivation preferences,***,cropland data in Europe with a spatial resolution of 5′×5′at five time sections from AD 1800 to 2000 were generated using the optimal allocation algorithm in accordance with the stages of the regional *** results were as follows:(1)The dominant factors governing the distribution of croplands in Europe vary at different agricultural stages,but the results can be merged *** suitability was more optimal for allocation during the modern agricultural stage(AD 1950 and 2000);the priority index combined with land suitability and cultivation preference was more reasonable for allocation during the traditional agricultural stage(AD 1800).The average of the allocations by priority index and the land suitability could be adopted as the allocation results during the transitional stage(AD 1850 and 1900)because the grids for absolute differences within±10 and±20 percentage points between the results obtained from the above two allocations were above 80% and 95%,respectively,which means the two allocation results could be merged.(2)Over the past 200 years,the total cropland area in Europe first increased to a peak in AD 1900 and then ***,the centre of the higher cropland fraction shifted from the western part of Europe in AD 1800 to the eastern part of the continent after AD 1950.(3)

The dynamic effect is a very important issue widely debated by scholars when studying the genetic and disaster-causing mechanisms of earthquake-triggered ***,the dynamic effect mechanism and phenomena of earthquake-triggered landslides were summarized in this ***,the primary types of dynamic effects were further used to interpret the Mogangling landslide in Moxi Town of Luding County,China.A field investigation,remote sensing,numerical calculation and theoretical analysis were carried out to illustrate the failure mechanism of slope rock masses affected by *** interaction between seismic waves and slope rock masses and the induced dynamic effect of slope rock masses were primarily accounted for in the *** slope topography,rock mass weathering and unloading characteristics,river erosion,regional seismogenic structure,and rock mass structure characteristics were also *** results showed that the formation of the Mogangling landslide was mainly related to the high amplification effect of seismic acceleration and back slope effects,interface dynamic stress effects,and double-sided slope effects of seismic waves caused by the catastrophic Ms 7.75 Moxi Earthquake in *** principles for the site and route selection of large-scale infrastructure in the planning stage and the scientific prevention of seismic geological disasters were proposed on the basis of the dynamic effect of earthquake-induced landslides.

In this work,low-softening-temperature and low-modulus SiO_(2) was introduced as an embedded interface between SiC nanowires(SiCnws)and a Ba_(x)Sr_(1−x)Al_(2)Si_(2)O_(8) ceramic matrix to enhance strength and toughness of the *** the sintering process,molten SiO_(2) enhances the flowability of the ceramic powders and modifies the dispersion of *** strengthening effect of SiCnws was fully realized,and the flexural strength of the optimized ceramics reached 193±16 MPa,which represents an increase of 52.6%.After the formation of the embedded SiO_(2) interface with a low modulus,cracks can deflect along the SiCnws surface,which is consistent with the criterion of He and *** can effectively extend the crack propagation path,and the fracture toughness(K_(IC))is thus improved by 94.0%,reaching 3.1±0.5 MPa·m^(1/2).

In order to analyze the influence of the addition of yttrium and manganese on the soot combustion performance and high temperature stability of CeO_(2) catalyst,a series of Y/Mn-modified CeO_(2) catalysts were *** effects of structural properties,textural properties,oxygen vacancies,Ce^(3+),surface adsorbed oxygen species,reduction properties and desorption properties of oxygen species on the activity were analyzed by various characterization *** results of the activity test show that the addition of manganese is beneficial to enhancement of the activity,while the addition of yttrium increases the amount of reactive oxygen species,but decreases the *** aging at 700℃,the activity of the CeMn catalyst decreases most sharply,while the catalytic activity of the CeY catalyst can be maintained to a certain ***,the addition of yttrium and manganese at the same time can stabilize the *** fundamental reason is that yttrium and manganese move to the surface of the solid solution after aging,which increases the reduction performance of the catalyst,thus contributing to the increase of *** the activity of CeYMn catalyst decreases after aging at 800℃,it is still higher than that of other catalysts aged at 700℃.
